Description
本願は、2010年8月20日に出願された米国仮特許出願第61/375,354号の優先権および利益を主張し、その内容全体が参照により本明細書中に組み込まれる。
脊柱は、人体に対する支持、ならびに脊髄および神経に対する保護を提供する、骨および結合組織から成る複雑なシステムである。大人の背骨は、上部および下部で構成されている。上部は、7つの頚椎、12の胸椎、5つの腰椎を含む3つの領域に細分化された、24の個々の骨を含む。下部は、仙骨および尾骨で構成される。椎体と呼ばれる円筒形の形をした骨が、上部から下部へと下方にいくにつれ、徐々に大きさが増大する。
対向する一対のフラップ436のそれぞれは、一対の切込436b間に配置される突起436aを含む。突起436aおよび一対の切込436bは、図15に示すように、それぞれ、突起438および一対の切込435と一列に配列され得る、または略一列に配列され得る。
Claims (12)
- 多軸椎弓根スクリューであって、
止めネジを受容するように構成されたハウジングであって、前記止めネジは脊椎ロッドを前記ハウジング内で支持し、前記ハウジングは先頭端および末尾端を有し、前記ハウジングは、遠位開口部ならびに前記先頭端および前記末尾端を通って延在する縦軸を画定する、ハウジングと、
ヘッドおよび前記ヘッドから延在するネジ山付きシャフトを含む骨スクリュー部材であって、前記ヘッドが、第1の円筒形の部分および第2の球形の部分を含み、前記第1の円筒形の部分のみが、前記ヘッドが前記ハウジング内に画定された開口部を通って嵌合することができるようにするように構成され、前記第2の球形の部分は、一度挿入されると、前記ハウジング内で前記ヘッドを維持するように構成される、骨スクリュー部材と、
前記骨スクリュー部材のヘッドの第2の球形の部分が、前記縦軸が前記第2の球形の部分を通って延在する状態で前記ハウジングの遠位開口部に向いている位置に前記骨スクリュー部材が再配向するのを防ぐよう、前記ハウジングに固定可能なキャップと、
アンビルであって、前記アンビルは、前記アンビルおよび前記骨スクリュー部材のヘッドが前記ハウジング内に配置される場合に、前記骨スクリュー部材のヘッドに隣接して前記ハウジング内に配置可能であり、前記アンビルは前記アンビルの外面の近位側に少なくとも1つの溝を画定し、前記少なくとも1つの溝は前記ハウジングの縦軸に垂直に延在し、かつ前記少なくとも1つの溝の全体にわたり延在する少なくとも1つのフラップを画定し、前記少なくとも1つのフラップは柔軟に前記アンビルに取り付けられ、前記骨スクリュー部材が前記アンビルに対して相対的に動かされる場合、前記骨スクリュー部材のヘッドを前記アンビルと常に接触した状態に維持するのに十分な量で前記アンビルが屈曲するのを可能にする、アンビルと、
を含む、多軸椎弓根スクリュー。 - 前記アンビルが、様々なサイズのロッド部材を収容するよう、複数の曲率半径を備えた面を有する凹部を画定する、請求項1に記載の多軸椎弓根スクリュー。
- 前記凹部の面が、前記複数の曲率半径が異なる直径の複数のロッド部材の少なくとも2つの接線を提供する複合曲線を画定するよう、第1の曲率半径を備えた第1の部分、第2の曲率半径を備えた第2の部分、第3の曲率半径を備えた第3の部分を画定する、請求項2に記載の多軸椎弓根スクリュー。
- 前記アンビルの外面が、前記アンビルに隣接してロッド部材を配置する際に前記アンビルの誤配向を防ぐよう、非円形の形状を有する、請求項1〜3のいずれか1項に記載の多軸椎弓根スクリュー。
- 前記アンビルに隣接して前記ハウジング内にロッド部材を固定するよう、前記ハウジング内に配置可能な止めネジをさらに含む、請求項1〜3のいずれか1項に記載の多軸椎弓根スクリュー。
- 前記骨スクリュー部材が、前記ハウジングに係合される場合、前記ハウジングについて複数の角度で配置可能であり、最大40度の円錐角内の角度で前記ハウジングについて固定可能である、請求項1〜3のいずれか1項に記載の多軸椎弓根スクリュー。
- 前記ヘッドの第1の円筒形の部分が、前記ハウジングを通って画定された縦軸と一直線上にある位置に前記骨スクリュー部材が再配向するのを防ぐよう、前記ハウジングに固定可能なキャップをさらに含む、請求項1に記載の多軸椎弓根スクリュー。
- 前記ハウジングが、前記ハウジングの先頭端から延在する鍔を含み、前記鍔は前記ハウジング内に前記ヘッドの位置決めを容易にするための切欠を画定し、前記キャップは、前記ヘッドが前記ハウジング内に配置された後に、前記切欠を覆うように前記鍔に固定可能である、請求項7に記載の多軸椎弓根スクリュー。
- 前記アンビルが、前記アンビルの外面上の突起を含み、前記ハウジングが、前記ハウジングの内面上のスロットを画定し、前記突起および前記スロットは、前記ハウジングに対して前記アンビルの配置を維持するよう係合可能である、請求項1〜3のいずれか1項に記載の多軸椎弓根スクリュー。
- 前記骨スクリュー部材が前記アンビルとの係合によって配置される場合、前記アンビルに対して相対的に前記骨スクリュー部材を再配置するには使用者の加力が必要であるように摩擦によって前記アンビルに係合する表面構造を、前記骨スクリュー部材のヘッドが含む、請求項1〜3のいずれか1項に記載の多軸椎弓根スクリュー。
- 多軸椎弓根スクリューを組み立てる方法であって、前記方法は、
多軸椎弓根スクリューを提供する工程であって、前記多軸椎弓根スクリューは、
ヘッドから延在するネジ山付きシャフトを有する前記ヘッドを含む骨スクリュー部材であって、前記ヘッドは第1の部分および第2の部分を含む、骨スクリュー部材と、
前記骨スクリュー部材のヘッドの上に配置可能なハウジングであって、前記ハウジングは遠位開口部およびその全体に延在する縦軸を画定する、ハウジングと、を含み、
前記ヘッドの第1の部分が、前記ハウジングの遠位開口部を通過するように構成され、前記ヘッドの第2の部分が、前記ハウジングの遠位開口部を通過しないように構成される、工程と、
前記ヘッドの第2の部分が前記遠位開口部に向いておりかつ前記縦軸と整列された状態で、前記ハウジングの遠位開口部を通って前記骨スクリュー部材のヘッドを挿入する工程と、
前記ヘッドの第1の部分が、前記ハウジングの遠位開口部と整列されないよう、かつ、前記ヘッドの第2の部分が、前記ヘッドが前記遠位開口部を通って延在することを防ぐよう、前記ハウジングについて前記骨スクリュー部材を回転させる工程と、
前記骨スクリュー部材のヘッドが前記ハウジングの遠位開口部を通過することを防ぐように、前記骨スクリュー部材のヘッドが前記ハウジング内に配置された状態で、前記ハウジング上にキャップを装着する工程と、
を含む、方法。 - 前記ハウジングから延在する鍔に前記キャップを装着する工程をさらに含む、請求項11に記載の方法。
