Abstract

本发明涉及一种组配式肿瘤型人工胫距关节假体,包括:胫骨组件,用于与人体胫骨连接;距骨组件,主要由距骨底座、距骨连接柱和锁定螺钉组成,距骨底座为与距骨骨面形态匹配的壳式结构,距骨连接柱一体形成于距骨底座的下部且沿垂直轴向下延伸,锁定螺钉沿冠状轴穿过距骨底座、距骨连接柱和人体距骨;胫距铰链式连接组件,转动连接在胫骨组件和距骨组件之间,以使胫骨组件和距骨组件可在矢状面内相对转动,从而恢复胫距关节的跖屈和背伸功能。本发明能够广泛应用于胫骨远端或腓骨远端严重累及胫骨情况下肿瘤切除后肢体连续性中断的骨缺损重建,可实现肢体结构恢复目的,同时实现胫距关节即刻稳定和远期稳定的完美结合,从而使患者远期胫距关节功能得到显著改善。

Description

一种组配式肿瘤型人工胫距关节假体
技术领域
本发明涉及一种人工关节,具体是关于一种可以自由调节假体长度且提高骨长入的组配式肿瘤型人工胫距关节假体。
背景技术
胫骨是仅次于股骨的骨肉瘤第二好发部位,胫骨骨肉瘤约占全身骨肉瘤的19%,且其中20%的胫骨骨肉瘤发生于胫骨远端。胫骨远端由于有较多肌腱、血管和神经,故很难获得良好的肿瘤边界。因此,有学者推荐膝下截肢作为胫骨远端骨肉瘤首选的手术方式。近年来随着化疗和外科技术的进步,胫骨远端骨肉瘤的保肢治疗同样能够获得良好的生存率,并被广大骨肿瘤医生所认可和推荐。
目前胫骨远端恶性肿瘤瘤段切除后重建方式多样,主要包括异体骨重建、自体骨灭活再植、带血管蒂自体腓骨移植、不带血管蒂自体腓骨移植以及踝关节假体置换。异体骨重建存在感染风险高、异体骨吸收及排异反应等问题,从而增加伤口感染等并发症风险。自体骨灭活再植方法多样,国内外最为常用的三种灭活方式为酒精、液氮和高温法,不同灭活方式在并发症方面各有利弊。踝关节承受全身重量,并且通过踝关节的正常活动可完成日常行走活动,因此对于胫骨远端恶性肿瘤患者重建踝关节的功能性对于术后疗效具有重要意义。踝关节融合的优点可以恢复骨性结构的连续性,提供良好的踝关节稳定性,减轻负重时的疼痛。但依靠钢板螺钉系统实现胫骨远端固定和踝关节融合的患者,随着植骨后自异体骨的吸收,可增加术后灭活自体骨和大段异体骨的骨折发生率。虽然目前胫骨远端恶性肿瘤瘤段切除后重建方式众多,但大部分重建方式均以牺牲踝关节活动度为代价,对于大多数患者尤其是年轻患者而言,牺牲正常踝关节活动度仍然较难接受。但在切除胫骨远端恶性肿瘤大段瘤骨后,能够实现踝关节稳定和良好活动度的完美结合的重建方式,仍是骨肿瘤科医师面临的重大挑战。
踝关节置换术并不像髋膝关节置换一样得到广泛推广,重要原因是远期假体机械性失效和切口并发症发生率高。同时踝关节置换术后假体松动失效与踝周的疼痛、肿胀,严重影响患者术后功能。研究显示,人工踝关节机械性失效很大原因在于假体距骨侧距骨塌陷及假体骨界面骨吸收造成。
发明内容
针对上述问题,本发明的目的是提供一种可以自由调节假体长度且提高骨长入的组配式肿瘤型人工胫距关节假体。
为实现上述目的,本发明采取以下技术方案:一种组配式肿瘤型人工胫距关节假体,包括:胫骨组件,用于与人体胫骨连接;距骨组件,主要由距骨底座、距骨连接柱和锁定螺钉组成,所述距骨底座为与距骨骨面形态匹配的壳式结构,所述距骨连接柱一体形成于所述距骨底座的下部且沿垂直轴向下延伸,所述锁定螺钉沿冠状轴穿过所述距骨底座、距骨连接柱和人体距骨,以实现所述距骨组件与人体距骨的稳定连接;胫距铰链式连接组件,转动连接在所述胫骨组件和距骨组件之间,以使所述胫骨组件和距骨组件可在矢状面内相对转动,从而恢复胫距关节的跖屈和背伸功能。
所述的组配式肿瘤型人工胫距关节假体,优选地,所述胫骨组件包括:胫骨髓针,用于插入胫骨髓腔内,以使所述胫骨组件与人体胫骨连接;延长段,所述延长段的上端与所述胫骨髓针的下端相锥接;基本段,所述基本段的上端与所述延长段的下端相锥接。
所述的组配式肿瘤型人工胫距关节假体,优选地,所述胫距铰链式连接组件包括:铰接耳,一体形成于所述基本段的下端;铰接臂,一体形成于所述距骨底座的上部且伸入所述铰接耳中,并通过铰接轴与所述铰接耳转动连接。
所述的组配式肿瘤型人工胫距关节假体,优选地,所述胫骨髓针为生物型或水泥型。
所述的组配式肿瘤型人工胫距关节假体,优选地,所述距骨底座及距骨连接柱与人体距骨的接触界面为采用3D打印技术设计的孔隙结构。
本发明由于采取以上技术方案,其具有以下优点:1、本发明能够重建胫距关节跖屈和背伸功能,恢复肢体长度,为下肢瘤段切除后正常的力学传导提供条件。2、本发明采用组配式设计,可以根据患者胫骨远端瘤段切除范围、皮肤缺损情况和软组织张力自由调节胫骨组件长度,从而个体化重建胫骨远端缺损段。3、本发明的距骨底座及距骨连接柱为通过3D打印技术设计的孔隙结构,提高距骨底座及距骨连接柱与宿主骨界面间的融合率,从而为良好的骨长入和假体稳定性提供重要基础。4、本发明通过锁定螺钉机制及3D打印技术为基础的界面设计,实现胫距关节假体即刻稳定性和远期稳定性的完美结合。

具体实施方式
以下将结合附图对本发明的较佳实施例进行详细说明,以便更清楚理解本发明的目的、特点和优点。应理解的是,附图所示的实施例并不是对本发明范围的限制,而只是为了说明本发明技术方案的实质精神。
如图1至图3所示,本发明提供的组配式肿瘤型人工胫距关节假体,包括:胫骨组件1,用于与人体胫骨连接;距骨组件2,主要由距骨底座21、距骨连接柱22和锁定螺钉23组成,距骨底座21为与距骨骨面形态匹配的壳式结构,距骨连接柱22一体形成于距骨底座21的下部且沿垂直轴向下延伸,用于插入形成于人体距骨上的置入孔;锁定螺钉23沿冠状轴穿过距骨底座21、距骨连接柱22和人体距骨,以实现距骨组件2与人体距骨的稳定连接;胫距铰链式连接组件3,转动连接在胫骨组件1和距骨组件2之间,以使胫骨组件1和距骨组件2可在矢状面内相对转动,从而恢复胫距关节的跖屈和背伸功能。
在上述实施例中,优选地,胫骨组件1包括:胫骨髓针11,用于插入胫骨髓腔内,以使胫骨组件1与人体胫骨连接;延长段12,延长段12的上端与胫骨髓针11的下端相锥接,并可根据胫骨缺损长度,通过装配不同长度的延长段12实现术中不同胫骨缺损长度的组配,以适应不同胫骨缺损程度的患者;基本段13,基本段13的上端与延长段12的下端相锥接。
在上述实施例中,优选地,胫距铰链式连接组件3包括:铰接耳31,一体形成于基本段13的下端;铰接臂32,一体形成于距骨底座21的上部且伸入铰接耳31中,并通过铰接轴33与铰接耳31转动连接。
在上述实施例中,优选地,胫骨髓针5可以设计为生物型或水泥型两款髓针,以根据不同需要,选择不同的固定方式。
在上述实施例中,优选地,距骨底座21及距骨连接柱22与人体距骨的接触界面为采用3D打印技术设计的孔隙结构,便于距骨骨长入,从而提高距骨底座21及距骨连接柱22与宿主骨界面间的融合率,便于距骨骨长入,实现假体距骨界面的远期稳定。
最后应说明的是:以上实施例仅用以说明本发明的技术方案,而非对其限制;尽管参照前述实施例对本发明进行了详细的说明,本领域的普通技术人员应当理解:其依然可以对前述各实施例所记载的技术方案进行修改,或者对其中部分技术特征进行等同替换;而这些修改或者替换,并不使相应技术方案的本质脱离本发明各实施例技术方案的精神和范围。

Claims (5)

1.一种组配式肿瘤型人工胫距关节假体,其特征在于,包括:
胫骨组件(1),用于与人体胫骨连接;
距骨组件(2),主要由距骨底座(21)、距骨连接柱(22)和锁定螺钉(23)组成,所述距骨底座(21)为与距骨骨面形态匹配的壳式结构,所述距骨连接柱(22)一体形成于所述距骨底座(21)的下部且沿垂直轴向下延伸,所述锁定螺钉(23)沿冠状轴穿过所述距骨底座(21)、距骨连接柱(22)和人体距骨,以实现所述距骨组件(2)与人体距骨的稳定连接;
胫距铰链式连接组件(3),转动连接在所述胫骨组件(1)和距骨组件(2)之间,以使所述胫骨组件(1)和距骨组件(2)可在矢状面内相对转动,从而恢复胫距关节的跖屈和背伸功能。
2.根据权利要求1所述的组配式肿瘤型人工胫距关节假体,其特征在于,所述胫骨组件(1)包括:
胫骨髓针(11),用于插入胫骨髓腔内,以使所述胫骨组件(1)与人体胫骨连接;
延长段(12),所述延长段(12)的上端与所述胫骨髓针(11)的下端相锥接;
基本段(13),所述基本段(13)的上端与所述延长段(12)的下端相锥接。
3.根据权利要求1所述的组配式肿瘤型人工胫距关节假体,其特征在于,所述胫距铰链式连接组件(3)包括:
铰接耳(31),一体形成于所述基本段(13)的下端;
铰接臂(32),一体形成于所述距骨底座(21)的上部且伸入所述铰接耳(31)中,并通过铰接轴(33)与所述铰接耳(31)转动连接。
4.根据权利要求1所述的组配式肿瘤型人工胫距关节假体,其特征在于,所述胫骨髓针(5)为生物型或水泥型。
5.根据权利要求1所述的组配式肿瘤型人工胫距关节假体,其特征在于,所述距骨底座(21)及距骨连接柱(22)与人体距骨的接触界面为采用3D打印技术设计的孔隙结构。
