Abstract

本发明公开了一种保胆取石/息肉用胆道镜鞘管,其头部至尾部依次设有互相连接的引流管段Ⅰ、引流管段Ⅱ、引流管段Ⅲ和引流管段Ⅳ;引流管段Ⅰ内设有从鞘管头部穿出的通道Ⅰ,引流管段Ⅰ外壁依次设有上球囊和下球囊;引流管段Ⅱ内设有与通道Ⅰ连通的通道Ⅱ,通道Ⅱ内设有一端延伸至通道Ⅰ且与上球囊连通的通道Ⅲ和一端延伸至通道Ⅰ且与下球囊连通的通道Ⅳ;引流管段Ⅲ内设有与通道Ⅱ连通的通道Ⅴ,引流管段Ⅲ外壁上设有充气接头Ⅰ、充气接头Ⅱ和与通道Ⅴ连通的出水接头;引流管段Ⅳ内设有与通道Ⅴ连通的通道Ⅵ。充盈上球囊和下球囊后,上球囊和下球囊对胆囊切口产生弹性压力,具有压迫止血和避免胆囊胆汁和胆泥污染腹腔。

Description

保胆取石/息肉用胆道镜鞘管
技术领域
本发明涉及一种胆道镜鞘管,具体地说是一种保胆取石/息肉用胆道镜鞘管。
背景技术
胆囊结石和胆囊息肉是我国的常见病和多发病,腹腔镜胆囊切除术是治疗胆囊结石及胆囊息肉的传统手术方案,随着微创技术的发展及对胆囊功能认识的逐步深入,腹腔镜保胆取石/息肉术是目前治疗胆囊结石及胆囊息肉的另一种主流方案。
目前,采取腹腔镜及胆道镜治疗胆囊结石及胆囊息肉过程中,先置置入腹腔镜戳卡,再从戳卡外套管中置入胆道镜,胆道镜经胆囊切口进入胆囊内进行取石或摘除息肉,该方式存在诸多缺点:1.整个手术过程中均需建立气腹,而长时间的气腹容易影响膈肌的摆动,影响呼吸及循环,大量二氧化碳的吸收可发生酸中毒及离子紊乱,高压的二氧化碳经静脉回流,甚至发生气体栓塞,危及患者生命;2.胆道镜取胆囊结石及息肉过程中,大量感染性胆汁、碎石、胆泥及胆道镜冲洗液经切口流入腹腔内,容易导致腹腔感染、腹腔脓肿,而碎石及胆泥进入腹腔后,难以清除干净,导致腹腔残石、腹腔粘连、肠梗阻、内瘘、腹腔脓肿等并发症;3.胆囊切开后,胆囊压力降低并塌陷,增加了胆道镜进入胆囊的难度;4.胆道镜经戳卡外套管进入胆囊后,由于胆道镜镜身缺少支撑,胆道镜容易在腹腔及胆囊内弯曲,增加了胆道镜探查胆囊管的难度;5.戳卡外套管为硬质塑料,反复进出胆道镜,胆道镜镜身胶皮容易被戳开开口刮伤,导致胆道镜出现外漏而损伤其电器元件;5.胆道镜反复进出胆囊,容易导致胆囊切口出血。
发明内容
根据上述提出的技术问题,而提供一种保胆取石/息肉用胆道镜鞘管。本发明采用的技术手段如下:
一种保胆取石/息肉用胆道镜鞘管,其头部至尾部依次设有互相连接的引流管段Ⅰ、引流管段Ⅱ、引流管段Ⅲ和引流管段Ⅳ;
所述引流管段Ⅰ内设有从所述鞘管头部穿出的通道Ⅰ,所述引流管段Ⅰ外壁依次设有上球囊和下球囊;
所述引流管段Ⅱ内设有与所述通道Ⅰ连通的通道Ⅱ,所述通道Ⅱ内设有一端延伸至所述通道Ⅰ且与所述上球囊连通的通道Ⅲ和一端延伸至所述通道Ⅰ且与所述下球囊连通的通道Ⅳ;
所述引流管段Ⅲ内设有与所述通道Ⅱ连通的通道Ⅴ,所述引流管段Ⅲ外壁上设有充气接头Ⅰ、充气接头Ⅱ和与所述通道Ⅴ连通的出水接头;
所述通道Ⅲ的另一端与所述充气接头Ⅰ连通;
所述通道Ⅳ的另一端与所述充气接头Ⅱ连通;
所述引流管段Ⅳ内设有与所述通道Ⅴ连通的通道Ⅵ,所述引流管段Ⅳ外壁具有用于手持的粗糙面;
所述通道Ⅵ内壁设有防反流垫;
所述防反流垫的材质为硅胶;
所述防反流垫上设有用于胆道镜穿过的弹性孔,所述胆道镜穿过所述弹性孔后,所述弹性孔贴合所述胆道镜,构成密封,防反流。
所述上球囊和所述下球囊沿所述引流管段Ⅰ轴线方向的长度均为5mm;
所述上球囊和所述下球囊充盈时,所述上球囊和所述下球囊的最大直径均为1~3cm。
所述上球囊上下两端和所述下球囊上下两端均分别通过金属环与所述引流管段Ⅰ固定。
所述引流管段Ⅰ和所述引流管段Ⅱ的材质为硅胶,具有良好的弹性;
所述引流管段Ⅲ的材质为聚乙烯;
所述引流管段Ⅳ的材质为聚乙烯;
所述引流管段Ⅰ、所述引流管段Ⅱ和所述引流管段Ⅲ的外径为0.9cm;
所述引流管段Ⅳ的外径为1.5cm;
所述引流管段Ⅰ、所述引流管段Ⅱ和所述引流管段Ⅲ的壁厚为1mm,具有较好的支撑;
所述引流管段Ⅰ和所述引流管段Ⅱ的整体长为30~60cm,可经腹壁直达胆囊腔内。
与现有技术相比,本发明具有以下优点:
1.引流管段Ⅰ和引流管段Ⅱ为硅胶材质,避免了胆道镜取石过程中戳卡外套管壁对胆道镜的损伤;
2.充盈上球囊和下球囊后,上球囊和下球囊对胆囊切口产生弹性压力,具有压迫止血的作用,避免取石术中胆囊切口出血;
3.充盈上球囊和下球囊后,上球囊和下球囊对胆囊切口产生弹性压力,可避免鞘管的移位;
4.充盈上球囊和下球囊后,上球囊和下球囊封闭胆囊切口,可避免感染性胆汁、冲洗液、泥沙样结石及胆泥等进入腹腔;
5.鞘管进入胆囊后,即可关闭气腹及腹腔镜视频,避免了长期气腹产生的并发症及腹腔镜系统的损耗;
6.出水接头连接负压吸引管,可将感染性胆汁、冲洗液、泥沙样结石及胆泥等排出体外,降低上述物质进入腹腔所致的并发症;
7.鞘管对胆道镜具有良好的支撑作用,避免胆道镜对胆囊及胆囊管的探查;
8.上球囊和下球囊可分别控制其充盈大小,使用更灵活。
基于上述理由本发明可在医疗器具等领域广泛推广。

如图1-图3所示,一种保胆取石/息肉用胆道镜鞘管,其头部至尾部依次设有互相连接的引流管段Ⅰ1、引流管段Ⅱ2、引流管段Ⅲ3和引流管段Ⅳ4;
所述引流管段Ⅰ1内设有从所述鞘管头部穿出的通道Ⅰ5,所述引流管段Ⅰ1外壁依次设有上球囊6和下球囊7;
所述引流管段Ⅱ2内设有与所述通道Ⅰ5连通的通道Ⅱ8,所述通道Ⅱ8内设有一端延伸至所述通道Ⅰ5且与所述上球囊6连通的通道Ⅲ9和一端延伸至所述通道Ⅰ5且与所述下球囊7连通的通道Ⅳ10;
所述引流管段Ⅲ3内设有与所述通道Ⅱ8连通的通道Ⅴ11,所述引流管段Ⅲ3外壁上设有充气接头Ⅰ12、充气接头Ⅱ13和与所述通道Ⅴ11连通的出水接头14;
所述通道Ⅲ9的另一端与所述充气接头Ⅰ12连通;
所述通道Ⅳ10的另一端与所述充气接头Ⅱ13连通;
所述引流管段Ⅳ4内设有与所述通道Ⅴ11连通的通道Ⅵ15,所述引流管段Ⅳ4外壁具有用于手持的粗糙面;
所述通道Ⅵ15内壁设有防反流垫16;
所述防反流垫16上设有用于胆道镜17穿过的弹性孔18。
所述上球囊6和所述下球囊7沿所述引流管段Ⅰ1轴线方向的长度均为5mm;
所述上球囊6和所述下球囊7充盈时,所述上球囊6和所述下球囊7的最大直径均为1~3cm。

建立气腹并经腹壁20置入戳卡21,取出戳卡21推进杆,出水接头14连接负压吸引管,使胆囊22、通道Ⅰ5、通道Ⅱ6、通道Ⅴ11和负压吸引管连通;
经戳卡21外套管置入所述鞘管并逐步接近胆囊22切口,将鞘管经胆囊22切口进入到胆囊22内,经充气接头Ⅰ12注入空气或生理盐水并充盈所述上球囊6,回拉所述鞘管至所述上球囊6与胆囊22壁相贴,再经充气接头Ⅱ13注入空气或生理盐水并充盈所述下球囊7。
调整负压吸引管的负压吸引压力,使负压出水量与胆道镜17进水量及胆汁量相同,避免胆囊22塌陷。
关闭气腹,停止腹腔镜视频系统,经鞘管(通道Ⅰ5、通道Ⅱ6、通道Ⅴ11和通道Ⅵ15)内注入液体石蜡,从弹性孔18置入胆道镜17,胆道镜17经鞘管进入胆囊22腔内进行探查、取石/息肉。
取净胆囊22及胆囊管内结石/息肉后,再次建立气腹并打开腹腔镜系统,释放上球囊6和下球囊7,取出鞘管并缝合胆囊22切口。

1.一种保胆取石/息肉用胆道镜鞘管,其头部至尾部依次设有互相连接的引流管段Ⅰ、引流管段Ⅱ、引流管段Ⅲ和引流管段Ⅳ,其特征在于:
所述引流管段Ⅰ内设有从所述鞘管头部穿出的通道Ⅰ,所述引流管段Ⅰ外壁依次设有上球囊和下球囊;
所述引流管段Ⅱ内设有与所述通道Ⅰ连通的通道Ⅱ,所述通道Ⅱ内设有一端延伸至所述通道Ⅰ且与所述上球囊连通的通道Ⅲ和一端延伸至所述通道Ⅰ且与所述下球囊连通的通道Ⅳ;
所述引流管段Ⅲ内设有与所述通道Ⅱ连通的通道Ⅴ,所述引流管段Ⅲ外壁上设有充气接头Ⅰ、充气接头Ⅱ和与所述通道Ⅴ连通的出水接头;
所述通道Ⅲ的另一端与所述充气接头Ⅰ连通;
所述通道Ⅳ的另一端与所述充气接头Ⅱ连通;
所述引流管段Ⅳ内设有与所述通道Ⅴ连通的通道Ⅵ,所述引流管段Ⅳ外壁具有用于手持的粗糙面;
所述通道Ⅵ内壁设有防反流垫;
所述防反流垫上设有用于胆道镜穿过的弹性孔。
