Abstract

본 추골간 임플랜트는 상면(11)과 저면(12), 두 개의 측면(13, 14), 전방벽(16)과 후방벽(15)을 가지고 공간(20)을 둘러싸는 프레임형상의 케이지(1)를 갖는다. 상면과 저면(11, 12)은 다수의 관통공(24)을 가지고, 관통공(24)의 전체 면적은 상면과 저면(11, 12)의 전체 면적의 40 내지 55%이다. 각 관통공(24)의 개별면적은 최대한 상면과 저면(11, 12)의 전체 면적의 20%이다. 공간(20)의 체적(VH)과 케이지(1)의 전체 체적(VK) 간의 비 VH/VK는 70 내지 90%의 범위내에 있다. 케이지(1)는 전방벽(16)을 향하여 벌어지는 상면과 저면(11, 12)을 가진 거의 쐐기형상으로 형성되어 있다. 이에 의해서 상면과 저면의 뼈지지면이 커지기 때문에 임플랜트가 추골체의 단부판의 내부로 침하되는 것을 방지할 수 있다는 장점이 얻어진다.

Description

추골간 임플랜트
본 발명은 청구항 1의 전제부에 따른 추골간(椎骨間) 임플랜트 및 청구항 21의 전제부에 따른 조합임플랜트에 관한 것이다.
이러한 추골간 임플랜트는 두 추골체의 합병시 특히 요추영역에 삽입된다. 추골간 공간마다 하나 혹은 두 개의 임플랜트가 사용된다.
종래기술로부터 이미 다양한 형태의 추골간 임플랜트가 공지되어 있으며, 이들은 임플랜트가 관련 추골의 단부판내에 침하될 위험을 내포하고 있다는 단점을 가지고 있다. 예를 들어 US 5, 192, 327(브란티간)으로부터 알려져 있는 추골간 임플랜트는 상부와 하부가 개방된 링 혹은 이중링의 형태를 갖는다. 이중링 구성에 있어서는, 링임플랜트의 연부만이 그리고 기껏해야 좁은 연결리브가 부가적으로 뼈접촉면으로 되기 때문에, 그에 의해 이격되어 있는 추골체의 단부판이 침하될 위험이 매우 크다.

그리하여 본 발명은 그 해결책을 제공하고자 한다. 본 발명은, 추골간 공간내에 조절가능하게 도입시킬 수 있으며, 최적의 뼈접촉면적을 가지면서, 그럼에도 불구하고 뼈접촉면에 있는 다수의 관통공에 의하여 뼈의 고착상태를 양호하게 하는 추골간 임플랜트를 창작함으로써, 상기 목적을 해결한다.
상기 목적의 달성을 위하여, 서두에 언급한 장치는 독립청구항1의 특징부의 특징에 의해 개선된다.
이에 의하여, 상면과 저면의 뼈접촉면이 커지기 때문에 임플랜트가 추골체의 단부판내로 침하되는 것을 방지할 수 있다는 잇점이 얻어진다.
그러나, 이와 동시에, 상면 및/또는 저면에 형성된 다수의 관통공이 뼈의 고착을 가능하게 한다. 상면 및/또는 저면에 형성된 관통공들은 가장 중요한 뼈의 고착을 위한 것으로서, 인접한 추골체를 합병시키는 역할을 한다. 놀랍게도 관통공들의 기하학적 비는 임상결과에서 결정적인 의미를 갖는 것으로 나타났다. 이 관통공들의 전체면적이 너무 적으면 뼈가 필요한 만큼 고착되지 못하여 합병이 일어나지 아니한다. 한편 관통공들의 전체면적이 너무 크면, 인접한 추골체의 단부판에 대한 임플랜트의 상면 및 저면의 가용 접촉면적이 너무 작아져, 임플랜트와 단부판사이에 너무 큰 접촉압력이 발생하며, 이것은 임플랜트가 단부판내로 침하될 위험을 증대시킨다.
양호한 임상결과를 얻기 위해서는, 상면 및/또는 저면내 관통공의 전체 면적이 상면 및/또는 저면의 전체 면적의 40 내지 55%이어야 하는 것으로 나타났다. 상면 및/또는 저면내 관통공의 전체 면적이 상면 및/또는 저면의 전체 면적의 43 내지 51%, 통상적으로 45 내지 49%인 것이 바람직하다.
상면 및/또는 저면내 개별 관통공의 칫수도 얻고자 하는 임상결과에 대해 특히 의미를 갖는 것으로 입증되었다. 개별 관통공의 면적이 너무 작으면, 비록 관통공면적이 전체적으로 상당히 크더라도, 뼈의 고착이 곤란하게 된다. 한편, 상면 및/또는 저면 내 개별 관통공들이 너무 큰 관통면적을 가질 경우에도, 단부판의 균일한 지지를 해하여 임플랜트가 단부판내에 국부적으로 침하될 위험이 있기 때문에, 부정적인 영향을 미친다. 각 관통공의 개별면적은 양호한 임상결과를 얻기 위해서는 상면 및/또는 저면의 전체 면적의 최대한 20%이어야 하는 것으로 나타났다. 각 관통공의 개별 면적은 상면 및/또는 저면의 전체 면적의 5 내지 15% 통상적으로는 8 내지 13%인 것이 바람직하다.
관통공의 직경은 최대 9.0mm, 통상적으로는 최대 5.0mm인 것이 바람직하다. 상면 및/또는 저면의 연부영역에 배치된 관통공들은 대체로 상면 및/또는 저면의 중앙영역에 배치된 관통공들보다 작아야 하며, 외부로부터 내측으로 갈수록 점진적으로 직경이 증대하는 것이 바람직하다. 이에 의해, 중앙에 배치된 관통공은 뼈의 고착이 단부판의 가장 희박한(가장 적절한) 위치에서 이루어지고, 그런 한편 상면 및/또는 저면의 주변부분은 뼈의 단부판의 밀집된 연부부분에 대해 최대의 접촉면적을 얻을 수 있게 된다.
끝으로 또한, 중공체로서 형성된 임플랜트의 기하학적 비가 임상결과에 중요한 것으로 나타났다. 인접한 추골의 양호한 합병을 달성할 수 있기 위해서는, 공간의 체적 VH와 케이지의 전체 체적 VK 간의 비 VH/VK는 70 내지 90%의 높은 범위내에 반드시 유지되어야 한다. 이에 의해 제1의 최적전제를 제공하는 뼈조각 혹은 뼈대용재료의 용이한 도입이 보장된다. 공간의 체적 VH와 케이지의 전체 체적 VK 간의 비 VH/VK는 75 내지 85%, 통상적으로는 78 내지 82%의 범위내에 있는 것이 바람직하다.
케이지의 상면 및 저면의 삼차원 구조를 갖는 바람직한 실시예에서는, 임플랜트의 높은 지지안정성이 추가적으로 달성될 수 있다. 삼차원 구조는 치형, 그루브, 혹은 기타의 적절한 융기부 혹은 함몰부로서 이루어질 수 있다. 이 구조의 높이는 0.5 내지 2.0mm로서, 바람직하게는 1.0 내지 1.5mm이며, 예를 들어 치형 바람직하게는 균일하게 배치된 치형으로 구성될 수 있다.
삼차원 구조는 구조화된 하이드록실아파티트(hydroxylapatit) 적층물일 수 있다. 전체 케이지를 하이드록실아파티트 혹은 바이오액티브 재료로 적층할 수도 있다.
그러나, 이 삼차원 구조는 티탄, 티탄합금 혹은 기타 인체친화적 재료로 이루어진 구조화된 적층물일 수 있다.
상면 및 저면은 구조화부가 없는 자유연부를 갖는 것이 바람직하다.
또 다른 실시예에서, 상면과 저면이 외측을 향하여 볼록하게 만곡되도록 형성되어, 인접한 추골체의 단부판의 기하학적인 형상과 최적으로 일치되도록 되어 있다.
또 다른 실시예의 경우, 측면에 관통공이 형성되어 있고, 그 관통공의 전체면적은 측면의 전체면적의 최대 40%(통상적으로는 최대 30%), 최소 15%(통상적으로는 최소 20%)이다. 측면내의 관통공은 바람직하게는 장공절취부이다.
전방벽도 관통공 바람직하게는 장공절취부를 가질 수 있다.
또 다른 실시예에서는, 전방벽에 케이지를 조작할 수 있는 공구의 수용을 위한 수단이 마련되어 있다. 측면도 케이지를 조작할 수 있는 공구의 수용을 위한 수단을 가질 수 있다.
본 발명의 또 다른 실시예에서는, 두 개의 추골간 임플랜트가 하나의 조합임플랜트로 단일화되며, 여기서는 두 추골간 임플랜트가 그들의 비어있는 측면에서 일체로 상호 결합되어 있다. 조합된 전방벽은 단일의 장공절취부를 갖는 것이 바람직하다.
본 발명에 따른 임플랜트는 종래기술에 비해 다음과 같은 장점을 달성한다.
a) 이탈안전성;
b) 개선된 뢴트겐투과성;
측면 뿐만아니라 전방벽 및 후방벽에 있는 관통공에 의해 임플랜트의 합병상태를 뢴트겐촬영을 통하여 용이하게 조절할 수 있으며, 이것은 종래기술에 따른 임플랜트의 경우 측면이 폐쇄되어 있어 매우 곤란하였다.
c) 케이지내에 도입될 수 있는 뼈재료의 압축성.
본 발명 및 본 발명의 개선점은 몇가지 실시예의 일부 개략적인 도면들에 기초하여 이하에서 상세히 설명된다.
도 1 내지 도 3에 도시된 추골간 임플랜트는 대체로 각 하나의 장공(19)을 갖는 측면(13 및 14), 두 개의 홈(17)을 갖는 전방벽(16) 및 후방벽(15)을 가지고 그의 상면(11) 및 저면(12)(관통공들을 제외하고)에서 차폐되어 있는 프레임형상의 케이지(1)로 이루어져 있다. 홈(17)은 조작공구를 수용하는 역할을 한다. 케이지(1)의 형상은 쐐기형상으로 형성되어 있다. 즉 상면과 저면(11, 12)은 전방벽(16)으로 갈수록 확장되도록 되어 있다.
상면과 저면(11, 12)은 도 1 내지 도 3에 따른 실시예의 경우 삼차원 구조부(18)를 가지며, 이 삼차원 구조부(18)는 바람직하게는 날카로운 치형의 형태로 약 1.75mm의 높이를 가지고 균일하게 배치되어 임플랜트의 지지안정성을 향상시킨다. 상면과 저면(11, 12)은 구조부(18)가 없는 자유연부(32)를 갖는다. 자유연부(32)는 수술중 및 수술후의 상해위험을 방지한다.
상면과 저면(11, 12)은 다수의 관통공(24)을 가지며, 관통공(24)의 전체면적은 상면과 저면(11, 12)의 전체면적의 40%이다. 각 관통공(24)의 개별면적은 상면과 저면(11, 12)의 전체면적의 15%이다. 공간(20)의 체적 VH와 케이지(1)의 전체 체적 VK 간의 비 VH/VK는 0.22이다.
케이지(1)의 전방벽(16)은, 도 1에 명백히 나타나 있는 바와 같이, 공구의 수용을 위한 두 개의 홈(17)을 가지며, 이에 의해 케이지(1)가 추골간 공간내에 도입되어 위치설정될 수 있다.
본 발명의 다른 실시예가 도 4 내지 도 8에 도시되어 있으며, 이 실시예는 다음에 설명하는 상이점을 제외하고는 도 1 내지 도 3에 따른 실시예와 동일한 특징을 갖는다. 본 발명에 따른 임플랜트는 상면(11), 저면(12), 각 하나의 장공(19)을 갖는 두 개의 측면(13 및 14), 하나의 개구(25)를 갖는 전방벽(16) 및 하나의 개구(26)를 갖는 후방벽(15)을 구비한 프레임형상의 케이지(1)로 이루어져 있다. 개구(25)는 측방의 홈(27)을 가지며, 이 홈(27)은 적절한 조작공구를 수용할 수 있다. 측면(13, 14)에 위치한 장공(19)도 측방 홈(28)을 가지며, 이 홈(28)은 적절한 조작공구를 수용할 수 있다.
케이지(1)의 형상은 쐐기형상으로 형성되어 있다. 즉 상면과 저면(11, 12)은 전방벽(16)으로 갈수록 확장되도록 되어 있다.
도 4 내지 도 7에 따른 실시예에서, 상면과 저면(11, 12)은 다수의 관통공(24)을 가지며, 관통공(24)의 전체면적은 상면과 저면(11, 12)의 전체면적의 48%이다. 각 관통공(24)의 개별면적은 상면과 저면(11, 12)의 전체면적의 10%이다. 공간(20)의 체적 VH와 케이지(1)의 전체 체적 VK 간의 비 VH/VK는 0.21이다.
임플랜트의 상면과 저면(11, 12)에 있는 관통공(24)은 - 본 발명에 따른 범위내에서 - 다양한 관점에서 변동될 수 있다. 도 8은 도 4에 따른 임플랜트의 상면과 저면(11, 12)내 관통공(24)의 변형의 일예를 나타낸 것으로서, 여기서는 관통공(24)의 전체면적이 상면과 저면(11, 12)의 전체면적의 50%이다. 각 관통공(24)의 개별면적은 상면과 저면(11, 12)의 전체면적의 15%이다. 공간(20)의 체적 VH와 케이지(1)의 전체 체적 VK 간의 비 VH/VK는 0.22이다.
모든 실시예에서 케이지(1)는 티탄, 티탄합금, 세라믹 혹은 생체호환적인 플라스틱 예를 들어 폴리에틸렌으로 만들어질 수 있다.
이제 이하에서는 임상적 적용예를 상세히 설명한다.
도 1에 도시된 케이지(1)는 장공절취부의 형태로 된 측방의 관통공(19)을 통하여 뼈조각(bone graft) 혹은 뼈대용재료로 (경우에 따라서는 압축되어) 채워진다. 그런 다음, 충전된 케이지(1)는 분리공구의 도움으로 비어있는 추골간 공간내에 삽입된다. 이때 전방벽(16)내의 양 홈(17)내에 삽입된 공구는 조작기로서 역할한다.
케이지(1)는 도 1 내지 도 3에 도시된 바와 같이 반쪽임플랜트로서 형성되어 두 개의 임플랜트가 추골간 공간내에 삽입되도록 할 수 있다. 혹은 두 개의 반쪽임플랜트를 단일체로 형성하여 도 4 내지 도 7에 도시된 바와 같이 하나의 임플랜트만이 추골간 공간내에 도입되도록 할 수도 있다.

Claims (22)

  1. 뼈지지면인 관통된 상면(11)과 저면(12), 두 개의 측면(13, 14), 전방벽(16)과 후방벽(15)을 가지고 공간(20)을 둘러싸는 프레임형상의 케이지(1)를 갖는 추골간 임플랜트에 있어서,
    A) 상기 상면 및/또는 저면(11, 12)은 다수의 관통공(24)을 가지고, 상기 관통공(24)의 전체 면적은 상기 상면 및/또는 저면(11, 12)의 전체 면적의 40 내지 55%이며;
    B) 상기 각 관통공(24)의 개별면적은 상기 상면 및/또는 저면(11, 12)의 전체 면적의 최대한 20%이며;
    C) 상기 공간(20)의 체적 VH와 상기 케이지(1)의 전체 체적 VK 간의 비 VH/VK는 0.11 내지 0.42의 범위내에 있으며;
    D) 상기 케이지(1)는 상기 전방벽(16)으로 갈수록 확장되는 상면과 저면(11, 12)을 가진 거의 쐐기형상으로 형성되어 있는 것을 특징으로 하는 추골간 임플랜트.
  2. 제 1 항에 있어서, 상기 상면과 저면(11, 12)은 삼차원 구조부(18)를 갖는 것을 특징으로 하는 추골간 임플랜트.
  3. 제 1 항 또는 제 2 항에 있어서, 상기 상면 및/또는 저면(11,12)의 연부영역에 배치된 관통공(24)은 평균적으로 상기 상면 및/또는 저면(11,12)의 중앙영역에 배치된 관통공(24)보다 작은 것을 특징으로 하는 추골간 임플랜트.
  4. 제 1 항 또는 제 2 항에 있어서, 상기 상면과 저면(11,12)은 외측을 향하여 만곡되게 형성되어 있는 것을 특징으로 하는 추골간 임플랜트.
  5. 제 1 항 또는 제 2 항에 있어서, 상기 관통공(24)의 직경은 최대 9mm인 것을 특징으로 하는 추골간 임플랜트.
  6. 제 1 항 또는 제 2 항에 있어서, 상기 측면(13,14)에 관통공(19)이 형성되어 있으며, 상기 관통공(19)의 전체면적은 상기 측면(13,14)의 전체 면적의 최대 40%인 것을 특징으로 하는 추골간 임플랜트.
  7. 제 1 항 또는 제 2 항에 있어서, 상기 측면(13,14)에 관통공(19)이 형성되어 있으며, 상기 관통공(19)의 전체면적은 상기 측면(13,14)의 전체 면적의 최소 15%인 것을 특징으로 하는 추골간 임플랜트.
  8. 제 2 항에 있어서, 상기 삼차원 구조부(18)는 구조화된 하이드록실아파티트(hydroxylapatit) 적층물인 것을 특징으로 하는 추골간 임플랜트.
  9. 제 2 항에 있어서, 상기 삼차원 구조부(18)는 티탄, 티탄합금 혹은 기타 인체친화적 재료로 이루어진 구조화된 적층물인 것을 특징으로 하는 추골간 임플랜트.
  10. 제 2 항에 있어서, 상기 삼차원 구조부(18)는 치형들로 이루어지며 바람직하게는 균일하게 배치되어 있는 것을 특징으로 하는 추골간 임플랜트.
  11. 제 2 항, 8 항, 9 항 10 항중 어느 한 항에 있어서, 상기 삼차원 구조부(18)는 0.5 내지 2.0mm의 높이를 갖는 것을 특징으로 하는 추골간 임플랜트.
  12. 제 1 항 또는 제 2 항에 있어서, 상기 전방벽(16)은 상기 케이지(1)의 조작을 가능하게 하는 공구의 수용을 위한 수단(17; 27)을 갖는 것을 특징으로 하는 추골간 임플랜트.
  13. 제 1 항 또는 제 2 항에 있어서, 상기 측면(13, 14)은 상기 케이지(1)의 조작을 가능하게 하는 공구의 수용을 위한 수단(28)을 갖는 것을 특징으로 하는 추골간 임플랜트.
  14. 제 1 항 또는 제 2 항에 있어서, 상기 상면과 저면(11, 12)은 구조화부(18)가 없는 자유연부를 갖는 것을 특징으로 하는 추골간 임플랜트.
  15. 제 1항 또는 제 2 항에 있어서, 상기 관통공(19)은 장공절취부인 것을 특징으로 하는 추골간 임플랜트.
  16. 제 1 항 또는 제 2 항에 있어서, 상기 전방벽(16)은 관통공을 바람직하게는 장공절취부의 형태로 갖는 것을 특징으로 하는 추골간 임플랜트.
  17. 제 1항 또는 제 2 항에 있어서, 상기 케이지(1)는 전체가 하이드록실아파티트 혹은 바이오액티브 재료에 의해 적층되어 있는 것을 특징으로 하는 추골간 임플랜트.
  18. 제 1항 또는 제 2 항에 있어서, 각 관통공(24)의 개별 면적은 상기 상면 및/또는 저면(11, 12)의 전체면적의 5 내지 15%인 것을 특징으로 하는 추골간 임플랜트.
  19. 제 1 항 또는 제 2 항에 있어서, 공간(20)의 체적 VH와 케이지(1)의 전체체적 VK 사이의 비 VH/VK는 0.17 내지 0.33인 것을 특징으로 하는 추골간 임플랜트.
  20. 제 1 항 또는 제 2 항에 있어서, 상기 상면 및/또는 저면(11,12)내 관통공(24)의 전체 면적은 상기 상면 및/또는 저면(11,12)의 전체면적의 43% 내지 51%인 것을 특징으로 하는 추골간 임플랜트.
  21. 제 1 항 내지 제 20 항중 어느 한 항에 따른 두개의 추골간 임플랜트를 갖는 조합임플랜트에 있어서, 상기 두 추골간 임플랜트는 그들의 비어있는 측면(14)에서 일체로 상호 결합되어 있는 것을 특징으로 하는 조합임플랜트.
  22. 제 21 항에 있어서, 상기 조합된 전방벽(16)은 단일의 장공절취부를 갖는 것을 특징으로 하는 조합임플랜트.
