Definitions

  • the human knee joint contains two tibio-femoral pairs of articular surfaces, namely a medial and a lateral pair of articular surfaces, each of which is formed by a femoral condyle and a bowl-shaped tibial or meniscal articular surface interacting therewith.
  • these anatomical relationships are simulated by two femoral, condyle-shaped articular surfaces and two tibial-side, shell-shaped articular surfaces.
  • the lateral distance between the femoral articular surfaces must correspond exactly to the distance between the tibial articular surfaces.
  • the femoral joint surfaces on the one hand and the tibial joint surfaces on the other hand are generally made in one piece (US Pat. No. 4,081,866; O / 79/007339, FIG. 36). It is also known to use O / 79/007339, FIG. 34), single prostheses that are not connected on the femur side to replace the femoral articular surfaces, which interact with shell-shaped, tibial articular surfaces connected in one piece. However, this is rejected nowadays because it cannot be guaranteed that the distance and the orientation of the femoral sliding surfaces match the tibial prosthesis part.
  • a prosthetic technique is also considered obsolete, in which unconnected femoral and tibial prosthesis parts are implanted using one and the same insertion instrument, which ensures the same sliding surface distance on both sides (US Pat. No. 3,949,428). Furthermore, in those cases in which an articular surface replacement takes place only on one side (femoral or tibial) or where the precise alignment of the interacting articular surfaces is not important, it is known to implant the unconnected prosthesis parts by means of an insertion instrument which ensures their parallelism (EP- A 824 904;. Link: Brochure "SKI cup-knee joint system").
  • the system also includes an insertion tool for the condyle shells so that they also assume exactly the position relative to one another that the condyles of the one-piece component have to one another, which gives them a relative position matched to the tibial sliding recesses.
  • the invention makes it possible to use one and the same knee prosthesis system for different damage states. If the entire sliding surfaces require replacement, the one-piece condylar component is used. If, on the other hand, the patella sliding surfaces are still in good condition, only the condylar sliding surfaces are replaced. In addition, it is possible to replace the condyle shells with a one-piece femoral component by means of a comparatively smaller intervention if it should emerge over time that the patellar gliding surfaces also need to be replaced. Finally, an intraoperative change from the initially intended, less extensive variant to the further variant is also possible.
  • the condylar resection surfaces which are required for the insertion of the condylar shells, match the corresponding resection surfaces of the one-piece femoral component. This not only has the advantage that the same saw template can be used. Rather, it also makes the transition from one femoral prosthesis variant to another easier.
  • the associated saw cuts should not only be made in the area of the condyles, but also in the intermediate area. The latter is not necessary when using condyle shells. It can even be harmful if - depending on the position of the resection incisions - a part of the femoral sliding surface area of the kneecap could also be caught as a result.
  • the saw guide which is used for both variants of femoral prostheses, has a locking projection in the intermediate condyle area, which allows the surgeon to protect the intermediate condyle area or even to protect the condyle area. limit the cuts on the sides facing each other exactly according to the intended position of the edges of the condyle shells.
  • the limits of the condyle shells can be chosen in any form. However, it is useful if they turn parallel, straight side surfaces. Then their precise alignment on the instrument is easier if it has matching parallel connection surfaces.
  • the prosthesis system according to the invention normally contains several size levels with different distances between the condyle shells. Accordingly, the invention provides that the mutual distance between the parallel connection surfaces of the insert instrument can be set differently in stages. Their adjustability is expediently based on the use of interchangeable parts, which are each assigned to only one step, because this reduces the risk of incorrect setting of the distance between the condyle shells.

  • FIG. 2 shows the same with a two-piece femoral component

  • the tibial plateau 2 contains correspondingly shaped troughs 7 for receiving the condylar sliding surfaces 6.
  • Fig. 2 shows the embodiment ariante in which the one-piece femoral component 3 is replaced by a pair of condyle shells 10 which are shaped exactly the same as the condyle parts 5 of the one-piece femoral component 3; so to speak, they are cut off from the one-piece component.
  • Your Sliding surfaces 6 correspond exactly to the sliding surfaces 6 of the one-piece component and interact in the same way with the sliding troughs 7 of the tibial plateau, provided that their relative positioning corresponds exactly to that of the condyle parts 5 of the one-piece component.

It consists of a handle 31, a mounting plate 32 and two clamps 33 for receiving the condyle shells 10. • the clamps 33 have clamping jaws 35 on the inside, which form parallel connecting surfaces 36 for the inner edges 37 of the condyle shells.

  • the clamps 33 are formed by a movable jaw 40 which is guided parallel to the fixed inner jaw 35.
  • the guidance takes place in the example shown by guide rods 41, which interact with corresponding bores in the terminal base 42, which is rigidly connected to the inner, fixed jaw 35.
  • the presentation takes place by means of a threaded spindle 43 and a rotary knob 44 connected to it on the outside.
  • ken 35, 40 are provided with mutually facing projections 46.
  • the edges of the condyle shells contain recesses 47 which match these projections. The projections and recesses are elongated and thereby force the condyle shells into a predetermined direction in the insertion instrument.
  • a pin 45 protrudes from the underside of the terminal base 42 and is designed to match a receiving opening 48 in the holding plate 32.
  • the pin 45 and the receiving opening 48 are non-rotatable in relation to one another, in the example shown thanks to a square cross section.
  • the pins can be secured in the receiving openings by means of screws 49.
  • the two receiving openings 48 in the mounting plate 32 run parallel to one another.
  • a separate mounting plate 32 is provided for each size level belonging to the prosthesis system.
  • the mounting plates of different sizes differ in their width in the LM direction and in the spacing of their receiving openings 48.
  • the associated clamps 33 can match. Instead, it would also be possible to use different clamps and one and the same mounting plate, the arrangement of the pins 47 on the clamps providing the desired spacing between them. Finally, it is also conceivable to use only one mounting plate, which has a larger number of receiving openings 48, which make it possible to place clamps at different distances.
  • Each clamp 33 contains between the jaws 35, 40 the wide head 50 of a set screw guided in the clamp base 42. The surface of the head 50 is covered with a resilient material, such as polyethylene.
  • the set screw 50 is screwed against the sliding surface facing it and supports it. It is therefore also referred to in the claims as a sliding surface support. It serves to enable a force transmission from the insertion instrument to the condyle shells in the placement direction when the shells are placed on the bone, without the forces having to be transmitted from the projections 46.
  • the sliding surface stubs 50 contribute to the correct positioning of the condyle shells in the insertion instrument if the movable jaw 40 has play relative to the other jaw 35 or if the projections 46 have play relative to the recesses 47 accommodating them.
  • a pair of condyle shells 10 is to be placed on the resected bones, they are first gripped by the insertion instrument in the manner explained. As shown in FIG. 7, this is then placed on the bone with the condyle shells.
  • the mounting plate 32 with the handle part 31 of the insertion instrument is first removed by loosening the screws 49 and pulling off the mounting plate 32 from the clamps 33 and their pins 45. This is possible because both clamps 33 are connected to the holding plate 32 in the same release direction of their pins 45. Subsequently, the clamps 33 can be released from the shells by screwing the movable jaw 40 back after the sliding surface support 50 has been loosened beforehand, if necessary.
  • the insertion tool is removed after they have been adequately anchored to the bone with cement or without cement. If there are trial implants, they are previously attached to the bone by means of fixing pins 60, for which holes 61 are provided in the condyle shells. These lie outside the area covered by the clamps, so that the pins 60 can be inserted as long as the prosthesis shells are still fixed by the insertion instrument. At the point where the implants have an anchoring pin 14, the trial implants contain a bore 62 of corresponding size. If a hole for receiving the pins had not previously been made in the bone, this can be made through the hole 62
  • Trial implants are made in the right place.
  • a fixing bolt 63 is then pressed into the bore 62 and the bone bore located underneath it, which is now able to hold the trial implants instead of the removed fixing pins 60 and whose head disappears in the bore 62.
  • the implant position and the joint function can now be checked using the trial implants. After this has been removed, the final prosthesis shells are used in the same way.
