Adapter und Abziehgerät für Hüftgelenkendoprothese!!
Die Erfindung betrifft einen Adapter und entsprechendes Abziehgerät für Hüftgelenkendoprothesen, wobei der Adapter eine konusförmige Aussenkontur aufweist aufweicher eine Gelenkkugel mit gleichem Konus festsitzend angeordnet sein kann oder der Adapter auf dem konusförmigen Schaft einer Endoprothese aufsitzt, so dass zum Trennen des Adapters sowohl von der Gelenkkugel als auch von dem Schaft das Abziehgerät mit einem Kragen durch eine Öffnung im Boden des Adapters gesteckt wird und sich hinter der Öffnung verhakt, so dass ein durch den Kragen geschraubter Stift sich entweder am Gelenkkugelboden oder an der Stirnfläche des Schaftes abstützend beim Weiterschrauben des Stiftes die Teile voneinander trennt.
Durch die EP- 0498 518 ist ein Gelenkprothesensystem bekannt welches einen Prothesenschaft mit einem konusförmigen Hals, eine Gelenkkugel mit einer kegelförmig ausgebildeten Aufnahme ein napfartiges, zwischen Prothesenschaft und Gelenkkugel einsetzbares Zwischenstück (Adapter) und ein Werkzeug zum Lösen des Zwischenstücks von dem Hals des Prothesenschaft und/oder Gelenkkugel aufweist. Das Zwischenstück (Adapter) ist im wesentlichen napffbrmig ausgebildet, wobei seine Innenkontur der Außenkontur des konusförmigen Halses entspricht.
Im Bereich der Stirnfläche des Adapters ist eine durchgehende Gewindebohrung vorgesehen, in welche der Schaft des Werkzeuges, welcher an mindestens einem Ende mit einem Aussengewinde versehen ist, eingeschraubt werden kann. Zur Einleitung des entsprechenden Drehmomentes ist an dem anderen Ende des Werkzeugschafts ein Handgriff vorgesehen. Die Länge des Aussengewindes des Werkzeugschaftes ist größer als die Länge der Gewindebohrung im Boden des Adapters, so dass der Schaft des Werkzeuges beim Einschraubenin die Stirnfläche des Adapters eine Kraftwirkung zum Lösen des Adapters von der Gelenkkugel oder Prothesenschaft auslöst.
Diese bekannte Lösung weist den Nachteil auf, dass der zur Erzeugung der den Adapter lösenden Kraft vorgesehene Gewindeschaft unter Operationsbedingungen nur unter Schwierigkeiten mit dem zu lösenden Adapter verbindbar ist. Darüber hinaus ist die Handhabung des Werkzeuges sehr unbequem, da insbesondere keinerlei Elemente vorgesehen sind, mit welchen
&Igr;'·
das Werkzeug in einer optimalen Position bezüglich der Krafteinleitung in das jeweilige Prothesenteil gehalten werden kann.
Weiterhin ist durch die DE-OS 198 33 791.4 ein weiteres Gelenkprothesen-System bekannt, bestehend aus einem üblichen, mit einem Innen- und Außenkonus versehenem Adapter, in dessen Boden eine besonders ausgebildete Öffnung vorhanden ist. Diese besteht aus einer größeren zentralen Bohrung und zwei kleineren, einander diametral gegenüberliegenden, im wesentlichen schlitzförmigen Ausnehmungen. Das Abziehwerkzeug besteht aus einem hohlzylindrischen Schaftteil, durch das ein runder Dorn geführt ist, wobei das Schaftteil an der Spitze zwei einander gegenüberliegende Kragenteile aufweist, so dass der Schaft mit den Kragenteilen durch den Boden des Adapters gebracht werden kann. Wird der Schaft nach dem Durchstecken um 90° gedreht, verhaken sich die Kragenteile auf der Aussenseite des Adapters und der Dorn kann mittels Gewindeverschraubung und damit langsamer Vorwärtsbewegung gegen den Boden der Steckkugel gebracht werden, so dass ein Trennkraft erzeugt wird, die ein Lösen der Konusverbindung bewirkt.
Nachteilig an dieser Lösung zunächst ist, dass hier ein ganz bestimmtes Abziehwerkzeug ausschließlich nur zu bestimmten Adaptertypen passt, welche diesen besonders ausgebildeten Boden aufweisen. Weiterhin kann es nachteilig sein, dass sehr exakt darauf zu achten ist, dass nach dem durchstecken des Werkzeugschaftes durch den Boden eine Drehung um 90° erfolgt um ein sicheres Verhaken der Kragenteile auf der Adapteraußenseite oder alternativ im Adapterboden zu erreichen.
Der Erfindung liegt die Aufgabe zugrunde, eine Kombination von Adapter und dazu passendem Abziehgerät zu schaffen, dass sehr einfach und leicht zu handhaben ist und gleichzeitig ein sicheres Verhaken des Gerätes am bzw. im Adapter bewirkt.
Diese Aufgabe wird durch die im kennzeichnenden Teil des Anspruch 1 angegebenen Merkmale gelöst.
Ein besonderes Anliegen bei einer OP wird bestimmt durch die Schnelligkeit und Sicherheit der benutzten Geräte und Werkzeuge damit der Patient möglichst schnell eine Operation beenden kann. Gerade bei Revisionen am Hüftgelenk, bei welcher häufig ein Austausch oder Neuaufbau einzelner Gelenkimplantate erfolgt ist der Zeitfaktor erheblich.
Zunächst ist deshalb ein Adapter vorgesehen, der im Boden eine größere zentrale Bohrung aufweist, welche somit auch eine Gewichterleichterung des jeweiligen Adapters bewirkt.

also das Ende aus zwei, drei, vier oder mehr federnden Segmenten besteht. Durch die Hülse, die in Richtung des anderen Endes einen Handgriff besitzt, ist ein Dorn geführt, der durch den Handgriff hindurch verlängert ist und in einen Knebel zum Drehen endet. An einer bestimmten Position von Dorn und Hülse besitzen beide einen Gewindebereich, so dass beide Teile zueinander eine nahezu zwangsweise Relativbewegung ausführen können. Dorn und Hülse haben zueinander gerade soviel Spiel, das ein Gleiten des Domes in der Hülse erfolgen kann
Wird nun die Hülse in den Boden des Adapters gesteckt, verengen sich die federden Segmente am Ende der Hülse, wobei sich nach dem Durchstecken der Kragen auf die Innen- oder Aussenseite des Adapterbodens legt. Der Dom wird nun soweit in Richtung Adapter geschoben bis der Beginn des Gewindebereiches fühlbar erreicht ist, wobei die Domspitze nunmehr kurz vor dem Austritt am Kragen der Hülse steht. Das Voranschrauben des Domes bewirkt somit eine zwangsweise Verrieglung des Kragens am Adapterboden. Ein Zurückziehen des Abziehgerätes ist in dieser Position nicht mehr möglich. Somit wird ein absolut sicherer Abziehvorgang eingeleitet, wenn die Domspitze entweder gegen den Boden der Steckkugel oder den Hopf des Endoprothesenschaftes drückt, um den Adapter zu trennen.
Weitere Vorteile ergeben sich aus der nachfolgenden Beschreibung eines Ausfuhrungsbeispiels und werden anhand der Zeichnungen näher erläutert. Es zeigt:
Fig. 1 Einen möglichen Adapter auf dem Schaft mit Steckkugel, Fig. 2 das Abziehwerkzeug,
Fig. 3 eine weitere Ausgestaltung eines Abziehwerkzeug.
In Fig. 1 ist zunächst der Adapter 10 auf dem Schaft 21 einer Endoprothese 20 mit einer Steckkugel 30 dargestellt, wobei der Adapter 10 stellvertretend für eine Vielzahl von Adaptern unterschiedlicher Durchmesser und Längen steht. Der Adapter 10 hat eine konische Außenkontur 12 und eine entsprechende Innenkontur 11, die zur konischen Außenkontur 22 des Schaftes 21 korrespondiert. Der Boden 14 besitzt eine zentrische Bohrung 15, deren Kreiskanten nicht entgratet, also nahezu scharfkantig sind.
Figur 2 zeigt nunmehr das Abziehwerkzeug 50 mit dem Dom 51,welcher an einer Seite eine Drehknebel 52 besitzt und an der gegenüber liegenden Seite eine gerundete Spitze 53 aufweist. Von der Spitze fuhrt ein Schaftteil 54 zu einem Gewindebereich 55.
Die Hülse 60 ist mit einem Handgriff 61 fest verbunden, in dessen Ende eine Buchse 62 eingesetzt ist, zum besseren gleiten auf dem Dom 51. Femer besitzt die Hülse ebenfalls einen Gewindebereich 63 mit gleichem Gewinde 55 wie auf dem Dorn, sowie im. ^jjschlyß an den Gewindebereick eine; Bohrung 64.
Am vorderen Ende des Dorn 51 besitzt die Hülse 60 einen Kragen 65 mit einer Fase 66. weiterhin sind Schlitze 67 in dem Hülsenende vorgesehen, welche federnde Elemente 68 bilden. In dem dargestellten Ausführungsbeispiel ist eine Kreuzschlitzung vorgesehen, es könnten auch weniger oder mehr Schlitze sein. Dargestellt ist das Abziehwerkzeug 50 in einer Stellung, in welcher der Dorn 51 mit seinem Gewindebereich 55 im Gewindebereich 63 der Hülse 60 eingeschraubt ist. Es leuchtet ein, dass die Dornspitz 53 jedenfalls soweit zurück geschraubt werden kann, dass beim Durchstecken des Kragens 65,66 in die Bohrung 15 eines Adapters 10, die Segmente 68 sich federnd zunächst zusammendrücken und nach dem Durchtritt des Kragen 65 sich öffnen. Wird dann der Dorn 51 vorwärts geschraubt, verhindert die Dornspitze 53 bzw. der anschließende zylindrische Teil 54 das die Segmente 68 einwärts bewegen können, so dass der Kragen eine Zwangsabstützung auf dem Boden des Adapter sichert.
Die Figur 3 zeigt ein weiteres Ausfuhrungsbeispiel eines Abziehwerkzeuges 70, welches einen nahezu identischen Aufbau wie das Abziehwerkzeug 50 besitzt. Allerdings ist hier eine Hülse 71 vorgesehen, die nahe dem Austritt am Kragen 72 eine innere Verengung 72 mit Schräge 73 aufweist. Dies kann in der Anwendung dazu dienen, dass das Loch 15 im Boden des Adapters 10 so groß ist, das der Durchmesser des Kragen 72 mit kleinstem Wiederstand hindurch passt. Um ein sicheres Verhaken des Kragens im Adapterboden zu erzielen treibt beim Eindrehen des Doms 51 die Dornspitze 53 mit dem Schaftteil 54 die Segmente 68 auseinander.
In einer vorteilhaften Ausgestaltung der Erfindung ist die Wandung 15A der Bohrung 15 im Adapter 10 mit einer besonderen Rauhheit, beispielsweise Querrillen versehen und die Abziehgeräte 50,70 besitzen die Segmente im Bereich direkt unterhalb des Kragen 65,72 ebenfalls einen Rauhbereich. So erfolgt beim Einschrauben des Domes 51 in die Segmente und Auseinanderpressen dieser ein erhöhter Flächendruck zwischen der Adapterbohrung 15A und den Segmenten 58.
Der Adapter ist aus einem körperverträglichen Material, meist eine CrMo-Legierung, hergestellt.
Die Werkzeuge 50 und 70 sind aus Materialien hergestellt, welche für den medizinischen OP- Bereich zugelassen und entsprechend sterilsierbar sind.

Claims (10)

1. Adapter zum Verbinden von Steckkugel und Schaft von Endoprothesen und Abziehwerkzeug zum Trennen derselben, mit einem Dorn, der durch eine Hülse geführt ist und beide zueinander in Gewindeeingriff stehen, dadurch gekennzeichnet, dass die Hülse (60, 71) an ihrem in den Adapter (10) einzuführenden Ende einen Kragen (65, 72) besitzt und das Ende mindestens zwei Schlitze (67) aufweist.
2. Abziehwerkzeug zum Trennen zum Trennen eines Adapters von Steckkugel oder Schaft, dadurch gekennzeichnet, dass die Hülse (60, 71) vier federnde Elemente (68) trägt, die durch vier Schlitze (67) entstehen.
3. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass der Kragen (65) eine Anschrägung (66) besitzt.
4. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass der Dorn (51) und die Hülse partielle Gewindeabschnitte (55, 63) besitzen.
5. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass der Dorn (51) eine gerundete Spitze (53) und einen Knebel (52) besitzt.
6. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Hülse einen Handgriff (61) besitzt.
7. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Hülse (71) an der Seite im Bereich der Schlitze (67) eine innere Verengung (72), kleiner als der Dorndurchmesser (54) trägt.
8. Abziehwerkzeug nach einem der vorherigen Ansprüche, dadurch gekennzeichnet, dass die Hülse (60, 71) an ihrer Aussenseite unterhalb des Kragen (65, 72) einen Rauhbereich aufweist.
9. Adapter mit innerer und äußerer Konusaufnahme einer Gelenksteckkugel und eines Schaftes einer Endoprothese, dadurch gekennzeichnet, dass der Adapter (10) in seinem Boden (14) eine zentrische Bohrung (15) aufweist, deren Durchmesser Gleich oder Kleiner dem Durchmesser des Kragen (65, 72) ist.
10. Adapter nach Anspruch 8, dadurch gekennzeichnet, dass die Bohrung (15) in der Wandung (15A) eine besondere Rauhheit, beispielsweise Querrillen aufweist.
