Abstract

ステム(10)と、アーチ形の上面(21)と閉ざされた底面(22)を有するボール状のヘッド(20)をもつ股関節あるいは肩関節用の人工関節。ステム(10)の縦軸はボール状ヘッド(20)の軸と３０°以下の角度をなす。ステム(10)は次第に細くなり、端から端までの絞り角度が５〜３０°の範囲である。ボール状ヘッド(20)は半球より小さい、球の一部状の形状をとる。ステム(10)の長さはボール状ヘッド(20)の直径より短い。前記ステム(10)の縦軸が、前記ボール状ヘッド(20)の軸と一致していてもよい。

Description

本発明は、ステムおよび、該ステムと一体化した、あるいは該ステムに取付可能なボール状のヘッドを有する人工股関節及び人工肩関節に関する。前記ボール状ヘッドは、アーチ形の上面と閉じられた底面を有する。ステムおよびボール状ヘッドはそれぞれ縦軸を有する。ボール状ヘッドの軸はボールの中心を通り抜け、ボールの底部にほぼ垂直である。
従来の人工関節部品は、ヘッドの軸に対し特有の角度（例えば１３５度）で回転するステムによって髄内に固着される。前記ステムは１０〜２０ｃｍの長さであり、骨の髄腔の中に挿入される。人工関節のデザインにより、および、セメントを使用せず移植するかセメントを使用して移植するかにより、固着のメカニズムにおける問題点が生じる。もしこの人工関節が末端で髄内に支持されていれば、自然力の伝達は、ボール状部位の近接する皮質から髄内のステム領域へ逸れていく。結果として、骨が再編成する、すなわち骨が遠位の人工関節の端部周辺に成長し、ボール状ヘッドと骨頭（近位の骨）の間の境界領域の中で骨が変性する。これはよく緩みの原因として挙げられる。人工関節が遠位で支持されている場合は突然大きな負荷がかかることによって、人工関節が近位で支持されているときは遠位の人工関節領域における骨の劣化によって、遠位の人工関節末端領域で骨幹ステムが破砕するおそれもある。
解剖学的な開始状況の再現は、ステムに対するボール状ヘッドの傾斜と逆さねじれに多くのバリエーションがあるため、難しい。移植の際、人工関節のヘッドは不可避的に骨幹に揃えられるが、人工関節が傾斜および逆ねじれの状態で固定された場合、ボール状ヘッドは高価な器具の使用にもかかわらず、たいてい切除面に平らに置けない。その結果生じる不均一な力の伝達により、骨の再編成が起こり、最終的には緩みが生じる。より新しい世代の人工関節は（特に肩の義肢学において）、高いテクニカルコストが生じるものの、指定されたパラメータの設定が可能である。
この問題を解決するために、ステムを非常に短く構築し、ステムを骨の髄腔の中に挿入せず、短いステムをボール状ヘッドの軸と平行に揃えることが既に知られている。ドイツ特許公報第198 34 277号に記載された大腿骨頚部の人工関節の場合、切除面への支持と平行して、外皮質の開口部を経て、軸方向のガイドと固定がさらに行われている。固着は、ステムの両端を固定することによって行われる。
米国特許公報第4,042,980号は、短いステムを持つ上腕骨の部品であって、ボール状ヘッドが中空であっても閉ざされていてもよい部品を開示している。前記ステムは円形のリブを持つ。
ドイツ特許公開公報第37 07 518号は、切除面からステムの方向に異なった屈曲で走る複数の固着柱を有する人工関節について開示している。
ドイツ特許公開公報第 196 13 078号は、海綿質が成長できる隙間の中にケージ構造を設ける固着技術について開示している。この構造あるいは遠心に伸びたステムは同じようにボール体の軸を根幹としてステムの軸の方へ回転し、骨幹のステム領域の中へ広がる。
ドイツ特許公開公報第42 20 217号は、関節面（特に軟骨層）と置換するための人工関節について開示している。これによれば、ボール体は中空であり滑らかな表面で閉じられていない。その内側はくり抜かれキャップ状の形状をしている。このようなボール状人工関節では、骨溶解のリスクがある。
ESKA Implants GmbH & Co.によって市販されているRozingタイプの肩用人工関節も既知である。これはモジュール式に構成されており、その最も短い伸展ステージが基部の固着のために設計されている。しかしながら、より長い伸展ステージにおいて髄内の固着を可能にするために、ステムの軸はボール体の軸に対し固定された傾斜角で回転する。これは、従来の長いステムを持つ人工関節の場合と同様に、上述の固着における問題点を伴う。
本発明の課題は、確実に位置を合わせることができ、簡単な方法で恒久的に固定できる、股関節あるいは肩関節用の人工関節を提供することである。
上記課題は、ステムの縦軸がボール状ヘッドの軸に対して３０°以下の角度に形成され、ステムの円錐角が５°〜３０°であり、半球より小さい球形状をしたボール状ヘッドを有し、ステムの長さが前記ボール状ヘッドの直径より短く形成された本発明によって解決された。
前記ステムの円錐角は好ましくは約２０°である。
本発明にかかる短いステムの人工関節は、骨頭内に確実に固着され、ステムの軸は、ボールの中心を通り、その軸に対し３０°以下、好ましくは２０°以下で回転する。特に好ましい例では、ステムの軸とボール状ヘッドの軸は一致する。結果として、固着における問題点（余剰な固着）が回避され、切除面の皮質端部を経る伝達が促進され、さらに海綿質への圧入によって確実にされる。側面の穿孔を避けるために、ステムは非常に短く、好ましくはボール状ヘッドの直径より短く形成される。
ボール状ヘッドとステムは、一部品として構築されても二部品として構築されてもよい。二部品構成とした場合、ステムとボール状ヘッドは、互いに接続可能な別部品となる。接続は、ポジティブ・ロッキング（例えば、爪部品による）、あるいはポジティブ・フォース・ロッキング（例えば、ねじ接続）によって行うことができる。ステムとボール状ヘッドは、好ましくは、ステムの円錐状付属部品と、これに対応するボール状ヘッドの円錐状凹部によって接続される。この配置は逆にされても良く、円錐状の付属部品をボール状ヘッドに設け、ステムに円錐状の凹部を設けてもよい。ボール状ヘッドに面するステムの端部は、円盤形状に広がり切除面と接する支持面を有することが好ましい。
ボール状ヘッドあるいは支持面の底面は閉じられており、平らであってもよく、僅かに凸状であってもよく、僅かに凹状であってもよい。特に、底面が凸状に形成された場合は、関節から生じ皮質および海綿質に伝わる負荷は非常に均一に分配される。
本発明にかかる人工関節を用いることによって、力の伝達が、解剖学的な開始状況にほぼ対応して起こり、その結果として、回転運動の中心の最適な再現が達成できる。一方、従来のロングステム方式の場合は、髄内に固定されたステムがボール状ヘッドの回転運動の中心となる特定のエリアを事前にセットするため、しばしばボール状ヘッドの位置の解剖学的異常により回転運動の中心がマッチせず、力の伝達が解剖学的な開始状況から逸脱する結果となる。非常に高価な、設定可能な人工関節を用いても、これは回避できず複雑な手術テクニックを用いることが必要である。
ボール状ヘッドあるいは支持ディスクの底面から始まり、ステムはステム先端の方向に向けて（好ましくは円錐形にあるいは放射状に）次第に細くなる。ボール状ヘッドあるいは支持ディスクの底面に直接隣接する部分は、円筒形に形成されてもよい。
回転に伴う抵抗のため、ステムは、一般にステムの縦方向に添って放射状に伸びるリブを一以上有することが好ましい。回転に伴う抵抗に対し、ステムは滑らかな外面を持つ角柱として形成されても良い。
本発明によって達成可能な利点は、特に、関節を起点とする負荷が皮質および海綿質に均一に伝達され、その結果、骨の劣化あるいは再編成が起こらないということである。３０°以下、好ましくは約２０°の角度を持つステムの円錐性により、当該分野において「圧入”press fit”」と呼ばれる固定によって、海綿質内でステムが安定に固定される。
ステムの軸は、ボールの中央を通りボールの閉じられた表面あるいは底面を垂直に貫く軸と、同一軸上にあること好ましい。支持および力の伝達は、それゆえ切除面の皮質リングによって及び海綿質内へ又は接合中にセメントプラグ内へ圧入されることによっても、達成される。ヘッドは、ボールの閉じられた表面あるいはステムの支持面により、切除やステムが収まる開口部が不正確であっても、また、逆ねじれおよび傾斜の変動があっても、切除面に平らに置かれ、そのため、術前の元の状態に近い均一な力の伝達が確保される。従って、特に、内側をくり抜いたキャップ形状のボールを持つ表面置換人工関節と比較して、骨溶解のリスクが減る。
本発明にかかる人工関節を肩に移植する場合、上腕骨頭を完全に切除する。その結果、関節窩を供給するあるいは移植するために必要な空き領域ができる。
ステムを通す通路を開ける際、的確な器具を使用することによって、ボール状ヘッドをステムの背側正中に復元することができる。
骨組織の増殖を可能にするため、１以上の凹部がステムに何らかの配置（例えば、軸方向に、放射状に、あるいは対角線上に走る研削、孔あるいはスロット）で設けられてもよい。ステム軸の外周リング上のピンは、固着と剪断力の吸収および回転抵抗の一因となる。ボール状ヘッドの底面あるいは支持面は、好ましくは溝が設けられるかあるいは被覆された多孔質体として構成される。
ステム部分はチタンあるいはコバルト-クロム合金で形成することができ、関節頭部はコバルト-クロム合金あるいはセラミックで形成することができる。ステム部分及び／または関節頭部は、非-強化プラスチックあるいは繊維強化プラスチックで形成してもよい。
関節を人工関節でケアする場合、カップ状の人工関節が、弱くなった関節頭部と表面置換するために用いられている。ボール状ヘッドは現在も一般的に比較的よく用いられており、人工関節は、おおむね関節頭部にフィットする半球状のキャップ形状をしている。一方、関節頭部が大きく破損している場合は、長いステムを持つ人工関節が通常使用される。本発明にかかる人工関節は、多くの場合、そのような長いステムの人工関節とも、上述したキャップ形状のボールを持つ短いステムの人工関節とも代替可能であり、これまで肩の義肢学においてこのような一貫性をもって考えられていなかった概念を説明する。関節骨折の場合のみ、長いステムをもつ人工関節の使用が避けられないことがある。
本発明の実施例を、以下に図面を用いて説明する。
図１〜３は、ステム１０およびボール状ヘッド２０を有する一体型人工関節を示す。
ステム１０には、リブ１２が設けられ、各リブ間には丸い溝１１が設けられている（合計４つ）。リブ１２の外側エッジはわずかに凹面をなしている。リブ１２は下方へ行くにつれて次第に細くなり、先端部１３は丸くなっている。各リブ１２の外側エッジはステム１０の縦軸に対し約１０°の角度をなして伸びている。一方、溝１１の表面は、軸方向に揃っている。
ボール状ヘッド２０は、アーチ形の上面２１と平坦な底面２２を有し、全体としては半球より小さい球状の形をしている。中心の角度αは１７０°より小さい。
一例を挙げると、ボール状ヘッド２０の横径が４３ｍｍであり、一方ステム１０の長さはわずかに２５ｍｍである。ステム１０の縦溝によって、人工関節は回転抵抗を受ける。リブ１２が滑らかな先端１３にかけて次第に細くなっているため、海綿質内へ、いわば圧入される。ボール状ヘッドの底面２２は、切除面上に平らに置かれる。
同様に、図４〜６も一体型の人工関節の一実施例を示す。ステム１０の表面は円錐状であり滑らかである。先端部１３も同様に丸みを帯びている。ボール状ヘッド２０も同様に球の一部状の形状を有する。剪断力および回転に対し人工関節を固定する４本のピン２３が、底面２２から下方へ突出している。ピン２３は、約５〜２０ｍｍの長さであり、直径が約４〜１０ｍｍであり、表面に溝あるいはねじ山及び／又はコーティングが施され、これによりピンを海綿質の対応する孔によりよく固定することが可能になる。
図７〜１０は分離型人工関節の一実施例を示し、ステム１０およびボール状ヘッド２０は分離するように形成され、ステム１０の上端に設けられた円錐１４とボール状ヘッド２０の底面に設けられた対応する円錐状の凹部２４によって結合させることができる。ステム１０は、凹形の表面を持ち、滑らかな先端１３にかけて次第に先が細くなる軸ピン１５を有する。縦軸と交差する支持ディスク１６は、円錐１４およびピン１５の間に設けられている。４つのリブ１２は、その外側エッジが約２０〜７０°、好ましくは約６０°の角度で交わり、支持ディスク１６とピン１５の間で、規則的な配置で外周に設けられている。
図１１〜１３は、分離型人工関節の別の実施例を示す。ステム１０には、図１〜３の実施例と同様に丸溝が設けられている。ステム１０とボール状ヘッドの接合は、図７〜１０の実施例と同様、ステム１０の上端の円錐１４とボール状ヘッドの対応する円錐状の凹部によってなされる。支持ディスク１６は、本実施例でもステム１０と円錐１４の間に設けられている。
図１５〜１８は、図１１〜１４と同様の分離型人工関節のステムを示す。ステム１０には円形あるいは長円形の切断面を持つ孔１８が設けられている。孔１８は、骨物質が中に増殖してくるのを可能にする。
図１９および図２０は、ボール状ヘッドとショートステム１０をもつ人工関節の断面図を示すものであり、ボール状ヘッド２０の底面２２は、下方へ向けて凸状をなしておりフラットな円錐状になっているか（図１９）あるいは下方あるいは上方に向けてアーチ形をなしている（図２０）。ボール状ヘッド２０はそれゆえ、全体としてはレンズ状の形状をなしている。底面２２の凸状の形により、結果的に、関節から皮質および海綿質に移行する力の伝達が非常に均質になる。
図２１および２２は、図１〜３と同様の一体型人工関節であるが、ステム１０がボール状ヘッド２０の軸に対し約１０°傾いている人工関節を示す。図７〜９に示した実施例と同様、ステムには放射状のリブ１２が設けられている。ボール状ヘッド２０に対してある角度をなすステム１０の傾斜は、ときに切除面の向かい側の皮質にステムが接触するのを防止するのに都合がよい。
図２３は、ボール状ヘッド２０の底面がらせん状の溝２５を有する人工関節を、下側から斜めに見た図である。溝２５により、骨の増殖がより可能になる。
図２４は、移植された人工関節の断面図である。ボール状ヘッド２０の平坦な底面２２は、切除面Ｒ上に平らに乗り、ステムは海綿質Ｓの中に伸びており、その先端は皮質Ｋから適切な距離に維持されている。図２４において、ボール状ヘッド２０の底面２２および切除面の間、およびステム１０と海綿質Ｓの間の狭い領域２は、セメントプラグあるいは人工関節のコーティングでありうることが認識できる。このようなセメントプラグあるいはこのようなコーティングは、あってもよいが、全てのケースで必須ではない。

Claims (13)

  1. ステム(10)とボール状ヘッド(20)を有する股関節あるいは肩関節用の人工関節であって、ボール状ヘッド(20)はステム(10)と一体に形成されていてもよくまたはステム(10)に接続可能に形成されていてもよく、アーチ形の上面(21)と閉ざされた底面(22)を有し、ステム(10)とボール状ヘッド(20)はそれぞれ縦軸を有し、ボール状ヘッド(20)の軸はボールの中央を通り、底面(22)と垂直に交わり、さらに
    ステム(10)の縦軸はボール状ヘッド(20)の軸と３０°以下の角度をなし、
    ステム(10)は次第に細くなり、端から端までの絞り角度が５〜３０°の範囲であり、
    ボール状ヘッド(20)は半球より小さい、球の一部状の形状をとり、且つ
    ステム(10)の長さはボール状ヘッド(20)の直径より短いことを特徴とする、人工関節。

  9. 前記ステム(10)が、ボール状ヘッド(20)の底面あるいは支持ディスク(16)を基点とする円筒形状をなす、前記いずれかの請求項に記載の人工関節。
  10. 前記ステム(10)に、１以上のリブ(12)が設けられている、前記いずれかの請求項に記載の人工関節。
  11. 前記ステム(10)が、軸方向、放射状、あるいは対角線上に走る切削、穿孔あるいはスロットなどの凹部あるいは孔(18)を１以上有する、前記いずれかの請求項に記載の人工関節。
  12. 前記ボール状ヘッド(20)の底面(22)あるいは前記支持ディスク(16)の底面が平らである、前記いずれかの請求項に記載の人工関節。
  13. 前記ボール状ヘッド(20)の底面(22)あるいは前記支持ディスク(16)の底面が、平らな円錐状、アーチ状の凸面、あるいはアーチ状の凹面である、請求項１〜１２のいずれか１項に記載の人工関節。
