    💬 "Entre les ratures, les oublis et les différences de remplissage d'un patient à l'autre, il faut parfois autant de temps pour interpréter l'agenda que pour analyser réellement le sommeil." Cette remarque nous a marqué. Car l'agenda sommeil est un outil précieux. Mais lorsque les données sont difficiles à recueillir, à relire ou à comparer, une partie de leur valeur se perd.     Is a Single Night Enough? The Data Gap in Sleep Diagnostics We live in an era of personalized medicine, yet when it comes to sleep disorders, many patients still receive a diagnosis based on a single, unnatural night in a lab. The "First-Night Effect" (FNE) is well-documented: stress, anxiety, and being tethered to dozens of wires in a clinical environment significantly disrupt sleep architecture. This often means crucial REM sleep is suppressed, potentially masking severe apnea or other parasomnias. A Single-Night Study is a Snapshot. A Multi-Night Study is a Data-Rich Timeline. New trends in clinical diagnostics are moving rapidly toward average-based testing over multiple consecutive nights, often in the comfort of the patient’s home. The infographic attached to this post breaks down the fundamental differences between the traditional In-Lab Single-Night PSG and the new paradigm of At-Home Multi-Night HST. Here is why the clinical community is prioritizing consistency over a snapshot: Environmental Context (The Home Advantage): Natural, familiar sleep environments produce natural data. This is where the majority of your sleep happens, not in a sterile lab. Data Integrity (The First-Night Effect): Multi-night tests average data across 3–7 nights, completely bypassing the distortion caused by First-Night FNE. Capturing Variability (The Moving Average): Night-to-night variability in sleep disorders (e.g., respiratory events) is significant. Only multi-night tracking captures your actual "moving average" risk. AI Integration: Artificial Intelligence (AI) can analyze huge longitudinal datasets to identify trends that human scorers, working on a single night, might overlook. The move toward multi-night diagnostics is about closing the data gap and achieving high-fidelity clinical insights.     🧠 New paper out! Sleep variability as an early marker of Alzheimer’s disease? Sleep disturbances are common in Alzheimer’s disease, but they may begin long before clinical symptoms emerge. In our latest study, conducted by Blandine Montagne, within the Neuropresage team, we focused on 𝗻𝗶𝗴𝗵𝘁-𝘁𝗼-𝗻𝗶𝗴𝗵𝘁 𝘃𝗮𝗿𝗶𝗮𝗯𝗶𝗹𝗶𝘁𝘆 𝗶𝗻 𝘀𝗹𝗲𝗲𝗽, a dimension that is still underexplored, and assessed its association with amyloid deposition in cognitively unimpaired older adults. 🔍 What we did: We assessed 76 older adults using: - multi-night objective sleep recordings using the Somno-Art® wearable developed by PPRS, - amyloid PET imaging, - comprehensive cognitive and psychoaffective evaluations. 📊 What we found: - Individuals with amyloid deposition 𝘀𝗹𝗲𝗽𝘁 𝗹𝗲𝘀𝘀 𝗼𝘃𝗲𝗿𝗮𝗹𝗹. - More importantly, they showed 𝐠𝐫𝐞𝐚𝐭𝐞𝐫 𝐧𝐢𝐠𝐡𝐭-𝐭𝐨-𝐧𝐢𝐠𝐡𝐭 𝐯𝐚𝐫𝐢𝐚𝐛𝐢𝐥𝐢𝐭𝐲 𝐢𝐧 𝐑𝐄𝐌 𝐬𝐥𝐞𝐞𝐩 𝐝𝐮𝐫𝐚𝐭𝐢𝐨𝐧. . - Across participants, this 𝐑𝐄𝐌 𝐬𝐥𝐞𝐞𝐩 𝐯𝐚𝐫𝐢𝐚𝐛𝐢𝐥𝐢𝐭𝐲 was associated with 𝐡𝐢𝐠𝐡𝐞𝐫 𝐚𝐦𝐲𝐥𝐨𝐢𝐝 𝐛𝐮𝐫𝐝𝐞𝐧 𝐢𝐧 𝐰𝐢𝐝𝐞𝐬𝐩𝐫𝐞𝐚𝐝 𝐛𝐫𝐚𝐢𝐧 𝐫𝐞𝐠𝐢𝐨𝐧𝐬.  - These sleep changes were n𝐧𝐨𝐭 𝐲𝐞𝐭 𝐥𝐢𝐧𝐤𝐞𝐝 𝐭𝐨 𝐜𝐨𝐠𝐧𝐢𝐭𝐢𝐯𝐞 𝐨𝐫 𝐩𝐬𝐲𝐜𝐡𝐨𝐚𝐟𝐟𝐞𝐜𝐭𝐢𝐯𝐞 𝐝𝐢𝐟𝐟𝐞𝐫𝐞𝐧𝐜𝐞𝐬? 💡 Why it matters: Our findings highlight that 𝐧𝐢𝐠𝐡𝐭-𝐭𝐨-𝐧𝐢𝐠𝐡𝐭 𝐬𝐥𝐞𝐞𝐩 𝐯𝐚𝐫𝐢𝐚𝐛𝐢𝐥𝐢𝐭𝐲, 𝐞𝐬𝐩𝐞𝐜𝐢𝐚𝐥𝐥𝐲 𝐢𝐧 𝐑𝐄𝐌 𝐬𝐥𝐞𝐞𝐩, 𝐦𝐚𝐲 𝐛𝐞 𝐚 𝐬𝐞𝐧𝐬𝐢𝐭𝐢𝐯𝐞 𝐚𝐧𝐝 𝐞𝐚𝐫𝐥𝐲 𝐢𝐧𝐝𝐢𝐜𝐚𝐭𝐨𝐫 𝐨𝐟 𝐀𝐥𝐳𝐡𝐞𝐢𝐦𝐞𝐫’𝐬-𝐫𝐞𝐥𝐚𝐭𝐞𝐝 𝐛𝐫𝐚𝐢𝐧 𝐜𝐡𝐚𝐧𝐠𝐞𝐬, even before measurable clinical symptoms.     What if improving ICU outcomes started with one simple thing: letting patients sleep? Congratulations to Xavier Drouot and his team on the publication of “Improving sleep in ICUs through real-time sleep monitoring: a proof-of-concept study” ! 

    Thrilled to announce the publication of our paper "Improving sleep in ICUs through real-time sleep monitoring: a proof-of-concept study" in Annals of Intensive Care. https://lnkd.in/dSTSfTtb Sleep disturbances are frequent in the ICU and associated with poor outcomes. Nursing care during the night frequently disturb sleep.  We hypothesized that if caregivers were aware of the status of the patient (asleep or awake), they would be able to reorganise nursing care according to the patients’ sleep cycle. Using a real-time  sleep monitoring system in 46 non-sedated patients, we show a decrease of room entries while patients were awake and a considerable increase in long sleep episodes with a doubling of slow wave sleep in sleep-guided care group compared to usual care group. This opens exciting opportunities to test the impact of better sleep on weaning duration or delirium. This is a meaningful step forward — not just for ICU patients, but for all hospitalized patients whose recovery depends on quality sleep.
