Description
心細胞保護薬としてのこれらの誘導体の利点は、限られているようである。
−塩酸、臭化水素酸、硫酸、硝酸、リン酸などの無機酸で形成した酸付加塩、または、酢酸、ベンゼンスルホン酸、安息香酸、カンファースルホン酸、クエン酸、エタンスルホン酸、フマル酸、グルコヘプトン酸、グルコン酸、グルタミン酸、グリコール酸、ヒドロキシナフトエ酸、2−ヒドロキシエタンスルホン酸、乳酸、マレイン酸、リンゴ酸、マンデル酸、メタンスルホン酸、ムコン酸、2−ナフタレンスルホン酸、プロピオン酸、サリチル酸、コハク酸、ジベンゾイル−L−酒石酸、酒石酸、p−トルエンスルホン酸、トリメチル酢酸、トリフルオロ酢酸などの有機酸で形成した酸付加塩、または、
−酸性陽子が親化合物中に存在している場合、いずれかが、例えば、アルカリ金属イオン、アルカリ土類金属イオン、またはアルミニウムイオンなどの金属イオンにより置換されて形成された塩、または、有機もしくは無機塩基と配位して形成された塩。許容可能な有機塩基は、ジエタノールアミン、エタノールアミン、N−メチルグルカミン、トリエタノールアミン、トロメタミンなどを含んでなる。許容可能な無機塩基は、水酸化アルミニウム、水酸化カルシウム、水酸化カリウム、炭酸ナトリウムおよび水酸化ナトリウムを含んでなる。
本発明による医薬組成物は、より具体的には、収縮機能が保存された心不全の処置における使用のためのものである。
本発明による医薬組成物は、経口的に、または任意の他の投与の医薬的経路により投与することができる。
イバブラジンと組合せた3−(R)−[3−(2−メトキシフェニルチオ)−2−(S)−メチル−プロピル]アミノ−3,4−ジヒドロ−2H−1,5−ベンゾキサチエピンの抗虚血活性を、麻酔したウサギ中の冠動脈回旋枝の閉塞によるST部分上昇のモデルを用いて評価した。
オスのウサギ(2.2〜2.5kg)を、ペントバルビタールを用いて麻酔する。カテーテルを頸動脈に入れて、動物の血圧を連続的に測定する。血液サンプルを採取して、血中ガスを定期的に確認し、必要に応じてベンチレーションパラメータを修正する。耳介静脈中に入れたカテーテルにより、試験化合物または担体を投与する。構成IIで、心電図もまた連続的に記録し、ST部分の大きさの変動を測定する。動物の胸部を開き、左冠状動脈(回旋枝)の周りに結紮を配置し、局部的な心筋虚血を誘発する(Verscheure et al.,1995)。全ての血行動態パラメータの安定後、ゆっくりと化合物をボーラス投与する(1分)。化合物の投与から5分後、回旋枝を5分間塞ぎ、その後、結紮を解放する。心臓を取り除き、危険領域(回旋枝の結紮した領域)を評価する。4つのグループのウサギについて実験した:
−担体:40%ポリエチレン300、60%生理食塩水を受け取るグループ
−0.04mg/kgの投与量で、3−(R)−[3−(2−メトキシフェニルチオ)−2−(S)−メチル−プロピル]アミノ−3,4−ジヒドロ−2H−1,5−ベンゾキサチエピン(化合物A)を受け取るグループ
−0.63mg/kgの投与量でイバブラジン塩酸塩(化合物B)を受け取るグループ
−化合物A(0.04mg/kg)+化合物B(0.63mg/kg)を受け取るグループ。
Claims (2)
- 有効成分として、少なくとも1つの薬学的に許容可能な賦形剤と組合せて、3−(R)−[3−(2−メトキシフェニルチオ)−2−(S)−メチル−プロピル]アミノ−3,4−ジヒドロ−2H−1,5−ベンゾオキサチエピンまたはその薬学的に許容可能な塩と、イバブラジンまたはその薬学的に許容可能な塩との組合せを含んでなる、狭心症、虚血および/または心不全の処置において使用するための、医薬組成物。
- 収縮機能の保持された心不全の処置において使用するための、請求項1に記載の医薬組成物。
