Description
Problem: With conventional instruments for flushing the paranasal sinuses, the traumatic bone perforation of the lower maxillary sinus is accepted. This means a risk of infection, requires anesthesia and requires an additional healing process. In addition, this procedure creates excess pressure in the sinus, which the patient finds unpleasant and painful. With sinusoscopy using rigid optics, flushing of the sinuses may be possible, but the expensive instrument prohibits continuous flushing over several hours.
Advantageous effects: With the invention, in the specified application, it is possible to simultaneously flush and suction the secretion, thus sparing the patient painful negative and positive pressures in the sinus.
The design also enables outpatient care of the patient through the use of battery-operated infusion pumps and exchangeable vacuum bottles, thus reducing the costly inpatient admission. By adding antibiotics and/or disinfectants such as iodine preparations, etc., it is possible to reduce the pathological flora of the mucous membrane. Additives such as adrenaline or essential substances can also make local changes to the mucous membrane possible, without the sometimes undesirable systemic side effects.
Preoperative or postoperative irrigation could promote the therapeutic effect of the operation, and in individual cases irrigation could also provide an alternative to surgery.
Before sinusoscopy, a thorough clearance of mucus is possible with the catheter and would thus increase diagnostic accuracy.
Furthermore, closing the sinus with the inflatable balloon prevents mucus and rinsing fluid from draining out of the nose, thus improving the patient's comfort.
The metal insert of the suction catheter places the catheter at the bottom of the sinus, the secretion collection point. In addition, localization is possible using X-rays or diaphanoscopy and can be corrected using the guide wire.
The balloon of the irrigation catheter ensures that the irrigation fluid always flows into the sinus above the secretion, thus allowing the mucus to be continuously diluted and not sucked out past the mucus.
Further development of the invention: Different effects on the mucous membrane can be achieved by adding different additives to the rinsing liquid. Possible additives would be: antibiotics, analgesics, sympathomimetics, mucolytics, essential substances, cytoprotectives, Hi-antagonists, cytostatics (in CA), iodine.
It would also be possible to implant the catheter intraoperatively in bony cavities, e.g. ethmoid sinus, frontalis, possibly orbit, endocranium, in which the generation of negative pressure is undesirable or continuous rinsing is to be achieved.
