Abstract
【解決手段】ALIF法と関連して使用される固定装置は、隣接する椎骨の脊柱突起内に押し込まれる、その対面する面における一体的なスパイクを有する1対隔たった板を有する。板の1つは、ポストの球状ヘッド端部を拘束する球状受け口を有し、該ポストの他端は、他方の板の開口内に受け入れられる。その受け口の取り付け部は、ポストが制限された程度、少なくとも2°の自由度にて枢動することを可能にし、厚さ及び向きの異なる脊柱突起を隣接する椎骨の上に受け入れることを可能にするような2つの板の間の角度形成となるように配置されている。ポストを第二の板内に受け入れることは、圧縮器具によって組立体を脊柱突起の上に効果的に取り付け且つ、圧縮器具器を除去した後、その間隔を恒久的に且つ、確実に維持し得るよう板の間の間隔を調節することを可能にする。ポスト及び板の受け入れ開口の断面形態は、板がポストの軸線の周りでポストに対して回転するのを阻止し、また、開口が設けられた板内の止めねじは、ポストの平坦面と係合して板の間の空間を調節された通りに固定する。
Description
Claims (34)
- 脊柱突起を固定する植え込み型装置において、
第一及び第二の隔たった板であって、該第一の板が該第二の板の面に対面する面を有する前記第一及び第二の板と、
前記板の各々に接続され且つ、前記第一の板の前記対面する面から前記第二の板の前記対面する面まで伸びるポストとを備え、
前記第一の板に対する前記ポストの取り付け部が、枢動可能であり、前記第二の板に対する前記第一の板の姿勢を変化させることができ、
前記第二の板に対する前記ポストの接続部が、前記第一の板と前記第二の板との間の間隔を変化させ得るように調節可能である、脊柱突起を固定する植え込み型装置。 - 請求項1の装置において、
前記第二の板が開口を有し、
前記ポストの一部分が該開口内に摺動可能に受け入れられ、
止めねじが、前記第二の板内に受け入れられ且つ、前記ポストと係合してポストを前記第二の板に対して固定する、装置。 - 請求項2の装置において、
前記ポスト部分及び前記開口が、前記第二の板が前記ポストに対して回転するのを阻止する相互嵌合面を有する、装置。 - 請求項3の装置において、
相互嵌合面が、前記ポストにおける平坦面及び前記開口内の平坦面である、装置。 - 請求項1の装置において、
前記第二の板が開口を有し、
前記ポストが該開口内に摺動可能に受け入れられた一部分を有し、
前記摺動可能に受け入れられたポスト部分が、前記開口内に非回転可能に受け入れられて、前記第二の板が前記ポストに対して回転するのを阻止する、装置。 - 請求項5の装置において、
前記摺動可能に受け入れられたポスト部分が細長であり、また、該ポスト部分が、円筒状の形状にて長手方向に伸びる中断部分を有する全体として円筒状の外面を備え、
前記開口が、全体として円筒状の穴を備え、該円筒状の穴が、ポスト部分の中断部分に嵌まり且つ、第二の板がポスト上で回転するのを阻止する軸方向に伸びる中断部分を有する、装置。 - 請求項5の装置において、
前記ポスト部分が細長であり、また、該ポスト部分は、全体として円形の中実な断面形状であるが、円の1つのセグメントを欠く形状であり、
回転を阻止する手段であって、円のセグメントを欠く箇所における前記ポスト部分上にて長手方向に伸び且つ、前記開口の平坦面に嵌まり可能に係合する平坦面を有する、前記回転を阻止する手段を備える、装置。 - 請求項6の装置において、
前記第二の板に設けられて、前記ポストを前記第二の板に対して係止し、前記板の間の選んだ間隔を固定する手段を更に備える、装置。 - 請求項8の装置において、前記係止手段が、
前記第二の板内にねじ込まれた止めねじであって、前記ポスト部分が前記第二の板内で摺動するのを阻止するのに十分緊密に前記ポスト部分の平坦面と係合可能な前記止めねじを有する、装置。 - 請求項1の装置において、
前記対面する面が、前記板の間の空間内で脊柱突起に侵入し、前記板の間にて脊柱突起に対する固定を実現する、前記面から突き出す列状スパイクを有する、装置。 - 請求項10の装置において、
前記対面する面が平坦である、装置。 - 請求項1の装置において、
第一の板が中心を有する受け口を備え、
前記ポストが、長手方向軸線と、前記受け口内に受け入れられ且つ、該受け口内で枢動可能なヘッド部とを有し、これにより前記ポストが互いに垂直な2つの平面内で前記第一の板の前記対面する面に対して枢動可能であり、
前記ポストが前記ヘッド部に溝を有し、
前記第一の板が、前記受け口内に突き出し且つ、前記溝内に受け入れられて、前記ポストの軸線に対し垂直な平面内で前記第一の板が前記ポストに対して回転するのを阻止する、装置。 - 請求項12の装置において、
前記ポストが、一端における前記ヘッド部と、該ヘッド部から前記ポストの他端に向けて伸びる案内面とを有し、
前記第二の板が、開口を有し、該開口を通じて前記ポストを受け入れ、
該開口が、前記ポストにおける案内面と係合してポストの軸線に対し垂直な平面内で前記第二の板が前記ポストに対して回転するのを阻止する案内従動面を有する、装置。 - 請求項13の装置において、
案内面が平坦であり、
前記第二の板の開口が、前記第二の板内に締め付けられた止めねじであって、前記平坦な案内面と係合する一端を有して、前記第二の板が前記ポストに沿って平行移動するのを阻止し且つ、前記第二の板が前記ポストに対して回転するのを阻止する前記止めねじにより遮られるようにした装置。 - 請求項13の装置において、
前記第二の板が、細長であり、また、該第二の板は、前記2つの平面の第一の平面内に位置する長手方向軸線を有し、前記第二の板は、前記2つの平面の第二の平面内に位置する横断軸線を有し、
前記第二の平面が前記第一の平面に対して垂直であり、
前記第一の板が細長であり、また、該第一の板は、前記第一の平面内に位置し且つ該第一の平面内で枢動可能な長手方向軸線を有し、前記第一の板は、前記第二の平面内に位置し且つ、該第二の平面内で枢動可能な横断軸線を有する、装置。 - 請求項15の装置において、
前記受け口内にある止めリングであって、前記第一の板が前記第一の平面及び前記第二の平面内で枢動したとき、前記ポストと係合して前記第一及び第二の平面内で前記第一の板の前記軸線が角度形成する最大の程度を受け口の中心から25°以下に制限する寸法及び配置とされた前記止めリングを更に備える、装置。 - 請求項16の装置において、
前記第一の板及び前記第二の板が、前記ポストが前記板の間の空間を跨ぐように全体として平行な隔たった関係に配列され、
前記第一の板が前記第二の板の面に対面する面を有し、
前記対面する面が、前記板の間の空間内で脊柱突起に侵入し得るよう前記面から突き出す列状スパイクを有する、装置。 - 請求項17の装置において、
前記対面する面が平坦である、装置。 - 請求項18の装置において、
前記板がバナナ形状の輪郭外形を有する、装置。 - 請求項19の装置において、
前記第二の板の横断軸線が、前記第二の板から遠方の空間内の中心の周りで同心状の円内にある前記板の半面凹半面凸の中心端縁の半径と共直線状であり、これにより、前記端縁がその前記対面する面の平面内で前記第二の板の半面凹半面凸の形状を画成するようにした、装置。 - 請求項20の装置において、
前記中心に向けて前記凹状端縁から突き出す止めねじを更に備える、装置。 - 請求項21の装置において、
前記止めねじが、工具受け入れヘッド部を有する、装置。 - 請求項22の装置において、
前記止めねじが、前記係合端部にて開始し且つ、前記凹状端縁から中心に向けて離れる方向を向く第一の工具受け入れ開口部を有する第一のねじ付き部分を備え、
前記止めねじのヘッド部が、前記凹状端縁から中心に向けて離れる方向を向く第二の工具受け入れ開口部を有し、
前記止めねじが、治療箇所に取り付けた後、ヘッド部をねじ付き部分から破断させ易くする減少した厚さの一部分を有する、装置。 - 請求項1の装置において、
前記第一の板が受け口を有し、
該受け口が、前記第一の板の前記対面する面にて開放し、
前記ポストが、前記受け口内に受け入れられたヘッド部を有し、前記ポストが、前記第一の板の前記対面する面外に突き出し且つ、前記受け口内で枢動可能であり、これにより、前記ポストが前記第一の板に対して2つの平面内で枢動可能であり、
前記第一の板が、受け口へのアクセス開口部を有する背面を備え、
前記ポストのヘッド部の一部分が前記開口部内に露呈される、装置。 - 請求項24の装置において、
前記受け口内にあり、前記ヘッド部を該受け口内で止めるリテーナを更に備える、装置。 - 被験者の脊柱突起を固定する方法において、
第一及び第二の板を備え、ポストが第一の平面内で受け口内に枢動可能に取り付けられた一端と、第二の板内に摺動可能に受け入れられた他端とを有するインプラント組立体となるように構成要素を組み立てるステップと、
被験者に切開部を形成するステップと、
組立体を脊柱突起に隣接する位置まで挿入するステップと、
第二の板をポストの上で摺動させ、これにより、第一の板と第二の板との間に脊柱突起を受け入れる空間を第一の板と第二の板との間に提供するステップと、
板を脊柱突起に圧縮するステップと、
圧縮された板を脊柱突起に対して保持し且つ、前記第二の板を止めねじにてポスト上に締め止めするステップとを備える、被験者の脊柱突起を固定する方法。 - 請求項26の方法において、
前記板を前記脊柱突起上で圧縮する間、少なくとも1つの平面内で第一の板を第二の板に対して角度形成するステップを更に備える、方法。 - 請求項27の方法において、
前記板を前記脊柱突起の上に圧縮する間、2つの平面内で第一の板を第二の板に対して角度形成するステップを更に備える、方法。 - 請求項28の方法において、
板を脊柱突起に対して圧縮する間、前記板におけるスパイクを脊柱突起内に押し込むステップを更に備える、装置。 - 請求項28の方法において、
第二の板に対する第一の板の角度形成が冠状で且つ軸方向平面内にて行われる、方法。 - 請求項26の方法において、組立体を脊柱突起に隣接する位置に挿入する前に、
第二の板を第一の板に対して閉じるステップと、
第二の板を止めねじにて閉じた状態に係止し、該止めねじが第二の板をポストに締め止めするようにするステップと、
被験者の腰に切開部を形成するステップと、
次に、組立体を脊柱突起に隣接する位置に挿入するステップと、
スクリュードライバを挿入し且つ、止めねじを非係止状態にするステップと、
第二の板をポストの上で摺動させ、これにより前記板の間の間隔を増大させるステップと、
固定すべき脊柱突起に隣接する所望の位置まで動かすステップと、
次に、板を脊柱突起に対して圧縮するステップとを備える、方法。 - 請求項31の方法において、
前記板の対面する面のスパイクを前記脊柱突起と係合させるステップと、
板を脊柱突起に圧縮する間、スパイクを係合した脊柱突起内に押し込むステップとを更に備える、方法。 - 請求項32の方法において、
スパイクを係合した脊柱突起内に圧縮する間、スクリュードライバを使用して第二の板をポスト上に係止するステップを更に備える、方法。 - 請求項33の方法において、
ヘッド部を止めねじから破断させるステップと、
切開部を閉じるステップとを更に備える、方法。
