WHEN THE GUARD IS MISSING - THE PROCEDURE IS NOT ENOUGH Real-world case: Conveyor maintenance What happened? A maintenance engineer was working during a night shift when he was asked to investigate a conveyor problem. The conveyor was still running. He reached through an opening in the conveyor casing to clear material. His arm was pulled into the moving machinery. The opening had been created years earlier to allow workers to clear blockages while the conveyor remained operational. The site also lacked an emergency stop at that location. The worker was alone and eventually managed to free himself. He suffered catastrophic injuries to his forearm and hand and remains unable to return to work, according to the UK HSE. 🔎 The deeper lesson The company already had: ✔ Risk assessments ✔ Safe systems of work ✔ Routine inspections ✔ Management walkarounds ✔ Audits But the dangerous modification remained. That's an important safety lesson: A safety management system can exist on paper while a dangerous condition exists in the workplace. What should have prevented the exposure? Engineering control → Fixed guarding around dangerous moving parts. Safe access → A designed method for clearing blockages. Isolation → Machinery isolated before intervention. Emergency response → Accessible emergency stop provision. Change control → Unauthorised modifications identified and corrected. Verification → Inspections that examine how equipment is actually being used—not just whether paperwork exists. HSE reported that the company subsequently installed fixed mesh guarding and an emergency stop at the location. The company was fined £160,000 after pleading guilty to health and safety breaches. 🎯 SAFETYTOOLBOX LESSON Don't ask only: “Do we have a procedure?” Ask: “What happens when the real job doesn't fit the procedure?” That question can reveal hazards that an audit checklist misses. 🔍 BEFORE YOU ACCEPT A MACHINE GUARD, ASK 5 QUESTIONS A guard can look perfectly acceptable during an inspection and still leave a serious gap. Before signing off machine guarding, ask: 1. Can someone physically reach the danger zone? Don't just look at the guard. Consider openings, gaps and possible reach paths. 2. Can the machine start while someone is exposed? Check unexpected or automatic start-up scenarios. 3. What happens when there is a blockage? This is where people often create shortcuts. 4. What happens during cleaning and maintenance? Normal production guarding may not control maintenance risks. 5. Can the guard be easily bypassed or removed? If the safe system depends entirely on worker discipline, ask whether engineering controls can be improved. A recent UK HSE prosecution illustrates the point. A maintenance engineer suffered life-changing injuries after his arm was pulled into an unguarded conveyor. HSE found that a hole had been cut into the conveyor casing years earlier to allow material to be cleared while the machine was running. THE HAZARD IS NOT ALWAYS THE PROBLEM. SOMETIMES, THE SYSTEM AROUND IT IS. 🔸A worker may know the correct procedure. 🔸A supervisor may have completed the safety briefing. 🔸A risk assessment may exist. 🔸A permit may be signed. 🔸And yet the unsafe condition can still remain. Why? Because safety is not created by documents alone. Ask: Can the worker actually follow the safe method? 🔹Is the machine physically guarded? 🔹Is the energy genuinely isolated? 🔹Is the correct equipment available? 🔹Is there enough time to do the job safely? 🔹Can the worker stop the task without fear? 🔹Has the workplace changed since the risk assessment? 🔹Are production pressures pushing people toward shortcuts? A procedure says: “Do it safely.” A good safety system asks: “What makes the safe way the easiest way to work?” That is a much deeper question. 🚨 ONE SHIFT CAN CHANGE EVERYTHING. Look closely at this truck. 👀 The load appears to be leaning/shifted toward the cab. Now imagine this happening during: ⚠️ Sudden braking ⚠️ Sharp turning ⚠️ Uneven road conditions ⚠️ High speed ⚠️ Emergency manoeuvring The result could be catastrophic. 🧐 WHAT HAZARDS DO YOU SEE? 👉 Load shift 👉 Inadequate restraint 👉 Potential crush/struck-by zone 👉 Loss of vehicle stability 👉 Risk to the driver and other road users 🛡️ HOW CAN WE ELIMINATE / CONTROL THE HAZARD? Don't rely only on straps. 1️⃣ Engineer the load Use suitable blocking, chocks, dunnage and supports. Prevent movement in all directions. Keep the centre of gravity as low as practicable. 2️⃣ Use the right restraint system Select properly rated chains/straps and anchor points. Ensure restraints are suitable for the load type and weight. Protect restraints from sharp edges. 3️⃣ Secure before moving Conduct a pre-trip load-securing inspection. Check restraint tension and positioning. Recheck the load after starting the journey and at planned stops. 4️⃣ Protect the cab Where required, use an appropriately designed headboard/bulkhead or other engineered load-retention system. Never assume the trailer front structure alone will stop a moving load. 5️⃣ Control the journey Appropriate speed. Avoid sudden braking and aggressive manoeuvring. Consider road, weather and load characteristics. 🎯 SAFETY LESSON A load that is stable while parked may not remain stable when the vehicle is moving. YOUR RISK ASSESSMENT MAY BE COMPLETE. BUT IS THE RISK CONTROLLED? A signature doesn't make a risk assessment effective. A completed form doesn't make a job safe. The real question is: “What changed after the risk was identified?” A good risk assessment should lead to action: 🔹 Hazards identified 🔹 Risks evaluated 🔹 Controls selected 🔹 Controls implemented 🔹 Controls verified But there is one step we often underestimate: REASSESS WHEN CONDITIONS CHANGE. A new person joins the job. The equipment is modified. The sequence changes. Weather conditions change. Multiple activities start interacting. Production pressure increases. The risk can change, even when the task name remains the same. That's why risk assessment shouldn't be treated as a document completed before work. It should be a living decision-making process. ⚠️ Safe Distance Should Be Designed! Not Guessed. When something goes wrong at a workplace, simply telling employees to “stand back” is not enough. An effective risk assessment should ask: 🔹 What can fail? 🔹 What could be released, ejected, dropped, or projected? 🔹 How far could the hazard reach? 🔹 Where should people be positioned to remain safe? From there, we can apply engineering controls such as: ✅ Physical barriers and guarding ✅ Interlocked protection systems ✅ Safe operating zones ✅ Exclusion zones and controlled access ✅ Remote operation where practical ✅ Clearly defined safe distances The goal is simple: Don’t rely only on people to avoid the hazard. Design the workplace so people are protected from the hazard. A good risk assessment doesn't just identify what can hurt us.
