The pre m bike. I don’t want to. It’s raining. I am busy. Too much to do excuses. Then the this is amazing. Glad i am out in nature and rain and wind and sun. The peaks were fun today. Then I need to lose some weight to keep up with the formidable Dave Callan and look at the size of his calves. Then the thank goodness that’s over and let’s check the data and see how we did. Part of my wellbeing is being pushed out of my comfort zone in nature by people like Dave. A great bloke and a great coach.

The jobs market has turned a corner and is starting to show growth again after 45 consecutive quarters of decline. Good news, but the upturn is fragile and not every part of the UK is seeing the same level of growth....yet. But what's clear is that UK businesses are resilient and more and more, we are hearing about new plans for growth, diversification into new markets and businesses wanting to keep developing their employee's careers. Business sentiment is certinly moving the right way and with AI creating more jobs than it is taking, there is reason for increased optimism for jobseekers too. Six months after deployment, a review finds it has been systematically missing a specific patient cohort. The cases it missed were not random. They were predictable. This failure pattern has been documented across medical AI deployments globally, in diagnostic imaging, clinical decision support, and patient triage. What these systems share is not a flawed algorithm. They share a flawed data foundation. In our latest article, we look at the three ways medical AI breaks in production. Distributional shift. A model trained on one patient population is not automatically valid for another. That gap is set in the annotation dataset, long before the model is built. Edge cases. Rare pathologies are underrepresented by definition. When gaps in training data are not actively identified and filled, they become failure modes in the cases where early detection matters most. Data drift. Clinical guidelines change. A model calibrated to an earlier standard keeps classifying against a threshold that no longer exists, accumulating error invisibly until the gap is too large to ignore. The first consequence of getting this wrong is not a budget overrun or a compliance finding. It is a patient who receives the wrong diagnosis, a delayed treatment, or no referral at all. The question is not whether to invest in annotation quality.
