Description
本願は、2016年4月20日出願の「Meniscal Repair Devices,Systems,And Methods」と題する米国特許仮出願第62/325,028号に基づく優先権を主張するものであり、当該出願の全容を本願に援用するものである。
本開示は、全般的には半月板修復装置、システム、及び方法に関する。
患者の身体に埋め込まれるように構成された、本明細書においてプレジットとも呼ばれるインプラントが後述される。インプラントは、縫合糸と結合されるように、かつ半月板修復処置、及び特に膝での半月板断裂の修復のための外科的処置で使用されるように、構成される。後述されるインプラントは、例えば、肩又は股関節における組織修復の外科的処置のような縫合糸を使用して組織及び/又は他の構造を結び付ける処置などの他の外科的処置で使用することができるものの、このように半月板修復に関して述べられる。
関節半月修復は典型的には、1つ又は2つ以上の縫合糸を介して共に取り付けられた複数のインプラントを使用する。縫合糸(複数可)は、様々な方法でインプラントに取り付けることができる。複数のインプラントに縫合糸を取り付ける様々な技術が後述される。縫合糸(複数可)は、およそのサイズ#0~#2-0の範囲内にあるサイズなど、様々なサイズのいずれかを有することができる。
本明細書に記述されるインプラントを、多様な方法の任意のもので組織を通して進めることができる。例えば、1つ又は2つ以上の針を使用してインプラントを手術部位に送達することができる。インプラントは、組織を切断するように構成されない場合があるので、1つ又は2つ以上の針は、組織(例えば、関節半月)を切断して、関連するインプラント(複数可)の通過を容易にするように構成され得る。組織を切断するように構成されていないインプラント(例えば、インプラントが切断面を欠いているなど)は、手術後に患者の身体内で不用意にインプラントが組織及び/又は他の物質を損傷する可能性を低減する助けとなり得る。
本明細書に記述されるインプラントを、多様な方法の任意のもので組織を通して進めることができる。例えば、ドライバーを使用してインプラントを手術部位に送達することができる。インプラントは、組織(例えば、関節半月)を切断してインプラント及びそれと関連するドライバーが組織を通って通過するのを容易にするように構成され得る。
上述したように、1本又は2本以上の針を、1つ又は2つ以上のインプラントを手術部位に送達するように構成することができる。1本又は2本以上の針及び1つ又は2つ以上のインプラントを、カニューレを通して手術部位まで進めることができる。例示の実施形態において、カニューレは、それを通して1本又は2本以上の針及び1つ又は2つ以上のインプラントを送達する送達システム、例えば図56、62、69、及び82の送達システムなどを進めるように構成され得る。
1本又は2本以上の針及び1つ又は2つ以上のインプラントを、組織を通して送達する送達システムを、中に通して進めるように構成されたカニューレは、カニューレが挿入される組織に対してかつ/又は送達システムに対してロック可能であるように構成され得る。カニューレのロック可能性は、実質的に組織にカニューレの遠位端を保持する、例えば、組織の表面に対して垂直にカニューレの遠位端を保持する助けとなり得、このことは、針(複数可)及びインプラント(複数可)が組織を通り所望の位置に進められることを確実にする助けとなり得る。カニューレのロック可能性は、インプラント(複数可)及び針(複数可)の送達中にカニューレの反発を低減する助けとなることができ、そのことは、反発が外科医により触覚的に感じられ、通常カニューレが針(複数可)及びインプラント(複数可)の配備中に適所に移動していることを示すので、針(複数可)及びインプラント(複数可)が組織を通って所望の位置に進められているという外科医の自信を増大させる助けとなり得る。
当業者には、上述の実施形態に基づいた本発明の更なる特徴及び利点が認識されよう。したがって、本発明は、添付の特許請求の範囲によって示される場合を除き、具体的に示され説明された内容により限定されるものではない。本明細書に引用される全ての刊行物及び文献はそれらの全容を参照によって本明細書に明示的に援用するものである。
(1) 外科的方法であって、
ドライバーを回転させて、組織を通してそこに嵌合されたインプラントを駆動して、前記インプラントを組織の片側で第1の空洞内に全体が位置するところから、前記組織の反対側で第2の空洞内に全体が位置するところまで移動させることと、
前記ドライバーを前記インプラントから引き抜いて、前記ドライバーを前記第2の空洞から前記第1の空洞へ移動させ、前記インプラントが前記第2の空洞内に全体的に残ることと、を含み、
前記インプラントが、開放近位端を有し、前記インプラントと嵌合されている前記ドライバーが、前記開放近位端を通って延びる前記ドライバーの遠位部分を含み、
前記インプラントが、前記インプラントの組織通過中に前記組織を縫うように進む雄ねじを有する、方法。
(2) 前記インプラントが、尖った遠位先端を有し、非カニューレ状である、実施態様1に記載の方法。
(3) 前記インプラントは、前記インプラントがカニューレ状になるようにそれを通って延びる内側ルーメンを有し、前記ドライバーは、前記ドライバーと嵌合された前記インプラントから遠位に位置する尖った遠位先端を有する、実施態様1に記載の方法。
(4) 前記インプラントが、その中間部分に縫合糸と嵌合するように構成される縫合糸嵌合機構を有し、前記ドライバーの前記回転が、前記縫合糸嵌合機構と嵌合された前記縫合糸に対して前記インプラントを回転させる、実施態様1に記載の方法。
(5) 前記縫合糸嵌合機構が、柔らかい連結部と、前記インプラントの周りに周囲方向に延在する溝と、前記インプラントの近位剛性部分と遠位剛性部分との間に延びる複数の縫合糸と、前記インプラントの近位剛性部分と遠位剛性部分を共に取り付け、かつ周りの前記縫合糸の張力に応じて内側に放射状に曲がるように構成される材料の輪と、前記インプラントの近位剛性部分と遠位剛性部分との間に延在する複数の布地ストリップと、のうち1つを備える、実施態様4に記載の方法。
(7) 前記縫合糸が、第2のインプラントと結合され、前記インプラントを駆動するための前記ドライバーの前記回転が、前記第2のインプラントを回転させない、実施態様6に記載の方法。
(8) 前記インプラントから前記ドライバーを引き抜いた後に、前記ドライバーを前記第2のインプラントと嵌合させることと、前記ドライバーを回転させて、前記組織を通してそこに嵌合された前記第2のインプラントを駆動して、前記インプラントを、前記第1の空洞内に全体が位置するところから、前記第2の空洞内に全体が位置するところまで移動させることとを更に含む、実施態様7に記載の方法。
(9) 前記第2のインプラントを駆動するための前記ドライバーの前記回転が、前記インプラントを回転させない、実施態様8に記載の方法。
(10) 前記組織が関節半月である、実施態様1に記載の方法。
(12) 外科用システムであって、
開放近位端を有し、雄ねじを有するインプラントと、
前記インプラントをドライバーと嵌合させるために前記インプラントの前記開放近位端を通って延びるように構成される遠位部分を備えるドライバーであって、前記ドライバーは、前記インプラントがこれと嵌合されると回転して前記インプラントを、組織を通して駆動して、前記インプラントを組織の片側で第1の空洞内に全体が位置するところから、前記組織の反対側で第2の空洞内に全体が位置するところまで移動させるように構成され、前記インプラントを全体的に前記第2の空洞内に残すために前記インプラントから引き抜かれるように構成されている、ドライバーと、を備える、システム。
(13) 前記インプラントが、尖った遠位先端を有し、非カニューレ状である、実施態様12に記載のシステム。
(14) 前記インプラントは、前記インプラントがカニューレ状になるようにそれを通って延びる内側ルーメンを有し、前記ドライバーは、前記インプラントが前記ドライバーと嵌合されたとき、前記インプラントから遠位に位置する尖った遠位先端を有する、実施態様12に記載のシステム。
(15) 前記インプラントが、その中間部分に縫合糸と嵌合するように構成される縫合糸嵌合機構を有し、前記ドライバーの前記回転が、前記縫合糸嵌合機構と嵌合された前記縫合糸に対して前記インプラントを回転させるように構成されている、実施態様12に記載のシステム。
(17) 前記インプラントと結合し、前記インプラントが前記第2の空洞に移動するのに応じて前記第1の空洞から前記第2の空洞に延びるように構成された縫合糸を更に備える、実施態様12に記載のシステム。
(18) 前記縫合糸が結合された第2のインプラントを更に備え、前記インプラントを駆動するための前記ドライバーの前記回転が、前記第2のインプラントを回転させない、実施態様17に記載のシステム。
Claims (4)
- 外科用システムであって、
開放近位端を有し、雄ねじを有するインプラントと、
前記インプラントをドライバーと嵌合させるために前記インプラントの前記開放近位端を通って延びるように構成される遠位部分を備えるドライバーであって、前記ドライバーは、前記インプラントがこれと嵌合されると回転して前記インプラントを、組織を通して駆動して、前記インプラントを組織の片側で第1の空洞内に全体が位置するところから、前記組織の反対側で第2の空洞内に全体が位置するところまで移動させるように構成され、前記インプラントを全体的に前記第2の空洞内に残すために前記インプラントから引き抜かれるように構成されている、ドライバーと、を備えており、
前記インプラントが、縫合糸と嵌合するように構成される、柔らかい連結部を含む縫合糸嵌合機構をその中間部分に有し、前記柔らかい連結部は、前記柔らかい連結部の周りに締め付けられた縫合糸ループによって、前記柔らかい連結部の遠位側および近位側にある前記インプラントの剛性部分よりもより小さい径に折り畳み可能であり、そのことによって、前記ドライバーの前記回転が、前記縫合糸嵌合機構と嵌合された前記縫合糸に対して前記インプラントを回転させる、システム。 - 前記インプラントは、前記インプラントがカニューレ状になるようにそれを通って延びる内側ルーメンを有し、前記ドライバーは、前記インプラントが前記ドライバーと嵌合されたとき、前記インプラントから遠位に位置する尖った遠位先端を有する、請求項1に記載のシステム。
- 前記インプラントと結合し、前記インプラントが前記第2の空洞に移動するのに応じて前記第1の空洞から前記第2の空洞に延びるように構成された前記縫合糸を更に備える、請求項1に記載のシステム。
- 前記縫合糸が結合された第2のインプラントを更に備える、請求項3に記載のシステム。
