Description
本願は、2014年1月10日に出願された米国仮特許出願第61/925,939号、について優先権の利益を主張するものであり、この文献の内容は、参照することにより本明細書に組み込まれる。
I型のエンドリークは、グラフトの取付け部位での不十分なシールの結果として発生する。これは、基端部又は先端部で起こり得る。血流が、グラフトに沿って動脈瘤嚢内に漏れる。それらI型のエンドリークは、大抵の場合、EVAR手術に適さない患者(動脈瘤)の選択や装置の選択の結果であるが、グラフトが移動する場合に発生することがあり得る。I型のリークは、それらI型のリークが自然に消散する傾向がないので、常に重要であるとみなされる。
I型の漏れは、一般的に、検出されると直ぐに治療される。伸長カフ又は被覆されたステントは、シールを改善するために、グラフトの漏れが生じている端部に挿入することができ、又は接着剤又はコイルを用いて漏れ部位に塞栓を生じさせることができる。まれに、EVARの手術中にこの漏れが検出された場合に、観血手術への変更は、漏れをシールする血管内の方法が不成功となった場合に、必要となり得る。
III型の漏れ(グラフト機械的な故障)は、自然治癒によって消散しないため、通常は追加のステントグラフト部品を用いて直ぐに治療される。
IV型の漏れ(グラフトの多孔度)は、装置の選択を改善すること以外に治療することができない。

Claims (13)
- 管腔内処置のためのシステムであって、当該システムは、
血管の画像データを取り込むように動作可能なセンサと、
少なくとも1つのプロセッサと、を有しており、
該少なくとも1つのプロセッサは、
前記画像データを受信して処理し、
該処理した画像データに基づいて、前記血管の断面及びその内部の流体の流動特性を表す出力を提供し、
前記流動特性に基づいて、前記血管内のエンドリークの存在を検出するように構成され、
前記エンドリークは、血管内動脈瘤修復に関連し、前記プロセッサは、前記血管内の動脈瘤修復に関連する介入構造体を評価するように構成され、該介入構造体は、ステントグラフトを含み、前記プロセッサは、前記ステントグラフトの配置の前後の管腔に関する1つ又は複数の属性に基づいて、不完全密着及び/又はグラフト包込みを検出するように構成される、
システム。 - 前記プロセッサは、
前記エンドリークの1つ又は複数の属性をさらに特定し、
該エンドリークの1つ又は複数の属性に少なくとも部分的に基づいて、前記エンドリークを、I型、II型、III型、又はIV型の漏れのうちの少なくとも1つに分類する、ように構成される、
請求項1に記載のシステム。 - 前記エンドリークの1つ又は複数の属性は、流れの方向、流速、及び前記血管の壁に対する流れの位置を含む、請求項2に記載のシステム。
- 前記ステントグラフトの配置の前後の前記管腔に関する1つ又は複数の属性は、前記ステントグラフトの配置の前後の前記血管内の寸法、前記動脈瘤に対する前記ステントグラフトの位置、及び血管壁に対する前記ステントグラフトの位置から構成されるグループから選択される、請求項1に記載のシステム。
- 前記プロセッサは、前記エンドリークの1つ又は複数の属性及び前記ステントグラフトの配置の前後の前記管腔に関する属性に少なくとも部分的に基づいて、前記エンドリークを、I型、II型、III型、又はIV型のうちの少なくとも1つに分類するように構成される、請求項4に記載のシステム。
- 前記センサは、血管内超音響波(IVUS)プローブを含む、請求項1に記載のシステム。
- 前記IVUSプローブは、15メガヘルツ(MHz)未満の周波数の超音響波によって前記管腔内のデータを取り込むように動作可能である、請求項6に記載のシステム。
- 前記管腔内処置は、標準的な血管内動脈瘤修復(標準EVAR)、胸部血管内動脈瘤修復(TEVAR)、ハイブリッドEVAR、又は腸骨動脈EVARから選択される、請求項1に記載のシステム。
- 血管内動脈瘤修復に関連するエンドリークを検出及び分類するためのシステムの作動方法であって、当該作動方法は、
前記システムのプロセッサが、センサによって提供される血管の画像データを処理するステップと、
前記プロセッサが、前記処理した画像データに基づいて、前記血管の断面及びその内部の流体の流動特性を表す出力を提供するステップと、
前記プロセッサが、前記血管内の動脈瘤修復に関連する介入構造体を評価するステップであって、該介入構造体は、ステントグラフトを含み、前記評価するステップは、前記ステントグラフトの配置の前後の管腔に関する1つ又は複数の属性に基づいて、不完全密着及び/又はグラフト包込みを検出するステップを含む、評価するステップと、
前記流動特性に基づいて、前記血管内のエンドリークを検出するステップと、を含む、
作動方法。 - 前記プロセッサが、前記エンドリークの1つ又は複数の属性を特定するステップと、
前記プロセッサが、前記エンドリークの1つ又は複数の属性に少なくとも部分的に基づいて、前記エンドリークを、I型、II型、III型、又はIV型の漏れのうちの少なくとも1つとして分類するステップと、をさらに含む、
請求項9に記載の作動方法。 - 前記エンドリークの1つ又は複数の属性は、流れの方向、流速、及び前記血管の壁に対する流れの位置を含む、請求項10に記載の作動方法。
- 前記プロセッサが、前記エンドリーク及び前記ステントグラフトの配置の前後の前記管腔に関する属性に基づいて、前記エンドリークを、I型、II型、III型、又はIV型のうちの少なくとも1つとして分類するステップと、をさらに含む、
