test(session): count every sibling the concurrent test creates TestConcurrentSiblingCreation failed on Linux in CI: "Expected 10 unique paths (with 0 lock failures), got 8". Its goroutines send each path or error to one of two buffered channels, a goroutine closes both after the last one, and the test collected with a select that stopped at the first closed channel. Once both are closed, select chooses at random between the empty errors channel and a results channel that may still hold paths, so the count could miss some. The test now waits for the goroutines, closes both channels, and reads each to its end.
feat(lmc): support denial reasons and editable approval input Send operator denial reasons to the model with explicit user attribution, and preserve them in sessions and tool journals. Keep uncertain rerun outcomes distinct from tools that never ran. Reuse prompt continuation and editor helpers for tool approval input, including retained history selection and explicit submission after editing. Classify the final answer consistently and keep multiline feedback under its tool result. Cover terminal input, cancellation, EOF normalization, tool execution, provider messages, and persistence with regression tests.
