背景技术
股骨头坏死是一种严重影响生活的退行性疾病,尤其好发于20-50岁的中青年,其病理机制多样,表现为股骨头局部血运障碍,进而引起骨细胞缺血坏死,骨小梁断裂,直至股骨头塌陷并可进展为骨关节炎。股骨头坏死治疗的目的为保存股骨头,缓解疼痛,避免或延缓全髋关节置换术的时间。股骨头坏死的治疗策略主要取决于疾病分期以及医生的选择。对于股骨头坏死早期,可供选择的治疗方式包括限制负重,髓芯减压,截骨术,骨移植,带血管蒂的骨移植,电刺激等。
限制性负重为疾病早期患者及医生首先选择的保守治疗方式,但限制性负重只有小范围坏死的临床效果满意,大部分病例仍需保髋治疗或关节置换。故限制性负重仅可作为一种辅助手段以结合其它方案治疗。
髓芯减压在临床上应用最为广泛,髓芯减压后坏死区域的压力可得到缓解,坏死区域或能有所恢复。但有报道称应用髓芯减压术,有13%的患者在术中或术后即刻出现股骨颈骨折。对股骨头髓芯减压会使本已脆弱的的软骨下骨的机械支撑作用进一步被减弱,可能加速股骨头的塌陷;对于合并有基础疾病及应用激素的患者,髓芯减压将会使应力集中,加速股骨头塌陷并使股骨颈骨折的风险增加。
股骨头坏死的另一种治疗方法为带血管蒂的胖骨移植,但其支撑力较弱,远期有效率仅有52%。该手术要求较高的手术技术,且手术时间长出血多,该手术的并发症包括有胖骨部分截骨后中度至重度疼痛(15%的患者),仅有约39%的患者无症状。上述局限性对于该术式的普及与患者的接受程度造成了较大的限制。
对于早期股骨头坏死,上述方法都有不足,植入金属棒,则可较好的解决上述矛盾。植入金属棒可在股骨头内部减压后重建股骨头负重部位骨缺损时为软骨下骨板提供有效支撑。同时金属棒植入手术操作相对简单,手术创伤小,易为医生及患者所接受。目前国外的金属棒已在国内临床应用,如多孔钽金属棒。
发明专利CN 103462730 B 一种多孔钽棒及其应用介绍了一种多孔钽棒,用于治疗早、中期股骨头坏死。该多孔钽棒包括本体,本体由化学气相沉积方法制备的多孔钽制成,本体的一端设置有凹槽,可负载骨髓基质干细胞或骨髓基质干细胞联合生物膜复合体等,有利于新生骨的生成,另一端为螺纹结构,所述螺纹结构末端端面上设置有槽口。
多孔钽金属棒应用中,出现了一些问题,如患者于术后数周,因为难以忍受的持续患髋疼痛,而坚持改行人工全髋关节置换术。
发明内容
本发明的目的在于提供一种用于治疗早期股骨头坏死的术后避免疼痛的医用植入的多孔金属棒。
发明人认为,钽金属棒周围连续的骨长入,使钽金属棒周围形成了一个厚衣壳,完全堵塞了钽棒的孔隙,解除了钽金属棒置入术髓芯减压的效果,使髓内压逐渐增高,导致患者术后持续疼痛。为了避免钽棒的孔隙被堵塞而造成髓内压逐渐增高使患者术后持续疼痛,本发明提出了如下技术方案:
一种多孔金属棒,由多孔金属材料制备,该多孔金属棒包括两部分,一部分为圆棒状部分,与其相连的另一部分为螺纹部分,其外部具有螺纹,螺纹外径大于圆棒状外径,其特征在于:沿着所述多孔金属棒长度方向的中心从螺纹部分端面向里有深盲孔,所述的圆棒状部分外侧至少有两个孔,每个孔所在位置的圆棒状部分横截面上只有一个孔,孔与中心的深盲孔贯通,孔的直径不小于2mm,该孔可通过内外部连通实现减压,避免髓内压逐渐增高,从而避免患者术后疼痛。
进一步说,所述的多孔金属棒,所述的圆棒状部分外侧的孔在圆棒状部分上位于自圆棒状部分一端外侧向里至圆棒状部分长度的60%部分以内,并在该部分沿长度均布,这样的话,金属棒螺纹部分与圆棒状部分的交界处应力减小,同时也可以避免整个金属棒的应力增大。

进一步说,所述的多孔金属棒,所述的圆棒状部分外侧上孔的孔壁及孔外侧周边沿孔圆周径向至少1.5mm内的表面无孔隙,以避免骨长入圆棒状部分外侧上孔,实现持续减压。它可用3D打印技术制备而成,便于实现该种结构,或者在外侧孔内安装有致密材料制成的金属管,金属管在孔外侧端部有法兰,法兰孔周边沿孔圆周径向至少有1.5mm宽。
进一步说,所述的多孔金属棒,其多孔金属材料孔的腔壁上有一层微孔阿屈膦酸盐和微孔磷酸钙涂层,有助于显著地增加骨向金属棒内长入的程度。
进一步说,所述的多孔金属棒,其多孔金属材料的孔隙率为50%-85%,孔径为100µm-600µm,有利于骨组织长入。
进一步说,所述多孔金属棒由医用钛及合金、医用铌及合金、医用钽及合金、医用不锈钢、医用钴基合金制备。
进一步说,所述多孔金属棒中致密材料制成的金属管由医用钛及合金、医用铌及合金、医用钽及合金、医用不锈钢、医用钴基合金制备。
本发明的有益效果:
(1)本发明通过沿着所述多孔金属棒长度方向的中心从螺纹部分端面向里有深盲孔,所述的圆棒状部分外侧至少有两个孔,孔与中心的深盲孔贯通,从而使内外部连通,实现减压,避免髓内压逐渐增高造成病人疼痛。
(2)将圆棒状部分外侧的孔设置在圆棒状部分上位于自圆棒状部分一端外侧向里至圆棒状部分长度的60%部分以内,并在该部分沿长度、圆周均布,既可以减小金属棒螺纹部分与圆棒状部分的交界处应力,还可以避免整个金属棒的应力增大。
(3)使圆棒状部分外侧上孔的孔壁及孔外侧周边沿孔圆周径向至少1.5mm内的表面无孔隙,成为致密的材料表面,可以避免骨长入圆棒状部分外侧的孔,实现持续减压,避免髓内压逐渐增高。上述结构用3D打印技术通过事先的三维设计进行制造,易于制备,或者在外侧孔内安装致密材料制成的金属管,金属管在孔外侧端部有法兰,法兰孔周边沿孔圆周径向至少有1.5mm宽也能实现。
(4)在多孔金属棒的多孔金属材料孔的腔壁上有一层微孔阿屈膦酸盐和微孔磷酸钙涂层,有助于促进骨生长,显著地增加骨向金属棒内长入的程度。
(5)使多孔金属棒的多孔金属材料的孔隙率为50%-85%,孔径为100µm-600µm,有利于骨组织长入。

具体实施方式
下面结合附图对本发明的具体实施方式作说明,实施方式以本发明技术方案为前提,给出了详细的实施方式和具体的操作过程,但本发明的保护范围不仅限于下述的实施方式。
如图所示,1为圆棒状部分,2为螺纹部分,3为螺纹部分的端面上的槽口,4为多孔金属棒长度方向的中心的深盲孔,5为圆棒状部分外侧的孔,6为孔5内壁,7为实施例3中孔外侧周边沿孔圆周径向向外2mm内的表面(图5中阴影部分)无孔隙的部分,8为实施例4中外侧孔5内安装的致密材料制成的金属管,9为金属管法兰,10为法兰外表面,S为法兰宽度。
以下详细给出本发明的实施例。
实施例1
本实施例的多孔金属棒用金属钽制备,如图1所述,圆棒状部分1外侧的孔5有2个,直径为2mm,与在长度方向的中心的深盲孔4贯通,两个孔分别位于距圆棒状部分外端长度的30%、60%处,在圆周方向,两个孔轴线成180°夹角,制备方法如下:将聚乙烯醇、蒸馏水与平均粒径小于700nm的钽粉按照重量比7:125:150混合搅拌均匀,制成浆料,选用平均孔径为0.6mm,密度0.022g/cm3多孔聚氨酯泡沫放入浆料,使聚氨酯泡沫孔隙注满钽粉浆料,将聚氨酯泡沫放入真空干燥箱中干燥,干燥温度60℃,干燥时间10小时,真空度保持1Pa。然后通过真空度低于10-4Pa,温度600℃,保温时间120分钟进行脱脂处理,再进行真空烧结,烧结温度2200℃,保温2小时,真空度10-4Pa,烧结过程充氢气保护。制成的多孔钽的孔隙率为50%,孔径大小为100µm -300µm。将该材料进行机械加工,制成所需的多孔钽金属棒。
实施例2
本实施例与实施例1相似,不同之处为多孔钽的孔隙率为85%,孔径大小为300µm -600µm,圆棒状部分1外侧的孔直径为2.5mm。
实施例3
本实施例多孔金属棒采用了金属钛制备,平均孔径为300µm,参见图4、图5,圆棒状部分1外侧的孔5有3个,直径为2.3mm,与在长度方向的中心的深盲孔4贯通,3个孔分别位于距圆棒状部分外端长度的20%、40%、60%处,在圆周方向,3个孔均布,每个孔与相邻孔轴线夹角为120°,圆棒状部分1外侧孔5的孔壁6及孔外侧周边沿孔圆周径向2mm内的表面7(图5中阴影部分)无孔隙,是致密的,构成多孔金属棒的多孔金属材料孔的腔壁上有一层微孔阿屈膦酸盐和微孔磷酸钙涂层,制备方法如下:
(1)采用颗粒形状基本为球形的纯钛粉末,平均粒度为5μm;
(2)用CAD软件造型,作出上述的多孔金属棒模型,其中,如图3所示,所述的圆棒状部分外侧上孔的孔壁6及孔外侧周边沿孔圆周径向向外2mm内的表面7(图5中阴影部分)无孔隙;
(3)将多孔材料模型导入选择性激光熔化快速成形系统,按照CAD软件造型扫描,扫描速度为200mm/min,激光束每完成一层切片面积的扫描,工作缸相对于激光束焦平面(成形平面)相应地下降一个切片层厚的高度,切片层厚为20µm。
(4)进行去应力退火处理;
(5)喷砂处理。
(6)将多孔金属棒浸入阿屈膦酸盐、磷酸钙、聚乙烯醇、水按照重量比21:28:7:125制成的浆料中,然后取出晾干,在200℃下烘烤,在多孔金属棒的多孔金属材料孔的腔壁上形成具有厚度为1μm-5μm、孔径为600nm-900nm微孔的阿屈膦酸盐与磷酸钙涂层,将圆棒状部分外侧上孔5的孔壁6及孔外侧周边沿孔圆周径向向外2mm内的表面7上的涂层刮除。
实施例4
本实施例多孔金属棒采用了金属铌制备,平均孔径为450µm,圆棒状部分外侧的孔5有2个,与在长度方向的中心的深盲孔4贯通,两个孔分别位于距圆棒状部分外端长度的30%、60%处,在圆周方向,2个孔轴线成180°夹角,制备方法参照实施例1,参见图6、图7,外侧孔5内安装有致密材料制成的金属铌管8,管内径为2mm,管在孔外侧端部有法兰9,法兰外表面10孔周边沿孔圆周径向向外法兰宽度S为3mm。
Claims (11)
1.一种多孔金属棒,由多孔金属材料制备,该多孔金属棒包括两部分,一部分为圆棒状部分,与其相连的另一部分为螺纹部分,其外部具有螺纹,螺纹外径大于圆棒状外径,其特征在于:沿着所述多孔金属棒长度方向的中心从螺纹部分端面向里有深盲孔,所述的圆棒状部分外侧至少有两个孔,每个孔所在位置的圆棒状部分横截面上只有一个孔,孔与中心的深盲孔贯通,孔的直径不小于2mm。
2.如权利要求1所述的多孔金属棒,其特征在于:所述的圆棒状部分外侧的孔在圆棒状部分上位于自圆棒状部分一端外侧向里至圆棒状部分长度的60%部分以内,并在该部分沿长度均布。
3.如权利要求1或2所述的多孔金属棒,其特征在于:所述的圆棒状部分外侧的孔沿圆周也均布。
4.如权利要求1或2所述的多孔金属棒,其特征在于:所述的圆棒状部分外侧上孔的孔壁及孔外侧周边沿孔圆周径向至少1.5mm内的表面无孔隙。
5.如权利要求3所述的多孔金属棒,其特征在于:所述的圆棒状部分外侧上孔的孔壁及孔外侧周边沿孔圆周径向至少1.5mm内的表面无孔隙。
6.如权利要求4所述的多孔金属棒,其特征在于:该多孔金属棒是用3D打印技术制备而成。
7.如权利要求4所述的多孔金属棒,其特征在于:所述外侧孔内安装有致密材料制成的金属管,金属管在孔外侧端部有法兰,法兰孔周边沿孔圆周径向至少有1.5mm宽。
8.如权利要求1至4任一权利要求所述的多孔金属棒,其特征在于:在多孔金属棒的多孔金属材料孔的腔壁上有一层微孔阿屈膦酸盐和微孔磷酸钙涂层。
9.如权利要求1至4任一权利要求所述的多孔金属棒,其特征在于:所述多孔金属棒的多孔金属材料的孔隙率为50%-85%,孔径为100µm-600µm。
10.如权利要求1至4、8和9任一权利要求所述的多孔金属棒,其特征在于:所述多孔金属棒由医用钛及合金、医用铌及合金、医用钽及合金、医用不锈钢、医用钴基合金制备。
11.如权利要求7所述的多孔金属棒,其特征在于:所述金属管由医用钛及合金、医用铌及合金、医用钽及合金、医用不锈钢、医用钴基合金制备。
