Abstract

本发明公开了一种肠系膜上血管悬吊装置,包括支撑桥和悬吊带,悬吊带为一定宽带的弹性条带,横向设置在腹内肠系膜上血管内侧,悬吊带两端经腹直肌两侧穿出腹壁连接在腹外的支撑桥上。本发明更为符合解剖生理特点,操作简单,并发症少,能够获得广泛推广。

肠系膜上血管悬吊装置
技术领域
本发明属于医疗手术技术领域,尤其是涉及一种肠系膜上血管悬吊装置,用于解除肠系膜上动脉对十二指肠的压迫。
背景技术
肠系膜上动脉压迫综合征的发病因素较多,术前和术中的病因判断非常困难,导致术式选择并不能完全准确,这也是导致各种术式的疗效都未取得完全满意的结果。胃空肠吻合术、胃大部切除+胃空肠吻合术、十二指肠空肠吻合术、十二指肠血管前移位术、十二指肠环形引流术、十二指肠复位术等术式的设计都是从解决梗阻出发,而不是解除梗阻。十二指肠空肠吻合术获得最为广泛的应用,但是却有三成左右的病例效果不佳,究其主要原因是没有根本上解除肠系膜上动脉对十二指肠的压迫而遗留盲端。血管前移位术和十二指肠环形引流术操作复杂,发生并发症几率明显升高,没有获得较好推广。无论什么原因导致的肠系膜上动脉压迫综合征,最终的病理状态是肠系膜上动脉对十二指肠水平段的压迫。解除肠系膜上动脉对十二指肠的压迫是治疗这一疾病的基本要求,而目前所有的吻合术式都是绕道而走,缺乏一种简单实用、并发症少的装置,用于解除肠系膜上动脉对十二指肠的压迫。
发明内容
本发明的目的是:提供一种操作简单、并发症少的肠系膜上血管悬吊装置。
本发明的技术方案是:包括支撑桥和悬吊带,悬吊带为一定宽带的弹性条带,呈弓弦形,横向设置在腹内肠系膜上血管内侧,悬吊带两端经腹直肌两侧穿出腹壁,支撑桥呈弓背形,悬吊带两端连接在腹外的支撑桥上。
所述支撑桥可为管状结构,悬吊带两端缝合固定在支撑桥上。
所述支撑桥可为实体型态,支撑桥两端带有固定悬吊带的卡扣。
本发明的有益效果是:1、从根本上解除了肠系膜上动脉对十二指肠的病理压迫状态,悬吊后解除压迫,经过持续悬吊,使组织器官获得解剖铸型。2、创伤小,只是分离固有的解剖间隙,无血管消化管道的离断和损伤。3、恢复、维持了正常的解剖生理状态,不需要打乱消化管道的的解剖。4、操作简单,操作步骤少,避免消化管道的离断重建等复杂操作。5、并发症少,避免了复杂操作带来的周围脏器的损伤,以及各种瘘的发生,降低了出血、梗阻等发生率。

具体实施方式
从图1所示本发明的应用状态示意图可以看出,本发明由支撑桥1和悬吊带2两部分组成。悬吊带2为带有弹性的条带,宽0.4-0.6cm,呈弓弦形,悬吊带2横向设置在腹内肠系膜上血管3内侧,悬吊带2两端经腹直肌5两侧穿出腹壁4,连接在腹外的支撑桥1上。支撑桥1呈弓背形,长是双侧腹直肌外缘的距离,为6-15cm,宽0.5-0.8cm。支撑桥1的作用是防止悬吊带2两端靠近卡压血管,借助于腹直肌5实现悬吊,并保护腹直肌5免受切割损伤。支撑桥1可为管状或者实体型态结构,管状的支撑桥1可穿入悬吊带2,悬吊带2两端缝合固定在支撑桥1上,实体型态的支撑桥1两端带卡扣6,以便固定悬吊带。
图2为本发明的第一种实施方式,采用的管状结构支撑桥,为穿桥固定型;图3为本发明的第二种实施方式,采用的实体型态支撑桥,为两端卡扣固定型;图4为本发明的第三种实施方式,采用的实体型态支撑桥,为一端固定、另一端可伸缩固定,在支撑桥1一端具有多个卡扣6,悬吊带2在不同位置的卡扣6伸缩调节。
本发明在实际操作中的要点是:在十二指肠悬韧带松解术基础上,十二指肠水平部下缘向下分离肠系膜与腹膜后间隙约6cm,进入toldt筋膜间隙,使肠系膜可轻松抬起,用右手食指钝性分离,进入肠系膜上血管与十二指肠水平部间隙,并经肠系膜上静脉右侧向前分离,经横结肠系膜右段无血管区穿出,导入40cm长弹力悬吊带,悬吊带两端经腹直肌两侧外缘对应点穿出腹壁,制作与悬吊带两穿出点等长的硬直管状鞘作为悬吊带的腹外支撑桥,将悬吊带穿过管状支撑桥,两端缝合固定。术后经过3个月以上持续悬吊后,取出悬吊带。
本发明更为符合解剖生理特点,操作简单,并发症少,能够获得广泛推广。

1.一种肠系膜上血管悬吊装置,其特征是:包括支撑桥(1)和悬吊带(2),所述悬吊带(2)为一定宽带的弹性条带,呈弓弦形,横向设置在腹内肠系膜上血管(3)内侧,悬吊带(2)两端经腹直肌(5)两侧穿出腹壁(4),所述支撑桥(1)呈弓背形,悬吊带(2)两端连接在腹外的支撑桥(1)上。
2.根据权利要求1所述的肠系膜上血管悬吊装置,其特征是:所述支撑桥(1)为管状结构,悬吊带(2)两端缝合固定在支撑桥(1)上。
3.根据权利要求1所述的肠系膜上血管悬吊装置,其特征是:所述支撑桥(1)为实体型态,支撑桥(1)两端带有固定悬吊带(2)的卡扣(6)。
4.根据权利要求3所述的肠系膜上血管悬吊装置,其特征是:所述支撑桥(1)一端具有多个卡扣(6),悬吊带(2)在不同位置的卡扣(6)伸缩调节。
