Allgemeines:General:

Bei septischen und aseptischen Lockerungen von Hüftgelenksendoprothesen im Oberschenkel entsteht durch die unterschiedliche Bewegung zwischen Implantat und Oberschenkelschaft eine im ehemaligen Verankerungsbereich des Endoprothesenstieles ausgeschlagenes, d.h. stark erweitertes Knochenlager. Bei der notwendigen Austauschoperation (Revisionsoperation) ist es meist unmöglich, in diesem ausgeschlagenen Knochenlager erneut einen Endoprothesenstiel sicher zu verankern, daß keine feste und belastungsstabile Verbindung zwischen Oberschenkel und Implantat eintreten kann.In the case of septic and aseptic loosening of hip joint endoprostheses in the thigh, the different movement between the implant and the thigh shaft results in a bone bed that is knocked out, i.e. greatly expanded, in the former anchoring area of the endoprosthesis stem. Dazu besitzt der Revisionsstiel eine entsprechende Überlänge. Die entsprechende Form, die Überlänge sowie die dem einzelnen Oberschenkelknochen angeglichene Querschnittform des Revisionsschaftes muß geeignet sein, eine sichere und belastungsstabile diaphysäre Verklemmung und damit sichere Verbindung herzustellen. Darüber hinaus soll der Revisionsschaft in seinem hüftgelenknahen Abschnitt so geformt sein, daß er den ausgeschlagenen Teil des Oberschenkelknochens (s.o.) nach Möglichkeit weit genug ausfüllt, so daß auch dort - zumindest punktuell - eine mechanische Verbindung zwischen dem Revisionsschaft und dem Oberschenkelknochen bei der Revisionsoperation zustande kommt.The purpose of a revision stem is to achieve a firm anchoring in the medullary cavity of the femur below the knocked-out bone bed. Claims (1)

Revisionsschaft (Revisionsstiel) zu Prothesenwechseloperationen von gelockerten Hüftgelenkschäften, gekennzeichnet durch gerade verlaufenden Langstiel (Geradschaft) mit volumenbetontem hüftgelenksnahem Abschnitt.
Prothesenstiel und Prothesenhals bilden ein Werkstück. Der Revisionsschaft ist aus einer Titanlegierung, alternativ auch aus anderen biologisch verträglich und ausreichend belastbaren Materialien hergestellt.
Der Hals des Revisionsschaftes ist gekennzeichnet durch das konusförmige Ende, zur Aufnahme der gängigen künstlichen Hüftgelenkskugeln.
Der hüftgelenknahe Abschnitt des eigentlichen Revisionsschaftes ist gekennzeichnet durch einen rechteckigen Querschnitt und einer in frontal nach unten, in den distalen Bereich des Langstieles auslaufenden Dreiecksform. Die Oberfläche dieses Abschnittes, auf der ventralen und dorsalen Oberfläche ist gekennzeichnet durch längsverlaufende Flügel zur Oberflächenvergrößerung.
Der weiter in Richtung Kniegelenk verlaufende Abschnitt des Stieles des Revisionsschaftes ist gekennzeichnet als Rundstab, welcher in 4 Quadranten längsverlaufende Stege besitzt.
Stiellänge und Stieldurchmesser sind in Längen zwischen 250 mm und 360 mm und zwischen 12 mm und 16 mm Durchmesser dem Markraum des menschlichen Oberschenkels angepaßt.
