Abstract

Το σύστημα είναι σταθερόν και ελαστικόν, αποκαθιστά την αρχιτεκτονική και τις ενδοοστικές τάσεις σε κατάγματα μακρών οστών, και επιτυγχάνει την ουδετεροποίησιν των καταγμάτων, μέχρι του σχηματισμού πόρου. Μετά την πόρωσιν δεν είναι αναγκαία η αφαίρεσις του υλικού έστω κι αν αυτό είναι μεταλλικόν. Η σταθερότης του συστήματος εξασφαλίζεται δια του ενδομυελικού οδηγού, (σχ. 1, 1α και 1β) ο οποίος είναι οδηγός των φορέων προεντάσεως και των ενδοοστικών τάσεων προς αποκατάστασιν αυτών, (παραλαμβάνει τις τάσεις που προέρχονται από ροπές στρέψεως όπως και τις περιφερειακές τάσεις που δημιουργούνται λόγω της γωνίας του κατάγματος με τον μηχανικό άξονα του οστού, δηλαδή παρεμποδίζει την διολίσθησιν των κατεαγότων μεταξύ των. Επίσης φέρει επιμήκη σχισμήν δια την καθήλωσιν κατεαγότων ή παρασχίδιων δια κοχλιωτών ήλων, (σχ. Ι 4, και ΙV 4). Ο ενδομυελικός οδηγός επιτρέπει την εφαρμογήν αξονικής προεντάσεως ταυτιζομένης με τον μηχανικόν άξονα του ιστού. Η εφαρμοχή της προεντάσεως δημιουργεί τάσεις θλίψεως εις το οστούν τις μόνες που είναι βέβαιον ότι είναι οστεογεννετικές. Φορείς προεντάσεως είναι λεπτά μεταλλικά σύρματα ή Carbon Fibres ή Carbon fibre plastics ή άλλο απορροφήσιμο υλικό (6). Οι φορείς προεντάσεως αγκυρούνται εις τον ενδομυελικόν οδηγόν (σχ. Ι) και εντός του υπομοχλίου προστασίας του οστού (σχ. ΙΙ 6) και σταθεροποιούνται είτε δια του κώνου (σχ. ΙΙ 7) είτε δι' άλλου τρόπου. Η έδρασις της αγκυρώσεως γίνεται με την ροδέλλα κατανομής των τάσεων (σχ. The intramedullary guide allows the application of axial prestress identified with the mechanical axis of the tissue. The application of prestress creates compressive stresses in the bone that are certain to be osteogenic. Prestress carriers are thin metal wires or Carbon Fibers or Carbon fiber plastics or other absorbable material (6 ).The prestressing carriers are anchored to the intramedullary guide (fig. I) and within the asia of the bone (fig. II 6) and are stabilized either by the cone (fig. II 7) or in another way. The anchorage is seated with the tension distribution washer (fig.
