Reasoning round-trip: per-block replay with dialect and model provenance

Breaking: ReasoningBlock, ReasoningPartsFromBlocks and
GenerateResult.ReasoningProviderMetadata are gone; ReasoningBlocks is now
ReasoningParts. ReasoningPart gained ID, Format and Model. Unmarked reasoning
is no longer replayed, and Copilot switched to its real wire fields
(reasoning_opaque/reasoning_text).
