ADR: Upgrade to Asciidoctor 2

The upgrade to Asciidoctor 2 (by way of Asciidoctor.js 2) is significant. Therefore we want to capture this transition in an ADR. The ADR should describe why we want to upgrade, what impact the upgrade will have, and how the version of Asciidoctor.js should be managed going forward.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
