Description
アテロームプラークの治療に広く適用される第1の種類のステントは、ステンレス鋼で構成された開放メッシュ管として設計されたバルーン拡張型ステント(BES)であった。血管形成術用バルーン上に圧着されたときに、動脈樹を通して同軸にバルーン拡張型ステントを前進させてプラーク内に直接留置することができた。ステント移植により、バルーン血管形成術単独と比較して大きく且つ耐久性のある流路が作り出された。
早くも1969年には、血管内ステントが剛性ではなく可撓性を有するべきであると理論付けられた。最初に航空宇宙用途で開発された、ニチノールと呼ばれるニッケルチタン製の等原子合金が、血管の足場に理想的な機械的特性を示すと考えられていた。1つの特性は、超弾性、すなわち、実質的な変形後に金属が元の形状に戻る能力であった。この超弾性により、動いている動脈内での可撓性と、四肢屈曲による一時的な圧潰後に動脈内腔を再形成する能力とが確保された。もう1つの特性は、形状記憶、すなわち、ある温度でアニールされ、低温で実質的に変形し、次いで、元の温度に加熱されたときに元の形状に戻る合金の能力であった。これにより、自己拡張型管状ニチノール血管内ステント(SES)の開発が可能になった。これらのステントは、体温(37℃)でニチノール管をレーザ切断し、次いで、送達システムへの装填を可能にするために冷却時に管を変形させることにより作り出される。器具が最終的に体温で血管内に留置されたときに、器具は、拡張して元のアニールされた形状をとる。
BESとSESの両方がアテローム性動脈硬化動脈の治療を目的としていることが、基本的に、BESとSESの唯一の類似点である。2つの器具は、実質的に他の全ての点(すなわち、材料、設計、パターン、製造方法、送達様式及び血管反応)において異なる。BES及びSESにより発生する力がBES及びSESの直径と共にどのように変化するかが並べて描かれている図3には、BESとSESとの相違の例示が示されている。
与えられた式から分かるように、他の全ての因子が同じままであれば、各ステント要素間の距離は、ステント要素の直径の増加に伴って増加する。同様に、他の全ての因子が同じままであれば、各ステント要素間の距離は、SFA内よりも膝窩内においてより大きくなる。表2は、この式を使用して算出された隙間を示している。

複数のステント要素が膨張可能なバルーンの長手方向長さに沿って直列に位置決めされ且つ複数の前記ステント要素が互いに接触しないように、多数の個々の前記ステント要素を備える多要素ステントを前記バルーン上に装填することを含み、
複数の前記ステント要素は、前記ステント要素が骨格運動中に標的血管部位において互いに接触しないように間隔をおいて配置され、
各ステント要素間の距離(G)が、標的血管部位における拡張状態にある前記ステント要素の直径(D)と、骨格運動中の前記標的血管部位の最大屈曲中にステント要素間に生成された角度(θ)とに基づき、
前記ステントが、前記標的血管部位への移植後に半径方向に剛性を及び長手方向に可撓性を有するように構成され、

- 請求項1に記載の方法において、前記ステントが、ポリ(L−乳酸)(PLLA)、ポリ(D−乳酸)(PDLA)、ポリ(D,L−乳酸)(PDLLA)、半結晶性ポリラクチド、ポリグリコール酸(PGA)、ポリ(乳酸−コ−グリコール酸)(PLGA)、ポリ(ヨウ化デスアミノチロシル−チロシンエチルエステル)カーボネート、ポリカプロラクトン(PCL)、サリチル酸系ポリマー、ポリジオキサノン(PDS)、ポリ(ヒドロキシブチレート)、ポリ(ヒドロキシブチレート−コ−バレレート)、ポリオルトエステル、ポリ無水物、ポリ(グリコール酸−コ−トリメチレンカーボネート)、ポリ(ヨウ化デスアミノチロシル−チロシンエチルエステル)カーボネート、ポリホスホエステル、ポリホスホエステルウレタン、ポリ(アミノ酸)、シアノアクリレート、ポリ(トリメチレンカーボネート)、ポリ(イミノカーボネート)、ポリアルキレンオキサレート、ポリホスファゼン、ポリイミノカーボネート、及び脂肪族ポリカーボネート、フィブリン、フィブリノゲン、セルロース、デンプン、コラーゲン、ポリカーボネートウレタンを含むポリウレタン、ポリエチレン、ポリエチレンテレフタレート、エチレン酢酸ビニル、エチレンビニルアルコール、ポリシロキサン及び置換ポリシロキサンを含むシリコーン、ポリエチレンオキシド、ポリブチレンテレフタレート−コ−PEG、PCL−コ−PEG、PLA−コ−PEG、PLLA−コ−PCL、ポリアクリレート、ポリビニルピロリドン、ポリアクリルアミド、及びこれらの組み合わせを含む材料から形成されることを特徴とする方法。

- 請求項1に記載の方法において、前記ステント要素が抗増殖剤でコーティングされることを特徴とする方法。
- 請求項1に記載の方法において、前記ステント要素が、拡張前状態にあるときに半径方向よりも長手方向に長い複数の菱形クローズドセルを備えることを特徴とする方法。
- 請求項1に記載の方法において、前記ステント要素が、前記拡張前状態において長手方向よりも半径方向に長い複数の菱形クローズドセルを備えることを特徴とする方法。
- 請求項1に記載の方法において、拡張前状態にある各ステント要素間の距離が、前記拡張状態にある各ステント要素間の距離以下であることを特徴とする方法。
