Add error recall: have I hit this failure before, and what fixed it #15

Open

Description

@karanb192

What: At index time, fingerprint error text from failed tool_results and error events (normalize paths, hashes, and line numbers before hashing), and index fingerprint to occurrences. A recall surface in ⌘K: paste or select an error, see every past session that hit it and what happened next, jumping to the moment the fix landed.

Why: Engineers hit the same class of failure across months and projects, and the archive already contains both the error and its resolution. Nothing else on the machine can answer "I have seen this before, where" from real history.
