Description
本発明は、概括的には医療デバイスに関し、より具体的には組織中の穿孔を閉じるための組織アンカーに関する。
体内壁の穿孔は自然発生的に生じる、或いは意図的に又は意図的ではなく形成され得る。恒久的にこれらの穿孔を閉じて、組織が適切に治癒できるようにするために、縫合糸、接着剤、クリップ、ステープルなどを用いる多数の医療デバイス及び方法が開発されてきた。これらのデバイスの多くは、通常１本以上の縫合糸を用い、該縫合糸のストランドは、穿孔を閉じるために引き寄せられ所定の位置に固定されて、その後カットされ、患者の体内から除去される必要がある。
穿孔を閉じるために、縫合糸ストランドを手で互いに結びつけることは、非常に複雑で時間がかかる。例えば、穿孔及び縫合が、内視鏡又は腹腔鏡処置など、身体内でのアクセスが困難な場合は特に、医療専門家によって高い水準の技術と調整が要求される。手で縫合糸を結束して切断することに伴う数多くの問題が十分に立証されている。結束して切断する手による縫合のこれら及び他の問題に対処するために、様々な自動縫合結束システムが開発されている。残念ながら、このような自動システムは、多くの場合、複雑で費用がかかり、使用が困難であり、また特定の状況での使用に限られている。
本発明は、体内壁の穿孔を閉じるために、組織アンカーだけでなく、関連するデバイス及び方法を提供する。組織アンカーは使用が簡単で信頼性があり、穿孔の閉合を促進し、また、さまざまな穿孔の閉合状況に適応できる。本発明の教示に従って構築された、穿孔を閉じるためのテンション部材と係合する構造を有する組織アンカーの一実施形態は、概括的にはクロスバーとストランドを備えている。該クロスバーは、第１及び第２の対向する端部を有して長手方向軸線を画定している。該ストランドは、対向する端部の間の位置で該クロスバーに連結されている。ストランドはクロスバーに結合された遠位端から、第１のコネクタを有する近位端まで延在する約５ｍｍから約５０ｍｍの範囲の長さを有している。ストランドとその第１のコネクタは長手方向軸線から離れる方向に突き出ている。
本発明の別の実施形態は穿孔を閉じるための医療デバイスを含んでいる。医療デバイスは、概括的に一組の組織アンカーと細長いテンション部材を備えている。各組織アンカーは、クロスバーとストランドを含んでいる。該クロスバーは、第１及び第２の対向する端部を有して長手方向軸線を画定している。該ストランドは、対向する端部の間の位置において該クロスバーに連結され且つ長手方向軸線から離れる方向突き出ている。該ストランドの遠位端が該クロスバーに結合されており、該ストランドの近位端は第１のコネクタを有している。細長いテンション部材は第１のコネクタを選択的に係合及び解除するように構成されている。ストランドは、穿孔を閉じるために共に引っ張って固定できる。
医療デバイスの本実施形態のより詳細な態様によれば、該ストランドは約２０ｍｍから約３０ｍｍの範囲の長さを有している。
患者の体内壁の穿孔を閉じるための方法も本発明の教示に従って提供されている。上記のデバイスなどの医療デバイスが提供されている。各組織アンカーは穿孔の周囲に隣接する体内壁を貫通し、その結果、各組織アンカーのクロスバーが体内壁の遠位側にあり、且つ各組織アンカーの第１のコネクタは細長いテンション部材を選択的に係合及び解除するために体内壁の近位側にある。細長いテンション部材は各ストランドの第１のコネクタと係合し、互いに近接するストランドを配置するために操作される。ストランドは、体内壁の近位側に共に固定される。細長いテンション部材は、第１のコネクタから解放され、患者の中から取り除かれる。
引っ張った状態の一組のアンカーを固定する方法も本発明の教示に従って提供されている。各アンカーは、クロスバーとクロスバーに結合されているストランドを有している。ストランドは、クロスバーに結合されている遠位端を有している。ストランドの近位端は第１のコネクタを含んでいる。細長いテンション部材は、管状部材のルーメン内の各ストランドの第１のコネクタに連結している。細長いテンション部材を配置するために細長いテンション部材に対して管状部材を移動させ、且つ管状部材の遠位端を越えて遠位方向に一組のアンカーを移動させる。細長いテンション部材はストランドを共に引っ張るために操作され、次に、ストランドは引っ張った状態で共に固定される。次に、細長いテンション部材は、固定されたストランドの第１のコネクタから解放される。
本明細書に組み込まれて本明細書の一部を形成している添付の図面は、本発明の幾つかの態様を図示しており、記述と共に本発明の原理を説明するのに役立っている。
ここで図を参照すると、図１ａは本発明の教示に従って構成された組織アンカー２０を示している。アンカー２０は、体内壁１２の穿孔１０（図５−７）を閉じるために利用される。また、アンカー２０は、例えば逆流性食道炎（ＧＥＲＤ）治療、又は吻合が形成される肥満症治療で組織を併置するために、或いは他の治療で使用するために用いることができる。アンカー２０は、概括的に対向する端部２６及び２８を有し且つ長手方向軸線２２を画定するクロスバー２４を含む。柔軟性のあるストランド３０は、クロスバー２４の対向する端部２６と２８の間の位置でクロスバー２４に結合されている。ストランド３０は、クロスバー２４に結合される遠位端３２を含み、クロスバー２４の長手方向軸線２２から離れて、以下でさらに詳細に説明されるコネクタ３６で終端する近位端３４まで延在する。
クロスバー２４は、好ましくは細長であるが、棒、管、ディスク形状或いは他の細長い又は平面状の部材を含む、体内壁１２の穿孔１０を閉じるのに適した任意の形態を取ることができる。クロスバー２４は、管状カニューレで形成されるのが好ましいが、固体円柱、金属棒、プラスチック成形品、又は他の棒状の材料であってもよい。ストランド３０は、柔軟性のある縫合材料で形成されるのが好ましいが、単一フィラメント及びマルチフィラメント線、並びに巻回及び編組電線、プラスチック製ストリング、ロープ及び同種のものを含む金属線などの他の構成を有することもできる。
ストランド３０は、機械的ファスナー、接着剤或いは様々な溶接又ははんだ付け技法を含む、あらゆる現在公知の或いはこれ以降開発される取付け手段を使用して、クロスバー２４に固定され得ることが、当業者によって認識されるであろう。一つの好ましい構成では、クロスバー２４は対向する端部２６と２８の間を貫通して形成された開口部を有するカニューレで形成されており、ストランド３０の遠位端３２はカニューレの開口部内に収容され、所定の位置に固定される。また、ストランド３０は、一つの要素として、クロスバー２４と単一且つ一体的に形成されてもよい。したがって、組織アンカー２０全体を、単一のプラスチック又は金属材料、及び最も好ましくは吸収性材料で形成することができる。例えば、クロスバー２４とストランド３０を含むアンカー２０は、ナイロン又は吸収性材料などの長持ちする材料で射出成形されてもよい。また、アンカー２０の材料は、プラスチックの中に粒子を埋め込んだり、固有の又は形成された放射線不透過性又はエコー源性の特性を有する適切な材料を選択したりすることにより、例えば、放射線不透過性又はエコー源性を作ることができる。
本明細書において、用語「吸収性」は、組織及び／又は体液と接触した際に、組織及び／又は体液に吸収される物質の機能を指す。多数の吸収性材料が、当技術分野において既知であり、任意の適切な吸収性材料を使用することができる。吸収性材料の適切なタイプの例としては、吸収性ホモポリマー、コポリマー、又は吸収性ポリマーのブレンドがある。適切な吸収性材料の具体例としては、ポリ乳酸、ポリラクチド、ポリグリコール酸（ＰＧＡ）、又はポリグリコリドなどのポリ−αヒドロキシ酸；トリ−メチレンカーボネート（ｔｒｉ−ｍｅｔｈｌｙｅｎｅ ｃａｒｂｏｎａｔｅ）；ポリカプロラクトン；ポリヒドロキシ酪酸塩又はポリヒドロキシ吉草酸塩などのポリ−βヒドロキシ酸；或いはポリホスファゼンなどの他のポリマー、ポリ有機−ホスファゼン、ポリ無水物、ポリエステルアミド、ポリ−オルソエステル、ポリエチレンオキシド、ポリエステル−エーテル（例えば、ポリ−ジオキサノン）又はポリアミノ酸（例えば、ポリ−Ｌ−グルタミン酸又はポリ−Ｌ−リジン）が挙げられる。セルロース、キチン、及びデキストランなどの変性多糖類、並びに、フィブリン及びカゼインなどの変性プロタインを含む、多数の適切な天然由来の吸収性ポリマーもある。
ストランド３０の長さは、好ましくは約５ｍｍから約５０ｍｍ、最も好ましくは約２０ｍｍから約３０ｍｍの範囲である。ストランド３０の直径は、好ましくはクロスバー２４の直径の約５０％未満、最も好ましくは約３５％未満である。ストランド３０の直径は、好ましくは約０．２０ｍｍから約０．３５ｍｍの範囲の、最も好ましくは約０．２５４ｍｍである。クロスバー２４の直径は、好ましくは約０．５ｍｍから約１．０ｍｍの範囲の、最も好ましくは約０．８ｍｍである。クロスバー２４の長さは、典型的に約３．０ｍｍから約１００ｍｍの範囲である。クロスバー２４及び／又はストランド３０は、既知のプラスチックや親水性のコーティングなどの低摩擦材料で被覆してもよい。
図１ａに示すように、コネクタ３６は、ストランド３０の近位端３４で小穴又は閉ループの形態をとることが最も好ましい。閉ループのコネクタ３６は、好ましくは約０．５ｍｍから約１．５ｍｍの範囲の直径を有する。コネクタ３６は、図１ｂに示すように別の形状を持つことができる。図１ｂは、本発明の教示による組織アンカー２２０の実施形態を示し、図１ａと同様の図であり、同類の構成要素が２００を足した同様の参照番号で示されている。この実施形態では、コネクタ２３６はＪ字型フックの形態をとっている。
コネクタ３６は、アンカー２０の配備を補助するためにテンション部材３８を受け入れるように構成されている（図２ａ−ｃ）。コネクタ３６は、柔軟な縫合材料、金属ワイヤー、プラスチック、ロープ又は任意の適切な吸収性材料から形成することができる。図１に示すように、閉ループのコネクタ３６はストランド３０と一体的に形成されていることが好ましい。閉ループのコネクタ３６は、柔軟性があり、テンション部材３８によって引っ張られたときに、延びたり、或いはその形状の向きを調整したりすることが可能である。また、コネクタ３６は、ストランド３０とは異なる材料から形成されてもよく、ストランド３０より厚くてもよく、機械的ファスナー、結束、結合、溶接、又は接着を含む任意の適切な手段を用いてストランド３０に固定してもよい。例えば、図１ｂのＪ字型フックコネクタ２３６がテンション部材によって引っ張られたときにその形状を維持するために、コネクタ２３６は、硬質プラスチックや金属材料などのストランド２３０より強く剛性のある材料から作ることが好ましい。加えて、コネクタ３６は、アンカー２０が他の周囲の組織に対して非外傷性となるように、滑らかな、丸みを帯びた形状を有していることが好ましい。
図２ａ−ｃを参照すると、コネクタ３６を選択的に係合及び解除するように構成された様々な実施形態のテンション部材３８、１３８、及び２３８と共に、一組の組織アンカー２０ａと２０ｂが描かれている。図２ａに示すように、テンション部材３８は、縫合糸５２を、細長い保持部材４０と共に含んでいる。図２ａの縫合糸５２と細長い保持部材４０は、図１ａの実施形態に示すような閉ループ形態のコネクタ３６を有するアンカー２０ａと２０ｂと共に使用している状態で示されている。
縫合糸５２は、好ましくは身体の外部に配置され保持された第１及び第２の端部５４及び５６を含む。第１及び第２の端部５４及び５６は、例えば、トーイ・ボースト（ｔｏｕｈｙ ｂｏｒｓｔ）コネクタ又はクランプによって、組織アンカー送達デバイス（図４）の近位端のハンドル（図示せず）内に固定することができる。縫合糸５２の一部が閉ループコネクタ３６の各々を通して引っ張られ、且つ細長い保持部材４０によって維持されるように、縫合糸５２は閉ループコネクタ３６を通って事前装着されていることが好ましい。このように、縫合糸５２は、環状部分５８及び６０に折り返しを形成するために、それぞれの閉ループコネクタ３６を二度通っていることが好ましい。環状部分５８及び６０のこれらの折り返しは、遠位への前進及び組織アンカー２０ａ及び２０ｂの別々の位置決めの際に、縫合糸５２に適切な弛みを与えるのに十分な長さのものである。
図２ａに示すように、縫合糸５２は、第１の端部５４から延在して、第１の環状部分５８を形成するために折り返され、第１の環状部分５８から延在して、第２の環状部分６０を形成するために折り返され、さらに、第２の環状部分６０から第２の端部５６まで延在する。第１の環状部分５８は一対のアンカーのうちの第１のアンカー２０ａの閉ループコネクタ３６内に摺動可能に受け入れられ、第２の環状部分６０は一対のアンカーのうちの第２のアンカー２０ｂの閉ループコネクタ３６内に摺動可能に受け入れられる。細長い保持部材４０は近位端４２と遠位端４４を有し、該遠位端は、縫合糸５２の第１及び第２の環状部分５８及び６０を選択的に係合及び解除するように構成されているコネクタ４６で終端している。縫合糸５２は、単一フィラメント又はマルチフィラメント構造のものであってもよく、好ましくは約０．２０ｍｍから約０．２５ｍｍの範囲の直径を有している。
コネクタ４６は、任意の適切な形状又は環状部分５８及び６０との選択的な係合及び解除に適した形態をとり得ることが、当業者によって認識されるであろう。好ましくは、細長い部材４０の遠位端４４におけるコネクタ４６は、図示するように、フック状の形状、例えばＪ字型フックである。
図２ｂの別の実施形態に示すように、テンション部材１３８は、組織アンカー送達デバイスを貫通する第１及び第２の端部１５４及び１５６を有している縫合糸１５２を含み、医師が操作するために、送達デバイスの近位端に隣接した身体の外部に位置し、保持される。この実施形態では、縫合糸１５２は、第１の端部１５４から一対のアンカー２０ａ及び２０ｂの第１及び第２の閉ループコネクタ３６をそれぞれ通って延在している。縫合糸１５２を含むテンション部材１３８は、図２ｂに示す閉ループコネクタ３６以外のコネクタを有するアンカーと組み合わせて使用してもよい。例えば、縫合糸１５２は、選択的に図１ｂに示すＪ字型フックコネクタ２３６などのフック状のコネクタを係合することができ、縫合糸１５２はフック状のコネクタ２３６を通されることになる。縫合糸１５２は、単一フィラメント又はマルチフィラメント構造のものであってもよい。
図２ｃの別の実施形態に示すように、テンション部材２３８は、近位端２４２と遠位端２４４を有する細長い保持部材２４０を含み、該保持部材は、一対のアンカー２０ａ及び２０ｂの第１及び第２の閉ループコネクタ３６をそれぞれ選択的に係合して直接的に解除するように構成されたコネクタ２４６で終端している。好ましくは、細長い部材２４０の遠位端２４４におけるコネクタ２４６はＪ字型フックである。コネクタ２４６を有する細長い保持部材２４０は、図２ｃに示す閉ループコネクタ３６以外のコネクタを有するアンカーと組み合わせて使用してもよい。例えば、保持部材２４０のコネクタ２４６は、図１ｂのＪ字型フックコネクタ２３６などのフック状のコネクタを選択的に係合することができる。コネクタ２４６は任意の適当な形状又は様々なコネクタ３６との選択的な係合及び解除に適した形態を取り得ることが、当業者によって認識されるであろう。例えば、コネクタ２４６はカール状又は閉ループも可能であり、さらにコネクタ３６はフック形状も可能である（図１ｂ参照）。これらと他のバリエーションは当業者には容易に明らかとなるであろう。
図３と４を参照すると、組織アンカー２０は、少なくとも２個のアンカー２０ａ及び２０ｂを含む一組のアンカーとして配備されることが好ましい。以下詳細に説明するように、テンション部材３８は、体内壁１２の組織を貫いてアンカー２０ａと２０ｂを送達する際、アンカー２０ａと２０ｂの位置決めを補助し、その後除去される。図３に最も分かり易く示す様に、アンカー２０ａと２０ｂのクロスバー２４は体内壁１２の遠位側に配置される一方、クロスバー２４に接続されコネクタ３６を含むストランド３０の大部分は体内壁の近位側に配置される。アンカー２０ａと２０ｂが体内壁１２の組織を貫通して配置された後、アンカー２０ａと２０ｂのストランド３０は、穿孔１０を閉じるためにテンション部材３８によって引き寄せられ、縫合糸ロック６２によって、所定の位置に固定される。その後、テンション部材３８は除去される。このように、（穿孔を閉じるために所定の位置に固定され、その後手動で切断して除去しなければならない）アンカーを体内壁に固定するために、別個の縫合糸又は複数の個々の縫合糸を採用するのではなく、アンカー２０ａと２０ｂはストランド３０と縫合糸ロック６２によって共に直接固定される。このように、コネクタ３６を有するストランド３０を含むアンカー２０ａと２０ｂの構成により、アンカーを共に固定するために使用される縫合糸を手動で切断することに伴う多くの問題が軽減される。
図４を参照すると、本発明の教示に従って、アンカー２０ａと２０ｂとロック６２を用いる医療送達デバイス１００は、組織アンカー２０ａと２０ｂを摺動可能に収容できる大きさである針のルーメン８８を有する針８６を含む。好ましくは、吸収性スペーサー部材５０が針８６の遠位端８９に隣接した針のルーメン８８内のアンカー２０ａと２０ｂの間に配置されている。針８６は、アンカー２０ａと２０ｂのストランド３０を収容できる大きさである針のスロット８７を有している。医療送達デバイス１００は、一組の３つ以上のアンカーと２つ以上のスペーサー部材を含んでもよい。針のスロット８７が、針のルーメン８８内に位置するクロスバー２４のそれぞれに結合されたストランド３０を受け入れることができるように、針のスロット８７の長手方向の長さは、アンカーの数と、クロスバーの長さと、アンカーの間に配置されたスペーサー部材の数とスペーサー部材の長さに依存する。プッシャー９８は、最も近位のアンカー、即ち図４のアンカー２０ｂに係合してアンカー２０ａと２０ｂ並びにその間に配置されたスペーサー部材５０を送達デバイス１００から配備するために、針のルーメン８８内に摺動可能に収容されている。
医療デバイス１００は、針８６を摺動可能に収容できる大きさであるルーメン９２を有する内側シース９０と、内側シース９０を摺動可能に収容できる大きさであるルーメン９６を有する外側シース９４をさらに含んでいる。アンカー２０ａと２０ｂのストランド３０は、アンカー２０ａと２０ｂのクロスバー２４の長手方向軸線２２から離れて、針のスロット８７を通って外側シースのルーメン９６内で近位方向に延在する。本発明の教示に従って、アンカー２０ａと２０ｂのストランド３０のコネクタ３６を選択的に係合及び解除するために、テンション部材３８が外側シースのルーメン９６内に摺動可能に収容される。縫合糸５２と細長い保持部材４０を含む図２ａのテンション部材３８は、図４−９の医療送達デバイス１００の一部分として示されるとともに、図２ｂと２ｃのテンション部材１３８又は２３８はそれぞれ、医療送達デバイス１００の一部分として用いることができる。同様に、Ｊ字型コネクタ２３６を有する図１ｂのアンカー２２０は本発明の教示に従って医療送達デバイス１００を用いて送達することができる。
内側及び外側シース９０、９４は、好ましくはポリテトラフルオロエチレン（ＰＴＦＥ）、膨張ポリテトラフルオロエチレン（ＥＰＴＦＥ）、ポリエチレンエーテルケトン（ＰＥＥＫ）、ポリ塩化ビニル（ＰＶＣ）、ポリカーボネート（ＰＣ）、ナイロンなどのポリアミド、ポリイミド、ポリウレタン、ポリエチレン（高、中又は低密度）などのプラスチック、或いは補強ワイヤー、コイル又はフィラメントの有無に関係なく多層又は単層構造を含むサントプレン（登録商標）などのエラストマーから形成されることが好ましい。縫合糸５２と保持部材４０を含めて、針８６、内側及び外側シース９４、９０、プッシャー９８、並びにテンション部材３８は、柔軟で、内視鏡又は腹腔鏡処置中などの患者体内での進行を可能にする、細長い構造であることが好ましい。このようにして、当技術分野において知られているように、適切な操作又は制御機構がこれらのコンポーネントの相対的な並進移動のために、医療専門家によって針８６、シース９０と９４、並びにプッシャー９８の近位端に接続される。
好ましくは、アンカー２０ａと２０ｂを体内壁１２に貫通して送達した後のアンカー２０ａと２０ｂのストランド３０を固定する市販の針縫合糸ロック６２を、医療デバイス１００はさらに含む。本発明の教示に従って、アンカー２０ａと２０ｂを体内壁１２に貫通して送達する間に、市販の針縫合糸ロック６２により、一組のアンカー２０ａと２０ｂのストランド３０が縫合糸ロック６２内に事前装着できるようにする。縫合糸ロック６２は概括的にはロックピン又はプラグ６４と保持スリーブ６６を含み、該プラグとスリーブは、体内壁１２の穿孔１０を閉じるために、体内壁１２の組織に対してアンカー２０ａと２０ｂのストランド３０を固定するように協働する。保持スリーブ６６及びプラグ６４は、円形の断面、又は三角、四角などを含む任意の他の断面形状を持つことができる。
図４−９に最も分かり易く示す様に、保持スリーブ６６は、概括的には内部通路７０を画定する内部表面６９を有する管状本体６８を含む。さらに本明細書で詳細に議論されるように、周辺リム７２は、管状本体６８の遠位端に形成されており、保持スリーブ６６の配置に使用される肩７４を画定している。概括的には、保持スリーブ６６は、内部通路７０内にアンカー２０ａと２０ｂのストランド３０を収容する。ストランド３０は、その後プラグ６４を使用して所定の位置に固定され、該プラグは通路７０内に固定且つアンカー２０ａと２０ｂのストランド３０を挟む又は圧縮するように作られている。また、プラグ６４の一部分がストランド３０を固定するために保持スリーブ６６と協働する場合、プラグ６４は多くの形状（例えば、規則的又は不規則な形状）及び構造（例えば、鋳造、成形、機械加工、（ワイヤーを使用するなどの）巻上げ、等）を有してもよいことが認識されるであろう。好ましくは、プラグ６４と保持スリーブ６６は、ステンレス鋼又は他の適切な金属又は当技術分野で公知のプラスチック材料から形成される。
図４−９に最も分かり易く示す様に、プラグ６４は概括的には内部表面７７を有する本体７６を含み、該内部表面は針８６を摺動可能に収容できる大きさである内部通路７８を画定している。本体７６は、グリップ８０とストッパ８２を含み、各々が本体７６から半径方向に拡張している。図示の実施形態では、グリップ８０は、プラグ６４の遠位端に形成されているが、該プラグは、本体７６の長さに沿う近位方向に移動することができる。グリップ８０は、アンカー２０ａと２０ｂのストランド３０を係合するために使用される環状のエッジ７９を画定している。ストッパ８２はグリップ８０から長手方向に離間し、保持スリーブ６６内でのプラグ６４の位置を調整するために使用される。ストッパ８２は概ね近位方向に向いている面８３を含み、該面はプラグ６４を配置するために使用される肩８４を画定している。保持スリーブ６６の内部通路７０を通ってグリップ８０が完全に通過してしまうことを防ぐために、ストッパ８２はグリップ８０と関連して配置されている。組織アンカー２０のストランド３０の長さは約５ｍｍと５０ｍｍの間であることが好ましいが、プラグ６４と保持スリーブ６６間のストランド３０の十分な係合を提供するために、ストランド３０の長さはストランド３０のコネクタ３６が縫合糸ロック６２を抜け出るようなものであるべきである。
図４−７に示すように、内側シース９０はプラグ６４の肩８４に当接して配置できる大きさであり、外側シース９４は保持スリーブ６６の肩７４に当接して配置できる大きさである。外側シース９４に対する内側シース９０の並進移動により、プラグ６４が市販の針８６を摺動させて保持スリーブ６６の通路７８内に収容されるようにすることにより、プラグ６４の本体７６と、ストランド３０を固定するための保持スリーブ６６の内部表面６９の間でアンカー２０ａと２０ｂのストランド３０を係合する。
針アセンブリと市販の針縫合糸ロック６２の更なる詳細は、「組織アンカーの迅速な配備及び固定のための医療デバイス、システム、及び方法（Ｍｅｄｉｃａｌ Ｄｅｖｉｃｅｓ, Ｓｙｓｔｅｍｓ, ａｎｄ Ｍｅｔｈｏｄｓ ｆｏｒ Ｒａｐｉｄ Ｄｅｐｌｏｙｍｅｎｔ ａｎｄ Ｆｉｘａｔｉｏｎ ｏｆ Ｔｉｓｓｕｅ Ａｎｃｈｏｒｓ）」と題される、デュシャルメ（Ｄｕｃｈａｒｍｅ）への米国仮特許出願第６１／１６６，３６１号に見いだされることができ、該出願の全内容が参照により本明細書に組み込まれている。
医療デバイス１００は、内視鏡の付属チャンネルを通して又は内視鏡と一緒に、或いは内視鏡と組み合わせて使用される他のデバイス、例えば、内視鏡吸引装置や流体注入装置と併用して使用される大きさとされてもよい。
本発明の教示によれば、図５−７に示すように、穿孔１０を閉じる方法は、各組織アンカー２０ａと２０ｂを穿孔１０の周囲に隣接する体内壁１２の組織に貫通するステップを含む。好ましくは、アンカーは穿孔１０の周囲に順に配置される。図示のように、アンカー２０ａと２０ｂは、穿孔１０の両側に配置される。前述のように、２個のアンカー２０ａと２０ｂより多くの複数のアンカーを順に穿孔１０の周囲に配置することができる。
図５に示すように、医療送達デバイス１００は、本発明の教示に従って、体内壁１２の組織に隣接する位置に送達される。アンカー２０ａと２０ｂは針８６の遠位端８９において針のルーメン８８内に配置されており、吸収性スペーサー部材５０がアンカー２０ａと２０ｂの間に配置されている。スペーサー部材５０とアンカー２０ａ及び２０ｂの間のスペースが明確化のために示されているが、スペーサー部材５０とアンカー２０ａ及びと２０ｂは、概ね針ルーメン８８内で端部と端部とが当接することになる。アンカー２０ａと２０ｂのストランド３０は、針のスロット８７内に受け入れられ、アンカーのクロスバー２４の長手方向軸線２２から離れて突き出る。好ましくは、針８６は内側シースルーメン９２内に摺動可能に収容されており、テンション部材３８は外側シースルーメン内に摺動可能に収容される前に、針ルーメン８８から延在するアンカーストランド３０のコネクタ３６と事前に係合されている。このようにして、内側シース９０内の針８６とテンション部材３８は、医療送達デバイス１００の位置決めの前に、事前に係合され、外側シース９４に一緒に装填され、外側シース９６の遠位端９５に向かって進められるのが好ましい。したがって、図４−７に示すように、テンション部材３８は、外側シースルーメン９６内に配置されて、アンカー２０ａと２０ｂのストランド３０のコネクタ３６と選択的な係合状態になっている。
図５−７に示す方法は、一例として図２ａのテンション部材３８を含む医療送達デバイス１００を示す。この例では、細長い保持部材４０は、一旦外側シース９６の遠位端９５の近傍に位置すると、アンカーストランド３０のコネクタ３６の保持を、縫合糸５２の第１及び第２の環状部分５８及び６０によって維持している。医師が細長い保持部材４０の保持及び操作をしている間、縫合糸５２の第１及び第２の端部５４及び５６が、好ましくは、デバイス１００の近位側ハンドル（図示せず）内に保持される。折り返された環状部分５８及び６０は、組織アンカー２０ａ及び２０ｂを別々に位置決めする際、縫合糸５２に十分な弛みを与えるのに十分な長さのものであることが好ましい。
図５−６に示すように、針８６を内側及び外側シース９０、９４に対して並進移動させることで、針８６は体内壁１２の組織を貫通して配備される。最も遠位の組織アンカー、即ち図４−５のアンカー２０ａは、その後、アンカー２０aが針のルーメン８８を抜け出るようにアンカー２０ａを針８６に対して並進移動させることで針８６から配備される。図４−５に示すように、アンカー２０ａと２０ｂ、並びにそれらの間に配置されたスペーサー部材５０は、プッシャー９８が内側シースのルーメン９２内に摺動可能に収容され、且つ最も近位のアンカー、即ち図４−７のアンカー２０ｂに係合して推し進めるのに使用され得るように、針ルーメン８８の長手方向軸線に沿って針ルーメン８８内に整列して示されている。最も近位のアンカーは、スペーサー部材５０と最も遠位のアンカー２０aを介して順に力を伝達することになり、その結果、針のルーメン８８の最も遠位のアンカー２０ａを前進させる。アンカー２０aが針のルーメン８８を抜け出ると、アンカー２０aのストランド３０は、針のスロット８７から解放される。
従って、多数の組織アンカーとスペーサー部材を医療デバイス１００内で使用することができ、針のスロット８７の長手方向の長さは任意の数のアンカーストランド３０を収容できる大きさであり得ることが認識されるであろう。この方法では、医療デバイス１００は再装填されるために引き戻される必要はない。従って、本方法は、近位方向に針８６を並進移動させることで体内壁から針８６を引き戻すステップと、次に組織１２を貫通して針８６を並進移動させる段階を繰り返すステップと、該組織を貫通して組織アンカーを配備するステップとを含む。
図７を参照すると、針８６は、近位方向に針８６を並進移動させることで体内壁１２を通して引き戻され、穿孔１０の周りの異なる位置に再配置され、内側及び外側シース９０、９４に対して針８６を並進移動させることで、体内壁の組織を貫通して再配備される。次に、プッシャー９８をスペーサー部材５０と近位側アンカー２０ｂを配備するためにさらに遠位方向に前進させ、アンカー２０ｂのストランド３０は針のスロット８７内から解放される。その後、スペーサー部材５０は、患者の体内に吸収される。
アンカー２０ａと２０ｂは、体内壁１２の遠位側に配置された後、針８６が体内壁１２の近位側に貫通して引き戻されて、内側シースルーメン９２内から取り出される。アンカー２０ａと２０ｂのストランド３０を引っ張ってストランド３０を一緒に持って来て穿孔１０を閉じるために、細長い保持部材４０が用いられる。図８と図９に示すように、好ましくは、細長い保持部材４０が、引き戻され、縫合糸５２の第１及び第２の環状部分５８及び６０に牽引力を加え、次にコネクタ３６に牽引力を加え、その結果、アンカー２０ａと２０ｂの間の距離を縮めて穿孔１０の周囲の体内壁１２を圧縮するようにアンカー２０ａと２０ｂのストランド３０を引っ張る。
図９に最も分かり易く示す様に、アンカー２０ａと２０ｂのストランド３０は、縫合糸ロックを使用するなどして、体内壁１２の圧縮を維持するために固定されている。以上詳細に説明し図４−９に示した本発明の教示に従って、好ましくは、ストランド３０は市販の針縫合糸ロックの使用によって固定される。また、同時係属の米国特許出願第１２／１２５，５２５号及び第１２／１９１，００１号に開示された縫合糸ロックなどの他の縫合糸ロックがストランド３０を固定するために用いることができ、該出願の開示が参照によりその全体が本明細書に組み込まれている。アンカー２０ａと２０ｂのストランド３０を固定するための現在知られている或いは将来開発される締付け、結束、クランプ、リベット及び同種のものなどの任意の方法を使用できることが認識されるであろう。アンカー２０ａと２０ｂが効果的に体内壁１２の穿孔１０を閉じて且つストランド３０が固定された後、テンション部材３８を含む送達デバイス１００は患者の体内から取り出される。図８−９の例では、保持部材４０は、第１及び第２の環状部分５８及び６０からフックが外されており、送達デバイスの近位側ハンドルのトーイ・ボースト（ｔｏｕｈｙ ｂｏｒｓｔ）コネクタ又はクランプを開く又は解放する、及び縫合糸の一方の端部を引っ張りながら縫合糸の反対側の端部を解放することで縫合糸５２は取り外される。この方法では、縫合糸５２は、患者から除去されるように、閉ループコネクタ３６を通って摺動する。
また、前述の方法は概括的には体内管腔を通して組織に組織アンカーを配置するステップを含む一方、当業者によって認識されるであろうデバイス及び方法は、人間又は動物の身体、及び体内管腔に関連してもしなくてもよい任意の材料の層（例えば織物、布、ポリマー、エラストマー、プラスチック及びゴム）に使用してもよいことが理解され、これらのことは当業者によって認識されるであろう。例えば、本明細書に開示された装置及び方法は、人間又は動物の体への用途を見つけてもよいし見つけなくてもよい材料の１つ以上の層を貫通してデバイスを配置する、及び身体の組織ではない材料の層の穴又は穿孔を閉じるために、実験室及び工業環境での用途を見つけることができる。幾つかの例には、裁縫又は縫合並びに同類の製造、合成組織との協働、高分子シートの結合、動物実験、及び解剖作業が挙げられる。

- 身体の体内壁の穿孔を閉じるための医療デバイスであって、

- 前記細長い保持部材が第２のコネクタを有する遠位端を有し、前記第１及び第２のコネクタが互いに選択的に係合及び解除するように構成されている、請求項１に記載の医療デバイス。
- 前記第１及び第２の端部が、医療専門家が引っ張ることができるように前記身体の外部に位置しており、前記細長い縫合糸の前記第１の端部と第２の端部との間に位置する中間部は、前記第１及び第２の端部の少なくとも一方を医療専門家が離すことによって前記第１のコネクタとの係合を選択的に解除するように構成されている、請求項１に記載の医療デバイス。
- 前記第１の組織アンカーの前記第１のコネクタが第１の閉ループを有しており、前記第２の組織アンカーの前記第１のコネクタが第２の閉ループを有しており、前記第１の閉ループは前記第１の環状部分を選択的に受け入れ、前記第２の閉ループは前記第２の環状部分を選択的に受け入れるようにされた、請求項１に記載の医療デバイス。
- 前記第１のコネクタがＪ字型フックと閉ループのうちの１つの形をしている、請求項１に記載の組織アンカー。
