DESCRIPCIONDESCRIPTION
Dispositivo de asistencia ventricular.Ventricular assist device.
OBJETO DE LA INVENCIONOBJECT OF THE INVENTION
The present invention can be included within the technical field of the manufacture of medical devices, in particular, in the elaboration of devices used in cardiac surgery.
More specifically, the object of the invention relates to a ventricular assist device.
ANTECEDENTES DE LA INVENCIONBACKGROUND OF THE INVENTION
The failure of one of the ventricles of the heart in its function as a pump constitutes a dramatic situation that can lead to the death of a patient. Currently, one of the procedures used to treat cases of severe ventricular failure are ventricular assist devices, hereinafter referred to as VAD, by its acronym in English (Ventricular Assist Devices).
A VAD is a device capable of replacing the pumping function of a damaged ventricle. It comprises a pump intended to aspirate blood from the damaged ventricle through a first cannula inserted into the tip of the ventricle and propel said blood into the artery corresponding to said ventricle, through a second cannula anastomosed to said artery.
The implantation of a VAD requires the realization of a sternotomy, (opening of the sternum to access the mediastinum and the pericardium with opening of the latter) the dissection of a segment of the corresponding artery to perform the anastomosis of the second cannula, the performance of the said anastomosis with vascular suture, and the placement of the first cannula at the tip of the ventricle by means of a tobacco bag suture. On many occasions, implantation is even more complex, since it requires the establishment of an extracorporeal circulation.
DESCRIPCION DE LA INVENCIONDESCRIPTION OF THE INVENTION
The present invention solves the technical problem posed, by means of a ventricular assist device (VAD) equipped with a cannula with two concentric ducts and a pump to boost blood.
As explained below, the VAD device of the invention allows blood to be propelled from a ventricle affected by insufficiency (the left, normally), in the corresponding artery (in the case of the left ventricle, the aortic artery; in the case of the right ventricle, the pulmonary artery).
As anticipated above, the VAD according to the invention comprises a cannula incorporating an inner duct and an outer duct, where the outer duct is arranged externally and coaxially with respect to the inner duct.
En el caso del ventriculo derecho, la valvula sigmoidea es la pulmonar y en el caso del izquierdo, laThe inner duct is provided with a first proximal end intended to be inserted from the tip of the ventricle, crossing, in the direction of flow, a sigmoid valve that connects the ventricle with its corresponding artery, to a position inside the artery. In the case of the right ventricle, the sigmoid valve is the pulmonary and in the case of the left, the

aortica. El conducto interior comprende al menos una abertura de impulsion, preferentemente dispuesta en el primer extremo, para impulsar sangre hacia la arteria.aortic The inner duct comprises at least one discharge opening, preferably arranged at the first end, to drive blood into the artery.
The outer duct is provided with at least one suction opening, preferably arranged in the most proximal area of the outer duct, to aspirate blood from the ventricle, intended to be arranged inside the ventricle when the impulse opening is arranged in the artery.
The VAD additionally comprises a pump intended to be disposed externally to the patient's body, whose drive is connected to the impulsion openings of the inner duct and whose aspiration is connected to the aspiration openings of the outer duct, such that said pump sucks blood from the ventricle, through the aspiration openings, said blood conducts through the outer duct, externally to the inner duct, to the pump, and drives said blood inside the inner duct, into the artery, through the impulse openings .
The VAD of the invention, since it incorporates in a single cannula both the inner duct, connected to the pump drive, and the outer duct, connected to the suction of said pump, only requires for its implantation a single incision, which it is practiced to insert the cannula from the first end of the inner duct, through the tip of the ventricle. It is not necessary, therefore, to additionally practice an anastomosis in the artery.
Adicionalmente, la ubicacion de la bomba exteriormente al ventrlculo presenta una serie de efectos tecnicos ventajosos adicionales, entre los que se destacan los siguientes:Additionally, the location of the pump externally to the ventricle has a series of additional advantageous technical effects, among which the following stand out:
- el dimensionamiento de la bomba presenta una mayor libertad puesto que no esta sometida a las restricciones de espacio propias de estar alojada en el ventrlculo, por lo que se pueden emplear bombas suficientemente grandes como para generar el caudal de sangre necesario, sin restricciones de espacio;- the sizing of the pump has a greater freedom since it is not subject to the space restrictions of being housed in the ventricle, so that pumps large enough can be used to generate the necessary blood flow, without space restrictions ;
The device can be more compact, since the cannula only needs a length slightly longer than the minimum necessary to access the artery.

Next, a detailed description of a preferred embodiment of the invention is described with the aid of the attached figures 1 and 2.
La canula (1) comprende un conducto interior (3), as! como comprende adicionalmente un conducto exterior (4), que esta dispuesto exteriormente y de manera coaxial respecto delAs can be seen in Figure 1, the ventricular assist device (VAD) according to the present invention comprises a cannula (1) and an axial type rotary pump, for driving blood. conducto interior (3).inner duct (3).
The inner duct (3) comprises a first proximal end (7), provided with impulse openings (8) to allow blood to flow out, and a second distal end (9) (see Figure 2), as! as the outer duct (4) comprises a third proximal end (10), provided with aspiration openings (11) to allow blood to enter, and a fourth distal end (12) (see Figure 2). The dimensions and configurations of the inner duct (3) and the outer duct (4) are such that the impulse openings (8) occupy a position more proximal than the suction openings (11).
In a more distal position than the suction openings (11), a rotary pump of axial type is arranged. The pump comprises a rotor (13) and an axial impeller (14). The impeller (14) is located in a widening (15) of the second end (9) of the inner duct (3), while the rotor (13) is arranged in a chamber (16) located at the most distal end of the cannula (1), then and outside the first duct (3) and the second duct (4).
In the embodiment shown in the figures, the chamber (16) forms part of the most distal part of the cannula (1), but the invention is not limited to said arrangement, said chamber (16) being able to be independent of the cannula (1 ) and even be attached to said cannula (1) in a detachable manner.
El conducto interior (3) comprende unos pasajes (17) proximos al cuarto extremo (12) para permitir el paso de sangre desde el conducto exterior (4) hacia la camara (16).The inner duct (3) comprises passages (17) near the fourth end (12) to allow the passage of blood from the outer duct (4) to the chamber (16).
The widening (15) is, at its most proximal end, communicated with the second end (9) of the inner duct (3), so that blood can pass from said widening (15) to the inner duct (3). The figures show a support (20) intended to support the impeller (14), said support (20) defines channels (19) for the passage of blood. However, other options are possible, such as, for example, a magnetic levitation of the impeller (14), where support (20) would not be required, also avoiding the channels (19).
The widening (15) is isolated from the chamber (16) by means of a separator (18), to avoid contact between the rotor (13) and the blood. For this reason, the rotor (13) and the impeller (14) are magnetically coupled and not through a physical coupling.
Next, the operation of the VAD of the invention is described. The VAD is designed to replace the pumping function of a ventricle suffering from eventual or chronic insufficiency, during a more or less long period that can last an observation or a treatment.
The cannula (1) is introduced, from the first end (7) of the inner duct (3), through the tip of the patient's ventricle, according to some of the known techniques, for example, by means of a mini-throttle in the case that the patient does not have the thorax open.
The cannula (1) is guided through the ventricle to pass the first end (7) through the corresponding sigmoid valve, until the discharge openings (8) reach the corresponding artery. A pressure record helps determine if the first end has already accessed the artery, since the typical blood pressure records are differentiable from the typical ventricular pressure records.
Una vez dispuestas las aberturas de impulsion (8) en un punto de la arteria, la configuration delOnce the discharge openings (8) are arranged at a point in the artery, the configuration of the
La bomba 10 impulsa la sangre, a traves de los canales (19) ubicados en el soporte (20), hacia el conducto interior (3).At this time, once the VAD is correctly placed, the pump is activated, after the corresponding purging of the air, whereby the blood of the ventricle is aspirated through the suction openings (11) of the outer duct (4). The subsequent blood path is shown with arrows in Figure 2. From the aspiration openings (11) the blood is conducted along the outer duct (4) to the passages (17), where it accesses through the duct inside (3) to the widening (15) and from there to the impeller (14) of the pump. The pump 10 drives the blood, through the channels (19) located in the support (20), into the inner duct (3).
Finally, the driven blood travels through said inner duct (3) and access, through the impulse openings (8), to the artery.
