Abstract

Implante para insertar entre cuerpos vertebrales de la columna vertebral, como apartador para vértebras o porciones de vértebras, retiradas de la columna ver- tebral, con una primera parte (2) del implante, y una segunda parte (3) del implante, que pueden ajustarse una respecto a la otra en la dirección de su eje longitudinal co- axial, para la modificación de la longitud del implante (1), caracterizado porque a la primera parte (2) del implante está coordinado un anillo (4) roscado giratorio que en- grana con una rosca (5) anular en una rosca (6) coordinada a la segunda parte (3) del implante, y porque el anillo (4) roscado está provisto con un dentado (7) de rueda cónica.

La invención se refiere a un implante para insertar entre cuerpos vertebrales de la columna vertebral, como apartador para vértebras o porciones de vértebras, retiradas de la columna vertebral, con una primera parte del implante, y una segunda parte del implante, que pueden ajustarse una respecto a la otra en la dirección de su eje longitudinal coaxial, para la modificación de la longitud del implante. La invención se refiere, además, a un instrumento quirúrgico, con el que puede conseguirse la modificación de longitud del implante.
Un implante del tipo citado en el preámbulo, se conoce, por ejemplo, por el documento DE 44 23 257 A1, que se ha acreditado muy bien en la práctica, y con el que se pudieron obtener muy buenos éxitos curativos después de la implantación. Pero, no obstante, durante la implantación en la operación con la columna vertebral abierta, se ha mostrado como parcialmente problemático, que para el atornillado de la primera parte del implante respecto a la segunda parte del implante, se necesita un gran campo operativo, puesto que la rotación mutua tiene que conseguirse mediante movimientos de giro. Pero en todo caso, no se dispone de espacio suficiente para semejantes movimientos de giro, en especial, cuando en el sentido de una cirugía mínimamente invasiva, solamente se colocan accesos pequeños a la columna vertebral.
Por consiguiente la misión de la invención se basa en configurar un implante del tipo citado en el preámbulo, de manera que también pueda conseguirse una modificación de la longitud del implante, sin que para ello sea necesario un movimiento de giro del instrumento quirúrgico previsto para el accionamiento del implante. Además, es misión de la invención facilitar un instrumento quirúrgico que sea apropiado especialmente bien para la interacción con el implante según la invención.
La parte de la misión que se refiere al implante, se resuelve con un implante del tipo citado en el preámbulo, haciendo que a la primera parte del implante esté coordinado un anillo roscado giratorio que engrana con una rosca anular en una rosca coordinada a la segunda parte del implante, y que el anillo roscado esté provisto con un dentado de rueda cónica.
Este implante ofrece la gran ventaja de que a causa del dentado de rueda cónica, puede conseguirse una rotación de la segunda parte del implante mediante un instrumento quirúrgico aplicado de forma puramente radial al implante, tan sólo mediante su torsión. Con ello, para la modificación de la longitud es suficiente un acceso puntual al implante, con lo que se evita una herida aumentada tan sólo con el fin de la separación.
Según una forma preferente de realización de la invención, está previsto que la primera parte del implante y la segunda parte del implante estén moldeadas como casquillos tubulares, y que la primera parte del implante con el anillo roscado rodee por fuera la segunda parte del implante. Esta estructuración de la invención utilizando dos casquillos, ofrece la ventaja de que el espacio interior del implante está libre y puede utilizarse, por ejemplo, para rellenarlo con cemento para huesos o virutas óseas propias del paciente o ajenas, para dejar crecer eficazmente el implante. Para impedir un agarrotamiento o frenado automático de la rosca durante la modificación de la longitud del implante al atornillar la segunda parte del implante respecto a la primera parte del implante, el implante está estructurado de manera que en la segunda parte del implante esté configurada al menos una rendija de guía para el alojamiento de una espiga coordinada a la primera parte del implante. Con objeto de conseguir una guía mejorada, se ha acreditado aquí cuando la rendija de guía y la espiga están previstas por duplicado en disposición diametral.
Para facilitar el accionamiento del anillo roscado mediante un instrumento quirúrgico, en la pared de la primera parte del implante, de forma de casquillo, existe al menos una abertura que corta el borde coordinado al anillo roscado. Esta abertura puede utilizarse como un apoyo para el instrumento quirúrgico a aplicar al implante.
Para no tener que limitar la guía del instrumento quirúrgico al espesor de la pared del casquillo, alineados con la abertura, existen dos agujeros alargados en la segunda parte del implante. Estos agujeros alargados permiten también que opuesto a la abertura, en la pared de la primera parte del implante, esté configurado un taladro roscado, que entonces puede alcanzarse desde la abertura.
Con vistas a un crecimiento más rápido del implante se ha confirmado, además, como favorable, cuando en la segunda parte del implante, en el extremo más alejado de la primera parte del implante, está configurada una corona. Esta corona aumenta la superficie de apoyo de la segunda parte del implante en el cuerpo vertebral. Si la corona está dispuesta distanciada del extremo libre de la segunda parte del implante, puede servir como seguro para el cemento para huesos a aportar, y al mismo tiempo proteger la rosca contra el atasco por cemento para huesos. Como alternativa es posible igualmente que en la corona estén configurados agujeros que discurren paralelos al eje longitudinal del implante.
Para conseguir la mayor longitud posible de guía, del anillo roscado en la rosca, el implante está estructurado de manera que el anillo roscado presente un collarín anular en la periferia interior.
Para poder conseguir un aseguramiento de la posición del anillo roscado respecto a la primera parte del implante, el implante según la invención está estructurado, además, de tal manera que la primera parte del implante presente una ranura transversal configurada en el casquillo tubular, y que discurre perpendicular al eje longitudinal del implante. Por conveniencia, esta ranura transversal está situada paralela al eje de unión de la abertura y del taladro roscado. Además, es favorable cuando la ranura transversal está configurada a los dos lados del eje de unión, en el casquillo tubular de la primera parte del implante.
La parte de la misión que se refiere al instrumento quirúrgico se resuelve mediante un instrumento quirúrgico para la separación de un implante que presente un dentado de rueda cónica, y pueda ajustarse en longitud mediante una unión roscada, para insertar entre cuerpos vertebrales de la columna vertebral, como apartador para vértebras o porciones de vértebras, retiradas de la columna vertebral, estando dispuesto en el extremo libre de un árbol, un piñón con un dentado de rueda cónica, coaxial al eje del árbol. Gracias a este instrumento quirúrgico, mediante el árbol que se aleja radialmente del implante, y que está orientado perpendicular al eje longitudinal del implante, solamente por torsión alrededor del eje longitudinal del árbol, puede girarse el dentado de rueda cónica del piñón, que al interaccionar con el dentado de rueda cónica del anillo roscado del implante, provoca la rotación necesaria de la primera parte del implante y de la segunda parte del implante, una con relación a la otra.
Para la manipulación más fácil del instrumento quirúrgico durante la operación, está previsto que el árbol esté apoyado giratorio en un mango, y para el accionamiento esté provisto con una rueda moleteada en el extremo más alejado del piñón.
Una forma especialmente preferente de realización del instrumento quirúrgico, está caracterizada porque el árbol está estructurado como árbol hueco en el que está apoyada una barra giratoria respecto al árbol hueco, que atraviesa el piñón con su extremo libre, y en la que está configurado un espárrago roscado que corresponde a la rosca del taladro roscado. Esto ofrece la ventaja de que puede asegurarse la posición de la primera parte del implante respecto al instrumento quirúrgico, mediante el espárrago roscado, de manera que al ejercer la presión necesaria del piñón para engranar el dentado de rueda cónica del piñón con el dentado de rueda cónica del anillo roscado, no se provoque una modificación de longitud del apartador ya colocado entre los cuerpos vertebrales de la columna vertebral.
Otra forma preferente de realización de la invención, está caracterizada porque el árbol está dispuesto en un casquillo fijado al mango, que en el extremo coordinado al piñón, presenta una horquilla con puntas de la horquilla que producen un aseguramiento de la posición axial mutua del anillo roscado y de la primera parte del implante. Aquí se ha demostrado como favorable cuando la horquilla presenta en total cuatro puntas de la horquilla que sujetan el implante, dos de las cuales se agrupan en un primer par de horquilla y están previstas para el apoyo en el anillo roscado, y dos se agrupan en un segundo par de horquilla, y están previstas para la introducción en las ranuras transversales. Con un instrumento quirúrgico semejante puede asirse el implante con mucha seguridad, y sujetarse en un lugar predefinido, no existiendo, a pesar de la torsión requerida del árbol para la modificación de la longitud, peligro ninguno de que el implante se corra en el campo de la operación, o que se desplacen el primer implante y el segundo implante uno respecto al otro, y así adopten una extensión longitudinal que no sea provocada por un giro del anillo roscado, y esté protegido por este.
Con vistas a condiciones higiénicas mejoradas en la desinfección, es conveniente cuando el árbol con la rueda moleteada, la barra y el mango con el casquillo, estén montados desmontables.

El implante 1 representado en la figura 1, sirve para la inserción entre cuerpos vertebrales de la columna vertebral, no representados en la propia figura, como apartador para vértebras o porciones de vértebras, retiradas de la columna vertebral. Este implante 1 presenta una primera parte 2 del implante y una segunda parte 3 del implante, las cuales pueden ajustarse una respecto a otra en la dirección de su eje longitudinal coaxial, para en el sentido de una separación de los cuerpos vertebrales, poder conseguir una modificación de longitud. La primera parte 2 del implante y la segunda parte 3 del implante, están moldeadas como casquillos tubulares, rodeando la primera parte 2 del implante, la segunda parte 3 del implante por la parte de fuera. A la primera parte 2 del implante está coordinado un anillo 4 roscado giratorio que, en su superficie periférica interior, presenta una rosca 5 anular con la que aquella engrana en una rosca 6 coordinada a la segunda parte 3 del implante. En la superficie periférica exterior, el anillo 5 roscado está provisto con un dentado 7 de rueda cónica (figura 8).
Como se reconoce en especial en las figuras 3 y 6, en la segunda parte 3 del implante están configuradas dos rendijas 8 opuestas de guía, en las que penetran espigas 9 coordinadas a la primera parte 2 del implante (figura 2).
Junto a estas espigas 9, la primera parte 2 del implante presenta, además, una abertura 10 que corta el borde 11 coordinado al anillo 4 roscado, y está situada en una recta con un taladro 12 roscado configurado en el lado opuesto. En la segunda parte 3 del implante, con la abertura 10 están alineados dos agujeros 13 alargados, a cuyo través puede alcanzarse el taladro 12 roscado en cualquier posición en altura de la segunda parte 3 del implante respecto a la primera parte 2 del implante.
En la segunda parte 3 del implante, distanciada de su extremo libre, en el lado más alejado de la primera parte 2 del implante, está configurada una corona 14 en la que están configurados agujeros 15 que discurren paralelos al eje longitudinal del implante 1.
El anillo 4 roscado presenta en la periferia interior, un collarín 16 anular, mientras la primera parte 2 del implante presenta una ranura 17 transversal configurada en el casquillo tubular, que discurre perpendicular al eje longitudinal del implante 1, y que está situada paralela al eje de unión de la abertura 10 y el taladro 12 roscado, estando prevista la ranura 17 transversal, en el ejemplo de realización representado en el dibujo, duplicada a los dos lados del eje de unión.
El collarín 16 anular configurado en el anillo 4 roscado, así como las ranuras 17 transversales, sirven para la mejor orientación y determinación de la posición de un instrumento 18 quirúrgico a describir en detalle a continuación, y que sirve para la manipulación y accionamiento del implante 1 en el ajuste de la longitud.
Para ello, el instrumento 18 quirúrgico presenta en el extremo libre de un árbol 19, un piñón 20 coaxial al eje del árbol. En este piñón 20 está configurado un dentado 21 de rueda cónica (figura 9, figura 10). El árbol 19 mismo está estructurado como árbol hueco en el que está apoyada una barra 22 giratoria respecto al árbol hueco. Esta barra 22 atraviesa el piñón 20, y presenta en su extremo libre, un espárrago 23 roscado que corresponde al taladro 12 roscado. El mismo árbol hueco está apoyado giratorio en un mango 24 que presenta, además, un casquillo 25 en el que está introducido el árbol hueco. Este casquillo 25 posee en el extremo coordinado al piñón 20, una horquilla 26 con cuatro puntas 27 de la horquilla, dos de las cuales están agrupadas para formar un primer par 28 de horquilla, y dos lo están para formar un segundo par 29 de horquilla.
Después de haber expuesto precedentemente la estructura del implante 1 y del instrumento 18 quirúrgico, a continuación debe de explicarse detalladamente su interacción.
Para colocar el implante 1 según la invención, como apartador entre dos cuerpos vertebrales, primeramente se mete el implante 1 en la horquilla 26, introduciéndose el segundo par 29 de horquilla en las dos ranuras 17 transversales, y llegando a apoyar el primer par 28 de horquilla en el anillo 4 roscado, y radialmente hacia dentro, en el collarín 16 anular, de manera que esté bloqueado un desplazamiento axial del anillo 4 roscado en el eje longitudinal del implante 1, respecto a la primera parte 2 del implante. A continuación se lleva la barra 22 con el espárrago 23 a través de la abertura 10 de la primera parte 2 del implante, así como de los dos agujeros 13 alargados de la segunda parte 3 del implante, y se atornilla en el taladro 12 roscado de la primera parte 2 del implante, de manera que el instrumento 18 quirúrgico no pueda retirarse más del implante 1 en dirección radial, con lo que la posición del instrumento 18 quirúrgico está asegurada globalmente respecto al implante 1. Gracias al atornillado del espárrago 23 en el taladro 12 roscado, se engrana al mismo tiempo el dentado 21 de rueda cónica del piñón 20, con el dentado 7 de rueda cónica del anillo 4 roscado, de manera que en la subsiguiente rotación del árbol 19 alrededor de su eje del árbol mediante una rueda 30 moleteada, se lleve a cabo una rotación del anillo 4 roscado alrededor del eje longitudinal del implante 1, girado 90º respecto al eje del árbol.
Después de la colocación correcta y de la separación del implante 1, puede soltarse de nuevo el instrumento 18 quirúrgico en el orden inverso lógico de las etapas antes descritas, del implante 1 que gracias a las rendijas 8 de guía entonces accesibles libremente, los agujeros 13 alargados y los taladros 12 roscados, así como la abertura 10, ofrece suficientes posibilidades de acceso en el interior de la primera parte 2 del implante y de la segunda parte 3 del implante, de forma tubular, para introducir en el implante cemento para huesos o material óseo propio u homólogo.
La pieza adicional representada en la figura 1, designada con el símbolo 31 de referencia, sirve para el acoplamiento del instrumento 18 quirúrgico con transmisores de posición para la navegación computerizada.

Claims (20)

1. Implante para insertar entre cuerpos vertebrales de la columna vertebral, como apartador para vértebras o porciones de vértebras, retiradas de la columna vertebral, con una primera parte (2) del implante, y una segunda parte (3) del implante, que pueden ajustarse una respecto a la otra en la dirección de su eje longitudinal coaxial, para la modificación de la longitud del implante (1), caracterizado porque a la primera parte (2) del implante está coordinado un anillo (4) roscado giratorio que engrana con una rosca (5) anular en una rosca (6) coordinada a la segunda parte (3) del implante, y porque el anillo (4) roscado está provisto con un dentado (7) de rueda cónica.
2. Implante según la reivindicación 1, caracterizado porque la primera parte (2) del implante y la segunda parte (3) del implante están moldeadas como casquillos tubulares, y porque la primera parte (2) del implante con el anillo (4) roscado rodea por fuera la segunda parte (3) del implante.
3. Implante según la reivindicación 1 ó 2, caracterizado porque en la segunda parte (3) del implante está configurada al menos una rendija (8) de guía para el alojamiento de una espiga (9) coordinada a la primera parte (2) del implante.
4. Implante según la reivindicación 3, caracterizado porque la rendija (8) de guía y la espiga (9) están previstas por duplicado en disposición diametral.
5. Implante según alguna de las reivindicaciones 2 a 4, caracterizado porque en la pared de la primera parte (2) del implante, de forma de casquillo, existe al menos una abertura (10) que corta el borde (11) coordinado al anillo (4) roscado.
6. Implante según la reivindicación 5, caracterizado porque alineados con la abertura (10), existen dos agujeros (13) alargados en la segunda parte (3) del implante.
7. Implante según la reivindicación 6, caracterizado porque opuesto a la abertura (10), en la pared de la primera parte (2) del implante, está configurado un taladro (12) roscado.
8. Implante según alguna de las reivindicaciones 1 a 7, caracterizado porque en la segunda parte (3) del implante, en el extremo más alejado de la primera parte (2) del implante, está configurada una corona (14).
9. Implante según la reivindicación 8, caracterizado porque la corona (14) está dispuesta distanciada del extremo libre de la segunda parte (3) del implante.
10. Implante según la reivindicación 8 ó 9, caracterizado porque en la corona (14) están configurados agujeros (15) que discurren paralelos al eje longitudinal del implante (1).
11. Implante según alguna de las reivindicaciones 1 a 10, caracterizado porque el anillo (4) roscado presenta en la periferia interior un collarín (16) anular.
12. Implante según alguna de las reivindicaciones 2 a 11, caracterizado porque la primera parte (2) del implante presenta una ranura (17) transversal configurada en el casquillo tubular, y que discurre perpendicular al eje longitudinal del implante (1).
13. Implante según la reivindicación 12, caracterizado porque la ranura (17) transversal está situada paralela al eje de unión de la abertura (10) y del taladro (12) roscado.
14. Implante según la reivindicación 13, caracterizado porque la ranura (17) transversal está configurada a los dos lados del eje de unión en el casquillo tubular de la primera parte (2) del implante.
15. Instrumento quirúrgico para la separación de un implante (1) que presenta un dentado (7) de rueda cónica, y puede ajustarse en longitud mediante una unión roscada, para insertar entre cuerpos vertebrales de la columna vertebral, como apartador para vértebras o porciones de vértebras, retiradas de la columna vertebral, según alguna de las reivindicaciones 1 a 14, caracterizado porque en el extremo libre de un árbol (19), coaxial al eje del árbol está dispuesto un piñón (20) coaxial al eje del árbol, con un dentado (21) de rueda cónica.
16. Instrumento quirúrgico según la reivindicación 15, caracterizado porque el árbol (19) está apoyado giratorio en un mango (24), y para el accionamiento está provisto con una rueda (30) moleteada en el extremo más alejado del piñón (20).
17. Instrumento quirúrgico según la reivindicación 15 ó 16, caracterizado porque el árbol (19) está estructurado como árbol hueco en el que está apoyada una barra (22) giratoria respecto al árbol hueco, que atraviesa el piñón (20) con su extremo libre, y en la que está configurado un espárrago (23) roscado que corresponde a la rosca del taladro (12) roscado.
18. Instrumento quirúrgico según la reivindicación 16 ó 17, caracterizado porque el árbol (19) está dispuesto en un casquillo (25) fijado al mango (24), que en el extremo coordinado al piñón (20), presenta una horquilla (26) con puntas (27) de la horquilla que producen un aseguramiento de la posición axial mutua del anillo (4) roscado y de la primera parte (2) del implante.
19. Instrumento quirúrgico según la reivindicación 18, caracterizado porque la horquilla (26) presenta en total cuatro puntas (27) de la horquilla que sujetan el implante (1), dos de las cuales se agrupan para formar un primer par (28) de horquilla y están previstas para el apoyo en el anillo (4) roscado, y dos se agrupan para formar un segundo par (29) de horquilla, y están previstas para la introducción en las ranuras (17) transversales.
20. Instrumento quirúrgico según alguna de las reivindicaciones 15 a 19, caracterizado porque el árbol (19) con la rueda (30) moleteada, la barra (22) y el mango (24) con el casquillo (25), están montados desmontables.
