Description
本出願は、2010年12月7日に出願した、国際出願PCT/RU2010/000735号の優先権を主張し、その全内容は、本明細書の中に書いてあることを根拠として明確に本明細書に組み込まれる。
クリップ100の送達、操作、適用、取り外しおよび抜き取りを単一の装置で実行することができ、複数のツールを使用する必要性が排除されるため、外科用マニピュレータ151および内視鏡マニピュレータ161の機能性を改善する。
マニピュレータ151/161を扱っている際の労働集約度および外傷を低減すると共に、マニピュレータ151/161を単純化し、その信頼性を改善する。
48歳の患者Bが、慢性有石胆嚢炎を治療するための所定の手術のために外科に入院した。患者は、それまでの5年間この疾患を有しており、1年に約5〜7回、規定食を守らなかった時にいくつかの悪化を経験していた。
手術中、腹部領域が検査されたが、他の病変は見られなかった。胆嚢のサイズは、超音波による所見に対応し、すなわち、肥厚した壁を有する8×6×5cmのサイズ、ならびに慢性炎症の兆候および膀胱内の複数の石の存在を示した。また、胆嚢頚部および胆嚢体の周りに癒着が見られた。
さらに、標準的手順に従い、腹腔鏡下胆嚢摘出術を完了した。
術後の回復は順調であった。患者は4日目に退院した。
52歳の患者Lが、両足の静脈瘤の切除のための所定の手術のために外科に入院した。その状態の病歴は、約8年であった。過去2年間、歩行中および徒歩での長距離移動中のふくらはぎの筋肉の疼痛が増加した。静脈血栓症および静脈血栓塞栓症のための年一回の入院も留意された。過去に行われた保存療法は、十分効果的ではなかった。検査中、両下肢の深部静脈内の血流は、ブロックされていなかった。
手術は、従来の方法に従った。独特のステップは、ニチノール(ニッケル−チタンNiTi)製のクリップ100に類似した2つのクリップを、腰部の浅大腿静脈に対して、この静脈が深部静脈と交差する直前の領域に位置付けることであった。マニピュレータを利用して、クリップの耳部を40℃より高い温度に加熱して顎部を閉鎖することにより、静脈にクリップを適用した。
ニチノールクリップを脈管の遠位および近位部分に設置して、微小および中程度のサイズの他の脈管もまたクリップした。
術後の回復は順調であった。患者は、手術後2日目に退院した。
電力ユニットは、直流または交流電源である。ペルチェ熱電対モード切替ボックスは、前記枝状部の1つの中央部に設けられた強制加熱ボタンおよび強制冷却ボタンの形態であってもよく、それらの反対側に、強制加熱マイクロスイッチおよび強制冷却マイクロスイッチが、他方の枝状部上に設けられ、前記頂部枝状部の遠位端に設置された前記電力ユニットに接続されている。ペルチェ熱電対モード切替ボックスは、電力ユニットを有し、前記頂部および底部枝状部の遠位端上に設けられた電気ジャッキを介して、前記外科用マニピュレータホルダに接続される、フットスイッチペダルの形態である。
Claims (15)
- 基体部と、
該基体部に連結された、第1のアームおよびこれに対向する第2のアームと、
前記基体部に向かって延びる内側延在部を有し、前記第1のアームに連結される第1の顎部と、
前記基体部に向かって延びる内側延在部を有し、前記第2のアームに連結され、前記第1の顎部と対向する第2の顎部と、を備え、
少なくとも前記基体部は、前記基体部の温度が前記基体部の変態温度を満たす、またはそれを超える場合に、前記第1および第2のアームを互いに近接する方向に強制する形状記憶合金で形成され、
前記第1のアームおよび前記第2のアームは、それぞれ一端部で前記基体部に連結されており、
対向する前記第1の顎部および前記第2の顎部は、それぞれ、前記内側延在部の第1自由端と、前記第1自由端に対向して前記基体部から離れる方向に延びる第2自由端と、を有し、且つ、前記第1自由端と前記第2自由端の中間の場所で前記第1のアームおよび前記第2のアームにそれぞれ連結されているクリップ。 - 前記第1の顎部およびこれに対向する前記第2の顎部のそれぞれの前記内側延在部が、それぞれ前記第1のアームおよび前記第2のアームに向かって湾曲する内側端を有する、請求項1に記載のクリップ。
- 前記基体部ならびに前記第1のアームおよびこれに対向する前記第2のアームに沿った前記クリップの外側表面が、略平滑である、請求項1または請求項2に記載のクリップ。
- 生物学的に不活性な材料で形成されている外科用クリップである、請求項1から3のいずれか一項に記載のクリップ。
- 前記基体部から離れる方向に延びる前記端部が、丸みを付けられた先端を有する、請求項1から4のいずれか一項に記載のクリップ。
- 前記第1の顎部および前記第2の顎部は、これらの顎部が互いに対し鋭角を成す開放位置を取るように分離可能である、請求項1から5のいずれか一項に記載のクリップ。
- 前記基体部を加熱することにより前記基体部の形状記憶が活性化されると、前記基体部が、前記第1の顎部および前記第2の顎部を閉鎖位置に強制する、請求項6に記載のクリップ。

該クリップマニピュレータが、1つの前記クリップを保持する少なくとも1つのアームと、前記基体部の温度を、前記変態温度を満たす、またはそれを超えさせるのに十分な温度変化を、前記クリップの前記基体部に付与する少なくとも1つの熱電変換器とを備えるキット。 - 前記クリップマニピュレータの前記少なくとも1つのアームは、各前記顎部上に1つの前記熱電変換器が配置された対向する顎部の遠位対を備え、前記対向する顎部が、前記クリップの前記基体部を掴むと同時にそれに温度変化を付与して使用可能である、請求項14に記載のキット。
