Definitions
- the present invention relates to a device for haemostatic medication in anorectal or vaginal canals.
- the present invention relates to a device used for haemostatic purposes following surgical operations located within the anorectal or vaginal canal.
- haemostatic action inside anal or anorectal and vaginal canals is carried out manually by means of tampons generally constituted by sterile gauze adapted to the anatomic conformation of the anal and vaginal canal.
- the gauze is compacted and possibly imbibed with vasoconstrictor drugs (e.g. epinephrine) and with local anaesthetics. It is also possible to prepare, as the need arises, gauze tampons for haemostatic purposes coated with a medical product, such as oxidised cellulose.
- vasoconstrictor drugs e.g. epinephrine
- the medical product constituted by oxidised cellulose, regenerated to acid pH, with similar molecular structure to haemoglobin, has a haemostatic effect due to a mechanical action by fluid absorption and pressure.
- This device with the appearance of a white-yellowish fabric, can be conformed and adapted to all surfaces to be treated and can be positioned manually by itself or on a support made of gauze or another material, inside the anal and vaginal canal.
- tampons used in proctologic and gynaecologic surgery constituted by a sponge made of a gel of porcine origin (Spongostan), which is positioned manually or by means of an anoscope in the anal and vaginal canal for haemostatic purposes.
- the haemostatic principle of the sponge is based on its high absorption capacity that causes its swelling and hence the mechanical compression action on the area of interest, and on its ability to facilitate platelet aggregation and activation, once it is in contact with blood.
- the sponge is provided with a central hole through which a venting tube for intestinal gases can be inserted.
- thrombin splits the fibrinogen into fibrin that causes the collagen to adhere to the surface of the wound. Coagulation occurs in 3-5 minutes and it is facilitated by the presence of local compression.
- the formation of a stable clot entails the formation of a compact grating with good adhesive and sealing properties.
- the honeycomb structure of the collagen support fixes the sponge to the surface of the lesion, forming a barrier to liquids and gases. With the accompanying compression the structure of the sponge collapses and the result is a thin film provided with considerable elasticity and flexibility that gives it ability to adapt to the surface of the organs.
- haemostatic tampons described above present an important drawback, mainly linked to their application methodology.
- the technical task at the basis of the present invention is to propose a device for haemostatic medication in anorectal or vaginal canals that overcomes the aforementioned drawbacks of the prior art.
- an object of the present invention is to make available a device for haemostatic medication able to be applied correctly regardless of the positioning of the wound to be medicated.
- An additional object of the present invention is to make available a device for haemostatic medication that is able to measure the depth of insertion of the medicament suitable for haemostasis.
- the specified technical task and the objects set out above are substantially achieved by a device for haemostatic medication in anorectal or vaginal canals, comprising the technical characteristics exposed in one or more of the appended claims.
- FIG. 1 shows a perspective, partially sectioned view of a device for haemostatic medication in accordance with the present invention
- FIG. 1 is lateral elevation, partially sectioned views of the device of figure 1 in respective operating sequences of application to an anorectal canal;
- figure 5 is a perspective and partially sectioned view of the device of figure 1 in a respective condition of non utilization.
- the reference number 1 indicates in its entirety a device for haemostatic medication in anorectal or vaginal canals 2.
- the device 1 comprises a tampon 3 adapted to be inserted into an anorectal or vaginal canal 2 and suitable for a haemostatic action.
- the tampon 3 is inserted into the canal 2 at a surgical wound that determines the bleeding of the inner wall of the canal 2.
- the tampon 3 is constituted by a cylindrical portion 4 made of elastically yielding material and presenting a first end 4a oriented towards the interior of the respective canal 2, and a second end 4b opposite to the first one 4a.
- the cylindrical portion 4 is preferably made with a gauze compressed or rolled to obtain a cylindrical conformation with circular section.
- the cylindrical portion 4 can be constituted by a material with high absorbing power to guarantee the absorption of endoanal (or vaginal) fluids, and with soft consistency to assure tolerability and maximum comfort to the patient who temporarily bears it.
- the cylindrical portion 4 presents a respective outer surface 5, advantageously coated with a haemostatic drug.
- the cylindrical portion 4 can be coated with fibrinogen and human thrombin (latest generation TachoSil) or with a medical-surgical product (oxidized and regenerated cellulose: Tabotamp).
- the tampon 3 further comprises a tubular body 6 extending along the longitudinal development of the cylindrical portion 4.
- the cylindrical portion 4 is wound around a first terminal portion 6a of the tubular body 6, in such a way as to house said first portion 6a inside the cylindrical portion 4.
- the tubular body 6 presents a first opening 7 positioned at the first end 4a of the cylindrical portion 4. Said first opening 7 is obtained in an anchoring disk 8 able to fasten the first portion 6a of the tubular body 6 to the cylindrical portion 4.
- a second portion 6b develops, extending outside the cylindrical portion 4, away from the second end 4b of the cylindrical portion 4 itself.
- a second opening 9 able to place in fluid communication the interior of the canal 2 with the outside environment.
- the tubular body 6 allows to drain intestinal gases (or vaginal fluids) and enables to remove the cylindrical portion 4 at the end of its haemostatic function.
- the tampon 3 described above is therefore inserted into the respective canal 2 at the surgical site of interest (anal, rectal, vaginal sutures), with local haemostasis purposes, linked to the intrinsic haemostatic capacity of the drug or induced by the medical product that may be used.
- the haemostatic action is assured by the pressure action exerted by the cylindrical portion 4 made of elastically yielding material, which at the same time assures absorption of the fluids.
- the device 1 further comprises a member 10 for applying the tampon 3, to insert the tampon at a predetermined depth inside the respective canal.
- the application member 10 comprises an internally hollow body 1 1 able to contain the aforementioned tampon 3.
- the hollow body 11 has substantially tubular conformation with circular section and it defines an outlet opening 12 through which the tampon 3 is ejected from the hollow body 11 to be inserted into the canal 2.
- an inlet opening 13 develops, through which the tampon 3 is fed into the hollow body 11.
- the hollow body 1 1 presents, at the outlet opening 12, a tapered area 14, deformable at the passage of the tampon 3.
- the tapered area 14 constitutes the peripheral edge of the outlet opening 12 and it is advantageously constituted by an elastic membrane 15 adaptable to the conformation of the tampon 3 when the tampon is ejected from the hollow body 11.
- the tapered area 14 can also be inserted at least partially into the canal 2 to house the tampon 3 within the canal 2 in a predefined area.
- the application member 10 is also provided with means 16 for measuring the depth of insertion of the tampon 3 into the canal 2.
- means 16 for measuring the depth of insertion of the tampon 3 into the canal 2.
- the measuring means 16 present at least one graduated scale 17, extending on the outer surface of the hollow body 11 along the longitudinal development of the hollow body 1 1.
- the graduated scale 17 develop starting from the outlet opening 12 to the inlet opening 13 and it presents a series of reference notches 17a indicating a precise depth measurement of the hollow body 11 relative to the canal 2.
- the reference notch 17a externally visible and proximate to the canal 2 defines a depth value to which the tampon 3 is inserted (figure 3 and 4).
- the application member 10 further presents an element 18 for ejecting the tampon 3 able to actuate the tampon 3 towards the canal 2 through the outlet opening 12 of the hollow body 11 (figure 3).
