Description
本出願は、2017年12月20日に出願された欧州特許出願第17306847.9号の便益を主張する。この出願の内容は、参照によりその全体が本明細書に組み込まれる。
〔発明の分野〕
本開示は、医療用移植片に関する。より詳細には、本開示は、可塑性多孔質移植片固定システムに関する。
多孔質又は部分的に多孔質の骨移植片が、様々な状況下で使用される。重量を節約するため、熱伝導率を低下させるため、周囲の骨の機械的特性に近づき機械的特性を達成するため、及び/又はより良好な固定のために骨の内部成長を可能にするためといった異なる理由により、多孔質にしている。
図1は、ある態様による、一体化されたインターロッキング素子を用いる螺旋形の棒である本体を有する多孔質構造体の一例である。
以下の説明及び添付の図面は、ある特定の実施形態を対象とする。任意の特定の文脈において説明される実施形態は、本開示を特定の実施形態又は任意の特定の使用に限定することを意図しない。当業者は、開示された実施形態、態様、及び/又は特徴が、任意の特定の実施形態に限定されないことを認識するであろう。例えば、「或る(a)」層、構成素子、部品などへの言及は、ある態様では「1つ又は複数」を指すことがある。
-図21Aに示すような本体2102。この本体2102は、骨の一部を置換する。一実施形態において、本体2102は、WO/2017/042366の図1a~図7、図10a~図17b又は図19a~図19d及び明細書の9頁31行~15頁18行、17頁14行~23頁16行、又は25頁4~28行に記載される多孔質構造体、又は本願に記載される実施形態に基づく多孔質構造体を有する。本体2102は、骨の軸に実質的に垂直な骨との界面2104を有する。本体2102は、界面2104をボルト又はネジ受入素子2105に連結する連結素子2103を含む。界面2104と受入素子2105との間の空間は、ボルト又はネジが内部に位置することができる大きさである;
-図21Bに示すようなボルト又はネジ2106。寸法の限界の課題を克服し、ボルト又はネジ2106が骨/移植片の界面に揃うよう、且つ骨の軸に実質的に平行になるよう、ボルト又はネジ2106の頭部2110が、本体2102の体積内にあってもよい。界面2104及び受入素子2105は、ボルト又はネジ2106の骨の軸に垂直な方向の移動を制限する。本体2102は、ネジドライバの挿入を可能にするために、ボルト又はネジ2106の軸に沿ってさらに延在する開口部2108を有してもよい。代わりに、ボルト又はネジ2106の頭部2110は、側面から接近可能にされ、その結果、ボルト又はネジ2106は、レンチを用いて、又はボルト又はネジ2106のヘッド2110内の一連の開口部2112につかむピンを用いて固定されてもよい。一実施形態において、ボルト又はネジ2106は、上記のステム、ペグ、及びネジについて示されるような多孔質構造体を有する;
-任意選択で、図21Cに示すようなプラグ2120。特に、ねじ回しのための開口部2108を提供するための空間がなく、ボルト又はネジ2106を側面から締結しなければならない場合、外科医が、ボルト又はネジ2106に十分な圧力をかけて骨にそれを打ち込むことは困難である。従って、一実施形態において、移植片固定システムは、プラグ2120も備える。プラグ2120のための穴は、(患者固有の)ドリルガイドを使用するなどして、事前に穿孔することができる。次いで、プラグ2120は、例えば手動で、又はハンマーを使用して、穴に挿入され得る。プラグ2120は、回転(例えば、プラグの軸に平行な羽根)を制限するために、外部特徴2122を備える。プラグ2120はまた、移植片が骨に固定されたときに抜去を防止するための少なくとも1つの素子を備える。これは、ボルト2106がプラグ2120内に駆動されるときに外側に押し出される1つ又は複数の素子であり得る。代わりに、それは、髄内の釘と区別されるように、プラグ2120の軸から約90°で挿入された1つ以上の固定ネジ又はピンであってもよい。一実施形態において、プラグ2120は、上記のステム、ペグ、及び/又はネジについて示されるような多孔質構造体を有する。
Claims (10)
- 骨に移植される可塑性多孔質構造体であって、
螺旋形構造体と、
前記螺旋形構造体に結合しており、当該螺旋形構造体によって連結されている複数のインターロッキング素子と、
を備え、
前記複数のインターロッキング素子の各々がニュートラルポジションにあるときには、前記インターロッキング素子と前記螺旋形構造体との間、および隣り合う前記インターロッキング素子同士の間には、空間が形成される構成となっており、
前記可塑性多孔質構造体に圧縮力が加えられると、前記複数のインターロッキング素子は、前記螺旋形構造体に接触することによって前記螺旋形構造体の圧縮を制限する構成となっており、
前記可塑性多孔質構造体に延伸力が加えられると、前記複数のインターロッキング素子は、隣り合うインターロッキング素子同士を接触させることによって前記螺旋形構造体の延伸を制限する構成となっている、
骨に移植される可塑性多孔質構造体。 - 前記螺旋形構造体及び前記複数のインターロッキング素子は、中空の円柱の表面を形成する、
請求項1に記載の可塑性多孔質構造体。 - 前記複数のインターロッキング素子の各々は、T構造体を備え、
隣り合う前記インターロッキング素子は、互いに鏡像関係に配向されている、
請求項2に記載の可塑性多孔質構造体。 - 前記螺旋形構造体は、円柱の表面を形成し、
前記複数のインターロッキング素子は、前記円柱の内部に配置される、

前記螺旋形構造体は、前記ネジの外側シェルを備える、

請求項5に記載の可塑性多孔質構造体。 - 前記ネジ山は、前記螺旋形構造体と整列している、
請求項6に記載の可塑性多孔質構造体。 - 前記ネジ山は、前記螺旋形構造体と交差する螺旋形設計を形成する、
請求項6に記載の可塑性多孔質構造体。 - 前記ネジは、複数のスライディング素子を備えた内部コアを備え、
前記複数のスライディング素子は、前記ネジの頭部に結合された第1のスライディング素子と、前記ネジの先端に結合された第2のスライディング素子と、を含み、
前記第1のスライディング素子及び前記第2のスライディング素子は、前記ネジの長手方向の軸に沿って互いに摺動する構成となっている、
請求項5から8の何れか1項に記載の可塑性多孔質構造体。 - 前記螺旋形構造体は、少なくとも1つの前記螺旋形構造体の長さに沿って形成された支柱を備え、
前記少なくとも1つの支柱が、前記ネジが前記骨から引き抜くように回転されるときに成長した骨を切断又は分断するように位置決めされ、前記螺旋形構造体の長さに沿った、鋭い縁部を有する、
