Abstract

La invencion se relaciona con implantes esencialmente cilindricos, removibles, que estan caracterizados en que se pueden reducir en diametro y estan envueltos una vez o varias veces en uno o mas niveles mediante una o mas estructuras (2a, 2b) de forma de alambre, delgadas, elasticas, que incluyen un dispositivo (3a, 3b) de reten cuando menos en un extremo de las mismas.

Description

IMPLANTES ESENCIALMENTE CILINDRICOS, REMOVIBLES La invención se relaciona con implantes esencialmente cilindricos, removibles. Ocasionalmente, debido a diversas razones, estos implantes, es decir predominantemente llamados "stents", se deben remover del cuerpo después del' procedimiento quirúrgico. En muchos casos, esto ocasiona problemas a medida que crece el tejido recientemente formado y dependiendo' de la estructura - también a través de los implantes de manera que la remoción puede ocasionar complicaciones . Existe de esta manera la necesidad de configurar implantes cilindricos de este tipo de manera de permitir una remoción del cuerpo de una manera sencilla con tan poca complicación como sea posible. Sorprendentemente, se ha encontrado que las dificultades experimentadas hasta ahora durante la remoción de implantes respectivos se puede superar de una manera relativamente sencilla cuando estos implantes se pueden reducir en diámetro y se envuelven mediante una o más estructuras de forma de alambre elásticas, delgadas, en lo que sigue designadas como alambre por fines de simplicidad, y cuando este alambre tiene en cuando menos un extremo un dispositivo de retén en el que un instrumento quirúrgico apropiado se puede acoplar o que se puede abarcar mediante dicho instrumento. Jalando el dispositivo de retén, el diámetro del implante se reduce por el alambre en uno o más niveles del implante completo de manera que el implanta se separa del tejido o de las paredes de tejido y se puede retirar . Los implantes de conformidad con la invención, es decir, "stents" en particular, están hechos de metal o plástico fisiológicamente compatible,- en forma de red o en forma de malla y frecuentemente se proporcionan en un lado con una ranura abierta de manera de ser comprimibles en el último caso, pero por lo demás extensibles, para de esta manera asegurar una reducción en diámetro. Esto se logra de conformidad con la invención proporcionando -uno o más alambres de metal o plástico que son delgados y elásticos, envueltos cuando menos en un nivel del implante alrededor del implante y normalmente asegurados, debido a razones de seguridad, a un extremo del implante mediante, v.gr., técnicas de sujeción típicas tales como soldadura, cruzado, anudado, o manera similar. Desde luego, también es posible eliminar en ciertos casos la necesidad de dicha fijación, cuando el alambre está envuelto varias veces alrededor del implante en un nivel y, de esta manera, se impide que deslice hacia afuera. Cuando menos un extremo del alambre o alambres se proporciona con un dispositivo de retén que puede estar . configurado, por ejemplo, como extremo engrosado para ser abarcado por un instrumento quirúrgico o como un ojete o lazo para acoplamiento con un instrumento apropiado. De conformidad con una modalidad preferida, el implante está envuelto cuando menos por 2 alambres que se enrollan en forma de una hélice alrededor · del implante y cuyos dos extremos libres, es decir, los extremos que no están fijos, incluyen un dispositivo de retén respectivo. En cualquier caso, es importante que el dispositivo de retén se proporcione en un momento cuando se construye el implante, es decir, no debe haber requisitos de colocar posteriormente un lazo alrededor del implante, sino que el dispositivo de retén ya está provisto o incorporado durante la producción del implante. El alambre puede correr libremente alrededor del implante, en particular cuando se enrolla varias veces, o puede correr en ojetes de guia que están dispuestos en el exterior, interior o en la pared del implante. Opcionalmente, el alambre también se puede extender entre el trenzado o red del implante y una camisa externa. Cuando se fija el lazo en el exterior, el implante se presiona junto o se restringe, en particular cuando el implante tiene una ranura. Cuando se dispone el alambre de tracción en el interior, el "stent" se aplasta. Es importante en cualquier caso la reducción del diámetro axial mediante la tracción longitudinal, de manera que el implante sea capaz de separarse suavemente del tejido. La construcción de alambre se puede extender a través de la longitud completa del implante o solamente en el extremo superior. Opcionalmente, la construcción de alambre también se puede extender a través del lumen del implante, luego transversalmente, por ejemplo. Cuando se involucran implantes de metal o el uso de alambres de metal, la construcción está de preferencia configurada de manera que se puedan atrapar a través de visualización utilizando endoscopía o través del uso de control de rayos X sin visualización directa. La construcción de conformidad con la invención simplifica significativamente la remoción de implantes cilindricos ya que la remoción es significativamente menos consumidora de tiempo y menos complicada. La invención se describirá ahora con mayor detalle con referencia al dibujos, en el cual: La Figura 1 muestra una ilustración- esquemática de un implante de conformidad con la invención con un solo lazo en un extremo, La Figura 2 muestra una ilustración esquemática de un implante con un lazo helicoidal como un soló lazo, La Figura 3 muestra una ilustración esquemática de un implante con un lazo helicoidal como un solo lado y un solo lazo en el extremo, La Figura 4 también muestra una ilustración esquemática de un implante con lazos helicoidales como lazo doble. El implante 1 de configuración substancialmente cilindrica está envuelto ya sea en un extremo con un solo lazo en forma de alambre que se puede asegurar ya sea a la pared exterior del implante o se envuelve varias veces alrededor del implante. Este lazo 2a en forma de alambre tiene en cada extremo un dispositivo 3a y 3b de retén, respectivamente, que ocasionan cuando menos parcialmente una reducción en diámetro del implante cuando se está jalando en la dirección de las flechas indicadas. En lugar de un solo lazo, el alambre también puede estar configurado como lazo helicoidal, como se muestra en la Figura 2, que incluye un dispositivo 3a de retén y se asegura al extremo 4a del implante de manera que se realice una reducción en el diámetro total del implante cuando se jala en la dirección de la flecha indicada. Como se muestra en la Figura 3, también es posible combinar un lazo helicoidal y un solo lazo entre sí, proporcionando el implante 1, por una parte, con un lazo 2a helicoidal que está asegurado en el extremo 4a, y por otra parte, con un solo lazo 2b, mediante lo cual ambos lazos se proporcionan con dispositivos 3a, 3b y 3c de retén. De esta manera, el diámetro total se reduce cuando se jala, mediante lo cual la reducción es más pronunciada en el extremo superior, en donde el lazo 2b sencillo adicional está colocado que en la porción restante. De conformidad con una modalidad preferida, como se muestra en la Figura 4, el implanta 1 tiene dos lazos 2a y 2b helicoidales, que están asegurados en los extremos 4a y 4b del implante y terminan en dispositivos 3a y 3b de retén. El lazo helicoidal doble es capaz de alcanzar una . reducción particularmente pronunciada en el diámetro del implante.

Claims (11)

REIVINDICACIONES
1. - Implantes removióles, esencialmente cilindricos, que se pueden reducir en diámetro y están envueltos una vez o varias veces en uno o más niveles mediante una o más estructuras en forma de alambre, delgadas, elásticas, que tienen cuando menos en un ' extremo un dispositivo de retén, caracterizados en que los implantes se pueden reducir en diámetro cuando menos parcialmente a través de la tracción longitudinal de los dispositivos de retén.
2. - Los implantes cilindricos de conformidad con la reivindicación 1, caracterizados en que el implante está hecho de una red o malla de metal o plástico, opcionalmente con una ranura abierta.
3. - los implantes cilindricos de conformidad con la reivindicación 1 o 2, caracterizados en que las estructuras en forma de alambre están hechas de metal o plástico.
4. - Los implantes cilindricos de conformidad con la reivindicación 1 a 3, caracterizados en que uno o más extremos de las estructuras en forma de alambre están asegurados sobre o en el implante.
5. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 4, caracterizados en que el dispositivo de retén está configurado como extremo engrosado, ojete o lazo.
6. - Los implantes cilindricos de conformidad con las .reivindicaciones 1 a 5, caracterizados en que las estructuras en forma de alambre abarcan al implante en un nivel solamente. -
7. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 6, caracterizados én que las estructuras en forma de alambre abarcar al implante de una manera helicoidal.
8. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 7, caracterizados en que las estructuras en forma de alambre se extienden entre el implante y una camisa externa.
9. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 8, caracterizados en que las estructuras en forma de alambre están configuradas a través de la longitud completa del implante.
10. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 8, caracterizados en que las estructuras en forma de alambre se extienden solamente en el extremo superior del implante.
11. - Los implantes cilindricos de conformidad con las reivindicaciones 1 a 10, caracterizados, en que se pueden capturar a través del uso de control de ' rayos X sin visualización directa.
