Abstract
Description
図1に示される実施形態において、近位端12は患者の体外にとどまる。近位端12はハンドル30及び任意選択のプッシャースタイレット20を備える。
図1及び2に示される送達システム10は中間部送達装置14を有している。本発明において、中間部送達装置14は送達システム10の近位端12(図1、2)と遠位端13(図1、2)との中間にある。用語「中間にある」は、中間部送達装置14が2つの極部の間に介在する、挟まっている、存在している又は現れている、又は遠位端13の遠位先端と近位端12の近位先端との間で―必ずしも等距離であるとは限らないが―空間的に中間の位置、状態、又は性質にある本発明の実施形態を説明することが意図されている。さらに、中間部送達装置14は遠位端13及び/又は近位端12の一部に重ねられてもよく、又は部分的に導入されてもよい。別の実施形態において、中間部送達装置14(以下で説明されるシース50など)の一部及び遠位端外部誘導管部材80(以下で考察され、図4、5、6、7を参照)は細長いチューブ状カテーテル又は一体構造のフレクサー(Flexor)(登録商標)シースであってもよい。
ここで本発明における医療装置送達システムの遠位端13の実施形態を参照すると、図4、5、6、及び7は遠位端13が比較的チューブ状の本体であることを示す。脈管、脈管路、内視鏡の作業導管の形態、又は操縦されるべき内視鏡と併せて使用される外部付属導管装置にかんがみて、遠位がテーパされ、丸められ、面取りされ、又は矢尻形であるほぼチューブ状の遠位端が患者にはより許容し易いであろう。さらに、特定の実施形態において、遠位端13の遠位部は軟性で、丸められ、かつ可撓性であることにより、患者にさらなる保護及び配慮を提供し得る。
チューブ状内部圧迫部材41は約0.134cm〜約0.335cm(約0.0527〜約0.132インチ)の範囲をとる均一な内径を有してもよい。チューブ状内部圧迫部材41の壁厚は約0.0038cm(約0.0015インチ)である。これらの寸法は例示に過ぎないとともに、内径及び壁厚は送達システムが用いられる目的を達成するのに必要な任意のサイズで構築されてもよい(すなわち、装置が使用される脈管路又は作業導管により制限される)。
本発明は、更に、植え込むことのできる人工器官(例えば、自己拡張型、バルーン拡張型、又は非拡張型ステント、人工弁装置、及び植え込むことのできる他の物品)を患者の身体内部の選択された場所に展開するための医療装置送達システムに用いられる内部接合部アセンブリと、内部接合部アセンブリを作る方法を備えている。本発明は、更に、植え込むことのできる人工器官を展開するための医療装置送達システムを備えている。
植え込むことのできる人工器官(例えば、自己拡張型、バルーン拡張型、又は非拡張型ステント、人工弁装置、及び移植可能な他の物品)を、患者身体内部の選択された場所に展開するための医療装置送達システム用の内部接合部を製造する方法を更に提供する。図15は、本発明による医療装置送達システム用の内部接合部を製造する方法200を示すブロック図である。
Claims (31)
- 植え込むことのできる人工器官を、患者の身体の内部の選択された場所に展開するための医療装置送達システムで使用するための内部接合部において、
遠位第1の端部と近位第2の端部を有しており、前記第2の端部は、外径と外部表面を備えている、内部誘導管部材と、
内部固定部分と基部固定部分を備えている固定体であって、前記内部固定部分は、第1の端部と第2の端部を有しており、それらの間に、前記内部誘導管部材の第2の端部の外径を受け入れることのできる寸法に作られた収容空洞を画定しており、前記収容空洞は、前記内部誘導管部材の第2の端部の外部表面の周りに配置され、これと機能的に連結されている少なくとも1つの内部係合表面を有しており、前記基部固定部分は、外部係合表面を備えている、固定体と、
近位端と遠位嵌合端部を有する細長い内部圧迫部材であって、前記遠位嵌合端部は、係合表面と、前記基部固定部分の外部係合表面と相補形の輪郭形状とを備えている、内部圧迫部材と、を備えており、
前記基部固定部分の外部係合表面と前記遠位嵌合端部の係合表面は、境を接しており、前記基部固定部分と前記遠位嵌合端部は、機能的に連結されている、内部接合部。 - 前記固定体は、金属を備えている、請求項1に記載の装置。
- 前記固定体は、概ね管状のカニューレを備えている、請求項1又は2に記載の装置。
- 前記内部圧迫部材の遠位嵌合端部の輪郭形状は、縦溝が付けられており、湾曲している前記基部固定部分の外部係合表面と相補形である、請求項1に記載の装置。
- 前記内部圧迫部材の遠位嵌合端部の輪郭形状は、平坦であり、平坦である前記基部固定部分の外部係合表面と相補形である、請求項1に記載の装置。
- 前記固定体は、概ねリング型である、請求項1に記載の装置。
- 前記固定体は、概ねU字型である、請求項1に記載の装置。
- 前記基部固定部分と前記遠位嵌合端部は、溶接又ははんだ付けによって機能的に連結されている、請求項1に記載の装置。
- 前記基部固定部分と前記遠位嵌合端部は、前記基部固定部分の外部係合表面と前記遠位嵌合端部の外部係合表面を融合させることによって機能的に連結されている、請求項1に記載の装置。
- 前記収容空洞の内部係合表面と前記内部誘導管部材の第2の端部の外部係合表面は、クリンプ、摩擦嵌合、圧入嵌合、楔止め、固着剤、接着剤、樹脂、溶接、化学結合材、及びそれらの組み合わせから成るグループから選択された結合によって機能的に連結されている、請求項1に記載の装置。
- 前記内部誘導管部材の第1の端部は入口を備えており、前記内部誘導管部材の第2の端部は出口を備えており、前記入口と前記出口は、それらの間に誘導管を画定している、請求項1に記載の装置。
- 前記内部誘導管部材は、更に、前記内部誘導管部材の両端の前記第1の端部と第2の端部の中間に設けた外部ステントプラットフォームと、近位側ステント抑制部と、を備えている、請求項11に記載の装置。
- 前記内部誘導管部材は、更に、前記内部誘導管部材の両端の前記第1の端部と第2の端部の中間に傾斜部を備えており、前記傾斜部は、前記第2の端部の外径より小さい第2外径を有している、請求項11に記載の装置。

遠位第1の端部と近位第2の端部とを有している内部誘導管部材であって、前記近位第2の端部は外部表面を備えており、ステントプラットフォームが、前記内部誘導管部材の周りに、前記第1の端部と前記第2の端部の中間に円周状に配置されている、内部誘導管部材と、
前記内部誘導管部材の第1の端部に機能的に連結されており、遠位側ステント抑制部を備えている、非外傷性先端と、
前記内部誘導管部材の周りに、前記内部誘導管部材の第1の端部と第2の端部の中間に配置され、これと機能的に連結されている抑制リングであって、近位側ステント抑制部を備えている、抑制リングと、
近位端部と遠位嵌合端部を有する細長い内部圧迫部材であって、前記遠位嵌合端部は、係合表面と輪郭形状を備えている、内部圧迫部材と、
前記内部誘導管部材の第2の端部の周りに配置され、これと機能的に連結されている内部固定部分と、前記遠位嵌合端部の係合表面に押し付けて配置されている外部係合表面を有する基部部分とを備えているカニューレ付き固定体であって、前記基部固定部分と前記内部圧迫部材の遠位嵌合端部の輪郭形状は、機能的に連結されている、カニューレ付き固定体と、を備えているシステム。 - 前記カニューレ付き固定体の内部固定部分は、第1の端部と第2の端部を備えており、それらの間に収容空洞を画定し、内部係合表面を有している、請求項14に記載の装置。
- 前記収容空洞の内部係合表面と前記内部誘導管部材の第2の端部の外部係合表面は、クリンプ、摩擦嵌合、圧入嵌合、楔、固着剤、接着剤、樹脂、溶接、化学結合材、及びそれらの組み合わせから成るグループから選択された結合によって機能的に連結されている、請求項15に記載の装置。
- 前記カニューレ付き固定部材の前記基部固定部分と前記内部圧迫部材の遠位嵌合端部は、溶接、はんだ付け、固着剤付け、融合、及びそれらの組み合わせから成るグループから選択された結合部によって機能的に連結されている、請求項14に記載の装置。
- 前記内部圧迫部材の遠位嵌合端部の輪郭形状は、縦溝が付けられており、湾曲している前記基部固定部分の外部係合表面と相補形である、請求項17に記載の装置。
- 前記内部圧迫部材の遠位嵌合端部の輪郭形状は、平坦であり、平坦である前記基部固定部分の外部係合表面と相補形である、請求項17に記載の装置。
- 前記結合部と前記カニューレ付き固定部材の一部との周りに配置された収縮管を更に備えている、請求項17に記載の装置。
- 前記内部誘導管部材の第1の端部は入口を備えており、前記内部誘導管部材の第2の端部は出口を備えており、前記入口と出口は、それらの間に誘導管を画定している、請求項14に記載の装置。
- 前記非外傷性の先端は、入口を備えた遠位第1の端部と、出口を備えた近位第2の端部を備え、誘導管を画定しており、前記非外傷性の先端の第2の端部は、前記遠位側ステント抑制部を備え、前記非外傷性の先端の誘導管が前記内部誘導管部材の誘導管と流体連通するように前記内部誘導管部材の第1の端部と機能的に連結されている、請求項21に記載の装置。

- 植え込むことのできる人工器官を、患者の身体の内部の選択された場所に展開するための医療装置送達システムで使用するための内部接合部を作る方法において、
入口を備えた遠位第1の端部と、出口を備えた近位第2の端部を有している内部誘導管部材を提供する段階であって、前記入口と出口は、それらの間に誘導管を画定しており、前記第2の端部は、外径と外部表面を備えている、内部誘導管部材を提供する段階と、
内部固定部分と基部固定部分を備えた固定体を提供する段階であって、前記内部固定部分は、第1の端部と第2の端部を有しており、それらの間に、前記内部誘導管部材の第2の端部の外径を受け入れることのできる寸法に作られた収容空洞を画定しており、前記収容空洞は、少なくとも1つの内部係合表面を有しており、前記基部固定部分は、湾曲部と外部係合表面を備えている、固定体を提供する段階と、
近位端部と遠位嵌合端部を有する細長い内部圧迫部材を提供する段階と、
前記内部圧迫部材の遠位嵌合端部を、係合表面と、前記基部固定部分の外部係合表面と相補形の輪郭形状とを備えた形態に形成する段階と、
前記基部固定部分の外部係合表面を、前記遠位嵌合端部の係合表面と境を接する関係に配置する段階と、
前記内部誘導管部材の第2の端部を、前記固定体の内部固定部分の収容空洞内に導入する段階と、
前記収容空洞の内部係合表面と前記内部誘導管部材の第2の端部の外部表面を機能的に連結させる段階と、から成る方法。 - 前記基部固定部分と前記遠位嵌合端部を溶接する段階を更に含んでいる、請求項24に記載の方法。
- 前記基部固定部分と前記遠位嵌合端部をはんだ付けする段階を更に含んでいる、請求項24に記載の方法。
- 前記基部固定部分の外部係合表面と前記遠位嵌合端部の係合表面を融合させる段階を更に含んでいる、請求項24に記載の方法。
- 前記収容空洞の内部係合表面と前記内部誘導管部材の第2の端部の外部表面を機能的に連結させる段階は、クリンプ、摩擦嵌合、圧入嵌合、楔止め、固着剤、接着剤、樹脂、溶接、化学結合材、及びそれらの組み合わせから成るグループから選択された結合を形成する段階を含んでいる、請求項24に記載の方法。
- 前記輪郭形状を形成する段階は、前記内部圧迫部材の遠位嵌合端部を、円形から、前記基部固定部分の外部係合表面と相補形の縦溝付き形状に、圧延加工又は鍛造する段階を含んでいる、請求項24に記載の方法。
- 前記輪郭形状を形成する段階は、前記内部圧迫部材の遠位嵌合端部を、円形から、前記基部固定部分の外部係合表面と相補形の平坦な形状に、圧延加工又は鍛造する段階を含んでいる、請求項24に記載の方法。
- 前記輪郭形状を形成する段階は、前記内部圧迫部材の遠位嵌合端部を、円形から、前記基部固定部分の外部係合表面と相補形の平坦な形状に、鍛造する段階を含んでいる、請求項28に記載の方法。
