본 발명은 기구식 비구컵 항행장치에 관한 것으로서, 더 구체적으로는, 인공 고관절 수술시, 전방골반평면과 평행한 평면 또는 이와 소정의 각도로 경사진 평면을 표시하는 참조기구를 이용하여 비구컵이 삽입되는 방향을 안내하는 기구식 비구컵 항행장치에 관한 것이다.
고관절은 공 모양을 갖는 대퇴골의 골두와 이를 감싸고 있는 소켓 모양의 골반골의 비구로 이루어진 관절을 말한다. 이러한 고관절이 여러가지 이유로 심하게 손상되면 관절 부위를 제거하고 인공관절수술을 하게 되는데, 이를 인공고관절 수술 또는 고관절 전치환술이라고 한다. 인공관절은 대퇴골에 삽입되어 골두를 대체하는 부분과 골반에 삽입되어 비구를 대체하는 부분으로 이루어진다.
이러한 인공고관절 수술에서 골반의 비구를 대체하는 비구컵은 정확한 방향으로 골반에 삽입되어야 한다. 비구컵의 삽입 방향은 골반의 3개의 지점, 즉, 좌-, 우-위앞엉덩뼈가시와 두덩결합에 의하여 한정되는 전방골반평면을 기준으로 정해진다. 비구컵이 정확하게 삽입되지 않는 경우에는 인공고관절의 수명이 짧아지며, 심한 경우 탈골되는 등의 문제가 발생할 수 있다.
이에 따라, 비구컵을 정확하게 삽입할 수 있도록 안내하는 비구컵 항행장치가 제안되었다.
미국 특허 제5,141,512호에는 광학식 비구컵 안내장치가 개시되어 있다. 이 장치는 각도 조절부를 갖는 광원과 3개의 다리부를 구비한다. 다리부는 앞에서 설명한 골반의 3개의 지점에 각각 고정된다. 광원으로부터 방출되는 광선의 방향은 각도 조절부에 의하여 비구컵이 삽입되는 방향에 대응하도록 조절된다. 이 광선은 비구컵 삽입장치에 부착된 거울에서 반사되는데, 입사광과 반사광이 일치하는 경우 비구컵이 정확한 방향으로 정렬된다. 그러나, 이 장치를 사용하는 경우에는, 수술 중에 환자의 자세가 달라지는 경우가 많은데 프레임의 다리부가 고정되기 때문에 환자의 자세를 바꾸기가 어렵고, 광선이 다른 수술 장비로 인하여 차단되어 정렬 작업이 방해되는 등의 문제가 있다.
미국 특허 제6,711,431호에는 컴퓨터를 이용한 비구컵 안내장치가 개시되어 있다. 이 장치는 골반 상의 적어도 3개의 지점을 참조로 하여 골반평면을 한정하고, 골반에 고정되는 골반 추적 마커를 위치 추적 장치를 이용하여 추적함으로써 실시간으로 골반평면의 방향 변화를 추적한다. 이 장치를 이용하면, 환자의 자세가 바뀌어도 골반평면을 추적할 수 있다. 그러나, 전선 케이블에 의한 감염의 우려가 있으며, 장비의 크기가 커서 협소한 장소에서 수술하는 경우 방해받을 수 있고, 장비가 상대적으로 고가라는 등의 곤란함이 있다.
본 발명은 전술한 문제점을 해결하기 위한 것으로서, 구조가 복잡하고 고가인 광학식, 전자식 장치를 사용하지 않고 단순한 구조를 갖는 참조기구를 이용하여 비구컵의 삽입 방향을 안내하는 비구컵 항행장치 및 방법을 제공하는 것을 목적으로 한다.
또한, 본 발명은 수술 중 환자의 자세가 달라져도 비구컵 삽입시 기준이 되 는 평면을 계속 표시할 수 있는 참조기구를 구비하는 비구컵 항행장치 및 방법을 제공하는 것을 목적으로 한다.
본 발명의 일 형태에 따르면, 인공 고관절 수술시 비구컵이 골반에 삽입되는 방향을 안내하는 비구컵 항행장치로서, 전방골반평면 상에 놓인 골반의 3개의 특이점과 접촉하도록 배치되는 탐침부와 이 탐침부가 상기 특이점과 접촉하도록 놓인 경우 소정의 참조평면을 표시하도록 설치된 제1 참조기구부를 구비하는 골반위치 탐색장치, 및, 골반에 고정되도록 설치되고, 상기 골반위치 탐색장치의 제1 참조기구부에 의하여 표시되는 소정의 참조평면과 평행하는 평면, 이와 직교하는 평면 또는 양자 모두를 표시하도록 조정가능한 제2 참조기구부를 구비하는 골반위치 표시장치를 포함하는 참조기구를 이용하는 비구컵 항행장치가 제공된다.
본 발명의 다른 형태에 따르면, 청구항 1에 기재된 참조기구를 이용하는 비구컵 항행장치를 사용하여, 인공 고관절 수술시 비구컵이 골반에 삽입되는 방향을 안내하는 방법으로서, (a) 상기 골반위치 표시장치를 골반에 고정시키는 단계와, (b) 상기 골반위치 탐색장치의 탐침부가 골반의 3개의 특이점과 접촉하도록 상기 골반위치 탐색장치를 배열하여 상기 제1 참조기구부가 소정의 참조평면을 표시하도록 하는 단계와, (c) 상기 골반위치 탐색장치의 제1 참조기구부가 표시하는 상기 소정의 참조평면과 평행한 평면 및/또는 이와 직교하는 평면을 표시하도록 상기 골반위치 표시장치의 제2 참조기구부를 조정한 후 고정시키는 단계와, (d) 골반 상에 배열된 상기 골반위치 탐색장치를 제거하는 단계와, (e) 상기 골반위치 표시장치를 골반으로부터 분리시키는 단계와, (f) 골반을 원하는 위치로 배치시키는 단계와, (g) 상기 골반위치 표시장치를 상기 단계 (a)에서 골반에 고정되었던 지점에 다시 고정시키는 단계, 및 (h) 상기 골반위치 표시장치의 제2 참조기구부가 표시하는 평면을 참조로 하여 비구컵을 항행하는 단계를 포함하는 비구컵 항행방법이 제공된다.
본 발명의 전술한 특징 및 다른 특징과 이점은 이하에서 도면을 참조로 하여 설명하는 바람직한 실시예들로부터 더욱 분명해질 것이다.
도 1에는 본 발명의 일실시예에 따른 골반위치 탐색장치가 도시되어 있으며, 도 2는 본 발명의 일실시예에 따른 골반위치 표시장치의 분해사시도이고, 도 3은 도 2에 도시된 골반위치 표시장치의 단면도이다.
도 1에 도시된 바와 같이, 골반위치 탐색장치(100)는 대체로 Y자 형태를 갖도록 3개의 부재(101, 102, 103)가 결합되어 있으며, 각 부재에는 탐침(111, 112, 113)이 설치되고 제1 부재(101)의 한쪽 단부에는 참조기구(120)가 부착되어 있다.
중앙에 배치되어 다른 부재를 지지하는 제1 부재(101)에는 길이방향 구멍(106)이 마련된다. 좌, 우의 제2 및 제3 부재(102, 103)는 일단부가 연결핀으로 서로에 대하여 회전가능하도록 연결되고 이 연결핀은 제1 부재(101)의 위 길이방향 구멍(106)에 삽입된다. 연결핀은 제1 부재(101)의 이 길이방향 구멍(106)을 따라 이동할 수 있도록 설치된다. 또한, 제2 및 제3 부재보다 길이가 짧은 2개의 링크 부재(104, 105)가 각각 제2 및 제3 부재와 연결되고 이들 링크 부재(104, 105)의 단부도 서로 회전가능하게 핀 등을 수단으로 연결된 후 제1 부재(101)에 고정된다. 이에 따라, 연결핀이 제1 부재(101)의 길이방향 구멍(106)을 따라 이동하는 경우 제2 및 제3 부재(102, 103)는 링크식으로 간격이 넓어지거나 좁아질 수 있다. 한편, 적절한 배치가 이루어진 후에 제2 및 제3 부재(102, 103)가 움직이지 않는 것이 좋으므로, 나사 등을 이용하여 연결핀을 고정할 수 있도록 구성하는 것이 바람직하다. 길이방향 구멍(106)은 1개 이상이 마련될 수 있다.
제1 부재(101)의 길이방향 구멍(106)에는 추가로 탐침(111)이 구멍(106)의 길이방향을 따라 이동할 수 있도록 설치된다. 또한, 이 탐침(111)은 나사 등을 이용하여 사용자가 원하는 위치에 고정할 수 있도록 구성되는 것이 좋다. 제2 및 제3 부재(102, 103)에도 각각 탐침(112, 113)이 연결된다. 도 1에 도시된 바와 같이, 원하는 위치에 탐침을 고정할 수 있도록 복수개의 탐침 고정용 구멍이 이들 부재(102, 103)에 마련되는 것이 좋다. 각 탐침은 골반의 소정의 지점과 접촉하며, 사용자는 이들을 이용하여 전방골반평면을 탐색한다. 예를 들어, 사용자는, 제1 부재(101)에 고정된 탐침(111)이 골반의 두덩결합과 접촉하며, 제2 및 제3 부재(102, 103)에 고정된 탐침(112, 113)은 각각 골반의 좌-, 우- 위앞엉덩뼈가시와 접촉하도록 이들을 배치할 수 있다.
한편, 탐침(111)은 도 9에 도시된 바와 같이 L자 형태를 갖도록 두덩결합과 접촉하는 단부가 구부러지게 형성될 수 있다. 골반이 좁은 경우, 제2 및 제3 부재(102, 103)를 고정하는 고정 나사와 탐침(111)을 고정하는 고정 나사가 서로 간섭할 수 있다. 그러나 탐침(111)의 단부를 구부러지게 형성하고, 이 구부러진 단부의 일부를 두덩결합에 접촉하도록 하면, 구부러진 단부의 길이만큼 탐침(111)을 고정 하는 나사가 아래쪽으로 이동할 수 있다. 이에 따라, 탐침(111)을 고정하는 나사와 제2 및 제3 부재(102, 103)를 고정하는 나사가 서로 간섭하지 않게 된다.
제1 부재(101)의 한쪽 단부에는 평판 형태를 갖는 참조기구(120)가 설치된다. 위에서 설명한 바와 같이, 각 탐침(111, 112, 113)이 골반의 3개의 지점, 즉, 두덩결합 및 좌-, 우- 위앞엉덩뼈가시와 접촉하는 경우, 참조기구(120)는 전방골반평면과 평행하는 평면을 표시하게 된다. 본 실시예에서는 참조기구가 평판 형태를 갖는 것으로 도시되었지만, 다른 형상의 다양한 참조기구가 사용될 수 있다. 예를 들어, 막대 형상을 갖는 2개의 부재를 이용하여 1개의 부재는 전방골반평면의 종축, 다른 부재는 전방골반평면의 횡축을 표시하도록 배치할 수 있다. 또한, 본 실시예에서는 참조기구가 전방골반평면과 평행하는 평면을 표시하도록 설치되었지만, 반드시 이에 한정되는 것은 아니며, 시상면에 대하여 평행하도록, 즉, 전방골반평면에 대하여 평행하는 참조기구를 구비하는 것도 가능하다.
도 2 및 3에 도시된 바와 같이, 골반위치 표시장치는 1개 이상의 핀(P)과 같은 고정수단을 매개로 하여 골반에 고정되는 지지 블록(210), 이 지지 블록(210)에 고정된 볼 조인트(220), 볼 조인트(220)과 연결되는 고정 블록(230) 및 이 고정 블록(230)에 설치되는 참조기구(240)를 구비하고 있다.
지지 블록(210)은 핀(P)을 매개로 하여 골반에 고정되고, 다른 부재를 지지한다. 도면에서는 2개의 핀(P)이 사용되었지만, 반드시 이에 한정되는 것은 아니며, 1개 또는 3개 이상의 핀이 사용될 수도 있다. 지지 블록(210)은 그 길이방향을 따라 블록을 관통하는 슬롯이 마련되고 이 슬롯의 간격은 나사(211)에 의하여 조절 된다. 따라서, 골반에 고정된 핀을 지지 블록의 슬롯에 삽입한 후 나사(211)를 조이면 지지 블록(210)이 고정된다. 또한, 지지 블록에는 구멍이 마련되고, 이 구멍으로 볼 조인트(220)가 삽입된다.
볼 조인트(220)는 구형 헤드(222)와 이로부터 연장되어 지지 블록에 마련된 구멍에 삽입되는 다리부(221)를 구비한다. 볼 조인트(220)의 구형 헤드(222)는 고정 블록(230)과 연결된다.
고정 블록(230)은 볼 조인트(220)의 구형 헤드(222)와 연결되는 내면을 구비한다. 바람직하게는, 상온에서는 고정 블록(230)의 내면이 수축하여 볼 조인트(220)의 구형 헤드(222)를 단단하게 조이고, 소정의 온도범위에서는 고정 블록(230)의 내면이 볼 조인트(220)의 구형 헤드(222)를 단단하게 조이지 않아서 고정 블록(230)이 볼 조인트(220)에 대하여 회전할 수 있도록 형성된다. 예를 들어, 고정 블록(230)은 이러한 기능을 수행할 수 있도록 형상 기억 합금으로 이루어질 수 있다. 한편, 소정의 온도범위에서 고정 블록(230)이 볼 조인트(220)에 대하여 회전할 수 있지만 이로부터 이탈되지 않도록 구성되는 것이 좋다. 소정의 온도범위로는 수술기구의 소독온도가 포함되는 범위인 것이 편리하다.
참조기구(240)는 평판 형태를 가지며, 고정 블록(230)에 마련된 2개의 돌기부(231)에 의하여 고정 블록(230)에 연결된다. 이를 위하여 참조기구(240)에는 돌기부(231)가 삽입되는 2개의 구멍이 마련된다. 골반위치 탐색장치(100)와 마찬가지로, 참조기구(240)는 평판 형상이 아닌 다른 형상을 가질 수도 있다.
도 4에는 시중에서 판매되고 있는 비구컵 삽입장치의 일례가 도시되어 있다.
비구컵 삽입장치(300)는 선단부에 삽입될 비구컵(310)이 부착되는 봉형 부재(320)와 손잡이(330)를 구비하며, 삽입각도를 확인할 수 있도록 참조기구(360)가 부착되어 있다. 도 4에 도시된 장치에서는 봉형 부재(320)와 손잡이(330) 사이에 배치된 블록(340)으로부터 지지봉(350)이 봉형 부재(320)의 길이방향 축과 소정의 각도를 갖고 연장되며, 이 지지봉(350)의 단부에 대략 A자 형태를 갖는 참조기구(360)가 부착되어 있다.
이하에서는, 위와 같은 구성을 갖는 골반위치 탐색장치(100)와 골반위치 표시장치(200)를 이용하여 비구컵 삽입장치(300)의 삽입 방향을 안내하는 방법에 대하여 설명한다.
먼저, 사용자는 골반위치 탐색장치(100)의 3개의 탐침(111, 112, 113)을 골반 상의 3개의 지점, 즉, 두덩결합과 좌-, 우- 위앞엉덩뼈가시와 각각 접촉시킨다. 도 5 및 6에는 골반(400)의 정면도 및 측면도가 도시되어 있으며, 위 3개의 지점이 표시되어 있다. 도면에서, 두덩결합은 참조부호 420a 또는 420b로 표시되어 있으며, 좌-, 우- 위앞엉덩뼈가시는 각각 참조부호 430a와 430b로 표시되어 있다. 전방골반평면(410)은 이들 3개의 지점을 포함하는 평면을 말한다.
골반위치 탐색장치(100)의 탐침이 위 3개의 지점과 각각 접촉하면, 골반위치 탐색장치(100)의 참조기구(120)는 전방골반평면(410)과 평행하게 배치된다. 이러한 상태가 도 7에 도시되어 있다.
다음으로, 골반의 일 지점에 미리 고정된 핀을 매개로 하여 골반위치 표시장치(200)가 골반에 고정된다. 도 7에서는, 골반위치 표시장치(200)가 좌-위앞엉덩뼈 가시 부근에 고정되어 있다. 이 때, 고정 블록(230)은 소정의 온도범위로 가열된 상태이다. 따라서, 사용자는 고정 블록(230)을 조정하여 골반위치 표시장치(200)의 참조기구(240)가 골반위치 탐색장치(100)의 참조장치(120)와 표시하는 평면과 평행하는 평면을 표시하도록 배치할 수 있다.
고정 블록(230)의 온도가 상온으로 하강하여 볼 조인트(220)에 단단하게 고정되면, 사용자는 골반위에 놓은 골반위치 탐색장치(100)를 제거한다. 이어서, 사용자는 골반위치 표시장치(200)의 지지 블록(210)에 구비된 고정 나사(210)를 조정하여 고정 블록(210)을 핀(P)으로부터 분리시킨다. 그러면, 수술 대상자의 골반에는 핀만이 고정되어 있으므로 수술 대상자를 용이하게 원하는 자세로 변경할 수 있다.
수술 대상자를 원하는 자세로 배치한 후에 다시 골반위치 표시장치(200)를 골반에 고정시킨다. 고정 블록(230)과 볼 조인트(220)는 단단하게 결합된 상태이므로, 참조기구(240)는 여전히 전방골반평면과 평행한 평면을 표시하게 된다.
사용자는 골반위치 표시장치(200)의 참조기구(240)를 참조로 하여 비구컵 삽입장치(300)를 항행하여, 원하는 위치에 비구컵을 삽입한다.
도 8에는 골반위치 표시장치(200)의 다른 실시예가 나타나 있다. 이 장치는 앞에서 설명한 실시예와 비교할 때, 참조기구가 다른 형태를 갖는 것을 제외하고는 동일한 구성을 갖는다. 따라서, 이하의 설명에서 도 2에 도시된 실시예와 동일한 구성을 갖는 부분은 동일한 참조부호로 표시한다.
도 8에 도시된 골반위치 표시장치는 참조기구로서 2개의 막대 형상 부재 (251, 252)를 구비한다. 이들 막대 부재(251, 252)는 블록(250)에 삽입되고, 이 블록(250)은 고정 블록(230)에 분리가능하게 고정된다. 블록(250)에는 고정 블록(230)의 돌기부(231)가 관통하여 삽입되는 구멍이 마련된다. 또한, 블록의 중앙부에는 슬롯이 마련되며, 슬롯에는 고정핀(253)이 삽입된다. 고정핀(253)은 돌기부(231)에 마련된 홈과 맞물려 블록(250)이 고정 블록(230)으로부터 이탈되지 않도록 한다. 블록(250)의 한쪽 모서리에는 경사면이 형성되고, 이 경사면에 막대 형상을 갖는 참조기구가 삽입되는 구멍이 배치된다. 그러면, 2개의 참조기구는 서로 다른 방향을 향하도록 배치될 수 있다. 도 8에서는 막대 형상을 갖는 참조기구가 전방골반평면과 직교하는 평면을 표시하도록 배치되어 있다.
사용자는 필요에 따라 1가지 형태의 참조기구만을 이용하거나 2가지 형태의 참조기구를 모두 이용할 수 있다. 즉, 우선, 평판 형상의 참조기구를 이용하여 골반위치 표시장치가 전방골반평면을 표시하도록 배치한 후에 이를 기준으로 비구컵 삽입장치를 안내하고, 다음으로, 평판 형상의 참조기구를 제거한 후 막대 형상의 참조기구를 설치하고 비구컵 삽입장치를 추가로 안내할 수 있다.
도 10 및 11에는 본 발명의 또다른 실시예에 따른 골반위치 탐색장치 및 골반위치 표시장치가 도시되어 있다. 마찬가지로, 이하의 설명에서 도 2에 도시된 실시예와 동일한 구성을 갖는 부분은 동일한 참조부호로 표시한다.
본 실시예에 따른 골반위치 탐색장치(100)는 레이저광을 이용하여 참조평면을 표시한다. 골반위치 탐색장치(100)는 일정한 평면을 표시하는 레이저광을 방출하는 레이저 수평기(150)를 구비한다. 시판되고 있는 레이저 수평기는 수평인 평 면, 즉, 레이저 수평기가 설치된 면과 평행하는 평면을 표시하는 것으로부터, 수평인 평면, 수직인 평면, 기타 사용자가 설정한 높이의 평면을 표시하는 기능을 갖는 것까지 다양한 것이 나와있다. 본 실시예에서는 전방골반평면 및/또는 시상면을 표시할 수 있으면 충분하다. 예를 들어, 레이저 수평기(150)는 전방골반평면과 평행하는 평면을 표시하는 레이저광을 방출할 수 있다. 레이저 수평기(150)는 다른 부재의 간섭을 받지 않고 레이저광을 방출할 수 있는 위치라면 어디든지 배치될 수 있다.
본 실시예에 따른 골반위치 표시장치는 레이저 수평기(150)로부터 방출되는 레이저광을 참조로 레이저광이 표시하는 평면과 직교하는 평면을 표시하는 제2 참조기구(240a)를 구비한다. 제2 참조기구(240a)는 다각형 평판 형상을 가지며, 본 실시예에서는, 도 11에 도시된 바와 같이 4각형 평판 형상을 갖는다. 평판에는 4각형의 면을 따르는 방향으로 복수개의 관통구멍이 마련된다. 예를 들어, 관통구멍은 평판의 가로방향을 따라 100개 세로방향을 따라 100개가 마련될 수 있다.
본 실시예에 따른 장치를 이용하여 비구컵을 안내하는 방법을 설명하면 아래와 같다.
골반위치 탐색장치가 골반의 3개의 특이점과 접촉하도록 배치되면, 골반위치 탐색장치에 설치된 레이저 수평기(150)는 전방골반평면과 평행하는 평면을 표시하는 레이저광을 방출한다. 그러면, 골반위치 표시장치에 마련된 제2 참조기구(240a)를 조정하여 레이저 수평기(150)로부터 방출되는 레이저광이 제2 참조기구(240a)의 관통구멍을 통과하도록 한다. 제2 참조기구는 소정의 두께를 갖는 평판의 형태를 갖기 때문에, 레이저광과 직교하도록 배치되지 않으면 레이저광이 차단되어 관통구멍을 통과하지 못한다. 따라서, 레이저광이 제2 참조기구(240a)의 관통구멍을 통과하도록 배치되는 경우 제2 참조기구(240a)는 레이저광이 표시하는 평면과 직교하는 평면을 표시하게 된다. 제2 참조기구(240a)에 대한 조정이 완료되고 제2 참조기구의 위치가 고정되면, 골반위치 탐색장치와 골반위치 표시장치를 골반으로부터 제거한다. 그러면 사용자는 골반을 시술하기 편리한 위치로 배열하기 용이하다. 골반의 배열이 완료되면 다시 골반위치 표시장치를 고정시키고, 제2 참조기구(240a)가 표시하는 평면을 참조로 하여 비구컵의 항행을 안내한다.
한편, 본 발명의 또다른 실시예로서, 제2 참조기구는 복수의 관통구멍을 구비하는 평판이 아니라 거울면을 갖는 평판형상의 부재를 사용할 수 있다. 이 실시예에서, 제1 참조기구부는 소정의 참조평면에 대하여 평행하는 레이저광을 방출하는 레이저 지시기와 소정의 참조평면에 대하여 수직인 방향을 지시하는 막대 형상의 참조기구를 구비한다. 또한, 제2 참조기구부는 제1 참조기구부의 레이저 지시기로부터 방출되는 레이저광을 반사할 수 있도록 거울면을 갖는 참조기구와 이 거울면을 갖는 참조기구에 거울면과 평행하게 부착되는 막대 형상의 참조기구를 구비한다.
이러한 구성을 갖는 비구컵 항행장치는 다음과 같은 방식으로 작동된다.
제1 참조기구부에 대하여는 전술한 실시예들과 동일하다. 제1 참조기구부가 골반 위에 정렬되면, 레이저 지시기는 소정의 참조평면, 예를 들어, 전반 골반 평면에 대하여 평행한 방향으로 진행하는 레이저광을 방출하게 된다. 막대 형상의 참 조기구는 소정의 참조평면에 대하여 수직인 방향을 지시한다. 제2 참조기구부를 상기 제1 참조기구부에 대하여 정렬할 경우에는, 제1 참조기구부의 레이저 지시기로부터 방출되는 레이저광이 입사된 경로로 출사되고 상기 제2 참조기구부의 막대 형상의 참조기구가 상기 제1 참조기구부의 막대 형상의 참조기구와 평행하도록 상기 거울면을 갖는 참조기구를 정렬시킨다.
이상에서 본 발명이 바람직한 실시예를 참조로 하여 설명되었지만, 당해 기술분야에서 통상의 지식을 갖는 자라면 후술하는 청구범위를 벗어나지 않는 범위 내에서 다양하게 변경 또는 수정하여 실시할 수 있을 것이다.
전술한 바와 같이, 본 발명은 구조가 복잡하고 고가인 광학식, 전자식 장치를 사용하지 않고 단순한 구조를 갖는 참조기구를 이용하여 비구컵의 삽입 방향을 안내할 수 있다.
또한, 본 발명은 수술 중 환자의 자세가 달라져도 비구컵 삽입시 기준이 되는 평면을 계속 표시할 수 있으므로, 비구컵을 정확하게 안내할 수 있다.

Claims (15)

  1. 인공 고관절 수술시 비구컵이 골반에 삽입되는 방향을 안내하는 비구컵 항행장치로서,
    전방골반평면 상에 놓인 골반의 3개의 특이점 - 두덩결합, 좌-위앞엉덩뼈가시 및 우-위앞엉덩뼈가시 - 과 접촉하도록 배치되는 탐침부와 이 탐침부가 상기 특이점과 접촉하도록 놓인 경우 참조평면을 표시하도록 설치된 제1 참조기구부를 구비하는 골반위치 탐색장치; 및
    골반에 고정되도록 설치되고, 상기 골반위치 탐색장치의 제1 참조기구부에 의하여 표시되는 참조평면과 평행하는 평면, 이와 직교하는 평면 또는 양자 모두를 표시하도록 조정가능한 제2 참조기구부를 구비하는 골반위치 표시장치
    를 포함하는 참조기구를 이용하는 비구컵 항행장치.
  2. 청구항 1에 있어서,
    상기 골반위치 탐색기구의 탐침부는, 두덩결합과 접촉하는 제1 탐침과, 좌-위앞엉덩뼈가시와 접촉하는 제2 탐침, 및 우-위앞엉덩뼈가시와 접촉하는 제3 탐침을 갖는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  3. 청구항 2에 있어서,
    상기 제1 탐침은 두덩결합과 접촉하는 단부가 구부러져 L자 형상을 갖는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  4. 청구항 1에 있어서,
    상기 제1 참조기구부가 표시하는 참조평면은 전방골반평면 또는 시상면인 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  5. 청구항 1에 있어서,
    상기 골반위치 표시장치는, 골반에 고정되는 1개 이상의 핀과, 이 핀에 의하여 지지되는 기부와, 이 기부로부터 연장되도록 설치되는 볼 조인트와, 이 볼 조인트와 결합되고 상기 제2 참조기구가 배열되는 고정블록을 더 구비하는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  6. 청구항 5에 있어서,
    상기 고정블록은 형상기억합금으로 이루어지며, 상온보다 높은 온도 범위에서 변형되어 볼 조인트와 간격이 발생하고 상온에서는 볼 조인트와 밀착하여 고정되는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  7. 청구항 5에 있어서,
    상기 고정블록은 체결나사로 결합되는 2개의 블록을 구비하는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  8. 청구항 1에 있어서,
    상기 제1 및 제2 참조기구는 다각형 평판 형상을 갖는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  9. 청구항 1에 있어서,
    상기 제1 및 제2 참조기구는 1개 이상의 막대 형상을 갖는 부재인 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  10. 청구항 1에 있어서,
    상기 골반위치 표시장치는, 상기 제2 참조기구부가 표시하는 평면에 대하여 미리 정해진 각도로 경사진 평면을 표시하는 제3 참조기구부를 더 구비하는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  11. 청구항 10에 있어서,
    상기 골반위치 표시장치의 제3 참조기구부가 표시하는 평면은 상기 제2 참조기구부가 표시하는 평면과 서로 직교하는 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  12. 청구항 1에 있어서,
    상기 제1 참조기구부는 상기 참조평면을 표시하는 레이저광을 방출하는 레이저 수평기이며, 상기 제2 참조기구부는 상기 레이저 수평기로부터 방출되는 레이저광이 통과할 수 있도록 다수개의 관통구멍이 마련된 다각형 평판 형상을 갖는 부재인 것을 특징으로 하는 참조기구를 이용하는 비구컵 항행장치.
  13. 청구항 1에 있어서,
    상기 제1 참조기구부는 상기 참조평면을 표시하는 레이저광을 방출하는 레이저 지시기와 상기 참조평면에 대하여 수직인 방향을 지시하는 막대 형상의 참조기구를 구비하며, 상기 제2 참조기구부는 상기 레이저 지시기로부터 방출되는 레이저광을 반사할 수 있도록 거울면을 갖는 참조기구와 이 거울면을 갖는 참조기구에 거울면에 대하여 평행하게 부착되는 막대 형상의 참조기구를 구비하고,
    상기 제2 참조기구를 상기 제1 참조기구에 대하여 정렬할 경우, 상기 레이저 지시기로부터 방출되는 레이저광이 입사된 경로로 출사되고 상기 제2 참조기구부의 막대 형상의 참조기구부가 상기 제1 참조기구부의 막대 형상의 참조기구와 평행하도록 상기 거울면을 갖는 참조기구를 정렬시키는 것을 특징으로 하는 비구컵 항행장치.
