As a general principle, it tends to happen because people, who often
haven't read the standards, keep re-inventing things that break
fundamental design principles.
My guess, in the case of vertical centering, the problem is that it
breaks incremental rendering. Breaches are allowed, but most of the
current ones are in areas that would only be used by experts, who,
presumably can make a reasoned decision as to whether the effect
justifies the loss of incremental rendering. Vertical centering would
almost certainly result in a large proportion of pages using it and
therefore break incremental rendering on a large scale.
