QF1012 matches fmt functions by package identity but previously hard-coded the default fmt qualifier in suggested fixes. Use the qualifier from the matched call so aliases such as format produce valid fixes. Add regression coverage for both Write and WriteString cases.
Validation: go test ./quickfix/qf1012 passes.
