Description
これまで、埋込み型心臓治療システムは、心外膜システムまたは経静脈システムのいずれかである。たとえば、経静脈システムは図1Bに概略的に示されるように埋め込むことができる。しかしながら、以下本明細書で説明するように、本発明は、図1Aに示されるような皮下埋込み型心臓治療システムで機能するようにも適応される。
第1の閾値T1および第2の閾値T2は、先行のピークまたは先行のピーク検出信号の平均の定義率として選択される。
図5A〜5Cは、捕捉された心臓信号の調整方法の例をグラフで示す。図5Aを参照すると、QRS特徴を有する比較的正常な心臓事象に対応する受信信号200が示されている。信号200は基準線202を中心にアナログで示される。サンプル閾値204、206は基準線202を中心に示される。図5Bは、図5Aの信号200のサンプリングを示す。サンプル210は信号200を定期的に表示し、信号のデジタル操作を可能にすることが分かる。いくつかのサンプルは閾値204、206を超えない。図5Cを参照すると、サンプリングされた表示210のみが示されている。サンプル212などのいくつかのサンプルは、符号「X」で置き換えられている。これらのサンプルは、閾値204、206を超えなかったサンプルであるため、図示される方法を用いて、基準線値で置き換えられている。

Claims (5)
- 埋込み型心臓刺激装置(ICSD)であって、
前記ICSD用演算回路を含むハウジングと、
前記演算回路に接続される複数の電極と、
を備え、前記演算回路は、心臓信号解析処理を実行するように構成されており、前記心臓信号解析処理が、
患者内に配置された第1および第2の電極を用いて心臓信号を捕捉すること、
心臓事象を検知すること、
前記心臓信号をサンプリングし、そのサンプリング信号をサンプルベクトルとして処理し、前記サンプルベクトルに重み付けベクトルを乗算して、重み付けサンプルベクトルを得ること、
前記重み付けサンプルベクトルを解析して、前記患者が悪性の心臓状態にある可能性が高いか否かを判定すること、を備え、
前記重み付けベクトルが前記重み付けベクトル内にて少なくともいくつかの他の成分よりも大きな少なくともいくつかの成分を有するように前記演算回路が構成され、前記重み付けベクトルが予め決定された一連の値からなる、埋込み型心臓刺激装置。 - 前記サンプルベクトルが、前記サンプルベクトルの基準点として特定される成分を含み、前記重み付けベクトルが、前記サンプルベクトル内の前記基準点に対応するピーク成分を有し、前記ピーク成分が、前記重み付けベクトルのうちの他の成分よりも大きな振幅を有するように前記演算回路が構成されている、請求項1に記載の埋込み型心臓刺激装置。
- 埋込み型心臓刺激装置(ICSD)であって、
前記ICSD用演算回路を含むハウジングと、
前記演算回路に接続される複数の電極と、
を備え、前記演算回路は、患者が悪性の心臓状態にあるか否かを判定する処理を実行するように構成されており、当該処理が、
埋込み電極を用いて患者から心臓事象のある心臓信号を捕捉すること、
複数の信号サンプルを含む前記心臓信号をサンプリングすること、
前記心臓信号を記憶テンプレートと比較して、前記心臓信号と前記記憶テンプレートとの相関を示す記録を生成することであって、該比較中に前記複数の信号サンプルのうちの少なくともいくつかの信号サンプルに大きな重みを与え、前記複数の信号サンプルのうちの他の信号サンプルに小さな重みを与える、前記記録を生成すること、
を備え、前記重みを与える重み付けベクトルが予め決定された一連の値からなる、埋込み型心臓刺激装置。 - 前記心臓信号が基準点を含み、他のサンプルよりも前記基準点に近いサンプルに大きな重みが与えられるように前記演算回路が構成されている、請求項3に記載の埋込み型心臓刺激装置。
- 前記心臓信号が1つ以上の傾斜を含み、前記心臓信号の傾斜部分に沿ったサンプルに小さな重みが与えられるように前記演算回路が構成されている、請求項3又は4に記載の埋込み型心臓刺激装置。
