Abstract
Una unidad (100) de carga de grapado quirúrgico que incluye una primera mordaza (102) y una segunda mordaza (104) móviles entre sí para realizar una operación de grapado quirúrgico mediante la cual se disparan grapas en el tejido, en donde la unidad de carga comprende: un conjunto de cartucho (200) soportable en la primera mordaza y la segunda mordaza, en el que el conjunto de cartucho tiene una superficie de contacto con tejido que define una ranura (234) alargada que se extiende longitudinalmente y una pluralidad de ranuras de retención (222) de grapas formadas en la misma, en donde el conjunto de cartucho incluye una grapa (223) quirúrgica cargada en cada ranura de retención de grapas; un conjunto de yunque (300) soportable en otra mordaza, la primera mordaza y la segunda mordaza, en el que el conjunto de yunque incluye una placa de yunque que tiene una superficie de contacto con tejido que define una ranura alargada que se extiende longitudinalmente y una pluralidad de cavidades de formación de grapas; un refuerzo quirúrgico (500) dispuesto de forma desmontable contra al menos una de las superficies de contacto con tejido del conjunto de cartucho y la superficie de contacto con tejido del conjunto de yunque, en donde el o cada refuerzo quirúrgico incluye una parte de cuerpo configurada y dimensionada para cubrir la pluralidad de ranuras de retención de grapas o la pluralidad de cavidades de formación de grapas de los respectivos conjunto de cartucho y conjunto de yunque, en donde la parte de cuerpo define un borde distal y un borde proximal; una longitud respectiva de hilo de sutura ("S2") que asegura un refuerzo quirúrgico respectivo contra al menos una de las superficies de contacto con tejido del conjunto de cartucho y la superficie de contacto con tejido del conjunto de yunque, en donde la sutura se extiende transversalmente a través del refuerzo quirúrgico respectivo; y una característica (114) de retención que asegura la longitud de hilo de sutura al respectivo conjunto de cartucho y conjunto de yunque en el que la característica de retención comprende una pluralidad de surcos formados en una superficie lateral de los respectivos conjunto de cartucho y conjunto de yunque, en donde los surcos están formados cerca de bordes laterales distales opuestos y bordes laterales proximales opuestos de los respectivos conjunto de cartucho y conjunto de yunque; y caracterizado por que partes de la longitud de hilo de sutura están enroscadas a través del refuerzo quirúrgico a lo largo de los lados opuestos del refuerzo quirúrgico, y en que cada surco está configurado y dimensionado para recibir, y está en recepción de, una longitud de la longitud de hilo de sutura en el mismo.

1. Campo tecnico
La presente descripcion se refiere a un aparato grapador quirurgico y, mas particularmente, a un aparato grapador quirurgico que incluye un refuerzo quirurgico desmontable para un conjunto de yunque y/o un conjunto de cartucho de grapas.
2. Antecedentes de la tecnica relacionada
Los instrumentos de grapado quirurgico que son usados para aplicar secuencial o simultaneamente una o mas filas de sujetadores para unir segmentos de tejidos corporales son bien conocidos en la tecnica. Los sujetadores tienen tipicamente la forma de grapas quirurgicas, pero tambien pueden utilizarse sujetadores de dos partes. Tales dispositivos generalmente incluyen un par de mordazas para sujetar entre ellas los tejidos corporales a ser unidos. ^picamente, uno de los miembros de mordaza incluye o soporta un conjunto de cartucho de grapas que aloja una pluralidad de grapas dispuesta en al menos dos filas laterales mientras que el otro miembro de mordaza tiene un conjunto de yunque que define una superficie para formar las patas de grapas cuando las grapas son accionadas del conjunto de cartucho de grapas.
Cuando se acciona el instrumento de grapado, levas de traslacion longitudinalmente hacen contacto con miembros de accionamiento de grapas en una de las mordazas, que a su vez actuan sobre los empujadores de grapas para expulsar secuencial o simultaneamente las grapas del conjunto de cartucho de grapas. Una cuchilla puede desplazarse entre las filas de grapas para cortar y/o abrir longitudinalmente el tejido grapado entre las filas de grapas. Tales instrumentos son descritos, por ejemplo, en los documentos de patente U.S. No. 3.079.606 y U.S. No. 3.490.675.
Cuando se grapan tejidos relativamente delgados o fragiles, puede ser importante sellar eficazmente la lmea de grapas contra aire o fuga de fluido. Adicionalmente, a menudo es necesario reforzar la lmea de grapas contra el tejido para prevenir desgarros en el tejido o tirar de las grapas a traves del tejido. Un metodo para prevenir desgarros o tirones envuelve la colocacion de un material de refuerzo de tejido biocompatible, o un "refuerzo," entre las grapas y el tejido subyacente. En este metodo, una capa de material de refuerzo se coloca contra el tejido, y el tejido se grapa de forma convencional. En metodos mas recientes, la capa de refuerzo se posiciona en el propio aparato grapador quirurgico antes de grapar el tejido. Algunas grapadoras quirurgicas utilizan sujetadores o clips para conectar temporariamente el material de refuerzo a cada una de las mordazas de la grapadora, p. ej., una dispuesta en el conjunto de cartucho de grapas y la otra en el conjunto de yunque.
Es un deseo de la presente solicitud proporcionar un material de refuerzo de lmea de grapas que este unido de forma segura a la mordaza de un instrumento de grapado, siendo desmontable de la misma. Tambien sena deseable proporcionar un material de refuerzo de lmea de grapas o refuerzo que este unido de forma desmontable a una mordaza de grapadora por un miembro que se retira del lugar con la grapadora despues de su uso. Por consiguiente, es objeto de esta descripcion satisfacer los deseos mencionados anteriormente.
La patente europea EP 2236098 describe una unidad de carga quirurgica del tipo en el que se basa el preambulo de la reivindicacion independiente.
El documento de patente US 2011/087279 es un refuerzo quirurgico sujeto a un instrumento quirurgico por una sutura.
La patente europea EP2462880 describe un refuerzo quirurgico que esta configurado con bucles para asegurar el refuerzo a un instrumento quirurgico.
El documento de patente US2012/074199 describe un refuerzo quirurgico asegurado a un instrumento quirurgico mediante clips discretos que encajan muescas en el instrumento.
La patente europea EP2630922 disponible para consideraciones de innovacion describe un refuerzo asegurado por suturas discretas que encajan en muescas.
Se considera que la tecnica anterior mas cercana es la patente europea EP 2236098 A2.
Compendio
La presente descripcion se refiere a un aparato grapador quirurgico que incluye un refuerzo quirurgico. Segun la invencion se proporciona una unidad de carga de grapado quirurgico como se describe en la reivindicacion 1, con caractensticas preferidas como se describe en las reivindicaciones dependientes.

Segun un aspecto de la presente descripcion, se proporciona un aparato grapador quirurgico e incluye una primera mordaza y una segunda mordaza moviles entre s^ para realizar una operacion de grapado quirurgico mediante las cuales se disparan grapas en el tejido. El aparato grapador quirurgico comprende una carcasa; un mango soportado por la carcasa; un cuerpo alargado que se extiende distalmente desde la carcasa y que tiene un extremo distal adaptado para acoplar de forma desmontable una unidad de carga; y una unidad de carga soportable en un extremo distal de la carcasa.
Segun otro aspecto de la presente descripcion, se proporciona una unidad de carga de grapado quirurgico que incluye una primera mordaza y una segunda mordaza moviles entre sf para realizar una operacion de grapado quirurgico por donde se disparan grapas en el tejido.
La unidad de carga incluye un conjunto de cartucho soportable en una mordaza, la primera mordaza y la segunda mordaza, en donde el conjunto de cartucho tiene una superficie de contacto con tejido que define una ranura alargada que se extiende longitudinalmente y una pluralidad de ranuras de retencion de grapas formada en la misma, en donde el conjunto de cartucho incluye unas grapas quirurgicas cargadas en cada ranura de retencion de grapas. La unidad de carga incluye un conjunto de yunque soportado en otra mordaza, la primera mordaza y la segunda mordaza, en donde el conjunto de yunque incluye una placa de yunque que tiene una superficie de contacto con tejido que define una ranura alargada que se extiende longitudinalmente y una pluralidad de cavidades de formacion de grapas.
La unidad de carga incluye un refuerzo quirurgico dispuesto de forma desmontable contra al menos una de las superficies de contacto con tejido del conjunto de cartucho, y la superficie de contacto con tejido del conjunto de yunque, en el que cada refuerzo quirurgico incluye una parte de cuerpo configurada y dimensionada para cubrir la pluralidad de ranuras de retencion de grapas y la pluralidad de cavidades de formacion de grapas de los respectivos conjunto de cartucho y conjunto de yunque, en donde la parte de cuerpo define un borde distal y un borde proximal.
La unidad de carga incluye una longitud de hilo de sutura que asegura un refuerzo quirurgico respectivo contra al menos una de las superficies de contacto con tejido del conjunto de cartucho y la superficie de contacto con tejido del conjunto de yunque, en el que la sutura se extiende transversalmente a traves del refuerzo quirurgico respectivo.
La unidad de carga incluye una caractenstica de retencion que asegura cada sutura al respectivo conjunto de cartucho y conjunto de yunque.
Porciones de la sutura continua pueden extenderse a traves de ambas partes, una parte distal y una parte proximal de cada refuerzo quirurgico. Porciones de la sutura continua pueden extenderse a traves de una superficie posterior de los respectivos conjunto de cartucho y conjunto de yunque. Porciones de la sutura continua estan enroscadas a traves del refuerzo quirurgico a lo largo de los lados opuestos del refuerzo quirurgico.
La caractenstica de retencion puede incluir un adhesivo que adhiere la sutura a una superficie de los respectivos conjunto de cartucho y conjunto de yunque. El adhesivo puede adherir la sutura a multiples lugares a lo largo de la superficie de los respectivos conjunto de cartucho y conjunto de yunque. El adhesivo puede adherir la parte de la sutura a la superficie del conjunto de cartucho en un lugar cerca del borde distal del refuerzo quirurgico. El adhesivo puede adherir la parte de la sutura a la superficie del conjunto de yunque en un lugar cerca del borde proximal del refuerzo quirurgico.
Cada refuerzo quirurgico puede incluir una pestana que se extiende desde cada borde lateral de la parte de cuerpo, y una abertura formada en cada pestana, en donde la sutura se extiende a traves de cada abertura.
Cada conjunto de cartucho y conjunto de yunque incluye una pluralidad de surcos formados en una superficie lateral de los respectivos conjunto de cartucho y conjunto de yunque, en donde cada surco esta configurado y dimensionado para recibir una longitud de hilo de sutura en ellos, y en donde los surcos estan formados cerca de bordes laterales distales opuestos y bordes laterales proximales opuestos de los respectivos conjunto de cartucho y conjunto de yunque.
Segun un aspecto adicional de la presente descripcion, se proporciona un conjunto de cartucho de grapas quirurgico configurado para usarse con un aparato grapador quirurgico. El conjunto de cartucho de grapas comprende un cuerpo de cartucho de grapas que define una superficie de contacto con tejido; una ranura central de cuchilla que se extiende longitudinalmente formada en el cuerpo de cartucho; y una pluralidad filas de grapas de que se extienden longitudinalmente desde cavidades retenedoras de grapas formadas en el cuerpo de cartucho.
El conjunto de cartucho de grapas ademas comprende una grapa quirurgica cargada en cada ranura de retencion de grapas; una pluralidad de empujadores soportados deslizantemente para traslacion vertical en el cuerpo de cartucho y asociados operativamente con las grapas quirurgicas; y un trineo de accionamiento soportados deslizantemente en traslacion longitudinal en el cuerpo de cartucho y asociados operativamente con la pluralidad de empujadores.
El conjunto de cartucho de grapas tambien comprende un refuerzo quirurgico dispuesto de forma desmontable contra la superficie de contacto con tejido del cuerpo de cartucho, en el que el refuerzo quirurgico incluye una parte de cuerpo configurada y dimensionada para cubrir la pluralidad de ranuras de retencion de grapas del cuerpo de

cartucho, en donde la parte de cuerpo del refuerzo qulrurglco define un borde distal y un borde proximal.
El conjunto de cartucho de grapas adlclonalmente comprende una longltud continua de hllo de sutura que asegura el refuerzo qulrurglco contra la superflcle de contacto con tejldo del cuerpo de cartucho, en donde la sutura se extlende transversalmente a traves del refuerzo qulrurglco, en donde porclones de la sutura contlnua se extlenden a traves de ambas partes, una parte dlstal y una parte proxlmal de cada refuerzo qulrurglco; y una caractenstlca de retenclon que asegura la sutura al cuerpo de cartucho, en donde la caractenstlca de retenclon lncluye un adheslvo que adhlere la sutura a una superflcle del cuerpo de cartucho.
Porclones de sutura contlnua pueden extenderse a traves de una superflcle posterlor del cuerpo de cartucho. Porclones de sutura contlnua estan enroscadas a traves del refuerzo qulrurglco a lo largo de lados opuestos del refuerzo qulrurglco.
El adheslvo puede adherlr la sutura a lo largo de la superflcle del cuerpo de cartucho. El adheslvo puede adherlr la parte de la sutura a la superflcle del cuerpo de cartucho en un lugar cerca del borde dlstal del refuerzo qulrurglco.
El refuerzo qulrurglco puede lnclulr una pestana que se extlende desde cada borde lateral de la parte de cuerpo, y una abertura formada en cada pestana, en donde la sutura se extlende a traves de cada abertura.

Una DLU 100 incluye un cartucho que recibe la mordaza 102 configurada para recibir selectivamente un conjunto de cartucho 200 que aloja una pluralidad de sujetadores quirurgicos o grapas 223 (ver Fig. 4). Una DLU 100 incluye ademas una mordaza del yunque 104, que soporta un conjunto de yunque 300, fijado de forma movil en relacion al cartucho que recibe la mordaza 102. El conjunto de mango 12 incluye un miembro de mango fijo 22, un miembro de mango movil 24, y una parte cilmdrica 26. Una palanca de articulacion 30 esta montada en el extremo delantero de la parte cilmdrica 26 adyacente al miembro giratorio 28 para facilitar la articulacion de la DLU 100. Un par de perillas 32 estan colocadas de forma desplazable a lo largo de la parte cilmdrica 26. Las perillas 32 se hacen avanzar distalmente para aproximar o cerrar las mordazas 102, 104, y son retrafdas proximalmente para alejar o abrir las mordazas 102, 104. El accionamiento del miembro de mango movil 24 aplica Imeas de grapas 223 al tejido. Con el fin de orientar adecuadamente las mordazas 102, 104 con respecto al tejido a grapar, el aparato grapador quirurgico 10 esta provisto adicionalmente de un miembro giratorio 28 en el extremo delantero de la parte cilmdrica 26. La rotacion del miembro giratorio 28 con respecto al conjunto de mango 12 hace girar el cuerpo alargado 14 y la DLU 100 con respecto al conjunto de mango 12 para orientar adecuadamente las mordazas 102, 104 con respecto al tejido a grapar.
Como se muestra en las Figs. 2-4, el cartucho que recibe la mordaza 102 esta configurado y dimensionado para recibir selectivamente unas grapas un conjunto de cartucho 200 en el mismo. El conjunto de cartucho 200 de grapas incluye ranuras 222 de retencion formadas en el mismo para recibir una pluralidad de sujetadores o grapas 223 y empujadores 226. Se forma una ranura 234 longitudinal central y se extiende a lo largo de la longitud del conjunto de cartucho 200 de grapas para facilitar el paso a traves de una hoja de cuchilla 156 de una barra de accionamiento 150 del aparato grapador quirurgico 1o. Durante el uso del aparato grapador quirurgico 10, un trineo de accionamiento 228 se traslada a traves del conjunto de cartucho 20o de grapas para hacer avanzar las cunas de leva del trineo de accionamiento 228 en contacto secuencial con empujadores 226, para hacer que los empujadores 226 se trasladen verticalmente dentro de las ranuras 222 de retencion e impulsar las grapas 223 de las ranuras 222 en las cavidades de formacion de grapas 312 de la placa de yunque 310 del conjunto de yunque 300 (ver Figs. 2 y 8). Se puede hacer referencia nuevamente al documento de Patente U.S. No. 8.011.555, para una discusion detallada de la construccion y funcionamiento de un conjunto de cartucho 200 de grapas.
Como se muestra en las Figs. 4-9, al menos uno de los cartuchos que recibe la mordaza 102 (que incluye un conjunto de cartucho 200 de grapas), y la mordaza del yunque 104 (que incluye conjunto de yunque 300), incluyen un refuerzo quirurgico 500 asegurado operativamente a una respectiva superficie de contacto con el tejido.
Como se muestra en las Figs. 3, cada refuerzo quirurgico 500 incluye una parte del cuerpo 502 central rectangular configurada y dimensionada para cubrir al menos la parte de las ranuras 222 de retencion de grapas del conjunto de cartucho 200 de grapas, y cavidades de formacion de grapas 312 de la placa de yunque 310 del conjunto de yunque 300. La parte del cuerpo 502 define un borde distal 502a, un borde proximal 502b, y un par de bordes laterales 502c, 502d.
Cada refuerzo quirurgico 500 incluye una primera lengueta o pestana 504a que se extiende desde un primer borde lateral 502c de la parte del cuerpo 502, y una segunda lengueta o pestana 504b que se extiende desde un segundo borde lateral 502d de la parte del cuerpo 502. Cada lengueta 504a, 504b define una respectiva abertura 506a, 506b formada en la misma, en donde las aberturas 506a, 506b estan ubicadas aproximadamente en un punto medio de una longitud de la parte del cuerpo 502. Cada lengueta 504a, 504b tiene una configuracion sustancialmente triangular. Se contempla que el refuerzo 500 puede incluir un Imea de espesor reducido a lo largo de los bordes laterales 502c, 502d para permitir que las lenguetas 504a, 504b se doblen fuera del piano con respecto a un piano definido por la parte del cuerpo 502. La Imea de espesor reducido tambien puede usarse para facilitar la remocion de la lengueta o lenguetas de la parte principal o del cuerpo del refuerzo.
El material del cual se forma el refuerzo quirurgico 500 puede ser bioabsorbible o no bioabsorbible. Debe entenderse que cualquier combinacion de materiales naturales, sinteticos, bioabsorbibles y no bioabsorbibles puede usarse para formar el material de refuerzo. El material de refuerzo puede ser poroso o no poroso, combinacion de capas de poroso y no poroso. El material de refuerzo no poroso puede utilizarse para retrasar o impedir el crecimiento hacia el interior del tejido de los tejidos circundantes, actuando de ese modo como una barrera de adhesion e impidiendo la formacion de tejido cicatrizal no deseado.
Los materiales ejemplares adicionales para refuerzos quirurgicos 500 para usarse con dispositivos de grapado quirurgico descritos en la presente memoria, se exponen en la patente de titularidad compartida U.S. Nos. 5.542.594; 5.908.427; 5.964.774; y 6.045.560, y en la solicitud de patente de titularidad compartida U.S. Nos. 2006/0085034, presentada en Abril 20, 2006; y 2006/0135992, presentada en Junio 22, 2006.
En una realizacion, refuerzos quirurgicos 500 pueden fabricarse de un material biocompatible y bioabsorbible adecuados. Los refuerzos quirurgicos 500 tambien pueden fabricarse de un material no absorbente que no retenga fluido, por ejemplo, los refuerzos quirurgicos 500 pueden fabricarse de "BIOSYNTm" (un poliester sintetico, disponible comercialmente en Tyco Healthcare Group, LP d/b/a COVIDIEN, North Haven, CT), que esta hecho de "gLyCOMER 631" (un copoKmero en bloque) que es un poliester sintetico compuesto de glicosido, dioxanona y

carbonato de trimetileno. El refuerzo puede ser un material de pelfcula, malla, tejido, no tejido, trenzado material u otro material. Puede ser hecho por moldeo, estirado, tejido, utilizando tecnicas soplado fundido o hilado continuo, u otras tecnicas.
Un bloque del copoUmero resultante contiene unidades combinadas aleatoriamente derivadas de p-dioxanona (1.4- dioxan-2-ona) y carbonato de trimetileno (1.3-dioxan-2-ona). Un segundo bloque del copoKmero contiene unidades combinadas aleatoriamente derivadas de glicosido y p dioxanona. El poliester resultante es un terpolfmero tribloque ABA que posee aproximadamente 60% de glicosido, aproximadamente 14% de dioxanona, y aproximadamente 26% de carbonato de trimetileno. Puede usarse otro copolfmero de glicosido carbonato de trimetileno para hacer el refuerzo.
Los refuerzos quirurgicos 500 pueden asegurarse (p. ej., del fabricante) sobre el conjunto de yunque 300 y/o en el conjunto de cartucho que recibe la mordaza 102/conjunto del cartucho 200 durante el proceso de manufactura (p. ej., precargados) o antes del procedimiento quirurgico mientras se esta en la sala de cio^a. Refuerzos quirurgicos 500 adicionales o reemplazos para el conjunto de yunque 300 y/o conjunto de cartucho que recibe la mordaza 102/conjunto del cartucho 200 pueden asegurarse al conjunto de yunque 300 o al cartucho que recibe la mordaza 102/conjunto del cartucho 200 segun sea necesario.
Como se muestra en las Figs. 4-6, el refuerzo quirurgico 500 se asegura al cartucho que recibe la mordaza 102 y al conjunto de cartucho 200 quirurgico con una longitud de hilo de una primera sutura "SI." Se desea que la longitud de hilo de sutura sea continua de tal forma que solo se use una unica hebra para cada refuerzo. En cualquiera de las realizaciones descritas en la presente memoria, mas de una hebra puede usarse para asegurar el refuerzo. Con el refuerzo quirurgico 500 colocado contra la superficie de contacto con tejido de conjunto de cartucho 200 quirurgico, de tal forma que parte del cuerpo 502 de refuerzo quirurgico 500 se superpone a las ranuras 222 de retencion de grapas del conjunto de cartucho 200 de grapas, se usa una primera sutura "SI" para sujetar, atar o tambien asegurar el refuerzo quirurgico 500 al conjunto de cartucho que recibe la mordaza 102/conjunto del cartucho 200.
En particular, se extiende transversalmente una primera porcion "Sla" de una primera sutura "SI" a traves de un extremo distal del conjunto de cartucho 200 de grapas (y un extremo distal de cartucho que recibe la mordaza 102) en una proximidad relativa al borde distal 502a del refuerzo quirurgico 500. Una primera sutura "SI" se extiende despues para enrollarse alrededor de cada lado del conjunto de cartucho 200 de grapas (y un extremo distal de cartucho que recibe la mordaza 102) de tal forma que un par de segundas porciones "Sib" de una primera sutura "Si" se extienden por debajo del conjunto de cartucho 200 de grapas (y un extremo distal de cartucho que recibe la mordaza 102). El par de segundas porciones "Sib" de la primera sutura "Si" se cruzan y despues se enroscan a traves de las respectivas aberturas 506a, 506b de la lengueta 504a, 504b, de tal forma que un par de terceras porciones "S1c" de una primera sutura "Si" se engancha a las respectivas lenguetas 504a, 504b. La primera sutura "Si" se extiende una vez mas para enrollarse alrededor de cada lado de conjunto de cartucho 200 de grapas (y ahora, una parte media del cartucho que recibe la mordaza 102), de tal forma que un par de cuartas porciones “Sid” de una primera sutura "Si" se extienden por debajo del conjunto de cartucho 200 de grapas (y de media parte de cartucho que recibe la mordaza 102). El par de cuartas porciones “Sid” de una primera sutura "Si" se cruza y luego se extiende para enrollarse alrededor cada lado del conjunto de cartucho 200 de grapas (y un extremo proximal de cartucho que recibe la mordaza 102) de tal forma que el par de cuartas porciones “Sid” de una primera sutura "Si" se extiende por encima del conjunto de cartucho 200 de grapas (y el extremo proximal del cartucho que recibe la mordaza 102). Los extremos restantes libres "Sle" (p. ej., un par de quintas porciones) de una primera sutura "Si" se atan o aseguran entre sf, mientras se extiende transversalmente a traves del extremo proximal del conjunto de cartucho 200 de grapas (y el extremo proximal del cartucho que recibe la mordaza 102) en una proximidad relativamente cerca del borde proximal 502b de refuerzo quirurgico 500.
Segun la presente descripcion, se proporciona una caractenstica de retencion para asegurar una primera porcion "Sla" de una primera sutura "Si" en una proximidad relativamente cerca del borde distal 502a del refuerzo quirurgico 500. En particular, la caractenstica de retencion puede incluir un toque de adhesivo 110 aplicado contra el conjunto de cartucho 200 de grapas (o un extremo distal de cartucho que recibe la mordaza 102) en un lugar para adherir o asegurar la primera porcion "Sla" de una primera sutura "Si" en una proximidad relativamente cerca del borde distal 502a del refuerzo quirurgico 500 antes de que las segundas porciones "Sib" de una primera sutura "Si" se extiendan alrededor de cada lado y por debajo del conjunto de cartucho 200 de grapas (y un extremo distal del cartucho que recibe la mordaza 102). Alternativamente, en cualquiera de las realizaciones descritas en la presente memoria, la sutura puede unirse a la mordaza usando soldadura, propiedades mecanicas, u otras tecnicas. En realizaciones alternas, tambien puede proporcionarse una caractenstica de retencion para asegurar la quinta porcion "Sle" de una primera sutura "Si" en una proximidad relativamente cerca del borde proximal 502b del refuerzo quirurgico 500.
Adicionalmente, se adhiere o se asegura una primera porcion "Sla" de una primera sutura "Si" al conjunto del cartucho de grapas 200 (o un extremo distal del cartucho que recibe la mordaza 102) en un lugar de tal forma que una primera porcion "Sla" de una primera sutura "Si" se extiende transversalmente a traves de la ranura 234 longitudinal central del conjunto de cartucho 200 de grapas.
Segun la presente descripcion, se contempla que las caractensticas de retencion pueden incluir una virada de una

primera sutura "Si" contra el conjunto de cartucho 200 de grapas (o un extremo distal de cartucho que reclbe la mordaza 102), en un lugar para mantener una primera porcion "Sla" de una primera sutura "Si" en una proximidad relativamente cerca del borde distal 502a del refuerzo quirurgico 500, por un proceso de soldadura o similares.
Segun la invencion, como se muestra en la Fig. 10, se contempla que el conjunto de cartucho 200 puede incluir un surco o muesca 112a formada en cada superficie lateral del conjunto de cartucho 200 de grapas (y/o cartucho que recibe la mordaza 102), en un lugar para mantener primera porcion "Sla" de una primera sutura "SI" en una proximidad relativamente cerca del borde distal 502a del refuerzo quirurgico 500, y el cual esta configurado para recibir un segmento de una primera sutura "SI" en el mismo. Un surco o muesca ll2b tambien puede formarse en cada superficie lateral del conjunto de cartucho 200 de grapas (y/o cartucho que recibe la mordaza 102), en un lugar para mantener la quinta porcion "Sle" de una primera sutura "Si" en una proximidad relativamente cerca del borde proximal 502b de refuerzo quirurgico 500, y el cual esta configurado para recibir un segmento de una primera sutura "Si" en el mismo.
Puede hacerse referenda al documento de Solicitud de Patente numero de Serie U.S. 13/404.134, presentado en Febrero 24, 2012, y al documento de Patente U.S. 8.011.555, para una descripcion detallada de la configuracion de caractensticas de retencion de tipo de surco o tipo de muesca.
Segun la presente descripcion, se contempla que el refuerzo quirurgico 500 puede asegurarse o atarse unicamente al conjunto de cartucho 200 de grapas. Como tal, un conjunto de cartucho 200 de grapas, que incluye un refuerzo quirurgico 500 asegurado o atado al mismo, puede cargarse en el cartucho que recibe la mordaza 102 del aparato grapador quirurgico 10.
Como se muestra en las Figs. 7-9, se asegura el refuerzo quirurgico 500 a la mordaza del yunque 104 por una longitud continua de una segunda sutura "S2." Con el refuerzo quirurgico 500 ubicado contra la superficie de contacto con tejido de la placa de yunque 310 del conjunto de yunque 300 (o mordaza del yunque 104), de tal forma que parte del cuerpo 502 de refuerzo quirurgico 500 se superpone a cavidades de formacion de grapas 312 de la placa de yunque 31o, la segunda sutura "S2" se usa para sujetar, atar o tambien asegurar el refuerzo quirurgico 500 al conjunto de yunque 300 (y/o mordaza del yunque 104).
En particular, una primera porcion "S2a" de la segunda sutura "S2" se extiende transversalmente a traves de un extremo distal de la mordaza del yunque 104 en una proximidad relativa al borde distal 502a del refuerzo quirurgico 500. La segunda sutura "S2" se extiende despues para enrollarse alrededor de cada lado del conjunto de yunque 300 (y/o mordaza del yunque 104) de tal forma que un par de segundas porciones "S2b" de la segunda sutura "S2" se eXtiende por debajo del conjunto de yunque 300 (y/o mordaza del yunque 104). El par de segundas porciones "S2b" de la segunda sutura "S2" se cruzan y despues se enroscan a traves de las respectivas aberturas 506a, 506b de las lenguetas 504a, 504b, de tal forma que un par de terceras porciones "S2c" de la segunda sutura "S2" se engancha en las respectivas lenguetas 504a, 504b. La segunda sutura "S2" se extiende una vez mas para enrollarse alrededor de cada lado del conjunto de yunque 300 (y/o mordaza del yunque 104) de tal forma que un par de cuartas porciones "S2d" de la segunda sutura "S2" se extienden por debajo del conjunto de yunque 300 (y/o mordaza del yunque 104). El par de cuartas porciones "S2d" de la segunda sutura "S2" se cruzan y despues se extienden para enrollarse alrededor de cada lado del conjunto de yunque 300 (y/o mordaza del yunque 104) de tal forma que el par de cuartas porciones "S2d" de segunda sutura "S2" se extiende por encima del conjunto de yunque 300 (y/o mordaza del yunque 104). Los extremos libres restantes "S2e" (p. ej., un par de quintas porciones) de la segunda sutura "S2" se atan o sujetan entre s^ mientras se extiende transversalmente a traves del extremo proximal del conjunto de yunque 300 (y/o mordaza del yunque 104) en una proximidad relativa al borde proximal 502b del refuerzo quirurgico 500.
Tambien segun la presente descripcion, se proporciona una caractenstica de retencion para asegurar las quintas porciones "S2e" de la segunda sutura "S2" en una proximidad relativamente cerca del borde proximal 502b de refuerzo quirurgico 500. En particular, la caractenstica de retencion puede incluir un toque de adhesivo 120 aplicado contra el conjunto de yunque 300 (y/o mordaza del yunque 104) en un lugar para mantener las quintas porciones "S2e" de la segunda sutura "S2" en una proximidad relativamente cerca del borde proximal 502b de refuerzo quirurgico 500 despues de que las quintas porciones "S2e" estan unidas o tambien unidas entre sf sobre grapas al conjunto de yunque 300 (y/o mordaza del yunque 104). Preferiblemente, el toque de adhesivo 120 se aplica a una superficie posterior del conjunto de yunque 300 (y/o mordaza del yunque 104). En realizaciones alternativas una caractenstica de retencion tambien puede proporcionarse para asegurar la primera porcion "S2a" de la segunda sutura "S2" en una proximidad relativamente cerca del borde distal 502a de refuerzo quirurgico 500.
Adicionalmente, la primera porcion "S2a" de la segunda sutura "S2" se adhiere o se asegura al conjunto de yunque 300 (y/o mordaza del yunque 104) en un lugar de tal forma que la primera porcion "S2a" de la segunda sutura "S2" se eXtiende transversalmente a traves del centro de una cuchilla longitudinal que recibe la ranura formada en la placa del yunque 310 del conjunto de yunque 300.
En una realizacion, se contempla que las caractensticas de retencion pueden incluir la virada de la segunda sutura "S2" contra el conjunto de yunque 300 (y/o mordaza del yunque 104) en un lugar para mantener la quinta porcion "S2e" de la segunda sutura "S2" en una proximidad relativamente cerca del borde proximal 502a de refuerzo

Como se muestra en las Figs. 10, se contempla que el conjunto de yunque 300 puede incluir un surco o muesca 114a formado en una superficie lateral del conjunto de yunque 300 (y/o mordaza del yunque 104), en un lugar para mantener la primera porcion "S2a" de la segunda sutura "S2" en una proximidad relativamente cerca del borde distal 502a de refuerzo quirurgico 500, el cual esta configurado para recibir un segmento de la segunda sutura "S2" en el mismo. Un surco o muesca 114b tambien puede formarse en cada superficie lateral del conjunto de yunque 300 (y/o mordaza del yunque 104), en un lugar para mantener la quinta porcion "S2e" de la segunda sutura "S2" en una proximidad relativamente cerca del borde proximal 502b del refuerzo quirurgico 500, y el cual esta configurado para recibir un segmento de segunda sutura "s2" en el mismo.
Se puede hacer referenda nuevamente al documento de Solicitud de Patente numero de Serie U.S. 13/404.134, presentado en Febrero 24, 2012, y al documento de Patente U.S. 8.011.555, para una descripcion detallada de la configuracion de las caractensticas de retencion del tipo de surco o tipo de muesca.
Se proporcionan refuerzos 500 para reforzar y sellar Imeas de grapas aplicadas al tejido por un aparato grapador quirurgico 10. El conjunto de cartucho 200 y conjunto de yunque 300 estan particularmente configurados para permitir que refuerzos quirurgicos 500 se localicen en superficies enfrentadas hacia adentro del conjunto de cartucho 200 y del conjunto de yunque 300 con el fin de facilitar el paso del aparato grapador quirurgico 10 en el cuerpo de un paciente sin riesgo de desgarro o arrugamiento de los respectivos refuerzos cuando el aparato grapador quirurgico 10 se inserta y se manipula dentro del cuerpo de un paciente.
En funcionamiento, con la DLU 100 acoplada a un extremo distal del cuerpo alargado 14 del aparato grapador quirurgico 10, y con refuerzos 500 cargados sobre cada de conjunto de yunque 300 y conjunto de cartucho 200, el aparato grapador quirurgico 10 se usa segun metodos conocidos por expertos en la tecnica. Una vez el conjunto de yunque 300 y el conjunto de cartucho 200 estan sujetados sobre tejido, se dispara el aparato grapador quirurgico 10. Al disparar el aparato grapador quirurgico 10, una hoja de cuchilla 156
A medida que la hoja de cuchilla 156 comienza a desplazarse distalmente, la hoja de cuchilla 156 corta sustancialmente simultaneamente a traves de una seccion central de las quintas porciones "Sle, S2e" de las respectivas primera y segundas suturas "SI, S2" del conjunto de yunque 300 y conjunto de cartucho 200, de ese modo liberando respectivamente los extremos proximales del cartucho de refuerzo 500a del conjunto de cartucho 200. Aflojando asf la sutura "SI" alrededor del cartucho refuerzo 500a. A medida que la hoja de cuchilla 156 se mueve distalmente, la hoja de cuchilla troza o corta longitudinalmente a traves de ambos refuerzos 500, dividiendo sustancialmente de ese modo los refuerzos 500 por la mitad.
Adicionalmente, a medida que la hoja de cuchilla 156 se aproxima a la posicion mas distal, la hoja de cuchilla 156 corta a traves de la primera porcion "Sla" de la primera sutura "SI" que se extiende transversalmente a traves de la ranura 234 longitudinal central del conjunto de cartucho 200 de grapas y a traves de la primera porcion "S2a" de la segunda sutura "S2" que se extiende transversalmente a traves del centro de una cuchilla longitudinal que recibe la ranura 314 (ver Figs. 2 y 8) formada en la placa de yunque 310 del conjunto de yunque 300, de ese modo liberando respectivamente los extremos distales del refuerzo de yunque 500b del conjunto de yunque 300. Aflojando asf las suturas "S2" alrededor del refuerzo de yunque 500b.
Puesto que las suturas "SI, S2" estan adheridas o tambien aseguradas a los respectivos componentes subyacentes (p. ej., cartucho mordaza 102 y/o conjunto de cartucho 200, y conjunto de mordaza 104 y/o conjunto de yunque 300), las suturas "SI, S2" permanecen adheridas o tambien aseguradas al mismo despues de un disparo del aparato grapador quirurgico lO y durante la extraccion o retirada de la DLU 100 del lugar de operacion, retirando tambien de ese modo las suturas "Si, S2" del lugar de operacion.
En cualquiera de las realizaciones descritas en la presente memoria, las suturas pueden comprender grapas, cables, Imeas, pestanas, correas o similares y estan hechos de materiales no absorbibles o bioabsorbibles. En cualquiera de las realizaciones descritas en la presente memoria, la sutura se extiende a traves de la ranura de la cuchilla y se corta con la cuchilla por lo menos una vez. Deseablemente, la sutura se une a los componentes de grapadora tales como el conjunto de yunque, conjunto de cartucho o canal, o ambos, de tal forma que cada pieza de una o mas piezas de sutura por refuerzo se retira con el instrumento de grapado despues de que se dispara.
Aunque las realizaciones descritas anteriormente sobre grapadoras quirurgicas que incorporan el uso de hojas de cuchilla moviles para cortar y liberar refuerzos quirurgicos del tejido que contacta superficies del conjunto de yunque y el conjunto de cartucho se han mostrado y descrito en relacion con grapadoras quirurgicas endoscopicas, se preve, y dentro del alcance de la presente descripcion, que cualquiera de las realizaciones anteriores pueden incorporate a cualquier tipo de grapadora quirurgica, que incluye y no limita a grapadoras quirurgicas abiertas, tales como, por ejemplo, grapadoras quirurgicas lineales y grapadoras quirurgicas transversales.
Se entendera que pueden hacerse varias modificaciones a las realizaciones descritas en la presente memoria. Claims (6)
- 5101520253035404550reivindicaciones1. Una unidad (100) de carga de grapado quirurgico que incluye una primera mordaza (102) y una segunda mordaza (104) moviles entre sf para realizar una operacion de grapado quirurgico mediante la cual se disparan grapas en el tejido, en donde la unidad de carga comprende:un conjunto de cartucho (200) soportable en la primera mordaza y la segunda mordaza, en el que el conjunto de cartucho tiene una superficie de contacto con tejido que define una ranura (234) alargada que se extiende longitudinalmente y una pluralidad de ranuras de retencion (222) de grapas formadas en la misma, en donde el conjunto de cartucho incluye una grapa (223) quirurgica cargada en cada ranura de retencion de grapas;un conjunto de yunque (300) soportable en otra mordaza, la primera mordaza y la segunda mordaza, en el que el conjunto de yunque incluye una placa de yunque que tiene una superficie de contacto con tejido que define una ranura alargada que se extiende longitudinalmente y una pluralidad de cavidades de formacion de grapas;un refuerzo quirurgico (500) dispuesto de forma desmontable contra al menos una de las superficies de contacto con tejido del conjunto de cartucho y la superficie de contacto con tejido del conjunto de yunque, en donde el o cada refuerzo quirurgico incluye una parte de cuerpo configurada y dimensionada para cubrir la pluralidad de ranuras de retencion de grapas o la pluralidad de cavidades de formacion de grapas de los respectivos conjunto de cartucho y conjunto de yunque, en donde la parte de cuerpo define un borde distal y un borde proximal;una longitud respectiva de hilo de sutura ("S2") que asegura un refuerzo quirurgico respectivo contra al menos una de las superficies de contacto con tejido del conjunto de cartucho y la superficie de contacto con tejido del conjunto de yunque, en donde la sutura se extiende transversalmente a traves del refuerzo quirurgico respectivo; yuna caractenstica (114) de retencion que asegura la longitud de hilo de sutura al respectivo conjunto de cartucho y conjunto de yunque en el que la caractenstica de retencion comprende una pluralidad de surcosformados en una superficie lateral de los respectivos conjunto de cartucho y conjunto de yunque, en donde los surcos estan formados cerca de bordes laterales distales opuestos y bordes laterales proximales opuestos de los respectivos conjunto de cartucho y conjunto de yunque;ycaracterizado por quepartes de la longitud de hilo de sutura estan enroscadas a traves del refuerzo quirurgico a lo largo de los lados opuestos del refuerzo quirurgico, y en que cada surco esta configurado y dimensionado para recibir, y esta en recepcion de, una longitud de la longitud de hilo de sutura en el mismo.
- 2. La unidad de carga segun la reivindicacion 1, en la que partes de la longitud de hilo de sutura se extienden a traves de ambas partes, una parte distal y una parte proximal de cada refuerzo quirurgico; y/o en donde partes de la respectiva longitud de hilo de sutura se extienden a traves de una superficie posterior de los respectivos conjunto de cartucho y conjunto de yunque
- 3. La unidad de carga segun la reivindicacion 2, en la que la caractenstica de retencion incluye un adhesivo (12) que adhiere la longitud de hilo de sutura a una superficie de los respectivos conjunto de cartucho y conjunto de yunque.
- 4. La unidad de carga segun la reivindicacion 3 en la que el adhesivo adhiere la longitud de hilo de sutura a multiples lugares a lo largo de la superficie de los respectivos conjunto de cartucho y conjunto de yunque; y/oen donde el adhesivo adhiere la parte de la longitud de hilo de sutura a la superficie del conjunto de cartucho en un lugar cerca del borde distal del refuerzo quirurgico; y/oen donde el adhesivo adhiere la parte de la longitud de hilo de sutura a la superficie del conjunto de yunque en un lugar cerca del borde proximal del refuerzo quirurgico.
- 5. La unidad de carga segun cualquier reivindicacion 1 a 4, en la que cada refuerzo quirurgico incluye una pestana que se extiende desde cada borde lateral de la parte de cuerpo, y una abertura formada en cada pestana, en la que la longitud de hilo de sutura se extiende a traves de cada abertura.
- 6. Un aparato grapador quirurgico que comprende una unidad de carga de grapado quirurgico segun reivindicado en cualquiera las reivindicaciones 1 a 5 que comprende adicionalmente:una carcasa;un mango soportado por la carcasa; yun cuerpo alargado que se extiende distalmente desde la carcasa y que tiene un extremo distal adaptado para acoplarse de forma desmontable la unidad de carga.
