PatternMatcher ignores indexes, always full-scan

File: matcher.rs:61-74

Even though loads facts and the optimizer generates values, PatternMatcher always does a full scan.

Fix: Implement index-guided lookups using the from optimizer. See TODO at executor.rs:169-170.

Reopening: PR #69 wired real indexes into the optimizer and added an indexes field to PatternMatcher, but that field is marked #[allow(dead_code)] and PatternMatcher still does full scans on every pattern match. The PR body acknowledged this as 'Phase 6.2 work'. The full-scan problem from the original issue remains open.
