feat(notes): count note refinement and filing in the activity ledger - #133

Model calls that refine a note's title and summary, and the decisions that file it in PARA, count for the note (note_page:id) and its namespace. Requests to /api/v1/notes count for the note page rather than a recording's notes, readers of the namespace can see a note's history, and each note shows its costs and activity. Before: the model calls that refine a note's title and summary, and the decisions that file it, weren't counted against the note in the activity ledger. Requests to
/api/v1/notes/{id}were counted asnote:{id}, which is a recording's notes table.After: those calls count for the note (
note_page:5) and its namespace. Requests to note pages count for the note page, and a note can be made withPOST /api/v1/notes. Anyone who can read the namespace can see a note's history, and each note page ends with "Costs and activity".Motivation and Context
