BeschreibungDescription Stand der TechnikState of the art

The surgical treatment of sub- and pertrochanteric femoral fractures predominantly presents problems in the actual stabilization of the fracture, since the means available have so far only achieved inadequate fixation of the fragments due to the material and construction properties.

This resulted in the support plates bending under load and the fracture fragments slipping laterally due to a lack of support, resulting in a deterioration in gait, shortening of the leg and a longer period of inpatient treatment. On the other hand, in a construction with insufficient dynamization paths, the femoral head may be perturbed by a fixed bolt.

As a rule , this type of bone fracture occurs as a transverse fracture and therefore leads to a disability.

The mostly elderly patient can usually only be mobilized under full weight bearing in the *~~cope\ Ati'^n phase, since due to his often reduced mental capacity and lower physical strength, partial weight bearing is not possible in order to protect the osteosynthesis.

However, early mobilization of the patient is an essential prerequisite for recovery.

AufgabeTask

The aim of the development is to create a simple, stable, safe and quickly applicable osteosynthesis construction in order to avoid the above-mentioned disadvantages. The construction should be compatible with existing support screws and bone fixation screws.

VorteileAdvantages

Due to the suggested heart shape of the upper part, the tab can be easily pushed under the attachment of the vastus lateralis muscle on the trochanter major; the muscle attachment does not need to be detached as with other constructions. Spongiosa screws can be inserted via the round holes in the upper support plate for additional fragment fixation and prevention of rotation of the head and neck fragment.

The angle between the tab and the support screw sleeve is manufactured in three versions (135, 140 and 145 degrees). This sufficiently takes into account the range of anatomically specified angles.

The dynamization of the fracture fragments is achieved via a support screw sleeve inserted into the head and neck fragment. The trochanter fragments are held by the upper support plate and prevented from slipping. This results in compression of the fragments, which is necessary for fracture healing, and a largely anatomical fracture division.

The plate extension for fixation to the femoral shaft is provided with four, six or eight offset oval so-called self-compressing screw holes, so that in individual cases even deeper fractures extending into the middle femoral shaft can be treated with it.

Fig. 1 zeigt die Frontansicht der Dynamischen Trochanter - Lasche, Fig. 2 die Seitenansicht der Dynamischen Trochanter - Lasche. An example of the development is shown in Fig. 1. Fig. 1 shows the front view of the dynamic trochanter flap, Fig. 2 shows the side view of the dynamic trochanter flap.

In Fig. 1 and Fig. 2, 1 indicates the round holes for the cancellous bone screws.

In Fig. 1 and Fig. 2, 2 denotes the support screw sleeve.

In Fig. 1, the screw holes for fixing the tab to the femoral shaft are marked with 3.

In Fig. 1 and Fig. 2, 4 denotes the upper support plate and 5 the plate extension, both of which give rise to the term tab.
