The question Jeffrey Brown closes with is one we hear constantly, and it's exactly why we're working to close the gap between clinical and genomic data. Real-world data are extraordinary at capturing what happened. But "why" has always lived one layer deeper, in the biology. Connecting those two worlds, in a way that's auditable, governed, and tied back to the institutions that contributed the data, is what we believe unlocks the next generation of medical discovery. I’ve spent most of my career focused on one mission: How to efficiently enable multi-site health data research networks that can answer important public health questions no one can answer alone. Back when we built the initial implementation of the FDA Sentinel System, it was a massive gamble. Nobody knew if real-world clinical data - data never meant for research and scattered across different health systems - could answer questions of national consequence through a large, federated data network and partnership. We proved it could work with well understood and reasonably well-structured data. But now I'm thinking about what comes next. Clinical data tells us what happened to a patient, but it rarely tells us why. Take two patients. Same diagnosis, same treatment history, but completely different outcomes. The answer to why that happens usually lives in their biology, in a molecular layer the existing RWD sources simply weren't built to see. Genomics unlocks that layer. Yet for decades, biological and clinical data have lived in separate siloes, managed by separate teams, and stitched together by hand if at all. The barrier was never collecting the data. It was connecting it. That's exactly what we’ve been working to solve at TriNetX. I’m convinced that the most groundbreaking medical discoveries of the next decade are currently hiding right at the intersection of these two worlds. But getting there takes more than just buying up genomic data. It requires evidence you can defend. A complete picture of a patient is only valuable if it’s auditable, strictly governed, and still deeply connected to the institutions that contributed it. Terminal ileum resection is a common intervention in Crohn's disease but what are the long-term oncologic consequences? A TriNetX cohort study spanning nearly two decades found a 2.58-fold increase in colorectal cancer risk following resection, persisting regardless of colonic disease involvement and unaffected by biologic therapy.
