Abstract
A radiographic marker for arthroplasty and especially for hip surgery comprises a radio-opaque element 3, 4 for association with a non-radio-opaque joint member, the element being shaped to define at least two planes, so as to appear on a radiograph and indicate both the rate of wear of the join member and tilting of the joint member. <IMAGE>
Description
SPECIFICATION
Improvements relating to hip surgery
This invention relates to arthoplasty, and especially to hip surgery and particularly to the use of marking means to appear on X-ray plates relating to a joint replacement.
It is now well known in hip surgery to use a plastics material acetabular cup which is cemented into position in the pelvic bone structure. For example, the cup is made of high density polyethylene which is non-radio-opaque and thus a marker is required to appear on X-ray plates to indicate the thickness of the plastics material and thus the distance between the cup and the femoral head and for indicating the rate of any wear.
The main object of the present invention is to provide a marker for the following two main purposes:
(1) To function as a marker capable of assessing the rate of wear of plastics material cup.
(2) To measure the version of the cup indicating whether it is tilted forwardly (anteverted) or backwardly (retroverted).
The ability to assess these parameters on an X-ray plate is essential in the study of position, wear, stability, and any progressive tilting of the acetabular cup. In this way a two-dimensional image of the cup is obtained.
According to the present invention a radiographic marker for arthroplasty, comprises a radio-opaque element for association with a non-radio-opaque joint member, the element being shaped to define at least two planers.
Moreover there is also provided an acetabular prosthesis comprising a non-radio-opaque acetabular cup for receiving a femoral head and a radioopaque marker in accordance with the invention, the marker and cup being fixed relative to each other.
The marker is preferably spring steel wire or strip.
The marker can be held in position by location means. The marker can be located in grooves externally of the cup and its tail ends turned inwardly, to fit in holes in the acetabular cup.
The invention also provides a method of providing a marking on an X-ray photographic plate for use in relation to hip surgery comprising a radio-opaque marker in a fixed position relative to a non-radioopaque acetabular cup, so that the marker defines at least two planes to facilitate correct positioning and observation of the cup with regard to the angle and wear.
The marker is held in a fixed position relative to the joint member and the radio-opaque marker defines two planes which will allow the angular position of the joint to be found out. The relationship between the two planes defined by the marker is different for each angular position of the joint.
Two examples of the invention will now be more particularly described with reference to the accompanying drawings in which:

In the particular illustrated embodiment of this
invention, a marker 1 for association with an aceta
bular cup 2 is made of spring steel wire or strip which is shaped to form a bow 3 in the central region of the wire and then the end parts 4 are bent laterally towards each end into a plane at right angles to the plane of the bow and such end parts 4 are curved in their lengths to form two semi-circles of a two-part bow. The ends of said two parts finish in close proximity to one another and the tail end extremities 5 of such end parts are bent inwardly.
A radiographic marking element 1 formed in the above manner is located on the external periphery of an acetabular cup 2 in grooves 6, 7 produced in such peripheral surface. Conveniently the cup is provided with a pair of arched grooves 6 which cross one another at right angles in the surface of the exterior of the cup and these two grooves are linked together by annular grooves 7 of which the largest diameter of groove 7 is close to the gapped perimeter 8 of the cup.
A cup and marker formed in the above manner are associated by the central bow 3 of the marker 1 being inserted into one of the pair of grooves 6 to pass over the summit of the cup and the ends 4 forming the two-part bow of the marker are located in the aforesaid annular groove 7 near the perimeter 8 of the cup. The aforesaid tail ends 5 of the marker can pass into a hole at the junction of the grooves 6, 7 to assist in retaining the marker in position.
In use the marker will show clearly on an X-ray plate and the anterior bow of the marker will run transversely across the path of the X-rays, its movement being further magnified on the X-ray plate. This face ensures that the marker is extremely sensitive to the tilt cup and gives a two dimensional image of the acetabular cup. On the X-ray plate the anteversion of the cup approximates the two bows of the marker and retroversion separates them.
It is further considered that the two bows 3 and 4 being made of a single wire give a better X-ray appearance and addition of a single one, although effective, is less asthetic in appearance. Although the anterior bow may be placed posteriorly to reduce its sensitivity it is considered that this is undesirable and that it cannot be seen during the insertion of the cup.
A second example of a marker is shown in Figure 5. In this case only one of the ends 6 of the central bow 3 is bent round to form the bow at right angles to the central bow 3. In this case there are no inturned ends 5. This is a simpler arrangement than the first example.
Claims (17)
1. A radiographic marker for arthoplasty comprising a radio-opaque element for association with a non-radio-opaque joint member, the element being shaped to define at least two planes.
2. A marker according to claim 1, in which the element is a thin elongate member bent into a shape defining at least two planes.
3. A marker according to claim 2, in which the member is a wire or strip of sprung steel.
4. A marker according to any one of the preceding claims, in which is held in a fixed position relative to an acetabular cup by location means.
5. A marker according to claim 4 when dependant on claims 2 or 3, in which the element is located within grooves lying on the external surface of the acetabular cup.
6. A marker according to claim 5, in which at least one end of the element is turned inwardly to fit within a hole in the acetabular cup to retain the marker in position.
7. A marker according to claims 5 or 6, in which the element includes two semi-circular bows at right angles to each other with a common diameter, the marker being located on the acetabular cup with one semi-circular bow over the summit of the cup, and the other semi-circular bow extending around half of the circumference of the cup.
8. A method of providing a marking on an X-ray photographic plate for use in relation to hip surgery comprising locating a radio-opaque marker in a fixed position relative to a non-radio-opaque acetabular cup, so that the marker defines at least two planes to facilitate correct positioning and observation of the cup with regard to the angle and wear of the cup.
9. A method according to claim 8, in which the marker is in accordance with any one of claims 1 to 7.
10. A method according to claim 8 or 9, in which the acetabular cup is made of plastics.
11. An acetabular prosthesis comprising a nonradio-opaque acetabular cup for receiving a femoral head and a radio-opaque marker in accordance with any one of claims 1 to 7 the marker and cup being fixed relative to each other.
12. An acetabular prosthesis according to claim 10, in which the acetabular cup is made of plastics.
13. A radiographic marker for use in arthroplasty arranged substantially as herein described with reference to Figures 1 to 4 or 5 of the drawings.
14. A radiographic marker according to claim 1 arranged substantially as herein described.
15. A method in accordance with claim 8 substantially as herein described.
16. An acetabular prosthesis in accordance with claim 11 and arranged substantially as herein described.
