Description
本願は、米国仮特許出願第12/348,722号(2009年1月5日出願)の優先権を主張し、この出願の開示は、その全体が本明細書に参考として援用される。
当技術分野は、電気手術、具体的には、扁桃摘出術およびアデノイド切除術である。
例えば、本発明は以下の項目を提供する。
(項目1)
電気手術デバイスであって、
a)近位端、遠位端を有する細長い本体であって、自身を通って延在する吸入を印加するための管腔を画定する本体と、
b)近位端、遠位端を有する先端であって、該細長い本体の該遠位端において該吸入管腔に流体的に接続される先端管腔を画定する先端と、
c)該先端の該遠位端に固定される筐体であって、該先端管腔に流体的に接続される開口を画定する筐体と、
d)該筐体の中にあり、電極を備えるブレードアセンブリと
を備え、
該ブレードアセンブリまたはそれの一部は、該筐体の該開口を有する吸入ポートを形成する、デバイス。
(項目2)
上記先端は、可鍛性である、項目1に記載の電気手術デバイス。
(項目3)
上記先端を上記細長い本体の上記遠位端に取外し可能に取り付けるために、該先端の上記近位端にコネクタをさらに備える、項目1に記載の電気手術デバイス。
(項目4)
上記コネクタは、該コネクタの周囲に位置する、圧縮性バレルと1つ以上のタブとを備える、項目3に記載の電気手術デバイス。
(項目5)
上記1つ以上のタブは、フィンガーグリップを上記コネクタに取り付ける、項目4に記載の電気手術デバイス。
(項目6)
上記筐体は、上部分と下部分とを備えており、該上部分と下部分とは、相互に接合されたときに上記開口を画定する、項目1に記載の電気手術デバイス。
(項目7)
上記開口は、上記筐体の壁の中に位置している、項目1に記載の電気手術デバイス。
(項目8)
上記開口は、球形または楕円形である、項目7に記載の電気手術デバイス。
(項目9)
上記開口は、それの主軸に沿って約0.4cm〜約1.5cmである、項目7に記載の電気手術デバイス。
(項目10)
上記開口は、それの短軸に沿って約0.4cm〜約1.0cmである、項目7に記載の電気手術デバイス。
(項目11)
上記筐体は、透明材料を含む、項目1に記載の電気手術デバイス。
(項目12)
上記透明材料は、ポリマーである、項目11に記載の電気手術デバイス。
(項目13)
上記ポリマーは、アクリル共重合体、アクリロニトリルブタジエンスチレン(ABS)、ポリカーボネート、ポリスチレン、ポリ塩化ビニル(PVC)、ポリエチレンテレフタレートグリコール(PETG)、およびスチレンアクリロニトリル(SAN)から成る群から選択される、項目12に記載の電気手術デバイス。
(項目14)
上記ポリマーは、ポリメチルメタクリレート(PMMA)共重合体またはスチレンメチルメタクリレート(SMMA)共重合体である、項目13に記載の電気手術デバイス。
(項目15)
上記ブレードアセンブリは、自身を上記筐体に固定する複数のアームを備える、項目1に記載の電気手術デバイス。
(項目16)
上記複数のアームは、各々、第1の部分と第2の部分とを有し、該第1と第2の部分との間に約110°〜約140°の角度を有する、項目15に記載の電気手術デバイス。
(項目17)
上記電極は、切断縁部と、凝固表面とを備える、項目15に記載の電気手術デバイス。
(項目18)
上記切断縁部と筐体開口とは、上記吸入ポートを画定する、項目17に記載の電気手術デバイス。
(項目19)
上記電極は、上記筐体の残りの部分を越えて延在し、湾曲部分と、そこを通って画定される開口部とを有する、項目1に記載の電気手術デバイス。
(項目20)
上記開口部は、上記吸入ポートである、項目19に記載の電気手術デバイス。
(項目21)
上記活性電極は、モリブデン、ニッケル、プラチナ、ステンレス鋼、タンタル、チタン、タングステン、およびそれらの合金から成る群から選択される金属を含む、項目1に記載の電気手術デバイス。
(項目22)
上記デバイスは、電気的に単極である、項目1に記載の電気手術デバイス。
(項目23)
上記電極は、細長く、それの中央部分の中に上記吸入ポートを画定しており、該電極の切断部分は、該吸入ポートに隣接して位置し、細長く、それの遠位部分に向かって先細になっており、該遠位部分は、少なくとも20°の角度で該切断部分の近位部分によって画定される平面から離れて位置している、項目1に記載の電気手術デバイス。
(項目24)
項目1に記載のデバイスを使用して、扁桃腺またはアデノイド組織を切除する方法であって、
a)上記電極にパルス電気信号を印加することにより、それの上にプラズマを形成することと、
b)該プラズマによって該組織を切断するか、または凝固させることと
を含む、方法。
(項目25)
上記吸入ポートを介して上記組織に吸引を印加することをさらに含む、項目24に記載の方法。
(項目26)
吸引は、上記電極の近位移動と同時に印加されることにより、上記組織を吸引切断する、項目25に記載の方法。
(項目27)
外科手術中に上記先端を取り外し、それを第2の先端と交換することをさらに含む、項目24に記載の方法。
(項目28)
a)1つ以上の先端であって、該先端は筐体を備えており、該筐体は、開口を画定し、電極を備えるブレードアセンブリを自身の中に有しており、該ブレードアセンブリまたはその一部は、該筐体の該開口を有する吸入ポートを形成する、先端と、
b)使用説明書と
を備える、キット。
(項目29)
上記先端に連結可能である1つ以上の吸引ハンドルをさらに備える、項目28に記載のキット。
鼻/咽頭の後部に位置するアデノイド組織は、口腔または鼻を通して除去され得るが、一般的には、全身麻酔下で口腔を通して除去され得る。したがって、アデノイドに到達するのが困難であるだけでなく、気管内チューブの存在が、すでに制限されている手術野をさらに制約する。本明細書に記載するアデノイド用先端は、アデノイド切除術のこれらの問題の多くに対処するように特に構成される交換可能な先端である。
扁桃腺は、咽頭の両側に位置し、全身麻酔下で除去される。この手技において、扁桃腺の結合されていない端部は、通常、鉗子で把持され、同時に、咽頭から扁桃腺を除去するために、もう一方の端部で切断が行われる。したがって、アデノイド切除術と同様に、アクセスが困難であり、手術野は制約される。本明細書に記載する扁桃摘出術用先端は、扁桃摘出術のこれらの問題の多くに対処するように特に構成される、交換可能な先端である。
切除したヒト口蓋扁桃に、30Wの切断および30Wの凝固の設定で、従来の「ボビー」針先を使用した一連の外科的切開を行った。切断に対して切断5(20Wの出力)および凝固に対して凝固6(30Wの出力)の設定で、本明細書に記載する電気手術扁桃摘出術デバイス(「扁桃腺用ブレード」)を使用して、追加の切開を行った。切開の直後に組織学的試料を採取し、顕微鏡を使用して残りの熱傷に関して評価した。熱傷深度(mm)および熱傷の減少の割合の比較を表1に提供する。
Claims (21)
- 電気手術デバイスであって、
a)近位端、遠位端を有する細長い本体であって、自身を通って延在する吸入を印加するための管腔を画定する本体と、
b)近位端、遠位端を有する先端であって、該細長い本体の該遠位端において該吸入管腔に流体的に接続される先端管腔を画定する先端と、
c)該先端の該遠位端に固定される筐体であって、該先端管腔に流体的に接続される開口を画定する筐体と、
d)該筐体の中にあり、前記筺体の前記開口を横に横断するように延びる切断縁部と凝固表面とを有する電極を備え、該電極が、パルス電気信号が該電極に印加されたとき該電極上に形成されたプラズマを有するように構成されている、ブレードアセンブリと
を備え、
前記デバイスは、該プラズマによって扁桃腺またはアデノイド組織を切断するか、または凝固させることによって該扁桃腺またはアデノイド組織を切除するように構成されており、
該ブレードアセンブリの前記切断縁部は、該筐体の該開口を有する吸入ポートを形成し、
前記ブレードアセンブリが前記筺体に組み込まれた後、前記切断縁部と前記筺体の前記開口とが、前記吸入ポートを画定する、デバイス。 - 前記先端は、可鍛性である、請求項1に記載の電気手術デバイス。
- 前記先端を前記細長い本体の前記遠位端に取外し可能に取り付けるために、該先端の前記近位端にコネクタをさらに備える、請求項1または2に記載の電気手術デバイス。
- 前記コネクタは、該コネクタの周囲に位置する、圧縮性バレルと1つ以上のタブとを備える、請求項3に記載の電気手術デバイス。
- 前記筐体は、上部分と下部分とを備えており、該上部分と下部分とは、相互に接合されたときに前記開口を画定する、請求項1から4のいずれか1項に記載の電気手術デバイス。
- 前記開口は、前記筐体の壁の中に位置している、請求項1から4のいずれか1項に記載の電気手術デバイス。
- 前記開口は楕円形である、請求項6に記載の電気手術デバイス。
- 前記筐体は、透明材料を含む、請求項1から7のいずれか1項に記載の電気手術デバイス。
- 前記透明材料は、ポリマーである、請求項8に記載の電気手術デバイス。
- 前記ポリマーは、アクリル共重合体、アクリロニトリルブタジエンスチレン(ABS)、ポリカーボネート、ポリスチレン、ポリ塩化ビニル(PVC)、ポリエチレンテレフタレートグリコール(PETG)、およびスチレンアクリロニトリル(SAN)から成る群から選択される、請求項9に記載の電気手術デバイス。
- 前記ポリマーは、ポリメチルメタクリレート(PMMA)共重合体またはスチレンメチルメタクリレート(SMMA)共重合体である、請求項10に記載の電気手術デバイス。
- 前記ブレードアセンブリは、自身を前記筐体に固定する複数のアームを備える、請求項1から11のいずれか1項に記載の電気手術デバイス。
- 前記複数のアームは、各々、第1の部分と第2の部分とを有し、該第1と第2の部分との間に約110°〜約140°の角度を有する、請求項12に記載の電気手術デバイス。
- 前記活性電極は、モリブデン、ニッケル、プラチナ、ステンレス鋼、タンタル、チタン、タングステン、およびそれらの合金から成る群から選択される金属を含む、請求項1から13のいずれか1項に記載の電気手術デバイス。
- 前記デバイスは、電気的に単極である、請求項1から14のいずれか1項に記載の電気手術デバイス。
- 前記電極は、細長く、それの中央部分の中に前記吸入ポートを画定しており、該電極の切断部分は、該吸入ポートに隣接して位置し、細長く、それの遠位部分に向かって先細になっており、該遠位部分は、少なくとも20°の角度で該切断部分の近位部分によって画定される平面から離れて位置している、請求項1から11のいずれか1項に記載の電気手術デバイス。
- 前記吸入ポートを介して前記組織に吸引を適用するようにさらに構成されている、請求項1から16のいずれか1項に記載の電気手術デバイス。
- 前記電極の近位移動と同時に吸引を適用することにより、前記組織を吸引切断するようにさらに構成されている、請求項17に記載の電気手術デバイス。
- 前記先端は、外科手術中に取り外され、第2の先端と交換されるように構成されている、請求項1から18のいずれか1項に記載の電気手術デバイス。
- a)請求項1に記載の電気手術デバイスと、
b)使用説明書と
を備える、キット。 - 前記先端に連結可能である1つ以上の吸引ハンドルをさらに備える、請求項20に記載のキット。
