Abstract
Description
IMPLANT FEMORAL DE PROTHESE POUR
HANCHE DYSPLASIQUE
DYSPLASTIC HIP
The present invention relates to a femoral implant of prosthesis for dysplastic hip.
The installation of a total hip prosthesis in a dysplastic hip poses, with regard to the femoral implant, problems linked, on the one hand, to the reduced dimensions of the femur, and in particular of the medullary canal and, d on the other hand, at the pronounced anteversion or retroversion that it is necessary to give to the neck of the implant to reproduce the original femur.
Small implants are known which are adapted to dysplastic hips, of the so-called "Charnley or Muller dysplastic" type. The stem of such an implant is generally straight in order to be able to pivot about its axis in the medullary canal of the femur, to allow adjustment of the ante or the retroversion of the neck of the femur. These implants have the drawbacks of being held in the medullary canal of the femur only by means of their anchoring cement and of having cortical supports of small surface. The repeated stresses they undergo generate significant risks of loosening, in the relatively short term.
To remedy these drawbacks, one solution consists in producing a custom implant, the dimensions of which are determined by means of radiographic analyzes and modeling by computer processing. This process is however complex and costly to implement. In addition, the dimensions of the implant thus determined are not 100% accurate and there is a non-negligible risk that the implant has a defect in length of the rod or an error in the orientation of the neck relative to the femoral shaft.
The present invention aims to remedy these drawbacks by providing a femoral implant specifically adapted to dysplastic femurs, the stem of which is kept rotating in the medullary canal of the femur and the adjustment of the anterior or retroversion of the cervix being possible. simple, easy and fast way, without requiring the manufacture of many different parts adapted to the different shapes and possible dimensions of the joints to be reconstructed.
To this end, the implant according to the invention comprises, in combination
- a rod adapted to the specific dimensions of the medullary canals of dysplastic femurs, that is to say sufficiently thin at its distal end to be engaged in the medullary canal of the femur, comprising a flare at its metaphyseal part and having a pronounced anteroposterior curvature; and
at least one cervical part comprising an inclined neck, that is to say the axis of which is not contained in the longitudinal median plane of the implant, which can be assembled to the rod by engagement of a pin projecting from the assembly face either of the rod or of the cervical part, in a well of complementary shape opening into the assembly face, respectively, either of the cervical part, or of the rod, the pin, the well and the assembly faces being symmetrical in relation to the longitudinal and transverse median planes of the assembly faces and the dimensions of the latter being identical, so that the cervical part can be fitted onto the rod in two stable positions located at 1800 from one another, with, in one or the other of these positions, the coming of the side walls of the two parts in the extension of each other.
Thanks to its shape adapted to that of dysplastic femurs, the stem can be wedged in the medullary canal of the femur, without the possibility of pivoting along its longitudinal axis. Its anchoring cement is therefore only little stressed in torsion, which significantly limits the risks of loosening.
The possibility of mounting the cervical part with inclined neck in two positions located at 1800 from one another allows, with a single cervical part, to obtain either an anteverted neck or a retroverted neck, by simple inversion of this cervical part.
Thus, the invention makes it possible to constitute, in a simple, easy and rapid manner, a prosthesis specifically adapted to dysplastic femurs, with only three basic elements, that is to say a rod for the right femur, a rod for the left femur and the aforementioned cervical part. The implant according to the invention can advantageously be formed from a set of different cervical parts whose necks have different inclinations. This game can include a cervical neck section, used when anteversion or retroversion of the cervix is not necessary.
According to a preferred embodiment of the invention, the pin and the well have a frustoconical shape with a slight slope and an elliptical cross section, the major axis of the ellipse being contained in the longitudinal median plane of the rod.
The frustoconical shape of the pin and the well provides an axial connection, by conical wedging, of the cervical part and the rod, and the elliptical cross section of this pin and this well ensures perfect immobilization in rotation of the cervical part with respect to to the stem.
Advantageously, the diaphyseal and metaphyseal parts of the implant are curved in order to adapt to the particular shapes of the dysplastic femurs.
In any case, the invention will be clearly understood with the aid of the description which follows, with reference to the appended diagrammatic drawing representing, by way of nonlimiting example, a preferred embodiment of the implant which it concerns.
la figure 2 en est une vue en coupe partielle selon la ligne Il-Il de la figure 3 ;
Figure 1 is a side view, exploded
Figure 2 is a partial sectional view along the line II-II of Figure 3;
Figure 3 is a view along the axis of its cervical portion; and
Figures 4 and 5 are front views in two different relative positions of two of its components.
The figures represent from different angles a femoral implant 2 for dysplastic hip prosthesis.
The implant represented consists of a rod 3, a cervical part 4 and a spherical head 5.
The rod 3 is adapted to the specific dimensions of the medullary canals of dysplastic femurs, that is to say sufficiently thin at its distal end 3a to be able to be engaged in the medullary canal of the femur, comprising a flare at its part metaphyseal 3b and having a pronounced anteroposterior curvature.
As can be seen in FIGS. 4 and 5, its distal part 3a has an anatomical curve with a slight slope in the anteroposterior plane, ensuring good wedging of the rod 3 in the medullary canal of the femur and two lateral chamfers 6 avoiding the cortical supports of its extremity. In the lateral plane (FIG. 1), this distal part 3a has a conical shape with a slight slope, for good guidance of the implant during its introduction into the bone.
The metaphyseal part 3b of the rod 3 is flared to provide good anti-rotational stability, while being curved to adapt to the particular shapes of the dysplastic femurs, and has two lateral flats 7 bearing on the bone.
At the end of its metaphyseal part 3b, the rod 3 has a pin 10 projecting from its flat end face 11. This pin 10 is of frustoconical shape with a slight slope and of elliptical cross section, the pin 10 and the face 11 being symmetrical in relation to the longitudinal and transverse median planes of the assembly face 11.
The cervical part 4 has a curved end 4a limited, on the side opposite to the spherical head 5, by a planar face 12, perpendicular to the axis of the cervical part 4 and of the same dimension as the end face 11 of the rod. 3. In the face 12 is arranged a frustoconical well of shape complementary to that of the pin 10, the well and the face 12 also being of symmetrical shape with respect to the longitudinal and transverse planes of the face 12.
The cervical part 4 comprises, on the side opposite to the face 12, a frustoconical finger with a slight slope 4b, intended to be engaged with wedging effect in a complementary frustoconical well arranged in the femoral head 5.
As shown in the figures, the cervical part 4 is intended to be fitted onto the rod 3 in two possible positions, located 1800 from one another, with, in one or the other of these positions , the coming of the side walls of the two parts in the extension of each other.
The neck of the cervical part 4 that constitutes the finger 4b being inclined, that is to say its axis not being contained in the longitudinal median plane of the rod 3, the possibility of mounting the cervical part 4 in two positions located at 1800 one from the other allows, with a single cervical part 4, to obtain either an anversed neck (Figure 4), or a retroverted neck (Figure 5), by simply inverting this cervical part 4 on the end of the rod 3.
Thanks to its shape adapted to that of dysplastic femurs, the rod 3 can be wedged in the medullary canal of the femur, without the possibility of pivoting along its longitudinal axis. Its anchoring cement is therefore only little stressed in torsion, which significantly limits the risks of loosening.
The frustoconical shape of the pin 10 and the well which is complementary ensures an axial connection, by conical wedging, of the cervical part 4 and the rod 3, and the elliptical cross section of the pin 10 and of this well ensures perfect immobilization in rotation of the cervical part 4 relative to the rod 3.
Thanks to all of these characteristics, the implant according to the invention makes it possible to constitute in a simple, easy and rapid manner, a prosthesis specifically adapted to dysplastic femurs, with only three basic elements, that is to say a rod 3 for the right femur, a rod 3 for the left femur and a cervical part 4 with inclined neck. The implant 2 can of course be formed from a set of different cervical parts 4, the necks of which have different inclinations, this set being able, moreover, to include a cervical part with a straight neck, used when it is not necessary to perform anteversion or retroversion of the cervix.
