El conjunto de viga en I 154 incluye barras de cuchilla 156a que soportan un bloqueo 156b en una porción proximal de las mismas y una viga en I 156c en una porción de extremo distal de las barras de cuchilla 156a. Las barras de cuchilla 156a incluyen un labio 156d en una porción de extremo proximal de las mismas. Para una descripción más detallada del conjunto de viga en I 156, o componentes del mismo, puede hacerse referencia a la Patente '033 mencionada anteriormente. La varilla de articulación 158 del conjunto de árbol interior 150 define una ranura alargada 158a a su través que está dispuesta en alineación vertical con el canal alargado 154d de la carcasa superior 154.
Con referencia continuada a las figuras 1 a 10, el conjunto de árbol interior 150 del conjunto de árbol 140 soporta un conjunto de bloqueo de disparo 160 que está configurado para impedir que el aparato de grapado quirúrgico 10 se dispare cuando el efector final 110 no está en la posición articulada, por ejemplo, aproximadamente 90 grados con respecto al conjunto de árbol 140 y/o al eje longitudinal "X1-X1". El conjunto de bloqueo de disparo 160 incluye un inserto de carcasa 162, un resorte 164, y un elemento de bloqueo de disparo 166. El inserto de carcasa 162 está configurado para ser recibido dentro del rebaje de inserción 154b e incluye una base 162a y un dedo 162b que se extiende distalmente desde la base 162a. El resorte 164 del conjunto de bloqueo de disparo 160, que puede ser un resorte de compresión, puede recibirse dentro del recorte 152b de la carcasa inferior 152 del conjunto de árbol interior 150 y está configurado para acoplar el elemento de bloqueo de disparo 166 del conjunto de bloqueo de disparo 160. El elemento de bloqueo de disparo 166 incluye un brazo vertical 166a y un brazo lateral 166b que se extiende en una dirección transversal lateral desde una porción superior del brazo vertical 166a. El elemento de bloqueo de disparo 166 incluye, además, una primera rama 166c que se extiende proximalmente desde una porción inferior del brazo vertical 166a y una segunda rama 166d que se extiende proximalmente desde el brazo lateral 166b. La primera rama 166c está configurada para ser recibida dentro del recorte 152b del conjunto de árbol interior 150 y el resorte 164 del conjunto de bloqueo de disparo 160 para permitir que el resorte 164 empuje el elemento de bloqueo de disparo 166 distalmente. El brazo vertical 166a del elemento de bloqueo de disparo 166 se extiende a través del canal alargado 154d de la carcasa superior 154 de manera que el brazo lateral 166b del elemento de bloqueo de disparo 166 se extiende a través del dedo 162b del inserto de carcasa 162 del conjunto de bloqueo de disparo 160 y la segunda rama 166d del elemento de bloqueo de disparo 166 se asienta en el rebaje de bloqueo 154c del conjunto de árbol interno 150.
En operación, tal como se ilustra en las figuras 11 a 23, el conjunto de viga en I 156 del conjunto de árbol 140 está inicialmente en una posición proximal de manera que los conjuntos de yunque y cartucho 120, 130 del efector final 110 están en la posición sin sujetar (por ejemplo, espaciada o no aproximada) y el conjunto de bloqueo de disparo 160 está en una posición bloqueada. En la posición de bloqueo, la segunda rama 166d del elemento de bloqueo de disparo 166 está soportada en el rebaje de bloqueo 154c de la carcasa superior 154 e inclinada en un ángulo tal que el labio 156d de las barras de cuchilla 156a es coplanario con una superficie proximal 166e (figura 12) de la segunda rama 166d del elemento de bloqueo de disparo 166. Cuando los conjuntos de yunque y cartucho 120, 130 están en la posición sin sujetar, la superficie proximal 166e de la segunda rama 166d está separada una distancia "D" del labio 156d de las barras de cuchilla 156a para permitir que el conjunto de viga en I 160 se desplace distalmente la distancia "D", tal como se indica mediante las flechas "C" (figura 14) para sujetar los conjuntos de yunque y cartucho 120, 130 juntos. A medida que el conjunto de viga en I 160 se desplaza distalmente, la viga en I 156c del conjunto de viga en I 156 aproxima los conjuntos de yunque y cartucho 120, 130 a la posición de sujeción (véanse las figuras 13 y 14) del efector final 110, tal como se indica mediante las flechas "B" (figuras 1 y 14). Para una descripción más detallada de cómo la viga en I 156c aproxima los conjuntos de yunque y cartucho 120, 130, puede hacerse referencia a la Patente '033. Cuando el efector final 110 está en la posición sujeta, el labio 156d del conjunto de viga en I 156 se acopla con la superficie proximal 166e de la segunda rama 166d del elemento de bloqueo de disparo 166, de modo que el elemento de bloqueo de disparo 166 impide un mayor avance distal del conjunto de viga en I 156 (véase la figura 15) y el disparo del aparato de grapado quirúrgico 10. En la posición sujeta, la unidad de carga 100 puede avanzar a través del portal de acceso quirúrgico 200 (figura 13) para acceder al sitio quirúrgico dentro del paciente. Para una descripción más detallada de un ejemplo de portal de acceso quirúrgico, se puede hacer referencia a la Patente US 9,788,857 de Bettuchietal.Una vez que el efector final 110 se hace avanzar a través del portal de acceso quirúrgico 200, el conjunto de viga en I 156 del conjunto de árbol 140 se puede arrastrar proximalmente para desacoplar el efector final 110 para el subsiguiente reapriete del efector final 110 sobre el tejido que va a ser grapado por el efector final 110.
Para articular el efector final 110 de la unidad de carga 100 a la posición articulada (por ejemplo, unos 90 grados), la varilla de articulación 158 del conjunto de árbol interior 150 se estira proximalmente (al girar el pomo 14a del conjunto adaptador 14; véase la figura 2), tal como indica la flecha "D" (figuras 16 y 17). A medida que la varilla de articulación 158 se desplaza proximalmente con respecto al elemento de bloqueo de disparo 166 del conjunto de bloqueo de disparo 160, el brazo vertical 166a del elemento de bloqueo de disparo 166 se desplaza a través de la ranura alargada 158a de la varilla de articulación 158, de modo que un borde de apoyo 158b (figura 18) dispuesto en una porción del extremo distal de la ranura alargada 158a de la varilla de articulación 158 puede acoplar una superficie distal del brazo vertical 166a del elemento de bloqueo de disparo 166. La continua traslación proximal de la varilla de articulación 158 hace que el elemento de bloqueo de disparo 166 comprima el resorte 164 del conjunto de bloqueo de disparo 160 y pivote a una posición desbloqueada (figura 18), tal como se indica mediante las flechas "E". En la posición desbloqueada del elemento de bloqueo de disparo 166, el efector final 110 se dispone en la posición articulada y la segunda rama 166d del elemento de bloqueo de disparo 166 se posiciona por encima del labio 156d del conjunto de viga en I 156, de manera que el labio 156d del conjunto de viga en I 156 pueda moverse por debajo de la segunda rama 166d del elemento de bloqueo de disparo 166 para disparar el efector final 110 (figura 19) al accionar el conjunto de viga en I 156.
Tal como se ve en las figuras 20 a 23, la unidad de carga 100 incluye, además, una guía de pivote 170 que tiene una porción de extremo distal 172 acoplada de manera pivotante a una porción de extremo proximal del efector final 110 mediante un pasador 180, y una porción de extremo proximal 174 que está libre y se recibe de modo deslizante dentro del tubo exterior 142 del conjunto de árbol 140. La guía de pivote 170 incluye, además, una superficie interior arqueada 176 y una superficie exterior curvada 178. Cuando el efector final 110 está en la posición articulada (figura 22), la superficie exterior curvada 178 de la guía de pivote 170 acopla con una superficie interior del tubo exterior 142 de manera que el tubo exterior 142 mantiene la superficie interior arqueada 176 de la guía de pivote 170 en posición para guiar el conjunto de viga en I 156 a lo largo de una trayectoria curva "CP" hacia el interior del efector final 110. La trayectoria curva "CP" se extiende a lo largo de la superficie interior arqueada 176 de la guía de pivote 170 y una superficie interior arqueada 158y de una porción distal de la varilla de articulación 158. En particular, cuando el efector final 110 está en la posición articulada, la superficie interior arqueada 176 de la guía de pivote 170 y la superficie interior arqueada 158y de la varilla de articulación 158 definen un radio predeterminado "r", por ejemplo, un radio perfecto, para permitir que el conjunto de viga en I 156 se curve suavemente dentro de un canal de cuchilla 110y del efector final 110 (por ejemplo, sin pandeo) a medida que las barras de cuchilla 156a del conjunto de viga en I 156 se adaptan, por ejemplo se doblan, al radio predeterminado "r" a medida que el conjunto de viga en I 156 se avanza distalmente para disparar el efector final 110.
Con referencia a la figura 23, cuando el efector final 110 está en la posición no articulada, el tubo exterior 142 constriñe una porción proximal de la guía de pivote 170 hacia adentro de tal manera que un borde interior 176a de la superficie interior arqueada 176 y un borde interior 158z de la superficie interior arqueada 158y de la varilla de articulación 158 aplican fuerzas opuestas hacia adentro contra las superficies exteriores de las barras de cuchilla 156a del conjunto de viga en I 156 para restringir por fricción el movimiento axial del conjunto de viga en I 156.
La fijación de cualquiera de los componentes de los dispositivos dados a conocer aquí puede realizarse utilizando técnicas de fijación conocidas, tales como soldadura, engarce, pegado, sujeción, etc.
Las diversas realizaciones dadas a conocer en el presente documento también pueden configurarse para trabajar con sistemas quirúrgicos robóticos y lo que comúnmente se denomina "Telecirugía". Tales sistemas emplean diversos elementos robóticos para asistir al clínico y permitir la operación remota (o la operación remota parcial) de la instrumentación quirúrgica. Para ello pueden emplearse diversos brazos robóticos, engranajes, levas, poleas, motores eléctricos y mecánicos, etc., que pueden diseñarse con un sistema quirúrgico robótico para asistir al clínico durante el curso de una operación o tratamiento. Dichos sistemas robóticos pueden incluir sistemas dirigibles a distancia, sistemas quirúrgicos flexibles automáticamente, sistemas quirúrgicos flexibles a distancia, sistemas quirúrgicos articulados a distancia, sistemas quirúrgicos inalámbricos, sistemas quirúrgicos modulares o configurables selectivamente accionados a distancia, etc.
Los sistemas quirúrgicos robóticos pueden emplearse con una o varias consolas situadas junto al quirófano o en una ubicación remota. En este caso, un equipo de clínicos puede preparar al paciente para la cirugía y configurar el sistema quirúrgico robótico con uno o varios de los instrumentos dados a conocer en el presente documento, mientras que otro clínico (o grupo de clínicos) controla a distancia los instrumentos a través del sistema quirúrgico robótico. Como puede apreciarse, un clínico altamente cualificado puede realizar múltiples operaciones en múltiples ubicaciones sin abandonar su consola remota, lo que puede ser económicamente ventajoso y beneficioso para el paciente o una serie de pacientes. Para una descripción detallada de estaciones de trabajo médicas y/o componentes de las mismas a modo de ejemplo, se puede hacer referencia a la publicación de solicitud de Patente US 2012/0116416, y a la publicación de solicitud PCT WO2016/025132.
Los expertos en la materia comprenderán que las estructuras y procedimientos descritos específicamente en el presente documento y mostrados en las figuras que lo acompañan son realizaciones no limitativas a modo de ejemplo, y que la descripción, la divulgación y las figuras deben interpretarse meramente como ejemplos de realizaciones particulares. Debe entenderse, por lo tanto, que la presente invención no se limita a las realizaciones precisas descritas, y que otros cambios y modificaciones pueden ser efectuados por un experto en la materia sin apartarse del alcance o espíritu de la invención. Además, los elementos y características mostrados o descritos en relación con ciertas realizaciones pueden combinarse con los elementos y características de ciertas otras realizaciones sin apartarse del alcance de la presente invención, y que tales modificaciones y variaciones también se incluyen dentro del alcance de la presente invención. En consecuencia, la materia objeto de la presente invención no está limitada por lo que se ha mostrado y descrito en particular.
Claims (12)
1. Unidad de carga (100) para un aparato de grapado quirúrgico (10) que comprende:
un conjunto de árbol (140);
un efector final (110) acoplado al conjunto de árbol (140) y posicionado para articularse con respecto al conjunto de árbol (140) entre una posición no articulada y una posición articulada;
un conjunto de viga en I (156) soportado en el conjunto de árbol (140) y movible selectivamente en relación con el efector final (110) para disparar el efector final (11); y
un conjunto de bloqueo de disparo (160) soportado en el conjunto de árbol (140) y configurado para impedir que el conjunto de viga en I (156) dispare el efector final (110) cuando el efector final (110) está dispuesto en la posición no articulada, en la que el conjunto de bloqueo de disparo (160) incluye un elemento de bloqueo de disparo (166) y un resorte (164) que está posicionado para impulsar el elemento de bloqueo de disparo (166) hacia una primera posición, el elemento de bloqueo de disparo (166) posicionado para impedir que el conjunto de viga en I (156) avance a través del efector final (110) cuando está en la primera posición; y
una varilla de articulación (158) que es móvil para articular el efector final (110) con respecto al conjunto de árbol (140), en la que la varilla de articulación (158) está posicionada para mover el elemento de bloqueo de disparo (166) a una segunda posición a medida que el efector final (100) se mueve desde la posición no articulada a la posición articulada, el conjunto de viga en I (156) posicionado para avanzar a través del efector final (110) para disparar el efector final (110) cuando el elemento de bloqueo de disparo (166) está dispuesto en la segunda posición,
caracterizado por quela varilla de articulación (158) define una ranura alargada (158a) a su través, estando posicionada la ranura alargada (158a) para recibir el elemento de bloqueo de disparo (166) en la misma.
2. Unidad de carga (100), según la reivindicación 1, en la que el conjunto de viga en I (156) incluye al menos una barra de cuchilla (156a), soportando la barra de cuchilla (156a) una viga en I (156c) configurada para avanzar a través del efector final (110) para disparar el efector final (110).
3. Unidad de carga (100), según la reivindicación 2, en la que el elemento de bloqueo de disparo (166) está posicionado para acoplar la al menos una barra de cuchilla (156a) para impedir que la viga en I (156c) dispare el efector final (110) cuando el efector final (110) está en la posición no articulada.
4. Unidad de carga, según la reivindicación 3, en la que el elemento de bloqueo de disparo (166) y la al menos una barra de cuchilla (156a) están desacoplados cuando el efector final (110) está en la posición articulada de tal manera que la viga en I (156c) puede avanzar a través del efector final (110) para disparar el efector final (110).
5. Unidad de carga (100), según cualquier reivindicación anterior, en la que el efector final (110) incluye un conjunto de yunque (120) y un conjunto de cartucho (130) que están acoplados de manera pivotante entre sí y son móviles entre una posición sin sujetar y una posición sujeta.
6. Unidad de carga (100), según cualquier reivindicación anterior, en la que el conjunto de viga en I (156) está configurado para colocar los conjuntos de yunque y cartucho (120, 130) en la posición sujeta cuando el efector final (110) está dispuesto en la posición no articulada o en la posición articulada.
7. Aparato de grapado quirúrgico (10) que comprende:
un conjunto de árbol (140);
un efector final (110) acoplado al conjunto de árbol (140) y posicionado para articularse con respecto al conjunto de árbol (140) entre una posición no articulada y una posición articulada;
una barra de cuchilla (156a) que soporta una viga en I (156c), la barra de cuchilla (156a) movible selectivamente con respecto al conjunto de árbol (140); y
un elemento de bloqueo de disparo (166) configurado para acoplar la barra de cuchilla (156a) para impedir que la viga en I (156c) avance a través del efector final (110) cuando el efector final (110) está dispuesto en la posición no articulada; y
una varilla de articulación (158) soportada por el conjunto de árbol (140), la varilla de articulación (158) configurada para articular el efector final (110) con respecto al conjunto de árbol (140),
en el que la varilla de articulación (158) está posicionada para mover el elemento de bloqueo de disparo (166) a una posición desbloqueada a medida que el efector final (100) se mueve desde la posición no articulada a la posición articulada;
caracterizado por quela varilla de articulación (158) define una ranura alargada (158a) a su través, estando posicionada la ranura alargada (158a) para recibir el elemento de bloqueo de disparo (166) en la misma.
8. Aparato de grapado quirúrgico (10), según la reivindicación 7, en el que el efector final (110) incluye un conjunto de yunque (120) y un conjunto de cartucho (130) que son móviles entre una posición sin sujetar y una posición sujeta, en el que la viga en I (156c) está posicionada para avanzar hacia una porción de extremo proximal del efector final (110) para mover los conjuntos de yunque y cartucho (120, 130) desde la posición sin sujetar a la posición sujeta.
9. Aparato de grapado quirúrgico (10), según la reivindicación 8, en el que el elemento de bloqueo de disparo (166) se desacopla de la barra de cuchilla (156a) a medida que el efector final (110) se mueve desde la posición no articulada a la posición articulada; y/o en el que el elemento de bloqueo de disparo (166) impide que la viga en I (156c) avance distalmente más allá de la porción de extremo proximal del efector final (110) cuando el efector final (110) está dispuesto en la posición no articulada.
10. Aparato de grapado quirúrgico (10), según la reivindicación 8 o la reivindicación 9, en el que el efector final (110) está articulado aproximadamente 90 grados con respecto a un eje longitudinal (X1-X1) definido a través del conjunto de árbol (140) cuando está en la posición articulada.
11. Aparato de grapado quirúrgico (10), según cualquiera de las reivindicaciones 7 a 10, en el que el elemento de bloqueo de disparo (166) está posicionado para deslizarse a lo largo de la ranura alargada (158a) a medida que la varilla de articulación (158) mueve el efector final (110) con respecto al conjunto de árbol (140).
12. Aparato de grapado quirúrgico (10), según la reivindicación 11, en el que la varilla de articulación (158) está posicionada para acoplar el elemento de bloqueo de disparo (166) para pivotar el elemento de bloqueo de disparo (166) con respecto a la barra de cuchilla (156a).
