Die Erfindung betrifft eine Endoprothese für den Femurkopf eines menschlichen Hüftgelenkes, bei der der Femurkopf mit Hilfe eines Schraubenbolzens und einer Mutter auf den Schenkelhals gezogen wird, wobei der Schraubenbolzen mindestens teilweise in einer Hülse durch den Schenkelhals hindurchgeführt und mit Hilfe einer Trochanterplatte abgestützt ist.
Endoprothesen der genannten Art sind bekannt (z. B. US-PS 2,612,159; 2,682,265). Je nach dem Grad der Verletzung oder Zerstörung des natürlichen Knochengewebes dienen derartige Prothesen der Osteosynthese als kappenförmige Prothesenschalen oder als Ersatz des vollständigen Femurkopfes. Bei den genannten Implantaten kommt es bei den wechselnden Belastungen zu Relativbewegungen zwischen dem Schraubenbolzen und seiner Abstützung bzw. seiner Führungshülse, obwohl zwischen beiden Elementen eine Vorspannung vorhanden ist. Diese Relativbewegungen erzeugen mechanischen Abrieb und Verschleiss, sofern Schraubenbolzen und Hülsen - wie bei den meisten Prothesen üblich - aus Metall gefertigt sind.
Da darüberhinaus zwischen diesen Teilen immer ein kleiner Spalt vorhanden ist, in den Körperflüssigkeiten eindringen können, besteht in diesem Spalt darüberhinaus die Gefahr von Spaltkorrosionen.
Aufgabe der Erfindung ist es, die Verankerung bzw. Abstützung des Schraubenbolzens im Knochen bzw. in der Hülse oder der Trochanterplatte so auszugestalten, dass sowohl der mechanische Abrieb als auch Spaltkorrosionen zwischen den beiden Metallteilen verhindert werden. erfindungsgemäss dadurch, dass zwischen dem Schraubenbolzen und der Hülse ein Kunststoff-Lagerelement vorgesehen ist, das durch den Zug des Schraubenbolzens zusammengedrückt ist.
Auf diese Weise werden die Relativbewegungen von dem Lagerelement aufgenommen, so dass zwischen den Metallkörpern kein Spiel mehr vorhanden ist und kein mechanischer Verschleiss mehr auftritt. Als Folge des Zusammenpressen beginnt das Lagerelement, das vorzugsweise aus Polyäthylen der Klassifikationen HDPE (High density polyaethylen) oder UHMW (Ultra High Molekular Weight) besteht, sich elastisch zu verformen oder sogar plastisch zu fliessen; es dringt dabei in den erwähnten Spalt zwischen Schraubenbolzen und Hülse ein. Dadurch wird auch in diesem Spalt ein direkter Kontakt zwischen Schraubenbolzen und Hülse verhindert und darüberhinaus ein Eindringen von Körperflüssigkeit in den Spalt vermieden.
Reizungen der den Femur umgebenden Weichteile durch die bei Belastungen des Femurkopfes auftretenden Mikrobewegungen des Schraubenbolzens können verhindert werden, wenn das Lagerelement in einen hülsenartigen Ansatz der Trochanter- · platte eingebettet ist, in dem das gewindetragende Ende des Schraubenbolzens und die Mutter versenkt sind.
Im folgenden wird die Erfindung anhand eines Ausführungsbeispiels im Zusammenhang mit der Zeichnung näher erläutert.
Fig. 1 zeigt - teilweise im Schnitt - die Erfindung an einer kappenartigen Prothesenschale für den
Femurkopf;
Fig. 2 gibt vergrössert ebenfalls in einem Schnitt als Detail aus Fig. 1 die Lagerung des gewindetragenden Endes des Schraubenbolzens in einer Hülse der Trochanterplatte wieder.
Eine käppenartige Prothese 1 für den Gelenkkopf, die aus Metall oder Biokeramik gefertigt ist, ist auf den operativ

vorbereiteten Femurknochen 2 aufgesetzt. Sie wird durch einen Schraubenbolzen 3, dessen Kopf 4 in bekannter Weise unter Zwischenlegen einer gefangenen Dichtung 5 aus Polyäthylen in einer zentralen Bohrung 6 der Prothese abgestützt ist, auf dem Knochen unter Vorspannung gehalten, wobei der aus einem körperverträglichen Metall bestehende Schraubenbolzen 3 als Zuganker wirkt.
Das Widerlager für den "Zuganker" bildet eine metallische Hülse 7, die an eine, ebenfalls aus Metall bestehende Trochanterplatte 8 angeschweisst ist. Die Trochanterplatte kann, wie allgemein üblich, durch nicht gezeigte Schrauben, die durch Bohrungen 9 gesteckt werden, mit dem Femur 2 verschraubt werden.
An seinem unteren Ende trägt der Schraubenbolzen 3 ein Gewinde 10 (Fig. 2), auf das eine Mutter 11 geschraubt ist; diese zieht den Schraubenbolzen 3 nach unten und presst damit die Prothesenschale 1 auf den Femurhals. Die Mutter 11 drückt dabei über eine, wiederum aus Metall bestehende Zwischenscheibe 12 erfindungsgemäss ein Kunststoff-Lagerelement 13 gegen das konisch zulaufende Ende der Hülse 7. Wie bereits beschrieben, verformt sich das Lagerelement 13 unter dem Druck der Vorspannung und schiebt sich - sich elastisch oder plastisch verformend - zwischen die Metallteile und verhindert so Abrieb und Verschleiss, sowie das Eindringen von Körperflüssigkeit in den Spalt 14.

Claims (2)

  1. Patentansprüche
    ( iJ Endoprothese für den Femurkopf eines menschlichen Hüftgelenkes, bei der der Femurkopf mit Hilfe eines Schraubenbolzens und einer Mutter auf den Schenkelhals gezogen wird, wobei der Schraubenbolzen mindestens teilweise in einer Hülse durch den Schenkelhals hindurchgeführt und mit Hilfe einer Trochanterplatte abgestützt ist, dadurch gekennzeichnet, dass zwischen dem Schraubenbolzen (3) und der Hülse (7) ein Kunststoff-Lagerelement (13) vorgesehen ist, das durch den Zug des Schraubenbolzens (3) zusammengedrückt ist.
  2. 2. Endoprothese nach Anspruch 1, dadurch gekennzeichnet, dass das Lagerelement (13) in einen hülsenartigen Ansatz (7) der Trochanterplatte (8) eingebettet ist, in dem das gewindetragende Ende (10) des Schraubenbolzens (3) und die Mutter (11) versenkt sind.
