本開示は、外科的ステープリング装置、およびより詳細には、外科的ステープリング装置において使用するためのステープル−ギャップ調節部材に関する。
（２．関連技術の背景）

従って、一貫して狭い範囲内である均一な固定ステープルギャップを有し、発射ストロークの間、均一なままである、外科的ファスナー適用装置についての必要性が存在する。さらに、使い捨て可能なステープルカートリッジが装置内に適切に装填されないかまたは全く装置内に装填されていない場合に発射しないか、あるいは、外科的ファスナー適用装置が締め付けられていない状態にある場合に発射しない、外科的ファスナー適用装置についての必要性が存在する。完全に発射されたかまたは部分的に発射された使い捨て可能ステープルカートリッジを中に受容しない、外科的ファスナー適用装置についての必要性がやはり存在する。
（要旨）
改善された外科的ステープラーを提供すること、および改善された外科的ステープラーを作製するための改善された機構および方法を提供することが望まれ、そして本発明の主要な目的であり、このステープラーは、受容可能なステープルギャップ許容範囲に十分な範囲内である、狭い範囲内で一貫して均一な固定ステープルギャップを有し、そして受容可能なステープル許容範囲に十分な範囲内の狭い範囲内で形成されるステープルを一貫して作り出す。
使用のために販売されるときにまたは使用の前に、所定のモデルについてステープラー間でステープルギャップに実質的に変動を有さず、そして改善されたステープルを提供する外科的ステープラーを生成することが、本発明の目的である。
本開示は、外科的ステープリング装置において使用するための線形ステープル−ギャップ調節部材に関する。本開示はまた、外科的ステープリング装置のステープルギャップを調節および固定する方法に関する。
本開示の１つの局面に従って、互いに作動可能に連結される一対の顎構造を有し、そして間にステープルギャップを規定する外科的ステープリング装置において使用するためのステープルギャップ調節部材は、共通の回転軸を共有する１つ以上のベアリング部分、および周囲に沿って少なくとも２つの異なる点を有する周囲を有する１つ以上のカム表面部分を備える。好ましくは、この周囲に沿った各点は、回転軸とは異なる半径を有する。

ギャップ調節部材の３６０°の回転は、１つ以上のベアリング表面の回転軸と１つ以上のカム表面の中心軸との間の距離の約２倍に等しいステープルギャップのサイズの調節を生じる。好ましくは、ステープルギャップ調節部材は、カム調節部材の回転運動によって、ステープルギャップのサイズの調節を作り出すように構成および適合される。あるいは、線形（例えば、水平）運動によってステープルギャップ調節を作り出すカム調節部材（例えば、ウェッジ（ｗｅｄｇｅ））が使用され得ることが想定される。

外科的ステープリング装置が、複数の計測要素をさらに備え、各々が、異なる所定の固定された厚みを有する。複数の計測要素は、選択された計測要素の厚みに等しいサイズにステープルギャップを設定するように、ステープルギャップに選択的に挿入可能である。
ステープルギャップ調節部材が、顎構造の少なくとも１つ（好ましくは、一対の取り付け部材の少なくとも１つ）にしっかりと固定されることが想定される。
本開示はまた、外科的ステープリング装置におけるステープルギャップを調節する方法を提供する。本開示の方法は、第１の顎構造および第２の顎構造それぞれの前方部分の間に規定されるステープルギャップに計測要素を挿入する工程、第１の顎構造および第２の顎構造の間に配置されたギャップ調節部材を操作することによってステープルギャップの大きさを調節し、ステープルギャップの大きさを計測要素に近づける工程、ならびにギャップ調節部材を所定の位置に固定する工程、を包含する。本方法の別の局面において、計測要素は、第１の顎構造および第２の顎構造の向かい合った組織接触表面の間に挿入される。

本開示される直線状ステープラー状ギャップ調節機構の好ましい実施形態は、ここで、添付の図面を参照して詳細に記載され、この図面において、同一の参照番号は、いくつかの図面の各々において、同一の要素または対応する要素を示す。

図６に見られるように、外科的ステープリング装置１００の個々の要素をほとんど（全てではないにしても）組み立てた後、ギャップ「Ｙ」は、外科的ステープリング装置１００の前方部分について、設定される。ギャップ「Ｙ」を設定する１つの方法に従って、予め決定された固定された厚みを有する測定要素１６０が、外科的ステープリング装置１００の遠位先端のギャップ「Ｙ」に挿入される。偏心カム２００は固定されるか、または次いで、ギャップ「Ｙ」が測量手段１６０の予め決定された厚みに設定されるまで回転される。ギャップ「Ｙ」は、非負荷条件または負荷条件（好ましくは、非負荷条件（すなわち、組織提示のないかまたはある程度の負荷のある条件））で設定される。他の適切な手段および方法は、製造プロセスまたは組み立てプロセスの間またはその一部として、同時にまたは異なる時点で使用され得る。

図７〜１１に関して、偏心カム２００を回転させるための種々の手段がその上に提供されており、そしてここに記載する。図７に見られるように、偏心カム２００の前方部分２１０には、その端または上表面に形成される六角形の凹部２４０が提供される。六角形の凹部２４０は、偏心カム２００を回転させるために、回転ツール（図示せず）の「アレン鍵（Ａｌｌｅｎ Ｋｅｙ）」型端を受容する構成である。六角形の凹部２４０が示されているが、任意の多角形の凹部（すなわち、三角形、四角形、五角形など）が、前方部分２１０の端表面に形成される多角形の凹部に形状が対応する多角形の突出部を有する構成の回転ツールと共に提供され得る。

ここで、図１２〜１４に目を向けると、ステープリング装置に偏心カムをしっかりと固定する好ましい方法が開示されている。好ましい方法に従って、一対のヒンジプレートが、環状壁２３２により規定される後方部分２３０を有する偏心カム２００との使用について開示される。図１２に見られるように、カートリッジ受容構造１１２には、第１の直立ヒンジプレート１６０および第２の直立ヒンジプレート１６２が提供され、各々、カートリッジ受容構造１１２の側表面から延びている。第１のヒンジプレート１６０には、少なくとも１つのノッチ１６６を有する貫通穴１６４が提供され、その円周視野計に形成され、そして偏心カム２００の後方部分２３０の一部をそこにまたはそこを通してぴったりと受容するような大きさである。第２のヒンジプレート１６２には、偏心カム２００の前方部分２１０をそこにまたはそこを通してぴったりと受容するような大きさの貫通穴１６８が提供される。図１３は、貫通穴１６４、１６８の対の中の場所にぴったりと入った偏心カム２００を示すことが意図される。図１４は、偏心カム２００が所望の位置まで回転した後、偏心カム２００が、貫通穴１６４の円周視野計に沿って形成されたノッチ１６６に緊密に環状壁２３２を変形させることによって適所にしっかりと固定されることを示すことが意図される。
ここで、図１５〜１７に目を向けると、ヒンジプレート１６０、１６２の対に偏心カム２００をしっかりと固定する代替の方法が記載されている。図１５に見られるように、偏心カム２００の後方部分２３０は、１つ以上の点溶接３００（すなわち、金属不活性ガス「ＭＩＧ］、タングステン不活性ガス「ＴＩＧ」、アーク、レーザー、超音波、電子ビーム、真鍮、銀鑞、はんだなど）によって、後方部分２３０とヒンジプレート１６０との間の適所にしっかりと固定される。図１６に見られるように、偏心カム２００は、第１ヒンジプレート１６０のシート金属の一部の変形３１０によって（例えば、ピン固定（ｐｉｎｎｉｎｇ）、ピーニング（ｐｅｅｎｉｎｇ）、スウェージ加工、センターポンチ固定（ｃｅｎｔｅｒ ｐｕｎｃｈｉｎｇ）などによって）、後方部分２３０の円周に沿って形成される凹部２３４の少なくとも１つ（好ましくは各々）の適所にしっかりと固定され得る。図１７に見られるように、偏心カム２００は、後方部分２３０が貫通穴１６４を充填および制限し、その中での偏心カム２００の回転を防止するよう十分拡張するまで、偏心カム２００の後方部分２３０の周囲の１つ以上の覗き穴３２０の適所にしっかりと固定され得る。ヒンジプレート１６０のシート金属が、偏心カム２００の後方部分をしっかりとつかむまで、貫通穴１６４の周囲でピーニングされ得ることもまた想定される。あるいは、任意の適切な接着剤または結合剤（例えば、嫌気的接着剤（例えば、ＬｏｃｉｔｅＴＭ（Ａｍｅｒｉｃａｎ Ｓｅａｌａｎｔｓ Ｃｏｍｐａｎｙ，Ｃｏｒｐ．の商標））もしくはその等価物、または他の接着性液体セメント）が、それぞれ、前方部分と後方部分の表面２１０、２３０と、貫通穴１６４、１６８との間に適用され得る。
図１８のグラフに見られるように、本開示に従って製造された外科ステープラーと比較して、当該分野の現状に従って製造された外科ステープラーについての許容可能なステープルギャップが示される。当該分野の現状では、外科ステープラーの製造および組み立ては、得られるステープルギャップの範囲に広い偏差を有する。外科ステープラーの特定のラインおよびモデルの製造および組み立ての間、外科ステープラーの製造および組み立てに本来備わっているかまたは関与する種々の因子（例えば、材料、寸法、および条件の偏差）に起因して、図１８のベル型曲線「Ｄ」により示されるように、許容可能なステープルギャップ「ＡＳＧ」許容範囲の中に、ステープルギャップ「Ｙ」の大きさに広い偏差が存在していた。しかし、本開示に従って、上記の種々の因子にも関わらず、製造および組み立てられた外科ステープラーは、一貫して、図１８のベル型曲線「Ｅ」により示されるように、許容可能な範囲内に狭い範囲の偏差を有し得る。従って、本開示に従って製造および組み立てられる外科ステープラーは、当該分野の現状に従って製造（ｍａｎｕｆａｃｔｕｒｅまたはｐｒｏｄｕｃｅ）される外科ステープラーのステープルギャップよりも一貫していて、均一なステープルギャップを有する。
偏心カム２００は、１つのヒンジプレートに固定されることが示されるが、偏心カム２００は、両方のヒンジプレートに固定され得ることが想定される。さらに、円形カムが開示されているが、他の型のカム部材が、本開示の範囲から逸脱することなく使用され得ることが想定される。例えば、ランプカムが、カートリッジ受容構造とアンビル受容構造との間に配置され得、屈曲可能なタブが、それらの構造のうちの１つに構成、追加され得、そしてアンビル受容構造に対するカートリッジ受容構造の移動に適合され得、そして１つ以上のジャックスクリューが、アンビル受容構造と接触するよう移動するために使用され得る。さらに、本開示は特に、線形外科ステープラーに適切であるが、本開示は、単に、線形外科ステープラーのみに限定されないことが想定される。
種々の改良が、本明細書に開示される実施形態に対してなされ得ることが理解される。従って、上記は、限定として考慮されるべきでなく、単に、好ましい実施形態の例示として考慮されるべきである。当業者は、添付の特許請求の範囲および意図の中で他の改良を想像する。
本開示される外科的ステープリング装置および方法の好ましい実施形態は、図面を参照して本明細書中に記載される。しかし、図面は、例示の目的のみのために設計され、本発明を制限する定義として理解されるべきである。
