Abstract

Le support destiné à recevoir un organe glénoïdien coopérant avec une tête humérale, ledit support (1) étant fixé dans la glène de l'épaule au moyen de vis divergentes disposées diamétralement en combinaison avec un plot central cylindrique et creux (1c) engagé dans le col de la glène, est remarquable en ce qu'il présente au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène.

Description

Support destiné à recevoir un oraane çslénoïdien.
L'invention concerne les implants orthopédiques, notamment les prothèses d'épaule.
II est connu, pour reproduire la surface anatomique de l'articulation de l'épaule, d'utiliser différents types de prothèses.
Généralement, ces prothèses comprennent un élément huméral fixé dans le canal médullaire de l'humérus. Cet élément huméral présente une tête coopérant, soit directement en appui avec la glène, soit, ce qui est généralement le cas, avec un élément glénoïdien fixé au niveau de la glène de l'homoplate.
Ces prothèses permettent de traiter différents traumatismes de l'articulation de l'épaule.
Plus particulièrement, I'invention concerne une prothèse définie dans la demande de brevet FR 8913366 dont le demandeur de la présente est également titulaire. Cette demande de brevet définit un ensemble modulaire d'une prothèse d'épaule, comprenant un élément apte à être implanté dans le fût huméral et dont la tête est conformée pour recevoir, d'une manière rapportée, des éléments interchangeables, dont les formes et profils sont adaptés pour coopérer notamment avec des éléments complémentaires aptes à être implantés dans une partie de l'épaule, en fonction de la nature de la pathologie à traiter.
Notamment, la tête humérale coopère en appui avec un organe glénoidien agencé pour être solidaire d'un support conformé pour être implanté au niveau de la glène de l'épaule. Ce support est fixé dans la glène au moyen de vis divergentes disposées en combinaison avec notamment un plot central, de manière à être en alignement ou sensiblement en alignement, respectivement avec le pilier de l'omoplate, le pilier coracoïdien et le pilier acromial. L'organe glénoïdien peut présenter différentes formes de réalisation.
Le problème que se propose de résoudre l'invention est d'assurer une parfaite stabilité de l'organe glénoidien en prévoyant des agencements spéciaux pour la fixation du support. A cet égard, différentes formes de réalisation peuvent être envisagées.
Dans une première forme de réalisation, le support présente au niveau de son bord périphérique, une patte transversale formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène. La patte présente une série de trous pour le passage de vis de fixation, dans le plan de la glène, ladite patte étant en matériau maléable.
Dans une autre forme de réalisation, le plot équipant le support présente, selon ses génératrices, des fentes aptes à lui assurer une expansion diamétrale sous l'effet de l'introduction d'une tige impactée solidaire de l'organe glénoidien.
Dans une autre forme de réalisation, le plot présente sur une partie de sa hauteur, une portée filetée.
Dans une autre forme de réalisation et pour résoudre le problème posé de respecter le col de la glène, le support présente deux trous diamétralement opposés, décalés de 90" par rapport aux logements des vis divergentes, lesdits trous étant convergents pour l'engagement de vis antérieures et postérieures.
Bien évidemment, ces différentes formes de réalisation peuvent être combinées.
L'invention est exposée, ci-après plus en détail à l'aide des dessins annexés, dans lesquels:

On rappelle pour une meilleure compréhension de la suite de la description, qu'une prothèse d'épaule comprend un élément huméral (H) conformé pour être implanté dans la diaphyse de l'humérus. Cet élément (H) présente une tête (HI) apte à présenter différents agencements pour coopérer avec un organe glénoidien (G). L'organe (G), de tout type connu et approprié, et susceptible de présenter différentes formes de réalisation en fonction du cas pathologique à traiter, est fixé sur un support (1) conformé pour être implanté au niveau de la glène de l'épaule.
Ce support (1), comme indiqué dans la demande de brevet FR 8913366 est fixé dans la glène au moyen de vis divergentes (V) de manière à être en alignement ou sensiblement en alignement, respectivement avec le pilier de l'omoplate et le pilier coracoidien. Ces vis (2) sont engagées dans deux trous taraudés diamétralement opposés (la) et (1 b) permettant l'orientation angulaire de manière divergente desdites vis.
Avantageusement, le support (1) est de forme générale circulaire et présente coaxialement un plot central cylindrique (1 c) destiné à être engagé dans un logement complémentaire formé dans la glène.
Selon l'invention, le support (1) présente différents agencements pour être fixé dans le col de la glène en combinaison avec le plot central (1c) en ayant pour objectif d'assurer une parfaite stabilité de l'élément glénoïdien (G).
Dans ce but, comme le montrent les figures des dessins, différentes formes de réalisation peuvent être envisagées.
Dans la forme de réalisation illustrée figure 1, le support (1) présente, au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne. Cette patte (2) est orientée perpendiculairement aux génératrices du plot de centrage (in), pour venir en appui sur la partie externe de la glène, au niveau de sa zone antérieure. La patte (2) est réalisée en matériau maléable pour s'adapter à la forme anatomique de la glène, et présente une série de trous (2a) pour le passage de vis de fixation (3), dans le plan de la glène (figure 2).
Dans la forme de réalisation illustrée figure 3, le plot (lc) présente, selon ses génératrices, des fentes (ici) aptes à assurer une expansion diamétrale sous l'effet de l'introduction d'une tige impactée, solidaire de l'organe glénoidien (G) (figure 8). Le plot (lc) peut également présenter, sur une partie de sa hauteur, une portée filetée (1c2) (figure 5).
Dans la forme de réalisation illustrée figures 3 et 4, le support (1) présente des logements convergents (nid) (le) pour l'engagement de vis antérieures et postérieures (4) notamment à tête sphérique de façon à les orienter d'une manière convergente dans la glène. Ces dispositions permettent de respecter le col de la glène. A noter que les logements (nid) et (le) sont diamétralement opposés et décalés de 90" par rapport aux logements (la) et (lob) des vis divergentes (V).
Bien évidemment, ces différentes réalisations peuvent être combinées. Notamment, le support (1) peut présenter l'une quelconque des combinaisons suivantes:
- patte transversale (2) et plot (1 c) avec fentes (Ici),
- patte transversale (2) et plot (1 c) avec portée filetée (1 c2),
- patte transversale (2) et logements convergents pour l'engagement de vis antérieures et postérieures,
- plot (lc) avec fentes (Ici) et logements convergents pour
I'engagement de vis antérieures et postérieures,
- plot (tic) avec portée filetée (1c2) et logements convergents pour l'engagement de vis antérieures et postérieures.
Quelle que soit la forme de réalisation, le support (1) présente avantageusement, deux autres trous taraudés diamétralement opposés (if) (zig) décalés de 90" par rapport aux trous (la) et (lob)5 et aptes à recevoir des vis de fixation à tête filetée, s'étendant dans un plan sensiblement parallèle au plot de centrage (in).
Ainsi réalisé, le support reçoit tout type d'organe glénoidien.
Par exemple, dans le cas d'un traitement d'une omarthrose avec destruction de la glène, L'organe glénoïdien (5) présente un évidement en forme de calotte sphérique, dont le rayon de courbure correspond très sensiblement à celui d'une portée sphérique que présente la tête humérale (figures 7 et 9).
Dans le cas d'une rupture de coiffe notamment, L'organe glénoidien (6) est constitué par une pièce mâle sphérique apte à coopérer avec un évidement en forme de calotte sphérique, que présente une cupule formée au niveau de la tête de l'élément huméral (figure 8).
La fixation de l'organe glénoidien (5) ou (6) sur le support (1), s'effectue par exemple, au moyen d'une portée interne taraudée vissée sur la périphérie (1h) dudit support, qui est filetée à cet effet.
On prévoit tout agencement interdisant un dévissage accidentel.
Les avantages ressortent bien de la description.

Claims (10)

    REVENDICATIONS
  1. -1- Dispositif de stabilité d'un organe glénoïdien (G) coopérant avec une tête humérale (H) et agencé pour être solidaire d'un support (1) fixé dans la glène de l'épaule au moyen de vis divergentes disposées diamétralement en combinaison avec un plot central cylindrique et creux (lc) engagé dans le col de la glène, caractérisé en ce qu'il présente au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène.
  2. -2- Dispositif selon la revendication 1, caractérisé en ce que la patte (2) présente une série de trous (2a) pour le passage de vis de fixation, dans le plan de la glène, ladite patte étant en matériau maléable.
  3. -3- Dispositif de stabilité d'un organe glénoïdien (G) coopérant avec une tête humérale (H) et agencé pour être solidaire d'un support (1) fixé dans la glène de l'épaule au moyen de vis divergentes disposées diamétralement en combinaison avec un plot central cylindrique et creux (lc) engagé dans le col de la glène, caractérisé en ce que le plot (lc) présente, selon ses génératrices, des fentes (ici) aptes à lui assurer une expansion diamétrale sous l'effet de l'introduction d'une tige impactée solidaire de l'organe glénoïdien.
  4. -4- Dispositif selon la revendication 3, caractérisé en ce qu'il présente au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène.
  5. -5- Dispositif de stabilité d'un organe glénoidien (G) coopérant avec une tête humérale (H) et agencé pour être solidaire d'un support (1) fixé dans la glène de l'épaule au moyen de vis divergentes disposées diamétralement en combinaison avec un plot central cylindrique et creux (lc) engagé dans le col de la glène, caractérisé en ce que le plot (lc) présente sur une partie de sa hauteur, une portée filetée (1c2).
  6. -6- Dispositif selon la revendication 5, caractérisé en ce qu'il présente au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène.
  7. -7- Dispositif de stabilité d'un organe glénoidien (G) coopérant avec une tête humérale (H) et agencé pour être solidaire d'un support (1) fixé dans la glène de l'épaule au moyen de vis divergentes disposées diamétralement en combinaison avec un plot central cylindrique et creux (lc) engagé dans le col de la glène, caractérisé en ce qu'il présente deux trous diamétralement opposés (if) (in), décalés de 90" par rapport aux logements des vis divergentes, lesdits trous étant convergents pour l'engagement de vis antérieures et postérieures.
  8. -8- Dispositif selon la revendication 7, caractérisé en ce qu'il présente au niveau de son bord périphérique, une patte transversale (2) formée en débordement de sa face interne pour venir en appui sur la partie externe de la glène.
  9. -9- Dispositif selon la revendication 7, caractérisé en ce que le plot (tic) présente, selon ses génératrices, des fentes (ici) aptes à lui assurer une expansion diamétrale sous l'effet de l'introduction d'une tige impactée solidaire de l'organe glénoïdien.
  10. -10- Dispositif selon la revendication 7, caractérisé en ce que le plot (tic) présente sur une partie de sa hauteur une portée filetée (1c2).
