Abstract
【解決手段】本発明は、カテーテルを通って血管動脈瘤の位置へ送出することのできる動脈瘤治療デバイスを対象にする。治療デバイスは、一般的に、少なくとも一つの支持部材及び少なくとも一つの支持部材に選択的に適用される反応性材料によって形成された本体部材を有する。本体部材は、弱化又は無力な血管に機械的支持体を提供する。反応性材料は血管を通る血流に実質的に影響を及ぼすことなく動脈瘤への血流を制限又は閉塞することが可能である。

Claims (43)
- 血管動脈瘤を治療する装置であって、
少なくとも第1表面を有する少なくとも一つの支持部材と、
前記支持部材に選択的に適用される反応性材料と、
を有し、前記反応性材料が未反応状態及び反応状態を有し、前記反応状態が前記動脈瘤への血流を制限することができるような状態である装置。 - 前記支持部材は第1表面及び隣接する第2表面を有し、前記反応性材料は前記第1表面に適用される請求項1に記載の装置。
- 前記反応性材料は前記第1及び第2表面に適用される請求項2に記載の装置。
- 前記反応性材料は前記第1及び第2表面の一部分に適用される請求項2に記載の装置。
- 前記反応性材料は前記第1表面の一部分に適用される請求項1に記載の装置。
- 前記少なくとも一つの支持部材は前記反応性材料をさらに有する請求項1に記載の装置。
- 前記反応性材料は前記少なくとも一つの支持部材と一体的に形成されている請求項1に記載の装置。
- 前記反応性材料は織り込み構造を提供するために、前記少なくとも一つの支持部材に織り込まれている請求項1に記載の装置。
- 前記反応性材料は未反応体積V及び反応体積V’を有し、V’はVより大きくなっている請求項1に記載の装置。
- 前記反応性材料は約7.4の生理的pHの存在下で、反応体積V’を得ることが可能である請求項9に記載の装置。
- 前記少なくとも一つの支持部材は、半径方向及び軸方向の可撓性を有する本体部材を有し、前記本体部材は、第1曲率半径R及び第2曲率半径R’を有し、R’はRより大きくなっている請求項1に記載の装置。
- 前記第2曲率半径R’は、受け入れる血管の曲率半径とほぼ等しくなっている請求項11に記載の装置。
- 前記少なくとも一つの支持部材は血管パッチデバイスを有し、そのパッチデバイスは複数の連結支持部材によって形成された半径方向及び軸方向の可撓性を有するパッチ本体部材を有し、前記パッチ本体部材はその中に形成された複数の開窓を有する請求項11に記載の装置。
- 前記少なくとも一つの支持部材は半径方向及び軸方向の可撓性を有するコイル状ブリッジデバイスを有し、そのブリッジデバイスは弾性正弦本体部材を有し、その正弦本体部材が複数の開口部を限定している請求項11に記載の装置。
- 前記少なくとも一つの支持部材は半径方向及び軸方向の可撓性を有する円筒形本体部材を有し、前記円筒形本体部材は、第1直径D及び第2直径D’を有し、 D’はDよりも大きくなっている請求項1に記載の装置。
- 前記第2直径D’は受け入れる血管の直径とほぼ等しくなっている請求項15に記載の装置。
- 前記少なくとも一つの支持部材は半径方向及び軸方向に拡張する螺旋状ステントを有し、そのステントは第1端部及び第2端部の間に位置する円筒形本体部材を有し、前記円筒形本体部材は前記第1及び第2端部と連通する内腔をさらに限定している請求項15に記載の装置。
- 前記少なくとも一つの支持部材は半径方向及び軸方向に網状に拡張するステントを有し、そのステントは第1端部及び第2端部の間に位置する円筒形本体部材を有し、前記円筒形本体部材はその上に形成された複数の開窓を有するとともに前記第1端部及び第2端部と連通する内腔をさらに限定している請求項15に記載の装置。
- 前記少なくとも一つの支持部材は半径方向及び軸方向の可撓性を有する分岐支持デバイスを有し、そのデバイスは第1端部、第2端部及び第3端部の間に位置する分岐本体部材を有し、前記分岐本体部材はその上に形成された複数の開窓を有するとともに前記第1、第2及び第3端部と連通する内腔をさらに限定している請求項15に記載の装置。
- 前記少なくとも一つの支持部材は生体内の所定位置へ送出されることが可能であるとともに送出デバイスから制御可能に解放されることが可能であり、前記送出デバイスはカテーテル、マイクロカテーテル、バルーンカテーテル、拡張可能なカテーテル、ガイドワイヤ、ワイヤ及び延伸体からなる群から選択される請求項1に記載の装置。
- 前記少なくとも一つの支持部材は生体内の所定位置へ送出されることが可能であるとともに制御可能な解放機構を使用して送出デバイスから制御可能に解放されることが可能であり、解放機構は機械式、電解質、電気機械式、熱、水圧及び形状記憶による解放機構からなる群から選択される請求項20に記載の装置。
- 前記支持部材は少なくとも一つの取付デバイスをさらに有する請求項1に記載の装置。
- 前記少なくとも一つの取付デバイスはあご、フック、針、スパー及び接着領域からなる群から選択される請求項22に記載の装置。
- 前記支持部材は少なくとも一つの生体適合性の材料から製造され、それらの材料が白金、金、タンタル、チタン、ステンレス鋼、タングステン、ニチノール、形状記憶合金、ポリウレタン、ポリテトラフルオロエチレン、ポリビニルアルコール、ポリエステル、シリコーン又はアクリルからなる群から選択される請求項1に記載の装置。
- 前記支持部材は放射線不透性材料を有する請求項1に記載の装置。
- 前記支持部材は音響発生材料を有する請求項1に記載の装置。
- 前記少なくとも一つの支持部材は動脈瘤内頚部ブリッジデバイスを有し、そのデバイスは少なくとも二つの係合部材と連通するブリッジ本体部材を有し、前記少なくとも二つの係合部材は協働して継ぎ手を形成している請求項1に記載の装置。
- 前記動脈瘤内頚部ブリッジデバイスは血管動脈瘤内への挿入が可能である請求項27に記載の装置。
- 前記動脈瘤内頚部ブリッジデバイスは細長い送出デバイスに取り付けることが可能であるとともにそこから制御可能に取り外すことが可能であり、送出デバイスはガイドワイヤ、管又はワイヤからなる群から選択される請求項27に記載の装置。
- 血管動脈瘤を治療する装置であって、
複数の連結支持部材によって形成された半径方向及び軸方向の可撓性を有するパッチ本体部材を有する血管パッチデバイスと、
前記支持部材に選択的に適用された反応性材料と、
を有し、前記支持部材は血管組織を支持することが可能であり、前記支持部材は少なくとも第1表面を有し、
前記パッチ本体部材はその中に形成された複数の開窓を有するとともに第1曲率半径R及び第2曲率半径R’を有し、R’はRより大きくなっており、
前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるようになっている装置。 - 血管動脈瘤を治療する装置であって、
少なくとも一つの支持部材によって形成された半径方向及び軸方向の弾性を有する正弦本体部材を有するコイル状ブリッジデバイスと、
前記少なくとも一つの支持部材に選択的に適用された反応性材料と、
を有し、前記少なくとも一つの支持部材は血管組織を支持することが可能であるとともに少なくとも第1表面を有し、
前記正弦本体部材は複数の開口部を限定するとともに第1曲率半径R及び第2曲率半径R’を有し、R’はRより大きくなっており、
前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。 - 血管動脈瘤を治療する装置であって、
第1端部及び第2端部の間に位置する半径方向及び軸方向の可撓性を有する円筒形本体部材を有する螺旋状ステントと、
反応性材料と、
を有し、前記円筒形本体部材は前記第1及び第2端部と連通する内腔をさらに限定しており、
前記円筒形本体部材は少なくとも一つの支持部材によって形成され、前記少なくとも一つの支持部材は血管組織を支持することが可能であり、前記少なくとも一つの支持部材は少なくとも第1表面を有し、
前記円筒形本体部材は第1直径D及び第2直径D’を有し、 D’はDよりも大きくなっており、
反応性材料は前記少なくとも一つの支持部材に選択的に適用されており、前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。 - 血管動脈瘤を治療する装置であって、
第1及び第2端部の間に位置する半径方向及び軸方向の円筒形本体部材を有する網状拡張ステントと、
反応性材料と、
を有し、前記円筒形本体部材は前記第1及び第2端部と連通する内腔を限定しており、
前記円筒形本体部材は少なくとも一つの支持部材によって形成され、前記少なくとも一つの支持部材は血管組織を支持することが可能であり、前記少なくとも一つの支持部材は少なくとも第1表面を有し、
前記円筒形本体部材は第1直径D及び第2直径D’を有し、D’はDよりも大きくなっており、
反応性材料は前記少なくとも一つの支持部材に選択的に適用されており、前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。 - 血管動脈瘤を治療する装置であって、
第1端部、第2端部及び第3端部の間に位置する半径方向及び軸方向の可撓性を有する分岐本体部材を有する分岐血管支持デバイスと、
前記分岐本体部材内に位置するとともに前記第1、第2及び第3端部と連通している内腔と、
反応性材料と、
を有し、前記分岐本体部材は血管組織を支持することのできる少なくとも一つの支持部材を有し、前記少なくとも一つの支持部材は少なくとも第1表面を有し、
前記分岐本体部材は第1直径D及び第2直径D’を有し、D’はDよりも大きくなっており、
反応性材料は前記少なくとも一つの支持部材に選択的に適用されており、前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。 - 血管動脈瘤を治療する装置であって、
動脈瘤内へ送出されることのできる動脈瘤内ブリッジデバイスと、
少なくとも二つの係合部材と、
前記少なくとも二つの係合部材の協働によって形成された継ぎ手と、
反応性材料と、
を有し、前記ブリッジデバイスは可撓性のブリッジ本体部材を有し、
少なくとも二つの係合部材は前記ブリッジ本体部材と連通しており、前記少なくとも二つの係合部材は前記動脈瘤の少なくとも一つの壁に係合可能であり、
反応性材料は前記少なくとも二つの係合部材に選択的に適用されており、前記反応性材料は未反応状態及び反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。 - 前記動脈瘤内頚部ブリッジデバイスは細長い送出デバイスに取り付け可能であるとともにそれから制御可能に取り外すことが可能であり、送出デバイスはガイドワイヤ、管又はワイヤからなる群から選択される請求項35に記載の装置。
- 血管動脈瘤を治療する方法であって、
少なくとも一つの支持部材に選択的に適用された反応性材料を有するデバイスを提供する段階と、
デバイスを血管内から血管動脈瘤へ送出する段階と、
前記デバイスによって前記動脈瘤の近傍の組織を支持する段階と、
血液が前記血管を通って流れることを可能にする段階と、
前記動脈瘤への血流を制限するために、前記デバイスに配置された前記反応性材料を活性化する段階と、
を有する方法。 - 前記反応性材料を活性化する前記段階は、約7.4の生理的pHの存在下で起こる請求項37に記載の方法。
- 前記反応性材料を活性化する前記段階は、前記反応材料の体積を拡張させる段階をさらに有する請求項37に記載の方法。
- デバイスを送出する前記段階は、前記デバイスを前記動脈瘤へ送出するためにカテーテルを使用する段階をさらに有する請求項37に記載の方法。
- 前記デバイスを前記動脈瘤へ送出するためにカテーテルを使用する前記段階は、前記デバイスを前記動脈瘤へ送出するためにバルーンカテーテルを使用する段階をさらに有する請求項37に記載の方法。
- 前記デバイスを前記動脈瘤へ挿入する段階をさらに有する請求項37に記載の方法。
- 血管動脈瘤を治療する装置であって、
少なくとも一つの支持部材と、
反応性材料ストランドと、
を有し、反応性材料ストランドは前記少なくとも一つの支持部材に織り込まれることによって織り込み構造を形成しており、前記反応性材料は反応状態及び未反応状態を有し、前記反応状態は前記動脈瘤への血流を制限することができるような状態である装置。
