A fracture in a femur is secured with an appliance comprising a fluted nail 1 integral with a plate 2, and a rod 5 which can be inserted through the plate and secured so that it engages the nail. The rod is screw-threaded into a boss 4 on the plate and is formed with a pointed end 5' which fits into a conical depression in the nail.
